The Development of Enterovirus Sepsis Syndrome in a Preterm Neonate: A Case Report.

BACKGROUND: Enterovirus (EV) infections are common in pregnant patients and neonates. However, peripartum EV infections are significantly underdiagnosed. Infants with EV infection may be asymptomatic, present with a non-specific febrile illness, or develop life-threatening disease. CLINICAL FINDINGS: This infant had an unremarkable clinical course for a preterm neonate, until a rising direct bilirubin, elevated liver enzymes, coagulopathy, and thrombocytopenia were identified by the neonatology team. The infant was then transferred to the level IV NICU with concern for liver failure of unknow

Non hepatotropic virus induced hepatitis - rising importance in a changing world.

A knowledge of the epidemiology and clinical aspects of non-hepatotropic viruses is becoming increasingly important in lieu of the rising incidence of acute liver injury caused by them in various circumstances. Broadly, they include the Herpesviridae group, the hemorrhagic fever viruses and certain respiratory viruses that infect the liver. They can affect both the immunocompetent and the immunocompromised individual, more commonly the latter as part of disseminated systemic infection with symptoms ranging from self-limited transaminitis to acute liver failure Various reasons for their rising

Hepatitis E Virus Infection in a Liver Transplant Recipient Presenting With Late-Onset Acute Rejection and Treated With Ribavirin.

Hepatitis E virus (HEV) is increasingly recognized as a cause of allograft hepatitis in transplant recipients. Persistent HEV infection is often asymptomatic with nonspecific findings on liver biopsy, requiring high clinical awareness to order viral testing. We present a challenging case of HEV allograft hepatitis that presented with increasing liver enzymes and subtle biopsy findings of hepatocyte apoptosis and mitosis, diagnosed 1 month after treatment of acute rejection. Viral testing was positive for HEV IgM and HEV RNA, consistent with acute HEV infection. The patient was treated with a 1

Hepatitis E Virus ORF1 Polyprotein Harbors a Pocket-Like Cavity That Is Vital for Virus Replication and Represents a Novel Antiviral Target.

Hepatitis E virus (HEV) is the leading cause of acute viral hepatitis worldwide, yet no FDA-approved anti-HEV medication available. Elucidating HEV replication machinery is therefore crucial for identifying novel antiviral targets, and consequently developing potent antivirals. The nonstructural ORF1 polyprotein is pivotal for HEV replication. Herein, it is revealed that the ORF1 X domain is a critical component of HEV replication machinery. Interestingly, the ADP-ribose hydrolase activity of X domain per se is dispensable for HEV replication. Instead, the X domain supports HEV replication thr

Guillain-Barré Syndrome Mimicking Autoimmune Hepatitis: A Rare Entity.

This case report describes a rare association between autoimmune hepatitis (AIH) and Guillain-Barré syndrome. A 71-year-old woman with a history of hypertension, diabetes, and hyperlipidemia presented with elevated liver enzymes and progressive distal limb weakness. Electromyography confirmed acute motor axonal neuropathy, a variant of Guillain-Barré syndrome, without an identifiable trigger. Serologies for viral hepatitis were negative; however, autoantibodies, including antinuclear antibody, antimitochondrial antibody, and smooth muscle antibody, were positive. Liver biopsy demonstrated hist

First Evidence of Spillover of Rocahepevirus ratti Into Humans in Thailand.

Rocahepevirus ratti (hepatitis E virus [HEV]-C), originally discovered in rats in 2010, has been recently linked to hepatitis in humans. Although rare and typically detected in the immunocompromised, much like Paslahepevirus balayani (HEV-A), it can manifest as acute or persistent hepatitis. In a next-generation sequencing (NGS)-based screen for causes of acute febrile illness (AFI) in Thailand, we assembled a complete Rocahepevirus (rat HEV [rHEV]) genome from a patient admitted to the hospital who developed abnormal liver functions 2-3 months after a heart transplant. Despite withdrawal of m

Identifying people with chronic hepatitis B virus who are lost to clinical follow up: A retrospective case finding and re-engagement service improvement exercise.

BACKGROUND: Hepatitis B virus (HBV) infection is an important cause of liver disease-related mortality and morbidity. The World Health Organisation aims to eliminate this as a public health concern by 2030 and as such the key international guidelines recommend that all patients are reviewed regularly to observe for preventable signs of disease progression. This requires life-long engagement with specialist services, but some patients fall out of the care pathway and become lost to follow-up. OBJECTIVES: This study sought to identify and re-engage patients with HBV who were lost to follow up (

Therapeutic Plasma Exchange in Patients With Acute Liver Failure: A Pilot Randomized Controlled Trial.

BACKGROUND/AIMS: Acute liver failure (ALF) is a life-threatening condition with high short-term mortality without liver transplantation (LT). Prior studies on therapeutic plasma exchange (TPE) have yielded mixed survival results. In this randomized controlled trial (RCT), we evaluated the safety and efficacy of TPE on the 30-day outcomes in ALF. METHODS: In this single-center, open-label, pilot RCT, 40 adults with ALF were randomized in a 1:1 ratio to receive either standard medical therapy (SMT) alone or SMT plus TPE. TPE was performed daily for up to five days or stopped as per the predefin

Development and validation of a predictive model for invasive syndrome in patients with Klebsiella pneumoniae liver abscess.

INTRODUCTION: Pyogenic liver abscess (PLA) is a life-threatening liver bacterial infection causing suppurative lesions. In Asia, hypervirulent Klebsiella pneumoniae (hvKP) is the main PLA pathogen, linked to invasive syndromes. Severe Klebsiella pneumoniae liver abscess (KPLA) manifestations, called invasive KPLA syndrome (IKPLAS), have acute onset, rapid progression and non-specific symptoms, often leading to poor prognosis if untreated. This study aimed to find risk factors and create a validated nomogram for predicting invasive syndrome in KPLA patients. METHODS: We retrospectively analyze

Case Report: Elevated Aminotransferases and Altered Hepatic Perfusion due to Retroperitoneal Paraganglioma.

INTRODUCTION: Hepatocellular injury is characterized by elevated levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT). In contrast, alkaline phosphatase levels are usually elevated in cholestatic diseases. Due to the multiples causes, it is crucial to define the etiology for proper management. The most common diagnoses are viral hepatitis, autoimmune hepatitis, Wilson's disease, drug-induced liver injury, acute alcoholic hepatitis, and ischemic hepatitis. Extrahepatic causes such as endocrine and cardiovascular conditions should also be considered. CASE PRESENTATION:

Bioinformatics identification of key genes correlating NOD1 and Endoplasmic Reticulum stress in Hepatitis B virus-induced acute liver failure.

Endoplasmic reticulum stress (ERS) has been implicated in a range of biological processes, yet its specific involvement in Hepatitis B virus-associated acute liver failure (HBV-ALF) remains poorly understood. This study aimed to identify key ERS-related genes (ERGs) and elucidate their underlying mechanisms in HBV-ALF. Publicly available HBV-ALF-related datasets (GSE38941, GSE62029) and ERGs were analyzed. Intersection genes were determined by overlapping differentially expressed genes from both datasets with ERGs, and genes showing strong correlation with NOD1 were selected as candidates. The

Role of efferocytosis in liver transplantation via hepatic macrophage.

Liver transplantation has undergone progressive advancements since the 1960s, with optimized surgical techniques and improved outcomes resulting from standardized procedures and therapeutic regimens. However, postoperative complications-such as ischemia-reperfusion injury, acute rejection, hepatocellular carcinoma recurrence, and biliary injury-continue to significantly affect both survival rates and quality of life in liver transplant recipients. Efferocytosis, the phagocytic clearance of apoptotic cells, plays a critical role in preventing necrosis-induced inflammation and promoting tissue r
