Chronic liver disorders represent a serious health problem, considering that 300 million people worldwide are hepatitis B virus carriers, and 8,000-10,000 patients per year, in the U.S. alone, die as a result of liver failure caused by hepatitis C infection. Nitric oxide synthase (NOS) regulates hepatic vasculature; however, the patterns of expression and activity of NOS proteins in healthy and diseased human livers are unknown. Sections of diseased (n = 42) and control livers (n = 14) were collected during orthotopic liver transplants and partial hepatectomy. The diseased sections included al

OBJECTIVES: To determine the overall direct cost of liver transplantation in Canadian adults and to identify the factors that are associated with high cost.
METHODS: The direct cost of liver transplantation from the perspective of third-party payers was determined in a retrospective analysis of data from hospital charts and databases. A consecutive series of 119 adults who underwent liver transplantation between 1991 and 1992 was followed from the date of listing for transplantation to the second anniversary of the transplant. Patient-specific services during the pre-transplantation, transpla

This study aims at establishing the pattern of liver cirrhosis. Histology slides and duplicate copies of reports were retrieved and re-examined while fresh sections were processed from original paraffin blocks when necessary. Cirrhosis was the second commonest cause of chronic liver disease after hepatocellular carcinoma. The commonest morphological type was macronodular cirrhosis. Micronodular cirrhosis is not as common in black Africans as among the Caucasians. This is not unexpected since alcoholic liver disease that is of aetiopathogenetic importance is also not as common as what is often

The molecule SOX13 was initially identified as an autoantigen (ICA12) in Type 1 diabetes. SOX13 is a member of the SOX family of transcriptional regulatory proteins that contain a high mobility group (HMG) motif with structural similarity to HMG proteins 1 and 2. Antibodies to HMG 1 and 2 occur in autoimmune diseases of the liver and in ulcerative colitis. We measured the occurrence and levels of anti-SOX13 by radioimmunoprecipitation in primary biliary cirrhosis (PBC) and other diseases, and compared frequencies with anti-HMG measured by ELISA. Anti-SOX13 was detected in 18% of patients with

It is very difficult to predict new developments in hepatology so we have decided to analyse the most important issues related to three clinical conditions in hepatology. The first is chronic hepatitis. Here, we discuss the relevance of occult forms of hepatitis B virus infection in the development of cryptogenic liver disease and hepatocellular carcinoma. In addition, the role of genotyping in hepatitis B is analysed, indicating that patients with genotype A have a better prognosis than those with genotype D. The treatment of hepatitis B virus infection is also reviewed, and it has been sugge

To assess the extent of microfilaments in cholestatic liver diseases we examined the cytoplasmic microfilaments in intrahepatic and extrahepatic cholestasis in man by electron microscopy. Study subjects were two patients with drug-induced intrahepatic cholestasis, three patients with intrahepatic cholestasis due to viral hepatitis, four patients with extrahepatic cholestasis due to stones of the common bile duct and two patients with primary biliary cirrhosis. Two biopsied specimens from patients without clinical or histological evidence of liver disease served as noncholestatic controls. The

Primary biliary cirrhosis (PBC) may associate an axonal neuropathy, a somatic and autonomic neuropathy and a very infrequently sensory neuropathy (with or without xanthomata). The aim of this paper is to describe the case of a 46 year old man diagnosed with PBC in stage I-II and a progressive sensory neuropathy (axonopathy) confined to his upper limbs with distal predominance. It had progressed slowly an began asymmetrically. A complete clinical study excluded other causes of neuropathy. We followed him clinically and electromyographically and he remains stable after two years evolution. The s

There is an association between celiac disease (CD) and primary biliary cirrhosis, but there is little information regarding the association between CD and autoimmune cholangitis (antimitochondrial antibody-negative primary biliary cirrhosis). We describe a case of a 60-yr-old woman with chronic serum liver biochemistry elevations, recent onset of pruritus, and unexplained iron deficiency anemia. Liver biopsy was suggestive of stage 1 primary biliary cirrhosis, but serum antimitochondrial antibody testing was negative. Subsequent evaluation revealed CD based on markedly elevated antiendomysial

Studies comparing quantitative testing of liver function (QTLF) in large numbers of patients with defined etiology of cirrhosis are lacking. In all 316 patients with proven cirrhosis underwent QTLF, including aminopyrine breath test (ABT), galactose elimination capacity (GEC), sorbitol (SCI), and indocyanine green clearance (ICG). Values were correlated with the Child-Pugh classification (CP) and the etiology of liver cirrhosis. Fifty-five percent of the patients had alcoholic cirrhosis (ALC), 31% cirrhosis due to viral hepatitis (VIC), and 14% primary biliary cirrhosis (PBC). In all three gro

BACKGROUND: Fetal microchimerism (FM) has been implicated in the pathogenesis of primary biliary cirrhosis (PBC). The aim of the study was to investigate the presence of FM in PBC patients.
PATIENTS AND METHOD: FM was studied in 28 women, 14 with PBC and 14 healthy controls.DNA was extracted from peripheral blood cells, and a PCR analysis to detect a specific Y-chromosome sequence was performed. Specificity was confirmed by Southern-blot analysis.
RESULTS: The Y-chromosome sequence was amplified from peripheral-blood cell DNA in only one PBC patient and in none of the controls.
CONCLUSIONS:

Hepatic hyaluronan is a glycosaminoglycan synthesised in the liver mainly by mesenchymal cells and degraded by liver sinusoidal endothelial cells. Serum hyaluronan was measured, using enzyme-linked binding protein assay (Chugai, Japan) in 30 patients with cirrhosis of different etiologies (alcohol n = 15, primary biliary cirrhosis n = 4, hepatitis B n = 5, hepatitis C n = 6). Hyaluronan levels were significantly higher in patients with liver cirrhosis (398 +/- 375.4 ng/ml) than in the controls (36.5 +/- 26.9 ng/ml). There was no significant difference in hialuronan levels between the different

Transjugular intrahepatic portosystemic shunt (TIPS) is a useful procedure for patients with variceal bleeding and refractory ascites. Migration of TIPS can potentially complicate the subsequent transplant procedure. The aim of this study was to compare survival, operating time, and blood transfusion requirements in patients with migrated and nonmigrated TIPS undergoing liver transplantation. Of 152 patients, 21 received TIPS; stent migration was noted in seven patients-six distally and one proximally. Mean age of the patients was 54 +/- 11 years (range, 27-65 years), and there were 12 men and

Primary biliary cirrhosis (PBC) is a rare cholestatic liver disease with an autoimmune etiology. The present study was done to estimate the frequency of occurrence of pulmonary disturbances and to analyse the results of bronchoalveolar lavage (BAL) findings in patients with PBC. Thirteen patients (only women) aged 50.4 with histologically proved PBC were investigated. Mean values of lung function tests in the study group were within normal range. In 38% of patients the impairment of DICO was observed. Only in one patient decrease of lung compliance (Cdyn) was observed. BAL findings showed the

Primary biliary cirrhosis (PBC) is a chronic cholestatic liver disease characterized by the immunological destruction of intralobular bile ducts and serum anti-mitochondrial antibodies (AMA). Based upon previous work of oral tolerance and autoimmunity, we hypothesized that feeding the mitochondrial autoantigens of PBC would alter the clinical course and the level of antimitochondrial antibodies. The bovine pyruvate dehydrogenase complex (PDC) was purified and 5 mg fed in gelatin capsules to 6 patients with early stage PBC for 6 months. Antimitochondrial antibodies and liver biochemistries were

Primary biliary cirrhosis (PBC) is an autoimmune disease characterized by intrahepatic bile duct destruction and the production of anti-mitochondrial antibodies (AMA). The absence of an animal model has been a striking impedance in defining the molecular basis of disease. Previous work has suggested that SJL/J mice immunize with the pyruvate dehydrogenase complex (PDC-E2), the major mitochondrial autoantigen of PBC, leads to the development of lymphoid cell infiltration in portal tracts and a model system coined autoimmune cholangitis. We hypothesized that this pathology would be augmented if

The target antigens of anti-nuclear autoantibodies in autoimmune hepatitis (AIH) are poorly characterised. Since antibodies to the p53 nuclear protein have been reported in various autoimmune diseases, we have assessed the prevalence of these antibodies in patients with AIH (n = 45), primary biliary cirrhosis (n = 60), hepatitis B (n = 22), hepatitis C (n = 55), and in a control group of subjects with various non-liver diseases (n = 56). A significant proportion of patients with AIH (31%) had elevated levels of autoantibodies to the p53 protein. In contrast, the prevalence of these antibodies
