Autoimmune hepatitis and primary biliary cholangitis are among a subset of autoimmune diseases defined by profound co-prevalence and genetic risk association with the IL12 pathway.

BACKGROUND AND AIMS: Distinct autoimmune diseases can share symptoms and autoantibodies, but the molecular pathways underlying this shared etiology remain poorly understood. We assessed genetic support for IL12 signaling across 27 diseases. APPROACH AND RESULTS: We identified a rare variant upstream of IL12A with large risk-increasing effects on autoimmune hepatitis [AIH; per-allele odds ratio (OR)=2.98, p=3×10-9], primary biliary cholangitis (PBC; OR=2.80, p=2×10-8), systemic sclerosis (SSc; OR=2.64, p=2×10-11), Sjogren's disease (SjD; OR=1.94, p=10-11), systemic lupus erythematosus (SLE; OR

Cytomegalovirus Hepatitis Occurring in a Young Patient With Autoimmune Polyglandular Syndrome and Pediatric-Onset Autoimmune Hepatitis.

Cytomegalovirus (CMV) hepatitis is rare in patients with autoimmune hepatitis (AIH) on long-term immunosuppression. We report a 28-year-old woman with a history of autoimmune polyglandular syndrome and pediatric-onset AIH who presented with fatigue, fever, and elevated liver enzymes, found to have CMV hepatitis without active AIH. Early initiation of antiviral therapy led to normalization of liver function tests and resolution of viremia. This case highlights the importance of early diagnosis of CMV hepatitis and the therapeutic challenge of treating CMV while also reintroducing immunosuppress

Recurrence of autoimmune liver diseases after liver transplantation: exploratory analysis of a pooled Latin American cohort.

INTRODUCTION AND OBJECTIVES: Autoimmune liver diseases (AILDs) are an increasingly frequent indication for liver transplantation in Latin America. Recurrence after transplantation may affect graft function and long-term outcomes; however, recurrence patterns and associated factors remain incompletely characterized. We aimed to describe recurrence after liver transplantation in a pooled cohort of AILDs and to explore factors associated with recurrence, overall survival, and retransplantation. PATIENTS AND METHODS: We conducted a retrospective cohort study of adult patients with AILDs who under

Macrophage-derived IL-27 sustains autoreactive CD8 + cytotoxic T cells in autoimmune hepatitis.

Chronic or persistent T cell activation is widely thought to culminate in T cell exhaustion, yet how autoreactive CD8⁺ T cells remain functionally intact despite sustained self-antigen exposure is poorly understood. In chronic infection and cancer this durability is attributed to stem-like, TCF1⁺ PD-1⁺ progenitor-exhausted (Tpex) cells that reside in lymphoid tissue and continuously replenish functional effectors; whether such a reservoir operates within a chronically inflamed peripheral organ during sterile autoimmunity is unknown. Here we show that mice lacking the lysosomal nuclease DNase2a

Autoimmune Hepatitis: 2026 Update.

Autoimmune hepatitis (AIH) is an autoimmune liver disease characterized by immune-mediated hepatocellular destruction. This review provides a comprehensive update on the changes in the clinical landscape of and advancements in therapy for AIH up to 2026. Recent epidemiological data indicate a significant increase in both the incidence and prevalence of AIH worldwide, emphasizing potential shifts in environmental triggers and heightened disease awareness. Diagnosing AIH remains intricate owing to atypical manifestations; however, the newly updated 2025 European Association for the Study of the

Phenotype and prognosis of primary sclerosing cholangitis-autoimmune hepatitis variant syndrome.

BACKGROUND AND AIMS: A subset of patients with primary sclerosing cholangitis (PSC) exhibits features of autoimmune hepatitis (AIH), referred to as PSC-AIH variant syndrome (VS). Data on the phenotype and prognosis of VS remain limited. We compared clinical characteristics and outcomes of patients with VS and PSC without VS (non-VS). METHODS: This retrospective cohort study included patients with PSC followed at a tertiary referral center. VS was defined as moderate-to-severe interface hepatitis on liver histology combined with at least one of the following: alanine aminotransferase ≥5 × uppe

Cyclin-dependent kinase 4/6 (CDK4/6) inhibitor-induced hepatotoxicity and its resolution: a study of seven cases.

AIMS: Hepatotoxicity is an increasingly recognised and potentially dose-limiting adverse effect of CDK4/6 inhibitors, but their histopathological spectrum remains incompletely characterised. We aimed to describe the clinicopathological features of histologically characterised CDK4/6 inhibitor-induced liver injury. METHODS AND RESULTS: We retrospectively identified patients with metastatic breast cancer who developed liver enzyme elevation and underwent liver biopsy while receiving a CDK4/6 inhibitor at a single comprehensive cancer centre (2020-2024). Inclusion required a RUCAM score ≥6 and e

Detection of adverse event signals associated with key immunosuppressants for autoimmune hepatitis: insights from the food and drug administration adverse event reporting system database.

BACKGROUND: Autoimmune hepatitis (AIH) requires long-term immunosuppressive therapy, yet the comparative safety profiles of key drugs remain incompletely characterized. The objective of this study was to conduct a systematic evaluation of the adverse drug event (ADE) signals associated with glucocorticoids, specifically dexamethasone, methylprednisolone, and prednisolone, in addition to azathioprine and mycophenolate mofetil, in the treatment of AIH. METHODS: Disproportionality analysis was performed on ADE reports from the Food and Drug Administration (FDA) Adverse Event Reporting System (FA

Late TNFα blockade is associated with systemic immune rewiring without disease control in established experimental autoimmune hepatitis.

Recent human studies have identified TNFα as a therapeutically targetable pathway in autoimmune hepatitis (AIH), but the consequences of TNFα blockade may depend on treatment timing and disease context. Whether TNFα neutralization can restore disease control once autoimmune liver inflammation is already established remains unclear. Here, we investigated late TNFα blockade in experimental autoimmune hepatitis (emAIH). Anti-TNFα treatment was initiated after disease establishment, and mice were analyzed at early and late post-treatment time points by integrating serum biochemistry, hepatic infla

Seladelpar improves liver stiffness in PBC: ASSURE interim analysis.

BACKGROUND & AIMS: Liver stiffness measurement (LSM) by transient elastography can predict clinical outcomes in patients with primary biliary cholangitis (PBC), with ongoing disease activity associated with progressive LSM increases. Seladelpar is a first-in-class approved delpar with demonstrable biochemical and symptom improvements in PBC. We assessed LSM changes, with up to 3 years of seladelpar treatment, in an interim analysis of the ongoing, open-label ASSURE study. METHODS: Patients with PBC rolled over into ASSURE from the pivotal, placebo-controlled RESPONSE study or enrolled after p

Acute autoimmune hepatitis diagnosed concurrently with human immunodeficiency virus.

We report a woman in her 50s who presented with jaundice and severe hepatocellular injury, concurrently found to have newly diagnosed advanced HIV infection (CD4 count 17, viral load 195 600 copies/mL). In this context, infectious hepatitis or opportunistic infections were the important initial consideration, but viral serology, imaging and liver biopsy excluded opportunistic and viral causes. Markedly raised immunoglobulin G, positive anti-nuclear and smooth muscle antibodies, typical histology and rapid response to corticosteroids confirmed acute autoimmune hepatitis (AIH). She improved on p

Elective caesarean birth in a patient with autoimmune hepatitis with cirrhosis and extensive rectus sheath varices.

Cirrhosis in pregnancy poses significant anaesthetic challenges, particularly when complicated by extensive abdominal wall varices, an exceptionally uncommon and high-risk finding with minimal published guidance. We report a 30-year-old primigravida at 36 weeks' gestation with autoimmune cirrhosis and extensive rectus sheath varices undergoing elective caesarean birth. Given the anticipated potential for major haemorrhage, robust multidisciplinary planning was undertaken with hepatobiliary surgeons for performing modified abdominal entry in a specialist theatre environment. Intrahepatic Tissue-Resident CD8+ T-Cell Programmes Associate With Fibrosis and Local Immune Remodelling in Autoimmune Hepatitis.

BACKGROUND AND AIMS: Autoimmune hepatitis is an immune-mediated liver disease characterized by chronic inflammation and progressive fibrosis. Tissue-resident memory T cells mediate organ-specific immunity, but their role in autoimmune hepatitis remains poorly understood. We characterized tissue-residency-associated T-cell populations and their cytokine profiles, assessed associations with disease severity and determined whether these signatures were reflected in peripheral blood. METHODS: Paired blood and liver samples were obtained from 19 patients with autoimmune hepatitis. Comparator cohor

Hepatic involvement in children with diabetes: Pathophysiology, clinical manifestations, and current management.

Diabetes is a chronic disease characterized by hyperglycemia resulting from insulin deficiency or resistance to insulin action. Its onset may result from polygenic or monogenic mechanisms. The liver plays a central role in glucose metabolism and can be affected by various conditions. In poorly controlled type 1 diabetes, glycogenic liver disease presents with hepatomegaly and elevated liver enzymes, which is reversible with good metabolic control. Autoimmune hepatitis may coexist with type 1 diabetes due to shared immunological mechanisms or may develop during treatment with corticosteroids. I

Correlation and discriminatory performance of non-invasive serum fibrosis indices according to liver disease etiology using two-dimensional shear wave elastography as reference.

BACKGROUND/OBJECTIVES: Non-invasive serum fibrosis indices are widely used for liver fibrosis assessment; however, their correlation with liver stiffness and diagnostic performance may vary according to liver disease etiology. This study aimed to evaluate the etiology-dependent correlation between multiple serum Non-Invasive Test (NITs) indices and liver stiffness measured by two-dimensional Shear Wave Elastography (2D-SWE), and to explore their ability to discriminate patients according to 2D-SWE-based fibrosis categories. METHODS: We conducted a retrospective cross-sectional study including
