3D reconstruction of human liver tissue at cellular resolution.

The liver contains an intricate microstructure that is critical for proper liver function. Architectural disruption of this spatial structure is pathologic. Unfortunately, two-dimensional (2D) histopathology-the gold standard for pathological understanding of many liver diseases-can misrepresent or leave gaps in our understanding of complex 3D structural features. Here, we used immunostaining, tissue clearing, microscopy, and computational software to create 3D multilobular reconstructions of both nonfibrotic and cirrhotic human liver tissue. We found that spatial architecture in human cirrhot

The Utility of Elastography in Discriminating Stages of Liver Fibrosis and Risk Stratification in Patients With Primary Sclerosing Cholangitis: A Systematic Review.

BACKGROUND: Primary sclerosing cholangitis (PSC) is a chronic autoimmune disease involving inflammation in the liver and biliary system, manifesting as biliary strictures and cholestasis, and leading to hepatic fibrosis. Due to variable patterns of fibrosis, precise criteria for risk stratification of these patients have yet to be determined. Elasticity-based methods are widely used to estimate the stage of liver fibrosis in PSC. PURPOSE: To review the role of different modalities of quantitative elastography in relation to the clinical and fibrosis biomarkers of PSC for the diagnosis and pro

Case Report: Cystic fibrosis transmembrane conductance regulator gene heterozygous variation presenting with abdominal pain and hepatopancreatic lesions in a child.

BACKGROUND: Cystic fibrosis (CF) is an autosomal recessive disorder caused by mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene, leading to multi-system involvement. CASE REPORT: A 4-year-old girl presented with a 2-day history of abdominal pain. Clinical manifestations included recurrent epigastric pain and vomiting. Physical examination revealed abdominal distension, mild periumbilical tenderness, and hepatomegaly (liver palpable 4 cm below the right costal margin). Laboratory tests showed elevated pancreatic enzymes: serum amylase 607 U/L, lipase 634 U/L, and

How do oral pathogenic bacteria potentially contribute to inflammatory processes in autoimmune liver disease? Results from a systematic review.

BACKGROUND: Emerging evidence suggests that the oral-gut-liver axis may play a role in the immunopathogenesis of autoimmune liver diseases (AILDs), particularly autoimmune hepatitis (AIH) and primary biliary cholangitis (PBC). Alterations in the oral microbiota may influence hepatic immune responses; however, this relationship remains poorly defined. OBJECTIVE: This systematic review aimed to evaluate current evidence on the association between oral microbiota dysbiosis and AILDs, with a focus on microbial alterations, inflammatory profiles, and potential diagnostic implications. METHODS: A

Hypoxia-conditioned umbilical cord mesenchymal stem cell secretome attenuates hepatic fibrosis in a bile duct ligation model of biliary atresia.

PURPOSE: Biliary atresia (BA) is a progressive fibro-inflammatory cholangiopathy and the leading cause of neonatal cholestasis and pediatric liver transplantation. Mesenchymal stem cell (MSC)-derived secretome has emerged as promising acellular alternative to cell therapy. This study evaluated the potential of hypoxia-conditioned umbilical cord MSC (HU-UCMSC) secretome in rat model of biliary fibrosis induced by bile duct ligation (BDL). METHODS: Twenty-four male Sprague-Dawley rats were randomized into six groups: Sham, BDL, BDL + vehicle, and BDL treated with HU-UCMSC secretome at doses of

Pyroptosis Plays a Key Role Through Macrophages in Primary Biliary Cholangitis of Mice.

BACKGROUND AND AIMS: Primary biliary cholangitis (PBC) is an autoimmune intrahepatic cholestatic disease with both environmental and genetic participation. In this study, we aim to investigate the involvement of pyroptosis in PBC mice based on prior bioinformatic analysis of PBC patients and explore the immune cell populations potentially involved. METHODS: KEGG pathway enrichment analysis was performed using the GSE119600 dataset from the Gene Expression Omnibus (GEO) database, which includes whole-blood samples from PBC patients (n = 90) and non-liver disease CTRs (n = 47). The bioinformati

Impact of Alkaline Phosphatase Normalization on Complication-Free Survival in Primary Biliary Cholangitis.

INTRODUCTION: Despite the widespread use of ursodeoxycholic acid (UDCA) in primary biliary cholangitis (PBC), many patients remain at risk of progression. Emerging data suggest that normalization of alkaline phosphatase (ALP) and total bilirubin (TB)-particularly when TB < 0.6 times the upper limit of normal (× ULN)-is associated with improved outcomes compared with ALP reduction to < 1.5 × ULN. This study investigated the association between ALP normalization in response to UDCA and long-term clinical outcomes in a real-world cohort of patients with PBC. METHODS: We compared complication-fre

Australian best practice recommendations for transjugular intrahepatic portosystemic shunt (TIPS) in portal hypertension: a consensus statement.

BACKGROUND: Transjugular intrahepatic portosystemic shunt (TIPS) is one of the preferred interventional radiology techniques for reducing clinically significant portal pressures in patients with cirrhosis and complications from portal hypertension when pharmacological therapy or endoscopic interventions have failed or been insufficient. Recent advances in TIPS procedural techniques and stent technology, along with emerging indications for TIPS, warrant a review of current practices and establishment of consensus recommendations in Australia, where TIPS remains underused. This TIPS consensus st

Indian National Association for Study of the Liver Guidance Document on Difficult to Treat Autoimmune Hepatitis.

Autoimmune hepatitis (AIH) is an immune-mediated inflammatory disorder. It is a highly heterogeneous entity, having a wide range of presentations from asymptomatic chronic hepatitis to cirrhosis and acute liver failure. In consonance with the variabilities in presentation, there are also variations in response to treatment, depending on disease phenotype, presentation, extent of fibrosis, and the presence of comorbidities. In addition, pediatric AIH and AIH postliver transplant have their individual nuances. Addressing such areas to identify strategies for best practices is an unmet goal in th

ULTRASONOGRAPHY, SHEAR WAVE ELASTOGRAPHY AND LIVER HISTOLOGY FOR THE DIFFERENTIAL DIAGNOSIS BETWEEN BILIARY ATRESIA AND OTHER CAUSES OF CHOLESTASIS.

BACKGROUND: Biliary atresia (BA) is the leading cause of cholestatic jaundice in the first months of life. Liver stiffness measurement by shear wave elastography (2D-SWE) could help discriminate BA from other causes of cholestasis. OBJECTIVES: To assess the use of abdominal ultrasound with bidimensional shear wave elastography and liver histology to diagnose Biliary Atresia in cholestatic infants. To compare the use of elastography to estimate the stage of liver fibrosis with the histologic classification. METHODS: Cholestatic infants younger than three months were divided into BA and non-BA

Unveiling Hepatic Protein Alterations in Neonatal and Infant Biliary Atresia.

Pediatric populations differ from adults in drug elimination capacity. While current scaling methods account for enzyme and transporter maturation, they overlook comorbidities, such as biliary atresia (BA), a liver disease appearing within the first 2-8 weeks of life that can progress to cirrhosis. Such conditions may impair hepatic drug clearance, requiring dose adjustments. Physiologically based pharmacokinetic (PBPK) tools aim to address such cases and have been advocated to fill gaps in clinical data instead of less formalized and evidence-based guesswork. However, the paucity of systems d

Periductal iron-corrected T1 is a predictor of adverse outcomes in large-duct primary sclerosing cholangitis.

BACKGROUND: Concentric periductal fibrosis, commonly referred to as “onion skin” fibrosis, is a histological hallmark of primary sclerosing cholangitis (PSC) and associated with the development of biliary-type cirrhosis and end-stage liver disease. Our aim was to investigate whether non-contrast enhanced quantitative MRI can provide a noninvasive biomarker of periductal fibro-inflammation in large-duct PSC. METHODS: This prospective cohort study included adults with diagnosis of large-duct PSC, at baseline and 1-year follow-up, and healthy volunteers (HV) at baseline. MRCP + and LiverMultiScan

Decoding liver injury in cystic fibrosis: How to tell drug-induced liver injury from cystic fibrosis liver disease.

BACKGROUND: Cystic fibrosis liver disease (CFLD) is a significant comorbidity in individuals with cystic fibrosis (CF), marked by biliary fibrosis and progressive cholestasis. The advent of CF transmembrane conductance regulator (CFTR) modulators has revolutionized care for lung disease, but their impact on liver-specific disease and outcomes remain unclear. Additionally, the risk of comorbid cholestatic liver injury from medications and progression of CFLD complicates the diagnostic landscape. AIM: To provide a clinical framework for differentiating CFLD and drug induced liver injury (DILI),

Adipokine profiles reflect metabolic dysfunction but not fibrosis in patients with primary biliary cholangitis.

BACKGROUND: Primary biliary cholangitis (PBC) is a rare, nonsuppurative cholestatic disease that affects the small intrahepatic bile ducts. If not adequately managed, it may progress to liver cirrhosis and hepatocellular carcinoma. Only a few studies have explored the impact of cardiometabolic risk factors on liver fibrosis progression in PBC. Relevant data on the role of adipokines in these processes are also limited. AIM: To compare leptin and adiponectin levels in PBC patients stratified by the presence of metabolic dysfunction-associated steatotic liver disease (MASLD), metabolic syndrome

Dual cut-offs and beyond: Expanding the role of transient elastography in primary biliary cholangitis.

Accurate fibrosis staging is pivotal in the management of primary biliary cholangitis (PBC). Although liver biopsy is considered the diagnostic gold standard, its invasiveness limits widespread use in disease monitoring. Chen et al provided strong evidence validating vibration-controlled transient elastography (VCTE) as a noninvasive alternative for identifying advanced fibrosis in Chinese patients with PBC. By establishing dual cut-offs (≤ 10.0 kPa and > 14.5 kPa) with high diagnostic performance, the authors offer a clinically applicable tool that could substantially reduce the need for biop

Optimising Primary thErapy in pRimAry biliary cholangitis (OPERA): protocol for a randomised, double-blind, placebo-controlled trial of enhanced primary therapy with obeticholic acid.

INTRODUCTION: Primary biliary cholangitis (PBC) is a rare chronic cholestatic disease that despite current therapy has significant ongoing unmet needs, including risks of cirrhosis and life-impairing symptoms. The current treatment approach is a step-up model, wherein first-line therapy, ursodeoxycholic acid (UDCA), is given for a minimum of 12 months before the addition of second-line therapy is considered for non-responding patients. This 'waiting to fail' approach, focused on the needs of low-risk patients, allows, we postulate, a key process of biliary epithelial cell (BEC) senescence to b

Constructing a protein-protein interaction network for autoimmune liver diseases by integrating pQTL, rQTL, and mediation analyses.

Autoimmune liver diseases (AILDs), including primary biliary cholangitis (PBC), primary sclerosing cholangitis (PSC), and autoimmune hepatitis (AIH), pose significant diagnostic and therapeutic challenges due to poorly understood mechanisms. While most studies focus on absolute protein levels, protein-protein ratios (PPRs), reflecting the relative abundance of paired plasma proteins, emerge as critical yet understudied biomarkers for decoding disease-specific network perturbations. To harness this potential, we integrated protein quantitative trait loci (pQTLs), ratio QTLs (rQTLs), and mediati
