Survival outcomes after liver transplantation in acute-on-chronic liver failure: A prospective study.

BACKGROUND: The impact of liver transplantation (LT) on mortality among patients with advanced cirrhosis who have two organ failures (acute-on-chronic liver failure-2 [ACLF-2]) or ≥ three organ failures (ACLF-3) is unclear. METHODS: Adults with decompensated cirrhosis with or without ACLF (grades 1-3), undergoing LT at 63 liver transplant centers worldwide were prospectively enrolled between 2021 and 2023 and followed for one year. Patients with ACLF-2 or ACLF-3 who were declined for LT were also prospectively followed for one year. RESULTS: Among the 144 patients declined for LT, 73% of tho

Accelerated Biological Aging in Cholestatic Liver Disease: Telomere Attrition and Association With Disease Severity.

BACKGROUND: Primary sclerosing cholangitis (PSC) and primary biliary cholangitis (PBC) are rare, chronic cholestatic liver diseases with heterogeneous progression and limited biomarkers of disease biology. Epigenetic age acceleration has been demonstrated in PSC and PBC, suggesting accelerated biological aging. DNA methylation-derived telomere length (DNAmTL) reflects replicative aging and cellular turnover, but its role in PSC and PBC remains unclear. We aimed to evaluate DNAmTL in PSC and PBC and its association with disease severity. METHODS: We analysed and compared DNAmTL and age-adjuste

Serum Adiponectin and Fibroscan as Non‑invasive Post‑operative Prognostic Markers for Biliary Atresia.

BACKGROUND: Majority of patients with biliary atresia (BA), including those who undergo Kasai hepatico‑portoenterostomy, develop cirrhosis, hepatic fibrosis and eventually progress to end‑stage liver disease. Therefore, early assessment of hepatic status and fibrosis is essential. The study objective was to estimate serum adiponectin, liver function tests and to associate them with liver stiffness scores in post‑operative BA patients. MATERIALS AND METHODS: A total of 30children (20 post‑Kasai BA cases and 10 healthy controls) were enrolled in this prospective case-control study. Serum adipon

ISG15 deficiency in hepatic stellate cells promotes TGFβ2-induced liver fibrosis by counteracting CREB1 ISGylation.

BACKGROUND: Interferon (IFN)-stimulated gene 15 (ISG15) is a downstream molecule of the IFN pathways central to many cellular processes. ISG15 mainly exerts its function through a post-translational modification process known as ISGylation. OBJECTIVE: In this study, the role of ISG15 in the activation of hepatic stellate cells (HSCs) and liver fibrosis was examined. DESIGN: Liver fibrosis was established by carbon tetrachloride (CCl4), bile duct ligation (BDL) surgery and metabolic dysfunction-associated steatohepatitis (MASH) diet between HSC-specific deletion of ISG15 (ISG15cKO) and wild t

Primary biliary cirrhosis and inflammatory bowel disease: a two-sample bidirectional Mendelian randomization study.

Observational studies have frequently reported an association between primary biliary cirrhosis (PBC) and inflammatory bowel disease (IBD). In this study, we leveraged summary-level data from genome-wide association studies (GWAS) to conduct a two-sample bidirectional Mendelian randomization (MR) analysis, with the primary objective of investigating the genetic causal relationship between PBC and IBD, comprising ulcerative colitis (UC) and Crohn's disease (CD). Additionally, a validation analysis was performed by repeating the bidirectional MR framework, alternately defining PBC and IBD as the

Iodine-enhanced x-ray phase-contrast CT for three-dimensional virtual histopathology evaluation of human cirrhosis.

BACKGROUND: Cirrhosis is the leading cause of liver disease-related morbidity and mortality worldwide. The destruction and remodeling of fibrous tissue, regenerated nodules, and microvascular system are the key pathological changes in the course of cirrhosis. Therefore, comprehensive visualization and analysis of the three-dimensional (3D) morphology of these characteristic structures using an appropriate imaging technique can provide a new approach for deep understanding of the spatial pathology of cirrhosis. PURPOSE: To realize 3D virtual histopathology assessment of key microscopic structu

Acute-on-chronic liver failure: pathophysiological mechanisms and clinical management.

Acute-on-chronic liver failure (ACLF) is a complex syndrome characterized by acute hepatic decompensation superimposed on pre-existing chronic liver disease or cirrhosis that is associated with acute worsening of portal hypertension, increased risk of infection, organ dysfunction and high short-term mortality. This Review provides a comprehensive update on definitions, pathophysiological mechanisms, clinical presentation and management of ACLF. The severe hepatic injury in ACLF triggers systemic inflammation, which is driven by damage-associated molecular patterns, gut-derived microbial produc

Variation in microbiome and metabolites is associated with advantageous effects of cholestyramine on primary biliary cholangitis with pruritus.

Emerging evidence implicates bile acid-intestinal microbiota interactions in the pathogenesis of pruritus associated with primary biliary cholangitis (PBC). Cholestyramine, a bile acid sequestrant, is clinically recommended for pruritus alleviation. This study investigates its regulatory effects on gut microbiome composition and metabolite profiles. A prospective cohort of 54 pruritic PBC patients and 25 asymptomatic controls underwent longitudinal multi-omics profiling. Fecal 16S rRNA sequencing and untargeted metabolomics were performed pre-/post-4-week cholestyramine intervention (4 g twice

Gut microbiome in biliary atresia.

Biliary atresia (BA) is a progressive cholangiopathy of infancy and the leading cause of pediatric liver transplantation. Despite surgical intervention with the Kasai portoenterostomy, long-term outcomes remain poor, with many patients progressing to cirrhosis. Emerging evidence implicates the gut microbiota-a dynamic ecosystem crucial to immune development and liver homeostasis-in BA pathogenesis and clinical progression. This review synthesizes current literature on gut microbiota composition in BA before and after the Kasai procedure, highlighting consistent patterns of dysbiosis, including

Combined liver-kidney transplantation using a situs inversus totalis donor liver: Retroversus implantation: A case report.

Situs inversus totalis is a rare congenital anomaly where thoracic and abdominal organs are completely reversed in mirror-image orentation. While it does not preclude transplantation, it presents significant technical challenges, especially in liver transplantation, due to the altered orientation of vascular and biliary structures. We present a case involving a 50-year-old man with end-stage renal disease and advanced cholestatic cirrhosis. His model for end-stage liver disease score was 26, qualifying him for combined liver-kidney transplantation. A donor with SIT became available, and pre-do

ATF3 Mediates the Immunoregulatory Role of the Tim-3 Pathway in Primary Biliary Cholangitis.

OBJECTIVE: The immune checkpoint Tim-3 has been implicated in the pathogenesis of primary biliary cholangitis (PBC), but its precise role in modulating macrophage polarization and immune dysregulation remains unclear. This study aimed to define the expression patterns of Tim-3 in PBC patients and elucidate its functional impact on macrophage behavior and disease progression. METHODS: Bulk RNA sequencing was performed using peripheral blood mononuclear cells (PBMCs) derived from PBC patients. The expression of Tim-3 and Toll-like receptor (TLR) 2 was assessed using flow cytometry. A murine mod
