Primary biliary cirrhosis and inflammatory bowel disease: a two-sample bidirectional Mendelian randomization study.

Observational studies have frequently reported an association between primary biliary cirrhosis (PBC) and inflammatory bowel disease (IBD). In this study, we leveraged summary-level data from genome-wide association studies (GWAS) to conduct a two-sample bidirectional Mendelian randomization (MR) analysis, with the primary objective of investigating the genetic causal relationship between PBC and IBD, comprising ulcerative colitis (UC) and Crohn's disease (CD). Additionally, a validation analysis was performed by repeating the bidirectional MR framework, alternately defining PBC and IBD as the

Benign Transient Hyperphosphatasemia in Adults: Case Report and Literature Review.

BACKGROUND: Benign transient hyperphosphatasemia (BTH) in children is characterized by temporary highly elevated serum alkaline phosphatase (ALP) activity in the absence of liver or bone disease and a return to normal within 3 to 4 months. Since its first description in the 1950s, several cases of BTH in infants and children have been reported. Although there is no known etiology for this illness, it has been associated with viral infections. This condition has rarely been seen in the adult population, and there is scarce available information. The sialylation of the ALP isoenzymes decreases t

Cure rate estimation with insufficient follow-up: A median-based bootstrap correction approach.

The cure rate in clinical trials can be estimated using the Kaplan-Meier (KM) estimator. However, when the clinical trial follow-up period is insufficient and short, the KM estimator may overestimate the proportion of cured patients. Although a correction method was proposed by Escobar-Bach and Keilegom based on bootstrap sampling, this can also lead to bias when the bootstrap distribution is skewed. We propose a median-based approach for bootstrap samples to address these issues. Simulation results showed that the effect of the variation of the proposed method due to many outliers was smaller

Primary biliary cirrhosis and rheumatoid arthritis: a comprehensive study combining genetics and transcriptomics.

Observational investigations have discerned a correlation between primary biliary cirrhosis (PBC) and rheumatoid arthritis (RA). The primary objective of this study is to elucidate the disease risk correlation between them in terms of genetics, and to explore the genes and signaling pathways involved. We acquired genome-wide association studies (GWAS) summary data for PBC and RA from distinct repositories: PBC data sourced from the IEU OpenGWAS database, and RA data obtained from the Finnish consortium. Employing a bidirectional two-sample Mendelian randomization (MR) approach, we scrutinized

Causal inference of metabolites in autoimmune liver diseases: A Mendelian randomization analysis.

Autoimmune liver disease (AILD) is a chronic and insidious liver disease that can lead to cirrhosis. Metabolites have been shown to play an essential role in autoimmune diseases, but the causality between metabolites and AILD has not been completely investigated. This Mendelian randomization (MR) analysis was conducted to evaluate the causal relationship of metabolites on AILD. A genome-wide association study was conducted using 1400 metabolites as the exposure, and 3 AILD phenotypes primarily from European sources were identified as the results. Inverse-variance weighted method was implemente

Quantile inference for multivariate response regression in joint modeling of longitudinal and survival data.

Quantile regression (QR) offers a robust framework for analyzing covariate effects across the outcome distribution, particularly when the response variable exhibits skewness or heavy tails. To jointly model multivariate longitudinal biomarkers and a time-to-event outcome, we propose a novel joint model based on linear QR mixed models. The longitudinal submodel accounts for associations among multivariate responses by assuming a multivariate asymmetric Laplace distribution for the errors, utilizing its location-scale mixture representation for computational efficiency. For the event-time proces

Varying Treatment Effects in Subgroups: A Unified Framework for Longitudinal Data Analysis.

The rise of precision medicine has shifted statistical analysis from population-averaged treatment effects to subgroup-specific effects, highlighting the importance of tailoring treatments to distinct patient subgroups. Longitudinal data, in particular, provides an opportunity to study how treatment effects evolve over time. However, methods for analyzing subgroup-specific, time-varying treatment effects in longitudinal settings remain limited. Most current approaches rely on two-step procedures with restrictive assumptions, such as a fixed number of subgroups or prespecified correlation struc

Integrating preliminary test and Stein-type techniques to improve estimation in the time-dependent Cox model.

While shrinkage and preliminary test estimation have long been studied in linear and static Cox models, their theoretical integration within models featuring time-dependent covariates has remained unresolved due to the evolving risk set and nonhomogeneous information accumulation inherent in such data. In this study, we develop a unified framework for shrinkage estimation in the time-dependent Cox proportional hazards model, by extending the classical Stein-type theory to a dynamic semiparametric survival setting. Our theoretical analyses reveal that the positive-rule Stein estimator preserves

When Chest Pain Unmasks the Liver: Acute Pericarditis Leading to the Diagnosis of AIH-PBC Overlap Syndrome.

Autoimmune pericarditis is commonly idiopathic but may represent the initial manifestation of systemic autoimmune disease. Hepatic autoimmune disorders rarely present with primary cardiac involvement, and recognition of extrahepatic presentations is essential for early diagnosis and prevention of organ damage. Autoimmune hepatitis-primary biliary cholangitis (AIH-PBC) overlap syndrome is an uncommon entity characterized by combined hepatocellular and cholestatic immune-mediated injury. A previously healthy young woman presented with progressive pleuritic chest pain and dyspnea and was found to

BACKGROUND/AIM: Partial splenic embolization (PSE) is an established interventional radiology procedure for portal hypertension. However, in cases of splenomegaly with complex vascular anatomy, guide catheter placement in the splenic artery may be difficult. We aimed to evaluate the clinical characteristics and technical feasibility of microballoon catheter-assisted PSE in such challenging cases. PATIENTS AND METHODS: Among 279 patients who underwent PSE at our institution between November 2007 and March 2026, we retrospectively analyzed 12 patients in whom conventional guide catheter inserti

Temporal trends in the epidemiology of primary biliary cholangitis in the public healthcare system of Brazil: a large population-based study.

BACKGROUND: Population-based data on the epidemiology of primary biliary cholangitis (PBC) in Latin America are lacking. We aimed to characterise incidence, prevalence, and geographic patterns of PBC within Brazil's public healthcare system. METHODS: We performed a nationwide population-based retrospective study using de-identified administrative data from the Unified Health System (DATASUS), curated through an artificial intelligence platform. Adults (≥16 years) with ICD-10 code K74.3 and public dispensation of ursodeoxycholic acid between 1 January 2021 and 31 December 2024 were included. A

Preoperative Neutrophil-to-Lymphocyte Ratio is Associated with Early Postoperative Pain and Opioid Consumption after Total Knee Arthroplasty: A Retrospective Study.

BACKGROUND: Improving quality and effectiveness of health care is one of the priorities of health policies. Hospital or physician volume represents a measurable variable with a relevant impact on effectiveness of health care. A recent Italian law, the "spending review", calls for the definition of "qualitative, structural, technological and quantitative standards of hospital care". There is a need for an accurate evaluation of the available scientific evidence in order to identify these standards, including the volume of care above or below which the public and private hospitals may be accredi

Causal Associations Between Nine Autoimmune Diseases and Idiopathic Thrombocytopenic Purpura: A Mendelian Randomization Study.

INTRODUCTION: Clinical evidence suggests the presence of comorbidity between Autoimmune diseases (ADs) and secondary Idiopathic thrombocytopenic purpura (ITP), whereas the underlying causal relationship remains to be established. This study employed Mendelian Randomization (MR) analysis to investigate potential causal relationships between multiple common ADs and the risk of ITP. METHODS: Using criteria of P<5×10-8 , P<5×10-6 , and F-statistic>10, and excluding single nucleotide polymorphisms (SNPs) with confounding, palindromic sequences, and linkage disequilibrium as instrumental variables

Programmable Spatiotemporal Magnetic Fields for Selective Multimodal Actuation and Deterministic Decoupling of Heterogeneous Magnetic Robots.

Magnetic milli/microrobots hold strong potential for biomedical applications, but their deployment as multi-robot systems is limited by short-range magnetic coupling, which prevents independent control in confined environments. Here, we present an expanded field-free region (FFR)-based control framework for spatially selective actuation of multiple magnetic torque-driven robots in confined environments. A generalized selective-control criterion incorporating the minimum rotating magnetic field required for robot actuation enables the framework to accommodate robots with different magnetic resp
