INTRODUCTION: Chiglitazar was approved by the China National Medical Products Administration (NMPA) in 2021 for the treatment of patients with type 2 diabetes mellitus, and may also benefit from metabolic dysfunction-associated steatohepatitis (MASH).
METHODS: To characterise the role of Chiglitazar in MASH, in vitro experiments examining monocyte activation and migration were conducted, alongside liver and stellate cell proliferation assays. Three distinct MASH animal models (db/db mouse model, MCD model, CCl4 model) for conducting in vivo studies were established.
RESULTS: The results indi

Silymarin, a flavonolignan complex from Silybum marianum, and its principal constituent silybin have long been used as hepatoprotective and antifibrotic agents. In the liver, silymarin suppresses tissue inhibitor of metalloproteinases 1 (TIMP-1), a central regulator of matrix turnover in fibrogenesis. This effect is reproducible across several models of hepatic fibrosis, but its magnitude is inconsistent. Some studies report no or only a nonsignificant reduction in hepatic TIMP-1 with free silymarin, while others report a substantial decline. Much of this inconsistency tracks with formulation

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a growing global health challenge. Its advanced stage, metabolic dysfunction-associated steatohepatitis (MASH), involves complex processes such as metabolic dysregulation, inflammation, and fibrosis. Existing treatments mainly target steatosis but have limited success. Therefore, focusing on inflammation and fibrosis may offer more effective therapeutic options. Phosphodiesterases regulate cAMP/cGMP signaling and are key in MASH-related inflammation and fibrosis. Inhibiting specific PDE subtypes is a promising approach for dru

BACKGROUND AND STUDY AIMS: Methotrexate (MTX) is an immunosuppressive drug used to treat various chronic inflammatory diseases. While its association with liver fibrosis is concerning, several associated factors appear to play a major role. This study aimed to assess the frequency of fibrosis in MTX-treated patients and identifies its associated factors.
PATIENTS AND METHODS: A descriptive cross-sectional study was conducted including patients with chronic inflammatory diseases who had been on MTX for at least 6 months. Liver fibrosis was assessed using transient elastography. A comparative a

Metabolic dysfunction-associated steatotic liver disease (MASLD), previously known as NAFLD, is increasingly recognized as a prevalent global burden. Type 2 diabetes mellitus (T2DM), another important metabolic disease, is considered a major contributor to the development of MASLD. MASLD and T2DM have a strong association with each other due to shared pathogenic mechanisms. The co-existence of the 2 diseases increases the risk of liver-related adverse outcomes and imposes a heavier burden on extrahepatic outcomes, representing a substantial public health issue. Effective assessment and managem

Liver fibrosis is a progressive pathological condition closely linked to gut microbiota dysbiosis and impaired intestinal barrier integrity. Vildagliptin, a dipeptidyl peptidase-4 inhibitor used in type 2 diabetes, exhibits pleiotropic anti-inflammatory and antioxidant properties; however, its impact on the gut-liver axis during hepatic fibrogenesis remains insufficiently defined. This experimental study investigated whether concomitant vildagliptin administration attenuates the development of CCl₄-induced liver fibrosis and whether this effect is associated with alteration in gut microbial co

BACKGROUND: Hepatic fibrosis represents a critical pathological stage in the progression of chronic liver diseases, yet clinically effective and low-toxicity therapeutic options remain limited. Taraxacum mongolicum and Astragalus form a classic hepatoprotective herbal pair, and their derived polysaccharides-Taraxacum mongolicum polysaccharides (TMP) and Astragalus polysaccharides (APS)-both exhibit hepatoprotective activities. This study aimed to investigate the ameliorative effects of TMP combined with APS on CCl4-induced hepatic fibrosis in mice and the gut-liver axis-associated changes.

Ultrasound plays a central role in the diagnosis, monitoring, and management of liver diseases. Assessing hepatic fibrosis is essential in chronic liver disease, and several diagnostic modalities are available. The gold standard remains percutaneous liver biopsy, an invasive method using a 16-18-gauge needle. A breakthrough came in 2003 with transient elastography (TE), a non-invasive technique that measures liver stiffness (kPa) via elastic wave propagation. Later, shear wave elastography (SWE), integrated into modern ultrasound systems, was developed to assess tissue elasticity. SWE generate

Circular RNAs (circRNAs) are a subclass of noncoding RNAs characterized by their closed-loop structure without terminal 3' or 5' ends. Studies have shown that circRNAs play pivotal roles in the regulation of various cellular processes. These molecules function as microRNA (miRNA) sponges, interact with RNA-binding proteins, and modulate gene transcription. CircRNAs are vital for regulating liver homeostasis, and dysregulation of their expression is correlated with liver diseases such as hepatic fibrosis, steatosis, inflammation, and liver cancer. Elucidating the functional significance of circ

Metabolic dysfunction-associated steatotic liver disease (MASLD) has emerged as a critical global public health challenge, with limited therapeutic options underscoring the necessity for innovative therapies. Thyroid hormone receptor-β (THR-β), the predominant THR isoforms in the liver, plays a pivotal role in regulating lipid homeostasis, inflammation, and hepatic fibrosis, positioning it as a compelling therapeutic target. Although early THR-β agonists have demonstrated potential, their clinical application was restricted by systemic side effects and poor THR-β selectivity. Advances in medic

Plant-based therapeutics are of immense importance in modern drug discovery owing to their multi-targeted mechanisms and favourable safety profiles. Among these, Petroselinum crispum has long been utilised in traditional medicine, yet remains underexplored for its potential against liver fibrosis, a progressive condition lacking effective antifibrotic therapies. In this study, a comprehensive computational strategy was employed to systematically investigate the hepatoprotective potential of P. crispum metabolites. A total of 177 phytochemicals from P. crispum were screened based on drug-likene

BACKGROUND: The interplay between glomerular hyperfiltration (GHF) and downstream renal dysfunction and hepatic fibrosis in metabolic dysfunction–associated steatotic (or fatty) liver disease (MASLD or MAFLD) remains insufficiently understood. We investigated these associations in a large cohort of generally healthy adults with a low prevalence of obesity. METHODS: In a longitudinal health-screening cohort, 47,741 adults aged 18–70 years were followed from 2002 to 2019, with baseline liver ultrasonography and annual renal assessments. Hepatic steatosis was diagnosed by ultrasonography. GHF was

Quantitative viscoelasticity imaging via shear wave elastography (SWE) remains challenging due to complex wave physics and limitations of conventional reconstruction methods. To address this, we present SW-VEI-Net, a physics-informed neural network (PINN) that simultaneously reconstructs the shear elastic modulus and viscous modulus by integrating viscoelastic wave equations into a dual-network architecture. The framework employs a dual-loss function to balance data fidelity and physics-based regularization, significantly reducing reliance on empirical data while improving interpretability. Ex

BACKGROUND: There is limited evidence on the prevalence of, and factors contributing to, metabolic dysfunction-associated steatotic liver disease (MASLD) among individuals with type 1 (T1DM) and type 2 (T2DM) diabetes mellitus.
METHODS: We consecutively enrolled 1304 adult individuals with T1DM (n = 237) or T2DM (n = 1067) who underwent vibration-controlled transient elastography (VCTE) with liver stiffness measurement (LSM) and controlled attenuation parameter (CAP) assessment. MASLD was defined as a CAP ≥ 248 dB/m in the presence of diabetes. Significant and advanced liver fibrosis were def

BACKGROUND: Liver fibrosis is characterized by deposition of excessive extracellular matrix (ECM). The major source of ECM is activated hepatic stellate cells (HSCs). NAT10 is the only known acetyltransferase catalyzing ac4C RNA modification. The purpose of this study is to explore the role of NAT10 acting as ac4C acetyltransferase during HSC activation.
METHODS: NAT10 was detected in fibrotic liver tissues from S. japonicum infected mice with immunohistochemistry and TGF-β1 stimulated LX-2 human HSC cells with Western blot and immunofluorescent staining. NAT10 was inhibited with specific siR

Heart failure (HF) is a chronic progressive clinical condition characterized by structural or functional impairment of the ventricles. The prevalence of HF is increasing, and it continues to be a significant cause of morbidity and mortality. Liver involvement during fibrosis in HF patients is associated with adverse clinical outcomes. Our study aimed to evaluate the relationship between noninvasive liver fibrosis indices and HF mortality. Our study was a retrospective analysis of patients with HF admitted to our hospital between 2017 and 2023. Patients were followed up until 2024 and divided i

Hepatic fibrosis is a major driver of mortality in metabolic dysfunction-associated steatotic liver disease (MASLD)-previously known as non-alcoholic fatty liver disease (NAFLD). While hepatic stellate cell (HSC) activation and myofibroblast accumulation are central to fibrogenesis, the regulatory mechanisms remain incompletely understood. Acetyl-CoA acyltransferase 2 (ACAA2), a pivotal enzyme in fatty acid oxidation, has been implicated in lipid metabolism but has not been investigated as a therapeutic target in MASLD. Here, we show that ACAA2 upregulation in HSCs exacerbates hepatic fibrosis
