Clinical and pathological significance of serum anti-PD-1 autoantibodies in acute-onset and chronic autoimmune hepatitis.

BACKGROUND: Acute-onset autoimmune hepatitis (A-AIH) poses diagnostic challenges because autoantibodies are frequently absent or serum IgG levels may only be mildly elevated. Autoantibodies against the immune checkpoint molecule PD-1 have been detected in patients with chronic AIH (C-AIH). To clarify the clinical and pathological significance of anti-PD-1 autoantibodies, we examined their association with the clinical features of patients with liver diseases, including A-AIH, C-AIH, primary biliary cholangitis (PBC), and drug-induced liver injury (DILI). METHODS: We compared the clinical char

Demographic Profile and Clinical Presentation of Celiac Disease in the Pakistani Population: A Systematic Review and Meta-analysis.

BACKGROUND: Celiac disease (CD) is an increasingly recognized autoimmune enteropathy triggered by gluten ingestion. While its global prevalence is well-documented, data from South Asia, particularly Pakistan, remain fragmented. This systematic review and meta-analysis aimed to consolidate available evidence on the demographic characteristics and clinical presentations of CD in the Pakistani population. MATERIALS AND METHODS: A comprehensive literature search of PubMed, Google Scholar, and Cochrane databases was conducted in accordance with PRISMA guidelines. Studies were selected based on pre

A Practical Approach to Abnormal Liver Enzymes in Pediatric IBD in the Biologic and Targeted Therapy Era.

Inflammatory bowel disease (IBD) is increasingly diagnosed in children and adolescents, and abnormal liver enzymes are a common yet often underrecognized clinical challenge. In the era of biologic and targeted therapies, liver test abnormalities may reflect drug-induced liver injury (DILI), hepatobiliary manifestations of IBD such as primary sclerosing cholangitis (PSC), autoimmune liver disease, infection, metabolic dysfunction-associated steatotic liver disease (MASLD), or reactive inflammatory changes. Timely recognition of the underlying etiology is essential to avoid unnecessary treatment

A Novel BAFF/APRIL Dual Inhibitor in Treating Refractory Autoimmune Hepatitis.

A subset of autoimmune hepatitis (AIH) patients fail to respond to standard treatments or experience frequent relapses. This study evaluates the efficacy and safety of telitacicept, a BAFF/APRIL dual inhibitor, as a rescue therapy. Two AIH patients treated with telitacicept were included. The first, steroid-dependent and refractory to multiple immunosuppressants, showed significant reductions in transaminases and immunoglobulin G, enabling steroid tapering. The second, with comorbid systemic lupus erythematosus, maintained remission and reduced steroid and mycophenolate mofetil doses. No sever

Management of Severe Abemaciclib-induced Liver Dysfunction: Feasibility of Switching to Palbociclib.

BACKGROUND/AIM: Abemaciclib, a cyclin-dependent kinase 4/6 (CDK4/6) inhibitor, can cause severe liver injury, leading to treatment discontinuation. We report five cases of patients treated with a combined regimen of a CDK4/6 inhibitor and hormone therapy for metastatic breast cancer. Following the development of serious liver dysfunction (grade ≥3) during abemaciclib therapy, switching to palbociclib allowed continuation of CDK4/6 inhibitor treatment. CASE REPORT: The causative role of abemaciclib was assessed using the drug-induced liver injury scoring system (RECAM-J 2023), which evaluates

Genuine acute autoimmune hepatitis has better therapeutic response and prognosis.

BACKGROUND AND AIM: Heterogeneous diagnostic criteria for acute autoimmune hepatitis (AIH) across studies hinder the characterization of genuine acute AIH. We developed stringent and multidimensional criteria to distinguish acute, acute-on-chronic, and chronic autoimmune hepatitis (A-AIH, AC-AIH, and C-AIH), aiming to elucidate their clinical profiles. METHODS: We consecutively enrolled patients diagnosed with definite AIH between 2012 and 2024. All patients were stratified using stringent criteria encompassing onset time (A-AIH: < 6 months), acute presentation (total bilirubin (TB)  ≥ 5 mg/d

Discovery of Novel Nonsteroidal SGRMs of Sulfonamide-2-Oxo-Tetrahydroquinoline Derivatives by Carbonyl Migration.

Glucocorticoids (GCs) are limited by severe side effects, driving the development of selective glucocorticoid receptor modulators (SGRMs) with improved therapeutic profiles. We previously development the SGRM lead B53, which suffered from poor metabolic stability. In this study, structure-guided optimization of B53 yielded 43 novel sulfonamide derivatives. Among them, D8, which contained 2-oxo-tetrahydroquinoline by carbonyl migration form B53, manifests an excellent SGRM with remarkable transrepression potency (IC50NF-κB = 0.9 nM) superior to dexamethasone (IC50 NF-κB = 5.0 nM). Besides, D8 e

IgG4-Associated Autoimmune Hepatitis: a Systematic Review.

BACKGROUND: IgG4-associated autoimmune hepatitis (IgG4-AIH) is an emerging entity characterized by overlapping features of classical autoimmune hepatitis (AIH) and IgG4-related disease (IgG4-RD). However, significant heterogeneity in diagnostic criteria has obscured its true prevalence and clinical significance. This systematic review aimed to characterize the clinical, serological, and histopathological features of IgG4-AIH. METHODS: Following PRISMA guidelines, a systematic search was conducted across PubMed, Embase, and Web of Science. Study selection, data extraction, and quality assessme

Management of autoimmune hepatitis-related decompensated cirrhosis: current evidence and clinical decision-making.

PURPOSE OF REVIEW: Autoimmune hepatitis (AIH) presenting with decompensated cirrhosis poses a major therapeutic dilemma for clinicians. Although the prompt initiation of immunosuppression (IS) can reverse disease activity and lead to the clinical resolution of decompensation, avoiding the need for liver transplantation (LT), it may also be futile or even harmful. International guidelines and cohort studies have begun to address this challenge, making it timely to synthesize available evidence and provide practical guidance for clinicians. RECENT FINDINGS: Emerging data highlight that the bene

Retrospective, single-center analysis of autoimmune hepatitis in Yemeni adults: clinical features, treatment, and outcomes.

BACKGROUND: Autoimmune hepatitis (AIH) is a chronic liver disease in which the immune system of the body unintentionally targets liver cells, resulting in inflammation, liver damage, and cirrhosis if treatment is not received. Although the precise origin of AIH remains unknown, experts believe that environmental and genetic factors play significant roles in its pathogenesis. This study describes the clinical, biochemical, and long-term outcomes of patients with autoimmune hepatitis (AIH) at the Science and Technology Hospital in Sana'a, Yemen. METHODOLOGY: This was a retrospective, single-cen

Depression Aggravates Immune-Mediated Hepatitis Through NLRP3 Overactivation Induced by Intestinal Microbiota.

BACKGROUND: Depression is associated with adverse effects in patients with autoimmune hepatitis (AIH). However, the underlying mechanism remains unclear. This study explores the impact of depression and related intestinal microbiota on immune-mediated hepatitis. METHODS: We assessed depression in 260 AIH patients receiving 2-year standardized treatment and 173 healthy controls. In mice, depressive-like behaviors were induced by chronic unpredictable mild stress (CUMS), and immune-mediated hepatitis was induced by intravenous injection of concanavalin A (ConA). Fecal microbiota transplantation

Annexin A5 ameliorates immune-mediated liver injury by regulating ferritinophagy-ferroptosis in M2 macrophages via the NRF2/ERK pathway.

BACKGROUND & AIM: M2 macrophages are key drivers of immune-mediated liver injury. The aim of this study was to investigate the mechanisms by which Annexin A5 regulates ferroptosis in M2 macrophages and its protective effect against immune liver injury. METHODS: The colocalization of ferroptosis in M2 macrophages from autoimmune hepatitis (AIH) patients and mice with ConA-induced immune liver injury was analyzed via laser confocal microscopy. The effects of ferritinophagy and autophagosomal-lysosomal fusion on the regulation of ferroptosis in M2 macrophages by Annexin A5 were investigated thro

Liver Biopsy Features May Help Distinguish Drug-Induced Autoimmune-Like Hepatitis From Idiopathic Autoimmune Hepatitis.

CONTEXT.—: Drug-induced autoimmune-like hepatitis (DI-ALH) and idiopathic autoimmune hepatitis (iAIH) share similar clinical, biochemical, serologic, and histologic features. Differentiating between them is crucial for treatment and prognosis. OBJECTIVE.—: To identify clinical and histologic factors that distinguish DI-ALH from iAIH and develop a predictive scoring algorithm. DESIGN.—: We evaluated diagnostic laboratory data and histologic features from index liver biopsies of 14 well-characterized DI-ALH cases and compared them with those from 19 age- and sex-matched iAIH cases. Data includ

Sensitivity and specificity of magnetic resonance elastography in liver diseases in the pediatric age group.

OBJECTIVES: Liver fibrosis staging in pediatric patients traditionally relies on invasive methods, such as liver biopsy, which pose risks and limitations. Magnetic Resonance Elastography (MRE) has emerged as a promising noninvasive alternative. This study aimed to validate the use of MRE in pediatric liver diseases and assess its diagnostic accuracy. METHODS: A total of 110 participants (61 with autoimmune hepatitis (AIH), 33 post-liver transplantation (LTx), and 16 healthy controls) underwent MRE examinations. Liver biopsies were performed based on ESPGHAN indications in patients with AIH an

6-Mercaptopurine vs Mycophenolate Mofetil in Autoimmune Hepatitis and Azathioprine Intolerance: A Multicenter Study.

BACKGROUND & AIMS: Azathioprine (AZA) and corticosteroids are the recommended standard therapy for autoimmune hepatitis (AIH); however, a significant proportion of patients discontinue AZA due to intolerance. Although guidelines propose 6-mercaptopurine (6-MP) and mycophenolate mofetil (MMF) as alternatives, there is a lack of data on 6-MP and no comparative studies in AIH. We aimed to compare the efficacy and safety of 6-MP and MMF as second-line therapies in patients intolerant to AZA. METHODS: This multicenter retrospective cohort study involved patients with AIH from 11 tertiary care cent

Cleavage-Resistant CYLD Protects Against Autoimmune Hepatitis.

Autoimmune hepatitis (AIH) is an immune-mediated liver disease that can progress to fibrosis, cirrhosis, and hepatocellular carcinoma. However, the pathogenic mechanisms underlying AIH remain poorly understood, limiting the development of effective therapies. Here, using a concanavalin A-induced murine model of experimental autoimmune hepatitis (EAH), proteolytic cleavage of the deubiquitinase cylindromatosis (CYLD) at Asp215 is identified as a critical molecular event that promotes disease progression. Mice harboring a macrophage-specific, cleavage-resistant CyldD215A/D215A mutation are marke
