Ubiquitin-Conjugating Enzyme E2O Primes Hepatocytes to Restore Immune Tolerance in Autoimmune Hepatitis via Inhibiting Y-Box Binding Protein 1/Interleukin-6 Axis.

BACKGROUND & AIMS: Autoimmune hepatitis (AIH) is a chronic progressive inflammatory liver disease, with its incidence increasing continuously worldwide. The mechanisms underlying the regulation of immune tolerance in AIH remain largely unknown. This study investigated a novel regulatory pathway of regulatory T cell/T helper 17 (Treg/Th17) balance involving the ubiquitin-conjugating enzyme E2O (UBE2O) and underlying mechanisms. METHODS: UBE2O expression was analyzed in patients and mice with AIH. In vivo UBE2O overexpression in a chronic AIH model and naïve CD4+ T differentiation induction ass

Predicting VRE Infection After Liver Transplantation With a Time-Updated Colonization Score.

BACKGROUND: Vancomycin-resistant Enterococcus (VRE) infection is a life-threatening complication after liver transplantation (LT). This study aimed to identify risk factors and develop a predictive score for post-transplant VRE infection in LT recipients. METHODS: We conducted a single-center, retrospective cohort study including all adult patients who underwent LT between 2010 and 2022. Patients were followed from 90 days before to 180 days after LT. The primary endpoint was time to the first VRE infection after LT, with death before infection treated as a competing event. We applied a stack

An imbalance between CD80 and CD86 levels and CD4 regulatory T cell number and transendocytosis function exists in the liver in autoimmune hepatitis.

Impairment in Regulatory CD4 T cells (Treg) number and function have been implicated in autoimmune hepatitis (AIH). Treg are critical regulators of CD28 costimulation through CTLA4-mediated CD80 and CD86 control. We sought evidence for hepatic Treg frequency, phenotype, and function, and CD80/CD86 availability in AIH. Hepatic immune cells were isolated from eight patients with AIH and compared with cirrhotic and non-cirrhotic controls. Cells were assessed by flow cytometry and function was assessed by acquisition of CD80 from model antigen-presenting cells (APCs). We observed that Treg frequen

[Infliximab-induced autoimmune-like hepatitis developed during treatment for Crohn's disease:a case report].

A female patient in her 20s with abdominal pain, diarrhea, fever, and arthralgia was referred to our hospital. Colonoscopy revealed multiple ulcers in the right colon, and biopsy demonstrated non-caseating granulomas, leading to a Crohn's disease diagnosis. She initially received adalimumab, but secondary loss of response occurred after 5 years. Her treatment was then switched to infliximab (IFX), which induced remission. Liver dysfunction was noted at 9 months after IFX initiation. Laboratory tests revealed positive antinuclear antibody (ANA), and liver biopsy demonstrated lymphoplasmacytic i

Autoimmune Hepatitis and Pregnancy: A Case Report.

Autoimmune hepatitis (AIH) is a long-lasting liver ailment. It causes hepatocellular necrosis and inflammation, leading to fibrosis. It can develop into cirrhosis and liver failure. The disease predominantly affects young to middle-aged women more than men. AIH flares up during gestation and is linked with a high rate of embryonic and maternal problems. With maternal and antenatal care becoming advanced, this disorder should be identified and managed for successful maternal and embryonic outcomes. We present a case report of a primigravida diagnosed with AIH at 14 weeks antenatally. Our main a

Autoimmune Hepatitis: A Review of Molecular Mechanisms and Research Gaps in African Populations.

Autoimmune hepatitis (AIH) is an inflammatory liver disease characterised by immune-mediated hepatic injury, often leading to liver failure. The underlying molecular mechanisms of AIH remain poorly elucidated, hindering diagnostic and therapeutic advances. This review overviews the current understanding of AIH pathogenesis, which arises from a complex interplay of genetic predisposition, environmental triggers, and immune mechanisms (loss of tolerance, regulatory T cell dysfunction). Furthermore, current technologies and models which are being used to deconvolve the molecular profiles and path

Mast Cells at the Crossroad of Gut-Derived Signals Through Aryl Hydrocarbon Receptor Activation: A Microbial-Immune Dialogue in Liver Inflammation with Therapeutic Perspectives.

Mast cells (MCs) are multifunctional innate immune cells that regulate inflammation, tissue repair, and immune responses, and they are increasingly recognized as contributors to chronic liver disease. In parallel, the aryl hydrocarbon receptor (AhR) has emerged as a key environmental sensor activated by gut-derived tryptophan metabolites such as kynurenine and microbial indoles. The current literature separately describes the role of AhR in MC signaling, as well as the contributions of MCs to liver pathology and the disrupted gut-liver axis, which drives immune dysfunction in chronic liver dis

In vivo CAR-Tfh cell reprogramming restores tolerance in a mouse model of autoimmune hepatitis.

Autoimmune diseases involve the coordinated dysregulation of multiple types of immune cells, and single-cell targeted therapies often yield suboptimal results. Here, we developed chimeric antigen receptor (CAR)-engineered follicular helper T (Tfh) cells capable of simultaneously suppressing T cells, B cells, and dendritic cells (DCs) to restore immune homeostasis in an autoimmune hepatitis (AIH) mouse model. Using amino acid-derived lipid nanoparticles, we deliver self-amplifying RNA encoding Forkhead box protein P3 (Foxp3) and a cytochrome P4502D6 (CYP2D6)-specific CAR to Tfh cells, conferrin

Naringin alleviates autoimmune hepatitis in mice via the gut-liver Axis through modulation of microbiota, metabolites, and immune responses.

Autoimmune hepatitis (AIH) is an immune-mediated liver disease that could be impacted by gut microbiota dysbiosis. Naringin, a flavonoid derived from citrus fruits, has been reported to modulate gut microbial composition and alleviate liver diseases, but its role in AIH remains incompletely understood. In this study, we used multi-omics analysis and fecal microbiota transplantation (FMT) to examine the protective benefits of naringin in a ConA-induced AIH mouse model. To strengthen mechanistic conclusions, histopathological scoring, liver function indices, and immune cell profiling were determ

Association between serum uric acid and non-alcoholic fatty liver disease (NAFLD): an observational cross-sectional study in an Egyptian outpatient cohort.

BACKGROUND: Hepatic steatosis, characterized by excessive triglyceride accumulation within hepatocytes, is the central feature of non-alcoholic fatty liver disease (NAFLD). This spectrum encompasses simple steatosis, non-alcoholic steatohepatitis (NASH), progressive fibrosis, and cirrhosis. NAFLD pathogenesis involves an intricate interplay between nutritional factors, metabolic dysregulation, and genetic predisposition. Evidence suggests hyperuricemia is an independent risk factor for NAFLD, with elevated serum uric acid (SUA) levels associated with increased steatosis severity and fibrosis a

Research progress on the role of gut microbiota dysbiosis in the pathogenesis of immune-mediated liver diseases.

Gut microbiota dysbiosis plays a significant role in the pathogenesis of immune-mediated liver diseases (IMLDs), including autoimmune hepatitis (AIH), primary biliary cholangitis (PBC), and primary sclerosing cholangitis (PSC), through multiple gut-liver axis mechanisms. Microbial metabolites such as short-chain fatty acids (SCFAs) and secondary bile acids regulate hepatic immune homeostasis by activating G protein-coupled receptors (GPRs) and the farnesoid X receptor (FXR). Concurrently, disruption of the intestinal barrier integrity allows endotoxins (e.g., lipopolysaccharide) to activate he

Evaluation of large language models in supporting autoimmune liver disease diagnosis and clinical decision-making: advantages of reasoning-based models.

INTRODUCTION: Large-language models (LLMs) have demonstrated increasing potential in healthcare, including applications in clinical information retrieval, medical reasoning, and decision support. Autoimmune liver diseases (AILDs), including autoimmune hepatitis, primary biliary cholangitis, primary sclerosing cholangitis, and IgG4-related liver disease, are rare and heterogeneous conditions that often present with nonspecific features and require specialized expertise for accurate diagnosis and management. In resource-limited settings, limited access to hepatology subspecialists may contribute

Seronegative Autoimmune Hepatitis Presenting With Concurrent Autoimmune Hemolytic Anemia in a 26-Year-Old Woman: A Rare Overlap Syndrome.

Autoimmune hepatitis (AIH) is a chronic immune-mediated liver disease, and its seronegative variant poses diagnostic challenges due to absent autoantibodies. Autoimmune hemolytic anemia, characterized by immunoglobulin G-mediated erythrocyte destruction, rarely coexists with AIH, creating a complex clinical scenario. We report a 26-year-old woman presenting with jaundice, anemia, and elevated liver enzymes. Serologies for antinuclear antibody, smooth muscle antibody, and anti-liver kidney microsomal type 1 were negative, but immunoglobulin G was elevated. Liver biopsy revealed severe panlobula

Novel autoantibodies in autoimmune hepatitis: from diagnostic challenges to molecular and spatial pathology.

Autoimmune hepatitis (AIH) lacks a single disease-specific diagnostic test, particularly in acute-onset or seronegative cases with normal IgG levels. Recent progress in high-throughput autoantibody profiling has enabled systematic and unbiased identification of novel autoantibodies, offering new insights into disease diagnosis and underlying pathogenic mechanisms. This review summarizes emerging autoantibodies in AIH, primarily focusing on antibodies identified through proteome-wide approaches. We highlight anti-docking protein 2 (DOK2) antibodies, identified using a human protein microarray a

A novel non-invasive nomogram for predicting advanced liver fibrosis in autoimmune hepatitis.

Autoimmune hepatitis (AIH) is a relatively rare, immune-mediated chronic inflammatory liver disease characterized predominantly by hepatocellular injury. Liver fibrosis assessment is crucial in AIH management. We aimed to develop and validate a novel non-invasive nomogram to predict advanced liver fibrosis in AIH patients. Patients with AIH who had undergone liver biopsy and met the inclusion and exclusion criteria were included in this retrospective study; subsequently, they were randomly assigned to the training set and the validation set in a ratio of 7:3. Least absolute shrinkage and selec

Incidence, Prevalence, Medication Use, and Transplant Rates in Paediatric Autoimmune Hepatitis-A Nationwide Cohort Study.

BACKGROUND AND AIMS: Despite the aggressive nature of paediatric autoimmune hepatitis (P-AIH), population-based epidemiology is poorly described. We aimed to validate an algorithm to identify patients with P-AIH in the Danish National Patient Registry (DNPR) and report incidence rates, prevalence, and describe medication exposure and liver transplantation rates. METHODS: This was a nationwide, register-based study. We used a smaller population-based P-AIH cohort as true positives for validation. The best-performing algorithm was then used to identify all patients in Denmark with P-AIH (1978-2
