Second-line tacrolimus and mycophenolate mofetil in difficult-to-treat autoimmune hepatitis.

INTRODUCTION: Azathioprine with corticosteroids is the first-line treatment for patients with autoimmune hepatitis (AIH). However, around 20% of patients require treatment alternatives, and second-line therapy for AIH is less well defined due to a lack of randomized controlled trials. We evaluated the efficacy and safety of mycophenolate mofetil (MMF) and tacrolimus as second-line therapy. METHODS: We performed a retrospective analysis of second-line therapies with MMF or tacrolimus for patients with AIH. Biochemical parameters were collected at the change of therapy, after 6 and 12 months, a

From Immunization to Autoimmunity: A Deep Dive Into Post-COVID-19 Vaccine Reactions-A Narrative Review.

BACKGROUND AND AIMS: Coronavirus disease-2019 (COVID-19) emerged in 2019 from Wuhan, China, leading to the accelerated development of vaccines, which have proven instrumental in controlling the spread of the virus. While vaccination ceased the increasing trend of the viral infection, rare adverse autoimmune events emerged as important considerations. The present review captures the current evidence on reported cases of post-COVID-19 vaccination autoimmune and clinical presentations and their potential immunological mechanisms. We aim to inform healthcare providers about the spectrum of potenti

Performance evaluation of mainstream large language models in autoimmune hepatitis patient education: a comparative study of readability, quality, and reliability.

BACKGROUND: Autoimmune hepatitis (AIH) is a chronic immune-mediated liver disease that requires long-term management, in which effective patient education plays a critical role. With the rapid development of large language models (LLMs), AI-generated health information is increasingly accessed by patients; however, the readability, quality, and educational suitability of LLM-generated AIH-related content remain insufficiently evaluated. METHODS: Five widely used LLMs-ChatGPT, Doubao, DeepSeek, Wenxin Yiyan, and Tongyi Qianwen-were assessed based on their responses to 20 frequently asked AIH p

AI portal tract detection and characterisation for a regional analysis of steatosis and inflammation in MASLD, MASH and AIH.

AIMS: Annotation of liver biopsies for disease staging is increasingly aided by digital pathology; however, existing systems do not quantify inflammation and steatosis within an anatomical framework. We hypothesise that an artificial intelligence (AI) system that quantifies portal tracts (PT) and the anatomical distribution of steatotic vesicles and inflammatory cells will align with manual pathologist scoring and stratify liver diseases. METHODS: In this observational, cross-sectional study, digitised images of haematoxylin and eosin-stained specimens were pooled from four independent cohort

Quality of Life in Patients With Variant Syndromes of Autoimmune Liver Diseases-A Cross-Sectional Multicentre Study.

BACKGROUND & AIMS: Primary biliary cholangitis (PBC), primary sclerosing cholangitis (PSC) and autoimmune hepatitis (AIH) go along with reduced health-related quality of life (HRQOL). Variant syndromes, that is, conditions with features of both PBC/PSC and AIH, are associated with higher clinical complexity and worse prognosis. Studies on HRQOL in patients with variant syndromes are lacking. We aimed to provide large-scale evidence addressing this gap. METHODS: We included adult patients with clinical diagnoses of autoimmune liver diseases across nine countries in a cross-sectional study. We

How do oral pathogenic bacteria potentially contribute to inflammatory processes in autoimmune liver disease? Results from a systematic review.

BACKGROUND: Emerging evidence suggests that the oral-gut-liver axis may play a role in the immunopathogenesis of autoimmune liver diseases (AILDs), particularly autoimmune hepatitis (AIH) and primary biliary cholangitis (PBC). Alterations in the oral microbiota may influence hepatic immune responses; however, this relationship remains poorly defined. OBJECTIVE: This systematic review aimed to evaluate current evidence on the association between oral microbiota dysbiosis and AILDs, with a focus on microbial alterations, inflammatory profiles, and potential diagnostic implications. METHODS: A

Indian National Association for Study of the Liver Guidance Document on Difficult to Treat Autoimmune Hepatitis.

Autoimmune hepatitis (AIH) is an immune-mediated inflammatory disorder. It is a highly heterogeneous entity, having a wide range of presentations from asymptomatic chronic hepatitis to cirrhosis and acute liver failure. In consonance with the variabilities in presentation, there are also variations in response to treatment, depending on disease phenotype, presentation, extent of fibrosis, and the presence of comorbidities. In addition, pediatric AIH and AIH postliver transplant have their individual nuances. Addressing such areas to identify strategies for best practices is an unmet goal in th

Constructing a protein-protein interaction network for autoimmune liver diseases by integrating pQTL, rQTL, and mediation analyses.

Autoimmune liver diseases (AILDs), including primary biliary cholangitis (PBC), primary sclerosing cholangitis (PSC), and autoimmune hepatitis (AIH), pose significant diagnostic and therapeutic challenges due to poorly understood mechanisms. While most studies focus on absolute protein levels, protein-protein ratios (PPRs), reflecting the relative abundance of paired plasma proteins, emerge as critical yet understudied biomarkers for decoding disease-specific network perturbations. To harness this potential, we integrated protein quantitative trait loci (pQTLs), ratio QTLs (rQTLs), and mediati

Phage therapy targeting DNA-encapsulated membrane vesicle-producing intestinal symbiotic Klebsiella pneumoniae ameliorates autoimmune disease.

Autoimmune hepatitis (AIH) is a progressive and currently incurable inflammatory liver disorder. Accumulating evidence suggests that intestinal Klebsiella pneumoniae may contribute to AIH. However, the specific bacterial features underlying its role remain unclear. We isolated 32 strains of K. pneumoniae from both healthy and AIH patients. Genomic and phenotypic analyses identified a unique strain, KpA4-1 from an AIH patient, distinguished by its production of membrane vesicles (MVs) encapsulating plasmid DNA (p2). Administration of p2-containing MVs in mice elevated serum liver enzymes and an

[Exploring the research on the molecular mechanisms of autoimmune liver diseases].

Autoimmune liver diseases include subtypes such as autoimmune hepatitis and primary biliary cholangitis, which are caused by abnormal immune system attacks on hepatocytes or bile ducts. This article reviews their molecular mechanisms from three aspects: genetics, environment, and immunity. Human leukocyte antigen (HLA) gene polymorphisms such as DRB10301 and DRB10405 show regional specificity, and non-HLA gene variants such as CTLA4 and FAS also contribute to disease development in terms of genetic factors. Cross-immune responses are triggered through 'molecular mimicry," disturbances in bile
