Lamotrigine-associated severe cholestatic drug-induced liver injury requiring artificial liver support and percutaneous biliary drainage: a Case Report.

Lamotrigine is widely used as an antiseizure medication, but it may rarely cause severe hypersensitivity reactions with systemic involvement. We report a 29-year-old male with epilepsy confirmed by brain MRI and EEG, who developed fever and generalized maculopapular rash during lamotrigine titration (2024-11-01), followed by progressive jaundice, severe pruritus, marked direct hyperbilirubinemia, cholestatic liver enzyme elevation, and coagulation dysfunction. Viral hepatitis and autoimmune liver diseases were excluded. Complete RegiSCAR scoring, RUCAM scale and WHO-UMC causality assessment we

Increasing rate of liver transplantation in Iceland over the last decade: a nationwide study.

BACKGROUND AND AIMS: Liver transplantation in Iceland has historically been infrequent, with primary biliary cholangitis the leading indication during 1984-2012. We analyzed trends in the incidence, indications, etiology, and outcomes of liver transplantation in Iceland from 2013 to 2024 and compared transplant rates with other Nordic countries. METHODS: This nationwide retrospective cohort study included all Icelandic residents transplanted between January 2013 and December 2024, with adults and children analyzed separately. Indications, aetiology, and clinical outcomes were obtained from el

Statins are associated with decreased risk of disease progression and death in non-cirrhotic patients with primary biliary cholangitis.

BACKGROUND/AIMS: We evaluated whether statins in non-cirrhotic patients with primary biliary cholangitis (PBC) might influence disease progression, including cirrhosis development, hepatic decompensation, all-cause mortality, liver-related death (LRD) and non-liver-related death (NLRD), as such data are missing. MATERIALS AND METHODS: We retrospectively analyzed prospectively collected data in 507 non-cirrhotic PBC patients (445 females, age: 53 (16) years, follow-up: 85 (130) months) including 232 that received statins due to increased cardiovascular risk and 275 that had no treatment with s

Seladelpar improves liver stiffness in PBC: ASSURE interim analysis.

BACKGROUND & AIMS: Liver stiffness measurement (LSM) by transient elastography can predict clinical outcomes in patients with primary biliary cholangitis (PBC), with ongoing disease activity associated with progressive LSM increases. Seladelpar is a first-in-class approved delpar with demonstrable biochemical and symptom improvements in PBC. We assessed LSM changes, with up to 3 years of seladelpar treatment, in an interim analysis of the ongoing, open-label ASSURE study. METHODS: Patients with PBC rolled over into ASSURE from the pivotal, placebo-controlled RESPONSE study or enrolled after p

Temporal assessment of spatial and single-cell transcriptomes identifies CD226-NECTIN2 T cell-cholangiocyte interaction in bile duct inflammation.

BACKGROUND AND AIMS: Primary sclerosing cholangitis (PSC) and primary biliary cholangitis (PBC) are inflammatory bile duct disorders affecting mostly young adults that often lead to liver cirrhosis and cancer. Especially for PSC, pathogenic mechanisms remain unclear and treatment options are lacking. To better understand biliary disease processes in bile duct inflammation, we analyzed livers of NOD.c3c4 mice by spatial and single-cell (scRNA-seq) transcriptomics, identifying key cell types, interactions and potential changes over time that were validated in human liver explants. METHODS: Live

Chronic Liver Disease as a Risk Factor For Post-ERCP Complications: A Nationwide Retrospective Analysis.

INTRODUCTION: Endoscopic retrograde cholangiopancreatography (ERCP) is indicated for multiple pancreatic and biliary pathologies and carries a heightened risk profile compared with other endoscopic procedures. Considerable research has been directed towards discerning risk factors associated with complications such as post-ERCP pancreatitis and post-ERCP bleeding. Despite this, data on chronic liver disease (CLD) as a risk factor for complications is limited. We aimed to assess the outcomes of patients with CLD who underwent therapeutic or diagnostic ERCP to determine whether these patients ha

Multimodal Imaging Demonstrates Antifibrotic Effects of Targeting α v β 6 /α v β 1 Integrins in Biliary Fibrosis.

OBJECTIVES: Development of molecular therapies for liver fibrosis is slowed by a lack of noninvasive methods addressing questions of target expression, target engagement, and treatment response. Integrin α v β 6 is a biomarker of liver fibrosis that is upregulated in livers of patients with primary sclerosing cholangitis. It activates latent TGF-β and plays a critical role in regulating extracellular matrix expression, especially collagen. In this study, our aim was to use combined α v β 6 integrin-targeted positron emission tomography (PET) and collagen-specific magnetic resonance imaging (MR

Hepatic methionine-cycle dysregulation is associated with immune-inflammatory and fibrogenic remodeling in biliary atresia.

BACKGROUND: Biliary atresia (BA) is a progressive fibroinflammatory cholangiopathy leading to liver failure in infancy. The contribution of methionine metabolism to BA remains incompletely defined. This study investigated hepatic methionine-cycle alterations and the potential therapeutic effects of S-adenosylmethionine (SAMe). METHODS: Public bulk transcriptomic and single-cell RNA sequencing datasets were analyzed to characterize methionine-cycle-related alterations in BA. Selected genes were validated in clinical liver tissues. Methionine-cycle metabolites, oxidative stress indicators, and

Direct analysis of hepatic stellate cells with flow cytometry in specimens derived from the human liver.

Hepatic stellate cells (HSCs) play a crucial role in liver fibrosis. However, the methodology to directly assess the biology of primary HSCs in human liver specimens is yet to be established. In this study, we aimed to establish a robust methodology to analyse primary HSCs in human liver specimens with flow cytometry (FCM). We first applied FCM to HSCs directly isolated from liver tissues with Nycodenz density gradients. Then, we analysed HSCs in frozen/thawed liver perfusate samples and liver tissues. We also compared the phenotype of HSCs in primary biliary cholangitis (PBC) and those in hea

Prospective validation of the EASL-EASD algorithm for risk stratification in patients with metabolic dysfunction-associated steatotic liver disease and type 2 diabetes.

BACKGROUND AND AIM: individuals with type 2 diabetes are at increased risk of severe metabolic dysfunction-associated steatotic liver disease (MASLD). The European Association for the Study of Diabetes (EASD) recently proposed a two-step algorithm for risk stratification and referral, developed in collaboration with the European Association for the Study of the Liver (EASL), but its real-world performance remains unassessed. METHODS: in prospective study, adult patients with type 2 diabetes attending our Diabetology clinic were consecutively assessed using fibrosis index (FIB-4). Those with F

Autophagy of Kupffer cells modulates CD8+ T cell activation in primary biliary cholangitis.

BACKGROUND: Kupffer cells and monocyte-derived macrophages (MoMs) are difficult to study in human primary biliary cholangitis (PBC) even though they reflect a dynamic hepatic immune population. OBJECTIVE: We aim to investigate the role of hepatic macrophage and its therapeutic potential in human PBC and murine autoimmune cholangitis. DESIGN: Phenotypic analysis of hepatic macrophages in patients with PBC and dnTGFβRII mice model was performed by single-cell RNA sequencing, flow cytometry and immunohistochemistry. Depletion of hepatic macrophages and inhibition of MoMs were performed in murin

Cholecystocolonic Fistula With Acute Cholangitis: A Case Report.

Cholecystocolonic fistula (CCF) is a rare type of cholecystoenteric fistula, which are sometimes diagnosed incidentally in patients undergoing cholecystectomy. They are usually a complication of chronic cholecystitis, often secondary to gallstones, and they are difficult to diagnose due to their rarity and vague clinical presentation. If untreated, they can lead to complications such as acute cholangitis, biliary peritonitis, and biliary cirrhosis. This highlights the importance of early recognition of CCF and timely treatment. While the generally accepted management is surgical, there are no

Safety profile of elafibranor versus placebo in primary biliary cholangitis: A systematic review, meta-analysis, and trial sequential analysis of randomized controlled trials.

BACKGROUND: While elafibranor's safety has been addressed within a 3 outcome network meta-analysis of second-line primary biliary cholangitis (PBC) agents and within a mixed design review using serious adverse events (AEs) as its sole safety endpoint, no review has yet applied trial sequential analysis (TSA) or Grading of Recommendations Assessment, Development and Evaluation (GRADE) certainty assessment to a dedicated, 10-outcome, placebo-controlled elafibranor evidence base. PBC is a chronic autoimmune liver disease affecting the small bile ducts, with approximately 30% to 40% of patients fa

S100B in brain-gut-liver crosstalk: from glial activation to multiorgan inflammation.

S100 calcium-binding protein B (S100B) is conventionally viewed as a marker of astrocytic injury, blood-brain barrier (BBB) disruption, and neurological disorders. Emerging evidence places S100B within the broader brain-gut-liver axis of inflammatory signaling. This review proposes that S100B is a glial-derived integrating effector that couples enteric glial activation, barrier dysfunction, hepatic fibrosis, and hepatic encephalopathy (HE) into a multi-organ inflammatory network. Its spatial expression map, release kinetics, alarmin (damage-associated molecular pattern, DAMP) activity, and imm

Accelerated Biological Aging in Cholestatic Liver Disease: Telomere Attrition and Association With Disease Severity.

BACKGROUND: Primary sclerosing cholangitis (PSC) and primary biliary cholangitis (PBC) are rare, chronic cholestatic liver diseases with heterogeneous progression and limited biomarkers of disease biology. Epigenetic age acceleration has been demonstrated in PSC and PBC, suggesting accelerated biological aging. DNA methylation-derived telomere length (DNAmTL) reflects replicative aging and cellular turnover, but its role in PSC and PBC remains unclear. We aimed to evaluate DNAmTL in PSC and PBC and its association with disease severity. METHODS: We analysed and compared DNAmTL and age-adjuste
