Prevalence, risk factors, and clinical outcomes of extraintestinal manifestations in inflammatory bowel disease: a population-based study in china.

BACKGROUND AND AIMS: Extraintestinal manifestations (EIMs) complicate the management of inflammatory bowel disease, and their epidemiology in Chinese patients remains unclear. We aimed to determine their prevalence and examine associated risk factors and clinical outcomes. METHODS: We conducted a cross-sectional study of over 44 000 patients of all ages in eastern China using data from the China National Healthcare Big Data (East) Center (2019-2024). We examined diverse EIMs (articular, skin, ocular, and oral manifestations, primary sclerosing cholangitis, and autoimmune hepatitis) and compar

Elevated Polyreactive Immunoglobulin G in Immune-Mediated Liver Injuries With the Need for Immunosuppressive Therapy.

BACKGROUND & AIMS: The distinction of drug-induced liver injury (DILI), drug-induced autoimmune-like hepatitis (DI-ALH), and autoimmune hepatitis (AIH) can be challenging due to overlapping clinical characteristics. Recently, polyreactive immunoglobulin G (pIgG) was identified as a novel biomarker in AIH. This retrospective study aimed to evaluate the diagnostic accuracy of pIgG to distinguish between AIH, DI-ALH, and DILI and thus identify patients in need of immunosuppression. METHODS: Samples from 120 patients (AIH = 81, DI-ALH = 16, DILI = 23) were compared to a control group (non-AIH-non

Single-cell profiling uncovers kaempferol-mediated immunoregulation in the protection against experimental autoimmune hepatitis.

BACKGROUND: Autoimmune hepatitis (AIH) is triggered by an aberrant immune response directed against hepatocyte autoantigens, ultimately leading to liver inflammation and hepatocellular damage. Kaempferol (KAE), a natural flavonoid, exhibits potent anti-inflammatory and hepatoprotective properties. However, the precise immunoregulatory mechanisms of KAE in alleviating AIH are still undefined. OBJECTIVES: This study utilized comprehensive single-cell analysis to investigate the therapeutic efficacy and underlying immunoregulatory mechanisms of KAE in alleviating ConA-induced experimental AIH.

Assessing the associations between immunosuppressant use and COVID-19 severity in patients with autoimmune hepatitis.

BACKGROUND: Due to immunosuppression (IS) use, patients with autoimmune hepatitis (AIH) may be at high risk for poor coronavirus disease 2019 (COVID-19) outcomes. AIM: To investigate the associations between IS type and COVID-19 severity in AIH patients using the National Clinical Cohort Collaborative (N3C) COVID enclave. METHODS: We identified all AIH patients with COVID-19 in the N3C COVID enclave. We used adjusted logistic regressions to determine associations between IS type and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. We used Cox models to determine associ

A subset of hepatic fibroblasts marked by Gli1 expression generates portal fibrosis and promotes ductular reaction.

BACKGROUND & AIMS: Gli1+ cells were previously described as myofibroblast precursors. We aimed to delineate the identity of Gli1+ cells and their contribution to wound healing in liver. METHODS: Gli1+ cells were analyzed in normal mouse liver by single-cell RNA sequencing (n = 3). Genetic cell fate tracing and depletion of Gli1+ cells were achieved in male mice, undergoing cholestatic injury induced by 3,5-diethoxycarbonyl-1,4-dihydrocollidine (DDC) or bile duct ligation (BDL), and metabolic dysfunction-associated steatohepatitis (MASH). Gli1 expression was examined by fluorescence in-situ hy

Comparative waitlist mortality of autoimmune liver disease: A systematic review and meta-analysis.

BACKGROUND & AIMS: Autoimmune liver diseases (AILDs) such as autoimmune hepatitis (AIH), primary biliary cholangitis (PBC), and primary sclerosing cholangitis (PSC) show differing waitlist outcomes. This meta-analysis aimed to synthesize comparative waitlist mortality across AILD subtypes and non-autoimmune etiologies, such as metabolic-associated steatotic liver disease (MASLD) or alcohol-associated liver disease (ALD). METHODS: Following PRISMA guidelines, MEDLINE, PubMed, Embase, Scopus, and Cochrane CENTRAL were searched from inception to October 2025. Eligible studies included adults lis

Prevalence and clinical impact of hepatic steatosis on autoimmune liver disease: A systematic review and meta-analysis.

BACKGROUND: The clinical impact of hepatic steatosis (HS) among patients with autoimmune liver disease (AILD) remains unclear. We aim to determine the prevalence of HS and its clinical impact on treatment response and outcomes in patients with AILD. METHODS: We systematically searched 3 electronic databases until 17 December 2025, including all studies that reported the prevalence, clinical impact, and treatment response of AILD patients with concomitant HS. The temporal trend of HS prevalence was analyzed using a quasi-Poisson regression model, with annual percent changes (APC, %) calculated

Clec3b⁺ fibroblasts are the primary effectors of portal fibrosis following activation via a KLF4/periostin axis.

Portal fibrosis, a determinant of progression in virtually all chronic liver diseases, prototypically develops in biliary diseases. Using single-cell RNA sequencing and genetic cell fate tracing in mouse models, we identify Clec3b⁺ fibroblasts as a distinct subset of portal fibroblasts, which rapidly expand after biliary injury and give rise to the bulk of portal myofibroblasts. Mechanistic analyses reveal that Clec3b⁺ portal fibroblasts activation is governed by a Krüppel-like factor 4 (KLF4)/periostin (POSTN) axis, i.e., KLF4 directly binds to the Postn promoter and represses its transcripti

Seronegative Autoimmune Hepatitis in Adults Is a Rare Entity Often Obscured by Suboptimal Diagnostic Assessment.

Seronegative autoimmune hepatitis (SnAIH) lacks detectable conventional autoantibodies (ANA, SMA, anti-LKM, anti-LC1, and anti-SLA/LP), posing diagnostic challenges. Increasing evidence suggests SnAIH in adults reflects limitations in autoantibody detection rather than a distinct variant. We systematically re-evaluated all biopsy-proven SnAIH cases in a large international multicenter AIH cohort using standardized testing. Among 760 patients with baseline liver biopsy, 52 (6.8%) were initially classified as SnAIH. Forty-three lacked complete autoantibody testing, most commonly anti-SLA/LP. Aft

Causal relationship between circulating immune cells, cytokines and different types of autoimmune liver diseases: A univariable and multivariable Mendelian randomization study.

Autoimmune liver diseases (AILDs) are a group of liver diseases characterized by immune dysfunction. Recent studies have proposed a potential correlation among immune cells, cytokines, and AILDs. However, the causal nature of this relationship remains to be elucidated. Summary-level Genome-Wide Association Studies (GWAS) statistical data for primary biliary cholangitis and primary sclerosing cholangitis were obtained from FinnGen, while autoimmune hepatitis and immune trait statistics were sourced from the GWAS Catalog. GWAS data for 41 inflammatory cytokines were acquired from the University

COVID-19 vaccine-associated liver injury and autoimmune hepatitis: a case series.

BACKGROUND: Coronavirus disease 2019 (COVID-19) vaccines have significantly reduced global morbidity and mortality. While generally safe, rare immune-mediated liver injuries, including autoimmune hepatitis (AIH) and drug-induced liver injury have been reported post-vaccination. CASE SERIES: We present three cases that developed liver injury following Pfizer-BioNTech (BNT162b2) COVID-19 vaccination. Case 1 describes a 64-year-old female with new-onset AIH confirmed by serology, histology, and clinical response to corticosteroids. Case 2 is a 36-year-old female who developed transient liver inj

An update on drug-induced liver injury-What's new?

Drug-induced liver injury (DILI) is an important medical condition. Recently, development of novel causality assessment tools has improved the diagnostic accuracy of idiosyncratic DILI, particularly RECAM (Revised Electronic Version of Roussel Uclaf Causality Assessment [RUCAM]). RECAM was found to be better than RUCAM in difficult cases and increased the precision of the DILI diagnosis but needs further refinement. In recent years, there has been an increased incidence of DILI with drugs, such as oncological drugs and herbal and dietary supplements (HDSs). DILI is commonly associated with che

Predictors of recompensation in immunosuppressive therapy for biopsy-proven autoimmune hepatitis patients with decompensated cirrhosis.

INTRODUCTION AND OBJECTIVES: The potential for recompensation in treatment-naïve patients with autoimmune hepatitis (AIH) and decompensated cirrhosis, per Baveno VII criteria, remains underexplored. This study aimed to identify the clinical characteristics and prognostic factors associated with recompensation. MATERIALS AND METHODS: We enrolled 81 treatment-naïve patients with biopsy-proven AIH and decompensated cirrhosis (median follow-up 61.8 months). All patients were treatment-naïve at enrollment and had confirmed diagnoses based on liver biopsy. Using Baveno VII criteria, we assessed rec

A machine learning model incorporating the globulin-to-platelet index for predicting severe fibrosis in autoimmune hepatitis: A retrospective and prospective validation study.

Accurate staging of liver fibrosis in autoimmune hepatitis (AIH) remains challenging due to the invasive nature and sampling limitations of liver biopsy. This study aimed to identify readily available predictors of severe fibrosis and to develop an AIH-specific noninvasive machine-learning model. This two-stage study retrospectively enrolled 208 patients with biopsy-confirmed AIH, with prospective validation in 26 additional patients. Transient elastography (TE) was performed in 110 retrospective and 12 prospective patients. Severe fibrosis was defined as Scheuer stages S3 to S4. Candidate var
