Psoriasis: Which therapy for which patient, updated, part II. Focus on special populations and chronic infections.

Choosing systemic therapy for psoriasis remains challenging in populations underrepresented in clinical trials, including pregnant and pediatric patients, patients with chronic infections, and those with difficult-to-treat phenotypes. Since the 2019 version of this article, therapeutic options have expanded substantially, including new pediatric approvals, oral targeted agents, and biologics with increasingly phenotype-specific evidence. This second article in the continuing medical education series reviews systemic treatment selection for moderate-to-severe psoriasis in pregnancy, pediatric p

Molecular characterization and genetic diversity of hepatitis B virus among health care workers in the Bamenda Health District, Northwest Region, Cameroon.

OBJECTIVES: Hepatitis B virus (HBV) infection is endemic in Africa, with an estimated 82 million people living with chronic HBV infection. The outcome of exposure to HBV is a complex interplay between host immunity and viral factors, including HBV genotypes. Despite the high burden of HBV infection, data on HBV genotypes circulating in many African settings, including Cameroon, remain limited. This study investigated the genetic diversity of HBV among health care workers in the Bamenda Health District, Cameroon. METHODS: In this cross-sectional hospital-based study, HBV DNA was extracted from

Alpha-fetoprotein change after systemic treatments in advanced hepatocellular carcinoma: a systematic review and trial-level meta-analysis.

Alpha-fetoprotein (AFP) kinetics after systemic therapy for unresectable or metastatic hepatocarcinoma has been evaluated as an early prognostic factor in many studies, but the effect size has varied largely. The current study aims to synthesize the evidence about the relationship between AFP changes and overall survival (OS). A literature search of clinical studies of metastatic hepatocarcinoma patients receiving systemic treatments was conducted in two databases, PubMed and Web of Science. After selecting studies evaluating the relationship of any AFP changes with OS, a meta-analysis was per

Immune Thrombocytopenia in Low- and Middle-Income Countries: A Systematic Review.

BackgroundImmune thrombocytopenia (ITP) is an acquired immune-mediated platelet disorder. Most evidence informing its diagnosis and management comes from high-income settings as transferability to low- and middle-income countries (LMIC) may not be possible. We conducted a systematic review to characterise adult ITP in this setting.MethodsWe conducted this systematic review in accordance with PRISMA guidance. Studies reporting adult ITP in LMICs were included if they provided data on epidemiology, infections, first- or second-line treatment, or patient-reported outcomes.ResultsTwenty studies we

Integrated AI-Driven Multi-Objective Optimization of Sulfamoylbenzamide-Based HBV Capsid Assembly Modulators: Discovery of 11B-6 with Superior Potency and Drug-like Properties.

Current capsid assembly modulators (CAMs) require improvements in antiviral potency and drug-like properties. Herein, we report a multi-objective optimization workflow for the NVR 3-778 by integrating structure-based fragment growing with systematic medicinal chemistry optimization, activity prediction, and multitask prediction of drug-like properties. This led to 48 novel analogs. Among them, 11B-6 showed superior antiviral activity in HepAD38 cells (EC50 = 0.04 ± 0.02 μM), outperforming NVR 3-778 (EC50 = 0.50 ± 0.17 μM). In HBV-infected HepG2-NTCP cells, 11B-6 inhibited secreted HBV DNA with

ISG-15 promotes de novo HBV infection after pediatric liver transplant via IL-18/STAT1 from HBcAb-positive donors.

INTRODUCTION AND OBJECTIVES: Liver transplantation is the main therapy for pediatric end-stage liver disease, but use of hepatitis B core antibody-positive (HBcAb+) grafts increases the risk of de novo hepatitis B virus (HBV) infection. We examined whether ISG-15 promotes de novo HBV through IL-18/STAT1 signaling. PATIENTS AND METHODS: We retrospectively analyzed clinical data and conducted proteomics analysis. Liver biopsy specimens were randomly selected from eight pediatric recipients who developed de novo HBV infection and from 10 pediatric recipients who did not develop de novo HBV infec

Therapeutic advances in chronic hepatitis B and Delta: where are we now and where are we headed?

Chronic hepatitis B (HBV) and delta (HDV) infections remain major global health challenges. Nucleos(t)ide analogues (NUCs) and bulevirtide, a recently approved entry inhibitor, effectively suppress HBV and HDV replication, respectively, thereby reducing the risk of liver-related complications. However, they rarely induce functional cure, defined as sustained HBsAg loss, and generally require long-term administration. This limitation is primarily driven by the persistence of covalently closed circular DNA (cccDNA) and integrated HBV DNA, which are not targeted by existing treatments. A broad ra

Impact of nationwide parvovirus B19 DNA screening and identification of transfusion-transmitted infections.

BACKGROUND AND OBJECTIVES: Parvovirus B19 (B19V) testing is often not required to release blood components. However, the European Pharmacopoeia requires that plasma pools for fractionation contain no more than 104 IU/mL of B19V DNA. In France, pool B19 nucleic acid testing (NAT) screening was performed by Établissement Français du Sang since 2015. Since June 2024, positive pools are broken down into individual testing, allowing for the identification and management of infected blood donations. MATERIALS AND METHODS: B19V NAT is performed in pools of 96 donations (P96) with the DPX Parvo/hepat

Literature Review of Occult Hepatitis B Infection and Hepatitis B Core Antibody-Positive Blood Donors.

Hepatitis B virus (HBV) is one of transfusion-transmissible infection that is screened routinely in blood centers by serology and/or nucleic acid testing (NAT). Serology tests for HBV include antibody to hepatitis B core antigen (anti-HBc) and hepatitis B surface antigen (HBsAg). Blood centers around the world differ in their procedures for screening for HBV in blood donors. In high-income, HBV non-endemic countries, anti-HBc, HBsAg, and pooled NAT are usually used to screen blood donors, while in HBV endemic countries, anti-HBc is not usually used because it would lead to high discard rates.

Discovery of Thiazolamide-based PI4KIIIβ/HDAC Dual Inhibitors with Potent Activity against Hepatitis C Virus.

Chronic hepatitis C virus (HCV) infection remains a critical global public health burden affecting hundreds of millions of people worldwide. The emergence of antiviral resistance persistently undermines the long-term clinical efficacy of current direct-acting antiviral (DAA) regimens, creating an urgent need for therapeutics with different mechanisms of action. Host-directed multi-target therapy offers a promising avenue to overcome DAA resistance, yet dual inhibitors targeting HCV-essential host kinase phosphatidylinositol 4-kinase IIIβ (PI4KIIIβ) and epigenetic regulator histone deacetylases

Nitrogen-Functionalized Amphilectane-Type Diterpenoids with Anti-HBV Activity from the Okinawan Marine Sponge Stylissa sp.

Current treatments for hepatitis B virus (HBV) infection are constrained by adverse effects, drug resistance, and the need for long-term administration. Therefore, the development of safer and more effective therapeutic agents remains an important priority. In this study, two new amphilectane-type diterpenoids, 7-isocyanatoneoamphilecta-11,15-diene (1) and 7-isothiocyanatoneoamphilecta-11,15-diene (2), together with three known analogues (3-5), were isolated as anti-HBV agents from the Okinawan marine sponge Stylissa sp. The structures of 1 and 2 were elucidated by spectroscopic analysis, sing

Thermal inactivation kinetics of foodborne viruses across food matrices: a Bayesian meta-analysis and predictive framework.

Human norovirus and hepatitis A virus are leading causes of foodborne illness, yet their thermal resistance across foods remains difficult to predict. We conducted a Bayesian hierarchical meta-analysis of time-series thermal inactivation data from 29 studies identified following PRISMA guidelines to quantify the effects of food matrix, virus group, and measurement method. Data from simple buffers, molluscan shellfish, dairy products, berries, and vegetables were evaluated for human enteric viruses and cultivable surrogates. Infectivity- and RT-qPCR-based measurements were analyzed separately t

Hepatitis E virus genome plasticity: a marker of chronicity and therapy failure?

Hepatitis E virus (HEV) is a globally distributed zoonotic pathogen, spread by contaminated food and blood products, and a leading cause of acute viral hepatitis. Recent data indicate that the HEV genome contains a highly heterogeneous hypervariable region (HVR) that can tolerate extensive genomic rearrangements, especially among immunocompromised patients. These rearrangements can increase HEV replication in tissue-culture and have further been observed in HEV-infected patients. Insertions comprise either duplications of virus-derived sequences, or alternatively, host-derived sequence fragmen

Fiber-2 induces autolysosome dysfunction and impairs autophagic flux to promote FAdV-4 replication.

Fowl adenovirus serotype 4 (FAdV-4) causes hepatitis-hydropericardium syndrome in poultry, yet viral subversion of host autophagy remains unclear. Here, we investigated the role of the viral structural protein Fiber-2 in modulating the host autophagy-lysosome pathway. Transcriptomic profiling of LMH cells expressing Fiber-2 revealed coordinated upregulation of autophagy-related genes, validated by quantitative PCR. Functional assays demonstrated that Fiber-2 expression leads to autophagosome accumulation and impaired autophagic flux, as shown by LC3-II/p62 analysis, tandem fluorescent LC3 repo
