Acute Alcohol-Associated Hepatitis: The Role of Liver Transplantation.

Severe alcohol-associated hepatitis (AH) remains one of the most challenging syndromes in hepatology and critical care, with high short-term mortality despite advances in supportive management. Although corticosteroids remain the only guideline-endorsed pharmacologic therapy shown to improve early survival, their benefit is modest, limited to selected patients, and frequently complicated by infection. Consequently, contemporary management extends well beyond corticosteroid therapy and requires meticulous supportive care, nutritional optimization, infection surveillance, management of portal hy

Multidimensional regulation of hepatic macrophages in chronic liver disease: from metabolic reprogramming to intercellular and interorgan crosstalk.

Chronic liver diseases (CLDs), including metabolic dysfunction-associated steatotic liver disease (MASLD), alcohol-associated liver disease (ALD, viral hepatitis, liver fibrosis, cirrhosis and hepatocellular carcinoma (HCC)), impose enormous global public health burdens. Hepatic macrophages act as core orchestrators governing the initiation and progression of all CLD subtypes. This review systematically integrates recent advances regarding hepatic macrophage ontogeny, spatial heterogeneity, metabolic rewiring, multi-cellular communication, and systemic neuro-immune crosstalk along the gut-live

Spleen stiffness to rule out high-risk varices in advanced chronic liver disease: an individual patient data meta-analysis.

BACKGROUND & AIMS: Management of esophageal varices (EV) is of critical importance in compensated advanced chronic liver disease. We conducted an individual patient data (IPD) meta-analysis to (i) investigate the performance of spleen stiffness measurement (SSM)-based algorithms using vibration-controlled transient elastography (VCTE) to rule out high-risk varices (HRV); (ii) develop a new model for diagnosing any grade EV. METHODS: We systematically searched databases for studies reporting endoscopy and SSM-VCTE data in adult patients, collecting their IPD. The main outcome was ruling out HR

Stigma related to hepatitis B virus: A barrier to global viral hepatitis elimination.

Chronic hepatitis B virus (HBV) infection remains a major global health challenge, affecting over 300 million people worldwide and contributing substantially to liver-related morbidity and mortality. Despite the availability of effective vaccination, diagnostic tools, antiviral therapy, and surveillance strategies, progress toward the World Health Organization's hepatitis elimination targets has been slower than anticipated. HBV-related stigma has emerged as a critical and under-recognized barrier undermining efforts across the entire HBV care cascade. Stigma associated with HBV is multi-facet

Hepatitis B reactivation in HBsAg-/anti-HB-negative patients diagnosed with hematological malignancy: A case series.

BACKGROUND: Hepatitis B virus (HBV) reactivation is a well-recognized complication of immunosuppressive therapy in patients with hematologic malignancies. It usually occurs in those with chronic or resolved HBV infection identified by positive hepatitis B surface antigen (HBsAg) or hepatitis B core antibody (anti-HBc). Reactivation in patients who test negative for both markers is exceedingly rare and poses a diagnostic challenge. METHODS: We report on three patients with hematologic malignancies who developed HBV reactivation despite negative baseline serology. Cases were identified and mana

Causal associations between chronic hepatitis B and major depressive disorder in East Asian populations: a bidirectional two-sample Mendelian randomization study.

BACKGROUND: Epidemiological studies suggest that there is a strong association between chronic hepatitis B (CHB) and major depressive disorder (MDD). However, it remains uncertain whether these associations are causal, as much of the current evidence originates from observational studies that are prone to confounding and reverse causation. METHODS: Single nucleotide polymorphisms robustly associated with CHB were obtained from genome-wide association studies, including 212 453 individuals from the BioBank Japan Project and 4309 individuals of East Asian descent from Korea. Two-sample bidirect

Development of dihydroquinolizinone (DHQ)-based inhibitors for suppression of hepatitis B viral infections.

Chronic hepatitis B virus (HBV) infection is a serious public health burden, and current treatments fail to achieve satisfactory cure rates. RG7834 is a novel therapeutic agent with a differentiated mechanism of action (MOA) compared to current standard treatments. However, its Phase I clinical trial was terminated due to neurotoxicity issues. Furthermore, the pharmacokinetic performances of RG7834 are unsatisfactory, leaving significant room for improvement. This article reports some attempts and efforts based on the RG7834 molecule, including enhancing antiviral activity, improving pharmacok

High Frequency of Circulating IL-4R+SELL+ Naïve B Cells Is Associated with Functional Cure in Chronic Hepatitis B Patients Receiving Antiviral Treatment.

BACKGROUND AND AIMS: Although hepatitis B surface antigen (HBsAg)-specific B cells have been partially characterized in chronic hepatitis B (CHB), the role of naïve B cells and their potential relevance to functional cure remain insufficiently explored. Here, we aimed to characterize the naïve B-cell subsets in functionally cured and uncured patients with CHB. METHODS: In this retrospective study, we applied single-cell RNA sequencing to compare peripheral B-cell profiles in functionally cured and uncured CHB patients. Key findings were validated by flow cytometry in an independent cohort and

Empowerment in advanced liver disease: UK experiences of care from the perspectives of people with lived experience, carers and healthcare professionals.

ObjectivesLiver disease is a leading cause of morbidity and mortality, with alcohol-related liver disease, metabolic-associated steatotic liver disease, and viral hepatitis comprising most cases in the UK. Advanced liver disease presents challenges due to unpredictable disease trajectories, high symptom burden, psychosocial impacts, and stigma. This study aimed to develop an explanatory theory of care experiences in decompensated advanced liver disease from the perspectives of individuals living with the condition, carers, and healthcare professionals, with public involvement embedded througho

[Effects of gambogic acid on liver function in rats with chronic hepatitis B by adjusting Sirt1/FoxO1 pathway].

This study investigated the effects of gambogic acid(GA) on liver function in rats with chronic hepatitis B(CHB) via the silent information regulator 2 homolog 1(Sirt1)/forkhead box transcription factor O1(FoxO1) signaling pathway. A CHB rat model was established using hepatitis B virus(HBV) antigen. Another 10 healthy rats injected with an equal volume of physiological saline served as the control group. After successful modeling, rats were divided into a CHB group, low-dose GA(L-GA, 50 mg·kg~(-1) GA by gavage) group, high-dose GA(H-GA, 100 mg·kg~(-1) GA by gavage) group, and GA+EX527(100 mg·

Chronic Hepatitis B: Current Understanding and Clinical Implications of Immune Exhaustion.

Chronic hepatitis B (CHB) infection leads to progressive dysfunction and exhaustion of antiviral immune cells, which is a primary obstacle to clinical cure. Although the phenotypic hallmarks of exhausted T cells are well described, the hierarchical trajectory, subset-specific heterogeneity, and underlying mechanisms of exhaustion across the full spectrum of immune cells in the unique hepatic microenvironment remain poorly defined. This review provides a comprehensive and critical synthesis of recent advances in immune exhaustion during CHB. We delineate the common and distinct exhaustion progr

A Single Center Retrospective Study: Efficacy and Platelet Preservation of Double Plasma Molecular Adsorption System Therapy by Membrane Separation Versus Centrifugal Separation in Acute-on-Chronic Liver Failure Patients With Hepatocellular Carcinoma.

Hepatitis B virus (HBV) infection is a leading cause of hepatocellular carcinoma (HCC) and liver failure. The double plasma molecular adsorption system (DPMAS) reduces bilirubin and may create a therapeutic window for anti-tumor therapy in HCC patients. Two plasma separation techniques are used: membrane-based (MBS-DPMAS) and centrifugal (CPS-DPMAS), but no study has compared them specifically in HCC patients with acute-on-chronic liver failure (ACLF). This retrospective study included 203 HBV-related HCC patients who received DPMAS without plasma exchange (93 MBS-DPMAS, 110 CPS-DPMAS), with c

Mass Cytometry Reveals Cell Type-specific Immune Remodeling in Placentas of Women with Chronic Hepatitis B Infection.

BACKGROUND AND AIMS: Hepatitis B virus (HBV) vertical transmission remains a major health challenge, and the characteristics of the placental immune microenvironment in chronic infection remain unclear. This study aimed to use mass cytometry to comprehensively analyze phenotypic changes in placental immune cell subsets. METHODS: We collected placental tissues from 20 pregnant women (6 healthy controls and 14 with chronic HBV infection). CD45+ leukocytes were detected using a 35-marker antibody panel. Unsupervised clustering identified 14 clusters, of which 13 immune clusters were analyzed. R

Viral persistence and immune dysfunction in chronic hepatitis B: Mechanisms of post-treatment antigen asynchrony.

Chronic hepatitis B (CHB) remains a major global health burden. Following antiviral therapy, the dynamics of hepatitis B virus (HBV) DNA and hepatitis B surface antigen (HBsAg) may become discordant, a phenomenon referred to as post-treatment asynchrony. This review summarizes the three principal forms of post-treatment asynchrony and examines their underlying mechanisms. We integrate the limitations of current antiviral targets, host immune dysfunction, virological characteristics, and host genetic factors into a unified framework to explain the discordant dynamics between HBsAg and HBV DNA.

Experimental models of hepatic injury in Wilson disease: stage-specific mechanisms and translational applications.

BACKGROUND: Wilson disease (WD) is an autosomal recessive disorder of copper metabolism caused by pathogenic variants in ATP7B, with the liver being one of the earliest and most prominently affected organs. ATP7B dysfunction impairs copper incorporation into ceruloplasmin and biliary copper excretion, resulting in progressive hepatic copper accumulation and a cascade of pathological responses, including proteostasis disturbance, mitochondrial dysfunction, oxidative stress, metabolic remodeling, inflammation, and fibrogenesis. Given the dynamic progression of WD-associated liver injury from ear
