Clinical Performance of the Elecsys HCV Duo Immunoassay Compared with a Standard-of-Care Algorithm Using Alinity Anti-HCV Screening and HCV RNA Confirmation.

Background/Objectives: The Elecsys HCV Duo immunoassay simultaneously detects HCV core antigen (HCV Ag) and anti-HCV antibodies in a single sample. We evaluated its clinical performance against a standard-of-care reference algorithm of anti-HCV antibody and HCV RNA testing. Methods: We retrospectively analyzed 1332 residual serum or plasma samples tested with reference HCV RNA assays and the Alinity i Anti-HCV assay. Samples were classified as reference-positive, indicating current or past HCV infection (n = 166; 88 RNA+/Ab+ with active infection and 78 RNA-/Ab+ with resolved or treated infect

SARS-CoV-2 spike protein enhances HBV replication via autophagy and aggravates inflammation in CHB patients with liver dysfunction.

This study investigated the effects of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spike protein (S protein) on hepatitis B virus (HBV) replication and immune function in patients with chronic hepatitis B (CHB), aiming to elucidate the underlying mechanisms of S protein-HBV interaction. Through in vitro cell experiments and clinical sample analyses, S protein expression was induced in HepG2.2.15 cells, leading to enhanced HBV replication; this effect was reduced by autophagy inhibitors, confirming the critical mediating role of autophagy. Furthermore, peripheral blood mono

Single-cell landscape of alcohol-related hepatitis reveals serum amyloid a hepatocytes as key drivers of disease pathogenesis.

BACKGROUND AIMS: Alcohol-related hepatitis (AH) is a severe form of alcohol-related liver disease (ALD). The cellular subpopulations that contribute to AH and dynamics remain poorly understood. This study aimed to identify key cellular subpopulations and their interactions in AH and explore key mechanisms contributing to disease pathogenesis. APPROACH RESULTS: Single-cell RNA sequencing was performed starting from liver biopsies from patients with AH, early-stage ALD, and healthy individuals. Immunofluorescence was used to validate protein expression of selected markers. Liver biopsy-derived

Serum ammonia improves early risk stratification beyond West Haven criteria in acute liver failure.

BACKGROUND & AIMS: The prognosis of patients with acute liver failure (ALF) presenting with low grade hepatic encephalopathy (HE) defined by the West Haven criteria (WHC) is difficult to establish. We studied the association between HE severity, assessed by WHC and serum ammonia, with outcomes across different age groups. METHODS: Retrospective analysis of the prospective multicentre United States ALF Study Group registry (2000-2016). Exposures were WHC and serum ammonia on day 1 post inclusion. Endpoints were transplant-free survival (TFS) and mortality due to cerebral events on day 21. Find

Preventing cancer Down Under: Australian screening guidelines for prevention of anal cancer in people living with HIV: the pathologists' viewpoint.

Anal squamous cell carcinoma (ASCC) is a human papillomavirus (HPV)-associated cancer with a rising incidence. People living with human immunodeficiency virus (PLHIV) are at the highest risk. Recent evidence has shown that treatment of the precursor lesion, high-grade squamous intraepithelial lesion (HSIL), can prevent cancer development. Consequently, several countries have published screening guidelines for anal HSIL as a strategy to decrease cancer incidence. In Australia, the Australasian Society for HIV, Viral Hepatitis and Sexual Health Medicine (ASHM) has recently released guidelines fo

Enabling Ciprofloxacin Resistance-Guided Therapy for Neisseria gonorrhoeae: Aligning Diagnostics, Regulation, Guidelines, and Reimbursement.

Gonorrhea treatment in the United States relies on empirically administered intramuscular ceftriaxone. Diagnostics to predict Neisseria gonorrhoeae ciprofloxacin susceptibility/resistance could enable use of ciprofloxacin, a low-cost, single-dose oral antibiotic, to treat ciprofloxacin-susceptible gonorrhea. Ciprofloxacin resistance in N. gonorrhoeae is overwhelmingly mediated by a single mutation in gyrA at codon 91. Molecular interrogation of this site reliably predicts ciprofloxacin resistance and susceptibility. The Antibacterial Resistance Leadership Group (ARLG) is performing a multicent

Beware the Muscle: Silent Exercise-Induced Aminotransferase Elevation as a Diagnostic Pitfall in Hepatology and Pretreatment Screening-A Report of Two Cases.

BACKGROUND: In clinical hepatology practice, pretreatment liver enzyme screening plays a key role in making management decisions, such as initiation of antituberculosis therapy or surveillance of chronic hepatitis B. Unrecognized extrahepatic sources of aminotransferase elevations can lead to misdiagnosis, inappropriate treatment alteration, and unnecessary invasive or costly investigations. CASE PRESENTATION: We present two cases of active young males who were undergoing liver biochemical testing for therapeutic decision-making for other underlying illnesses. Patient one was undergoing basel

Health, workforce, and cost trade-offs of HIV programme prioritization strategies in Malawi.

BACKGROUND: Malawi has achieved substantial progress towards HIV epidemic control, reaching the UNAIDS 95-95-95 targets. However, declining external financing and health-workforce constraints threaten sustainability. Programme reconfiguration is being considered to reduce resource use. METHODS: We used the Thanzi La Onse (TLO) individual-based health system simulation model to evaluate HIV programme changes in Malawi from 2025 to 2050. HIV testing, treatment monitoring, prevention interventions, and tuberculosis preventive therapy (TPT) were assessed individually and in combination, and compa

Autoimmune hepatitis following long-term denosumab therapy in a patient with Hashimoto thyroiditis.

Denosumab, a receptor activator of nuclear factor-κB ligand (RANKL) inhibitor, is widely used to treat osteoporosis. Its role as a trigger for autoimmune hepatitis (AIH) remains uncertain. We report a 68-year-old woman with osteoporosis and Hashimoto thyroiditis who developed jaundice and severe hepatocellular liver injury after 3 years of denosumab therapy. Alternative viral, obstructive, and metabolic causes were excluded. Liver biopsy demonstrated interface hepatitis with a lymphoplasmacytic portal infiltrate consistent with AIH, and autoimmune serologies were positive. Hemophagocytic lymph

Benefits and risks of induction immunosuppression in pediatric autoimmune liver disease from SPLIT Registry.

The optimal induction regimen for pediatric liver transplantation (LT) in patients with Autoimmune Liver Disease (AILD) is not well-defined. This study compared outcomes between corticosteroids only, anti-thymocyte globulin (ATG), and IL-2 receptor antagonists (IL2RA; Basiliximab/Daclizumab). A retrospective review of 445 pediatric LT recipients with AILD (Autoimmune hepatitis, primary sclerosing cholangitis, or autoimmune sclerosing cholangitis) was conducted using the SPLIT database. Re-transplants were excluded. Primary outcomes included biopsy-confirmed rejection, culture-proven infection,

Hepatitis B screening among asylum seekers: a retrospective cohort study in Modena, Northern Italy.

Hepatitis B virus remains a major health challenge among migrants from high-endemic regions arriving in low-endemic countries like Italy. However, more evidence on both infection burden and immunity gaps among asylum seekers is needed to inform prevention policies. We conducted a retrospective cohort study of 739 asylum seekers screened for Hepatitis B virus (HBV) in Modena, Northern Italy, between 2019 and 2023. Serological testing included quantitative determination of HBsAg, HBcAb, and HBsAb, allowing identification of active infection, natural immunity, vaccine-induced immunity, and suscep

Malignant Ascites Harbors Immune Niche with IL-10 Enriched Effector Dysfunction and Persistent Cancer-Binding IgG.

Malignant ascites provides a window into cancer-immune interactions in the peritoneal cavity. Using Cytometry by Time of Flight (CyTOF), we analyzed ascitic immune cells collected from patients with advanced gastrointestinal cancers, nonmalignant forms of hepatitis (NMH), and healthy control peripheral blood mononuclear cells (HC PBMCs). Compositionally, ascitic immune cells shared a myeloid-rich, B and naïve T cell-low signature relative to HC PBMCs. Underneath the shared ascitic compositional signature, three major functional immunophenotypes emerged in response to exogenous stimulation alon

Biologic and small-molecule therapies for inflammatory bowel disease: mechanisms, comparative evidence and the emerging pipeline.

Inflammatory bowel disease (IBD), comprising ulcerative colitis (UC) and Crohn's disease (CD), is a chronic immune-mediated disorder of rising global prevalence, with newly industrialised nations now entering the phase of most rapid growth. Its therapeutic landscape has transformed since 2020. Three selective interleukin (IL)-23p19 antagonists have been approved for both UC and CD; two sphingosine-1-phosphate receptor modulators and a second-generation Janus kinase (JAK) inhibitor have entered routine practice; subcutaneous formulations have altered delivery; and the first formal head-to-head

A study using the REIN registry suggests a statin therapy benefits for secondary prevention in patients receiving dialysis.

INTRODUCTION: While statins are effective for cardiovascular prevention in the general population, their benefit in patients with CKD G5D (kidney failure treated by dialysis) remains uncertain. Previous randomized controlled trials included mixed populations for primary and secondary prevention while excluding patients with recent cardiovascular events, leaving a knowledge gap regarding statin effectiveness for secondary prevention immediately following an atherosclerotic cardiovascular disease (ASCVD) event. Here, we address this gap by providing real-world evidence. METHODS: We conducted a
