Evaluation of antiseptic drug candidate pyridosept in preclinical in vivo toxicity studies.

The purpose of this study is to assess the safety profile of the antiseptic drug candidate pyridosept in preclinical in vivo toxicity tests. The safety of pyridosept was assessed in acute toxicity tests in rats, mice and rabbits via dermal, intragastric, intraperitoneal, and inhalation routes of administration. Subchronic dermal toxicity of pyridosept was evaluated following cutaneous application for 90 days in rats and rabbits. Pyridosept was also examined in the accumulation test in rats and in micronucleus assay in the skin and bone marrow of mice after single and repeated administration. A

Macrophage-CD8⁺ T Cell Spatial Coupling Defines an Innate-Adaptive Injury Niche in Human Checkpoint Inhibitor Hepatotoxicity.

BACKGROUND & AIMS: Immune-mediated liver injury from immune checkpoint inhibitors (ILICI) is a major immune-related adverse event that limits cancer immunotherapy, yet its tissue-level immunobiology is poorly defined and its management is largely extrapolated from autoimmune hepatitis (AIH). We previously identified a tri-cellular CD8+ T cell-macrophage-hepatocyte injury niche in a murine model of ILICI; here, we tested whether this niche is recapitulated in human disease. METHODS: We applied imaging mass cytometry with a 32-marker panel to liver biopsies from patients with ILICI (n = 12), AI

Idiopathic hepatic granuloma: a case report.

BACKGROUND: Granulomatosis of the liver may present a diagnostic enigma. Granulomas are organized aggregates of macrophages that form as a chronic inflammatory response to various stimuli. CASE PRESENTATION: We report a case of a 69-year-old white male patient presenting with fever and lower extremity pain but no other symptoms of infection. Despite extensive diagnostic evaluations, no infectious source was identified. Initial empirical antibiotic therapy failed, prompting treatment with methylprednisolone and methotrexate, which improved clinical and laboratory parameters. Subsequent imaging

Early Diagnosis of Nieman-Pick Disease Type C and Rapid Response of Gelastic Cataplexy to Treatment With N-Acetyl-L-Leucine: A Case Report.

BACKGROUND Niemann-Pick disease type C (NPC) is a rare, progressive neurodegenerative lysosomal storage disorder often diagnosed in childhood after the onset of neurological symptoms. Gelastic cataplexy - the sudden loss of muscle tone associated with laughter but remaining consciousness - is quite specific for NPC. This report describes the case of a 4-year-old boy with NPC presenting with gelastic cataplexy who responded to treatment with N-acetyl-L-leucine (NALL). CASE REPORT We present the case of a patient who first presented in the genetics clinic due to hepatosplenomegaly, dysmorphic fe

An Unusual Case of Drug-Induced Hypersensitivity Syndrome With Multiple Atypical Histopathologic Findings.

Drug-induced hypersensitivity syndrome (DIHS) is a rare, severe cutaneous adverse reaction that presents with rash, fever, and multiorgan involvement after medication or supplement exposure. Diagnosis is primarily clinical, although a skin biopsy may provide supportive evidence. Histologic features vary but generally reflect nonspecific epidermal and dermal inflammatory patterns. Rarely, DIHS presents with atypical features such as pustules, imitating acute generalized exanthematous pustulosis. We describe a case of a 68-year-old man who was started on carbamazepine for postoperative seizures

Entecavir-Induced Lichenoid Drug Eruption: A Case Report.

Entecavir is a highly potent antiviral medication used in the treatment of chronic hepatitis B infection and has demonstrated an excellent safety profile in clinical practice. Cutaneous adverse reactions to entecavir are exceedingly rare, with only seven cases reported in the literature to our knowledge. Here, we report the case of an adult female receiving entecavir therapy for chronic hepatitis B who developed a lichenoid drug eruption secondary to the medication. The rash resolved promptly following drug discontinuation and supportive treatment. Various cutaneous morphological patterns asso

Not your usual drug-induced liver injury: Dolutegravir-associated granulomatous hepatitis.

Dolutegravir is an integrase inhibitor in first-line antiretroviral therapy in South Africa. We describe a patient who developed severe drug-induced liver injury on dolutegravir-containing antiretroviral treatment, in whom liver biopsy demonstrated subacute hepatic necrosis, non-necrotising granulomatous inflammation, and bile duct injury: an uncommon histological pattern in dolutegravir-associated hepatotoxicity.

Case Report: Multimodality imaging features and serial metabolic changes on 18 F-FDG PET/CT in a dog with granulomatous hepatitis.

Granulomatous hepatitis is an uncommon inflammatory liver disease that can mimic hepatic malignancy on diagnostic imaging. A 12-year-old spayed female Cocker Spaniel presented with multiple target-like hepatic and splenic lesions showing peripheral enhancement on triple-phase CT and markedly increased 18F-FDG uptake on PET/CT (SUVmax 11.2), raising strong suspicion for malignancy. However, histopathology confirmed granulomatous hepatitis. Serial PET-CT performed 4 months later demonstrated decreased metabolic activity, supporting a chronic inflammatory process. This case highlights the potenti

Systemic salmonellosis in 3 pine siskins from Louisiana during the 2020-2021 songbird mortality event.

During the 2020-2021 irruption year, pine siskins (Spinus pinus) experienced widespread mortality across North America, attributed to Salmonella enterica subsp. enterica serovar Typhimurium. In March 2021, 3 pine siskins were admitted to the Louisiana State University Wildlife Hospital (Baton Rouge, LA, USA) because of lethargy, respiratory distress, diarrhea, and inability to fly. All birds died on or shortly after presentation. Postmortem examination revealed necrotizing and granulomatous esophagitis (3), hepatitis (3), splenitis (2), enteritis (1), cloacitis (1), and stomatitis/rhinitis (1)

Hepatic granulomas as a manifestation of ANCA-associated vasculitis:a systematic review.

BACKGROUND: ANCA-associated vasculitides (AAV) - granulomatosis with polyangiitis (GPA), eosinophilic granulomatosis with polyangiitis (EGPA), and microscopic polyangiitis (MPA) - are rare small-vessel autoimmune diseases. Liver involvement in AAV is uncommon and generally manifests as biochemical hepatitis; true hepatic granulomatosis is exceedingly rare and diagnostically challenging. METHODS: We conducted a systematic scoping review following PRISMA-ScR guidelines and the Arksey & O'Malley framework, searching PubMed, Google Scholar, ScienceDirect, and Scopus without date restriction (thro

Case Report: Epidemiologically inferred reverse zoonosis and intra-household zoonotic re-exposure of Mycobacterium tuberculosis in a domestic dog: a one health alert and the diagnostic potential of hypercalcaemia.

BACKGROUND: Canine tuberculosis caused by Mycobacterium tuberculosis is rare and often underdiagnosed. This case report aims to describe the clinical, pathological, and molecular features of a dog with systemic tuberculosis, highlighting its epidemiological relevance within a One Health framework. The specific objective is to characterise a confirmed case of systemic M. tuberculosis infection in a dog, emphasising its unique clinicopathological features, including hypercalcaemia, and documenting an inferred reverse zoonotic transmission with subsequent intra-household zoonotic re-exposure. Pulmonary Tuberculosis and Sarcoidosis Presented With Pleuropericardial Effusion: A Challenging Case Report.

Differentiating tuberculosis (TB) from sarcoidosis, especially in TB endemic regions, is one of the major clinical challenges because these two diseases may show considerable similarity in terms of clinical manifestations, imaging findings, and granulomatous pattern. In addition, the presence of noncaseating granuloma alone is not sufficient for the definitive diagnosis of sarcoidosis because in some cases, TB may also show granulomas without necrosis. In this report, a 83-year-old man is presented with fever, productive cough, chest pain, weakness, and mild dyspnea, who had a history of pleur

Hepatic histoplasmosis presenting as portal hypertension in an immunocompetent elderly male: A multidisciplinary case report.

Histoplasmosis, caused by Histoplasma capsulatum, is a dimorphic fungus typically endemic to parts of the Americas, Africa, and Asia. Inhalational exposure leads primarily to pulmonary infection, and extrapulmonary or isolated hepatic presentations are rare, particularly in immuno-competent individuals. We present an unusual case of hepatic histoplasmosis with no evident lung involvement, highlighting a diagnostic challenge and a rewarding pathologic discovery.

Isolated Hepatic Cryptococcosis in an Immunocompetent Child: A Rare Cause of Granulomatous Hepatitis with Cholestasis.

Cryptococcosis is an opportunistic fungal infection acquired through inhalation of Cryptococcus neoformans and is typically encountered in immunocompromised hosts. In children, reported cases most commonly involve the central nervous system or lungs, with hepatic disease usually occurring as part of disseminated infection. Isolated hepatic cryptococcosis in an immunocompetent child is exceptionally rare and provides insight into developmental immune responses in early childhood. We report a young immunocompetent child presenting with fever, hepatomegaly, and cholestatic liver dysfunction. Live
