Genome-wide identification and characterization of the PE/PPE gene family in Mycobacterium abscessus.

Mycobacterium abscessus (Mab) is a rapidly growing non-tuberculous mycobacterium that causes opportunistic infections and exhibits extensive intrinsic antibiotic resistance. The proline-glutamate (PE) and proline-proline-glutamate (PPE) protein families are mycobacteria-specific virulence factors implicated in immune modulation, antigenic variation, and persistence. While Mycobacterium tuberculosis (Mtb) harbors approximately 170 PE/PPE members, the repertoire of PE/PPE genes in Mab remains largely uncharacterized. Here, we performed a genome-wide survey of PE/PPE genes in Mab ATCC 19,977 and

Early hemodynamic changes after the use of push-dose noradrenaline in patients presenting to the emergency department with septic shock: a descriptive study.

BACKGROUND: Although continuous noradrenaline infusion (CNI) is standard vasopressor therapy during the initial resuscitation of patients with septic shock, hemodynamic stabilization may not be immediate. Push-dose noradrenaline (PDNA) has been proposed as a temporary measure while CNI is being established. However, its clinical use, dosing, early hemodynamic response, and short-term safety in the emergency department (ED) are not well described. METHODS: To address this gap, we conducted a single-center retrospective descriptive study at a tertiary care hospital in Japan in 2019. Adults with

Passive acquisition of hepatitis B virus antibodies after allogeneic hematopoietic cell transplantation.

BACKGROUND: Newly positive hepatitis B virus (HBV) serologic results after allogeneic hematopoietic cell transplantation (HCT) may reflect passive antibody transfer from blood-derived products rather than HBV infection. We investigated the incidence and temporal patterns of hepatitis B surface antibody (anti-HBs) and antibody to hepatitis B core antibody (anti-HBc) acquisition after allogeneic HCT. STUDY DESIGN AND METHODS: We retrospectively evaluated 193 adults who underwent allogeneic HCT for hematologic diseases between May 2012 and March 2025 at a single institution in Japan. Anti-HBs, a

Case Report: Light chain amyloidosis in a patient with low-grade B-cell lymphoma.

BACKGROUND: Light chain (AL) amyloidosis is typically associated with plasma cell dyscrasias but can rarely arise from low-grade B-cell non-Hodgkin lymphoma. Renal involvement may present with nephrotic-range proteinuria, acute kidney injury, edema, anasarca, and hypoalbuminemia. Recognizing lymphoma-associated AL amyloidosis is essential because diagnosis requires tissue confirmation, and treatment must target the underlying clonal B-cell disorder. CASE: We report a rare case of severe renal AL amyloidosis in a patient with known low-grade B-cell non-Hodgkin lymphoma previously treated with

Synchronous Early Gastric Adenocarcinoma and Cervical Carcinoma of Unknown Primary with Metachronous Hepatocellular Carcinoma: A Case Report.

INTRODUCTION: Diagnosing multiple primary malignancies requires integration of chronology, histology, immunophenotype, imaging findings, and clinical course, and must be distinguished from recurrence or metastatic disease. CASE PRESENTATION: A 69 year-old male patient presented in October 2022 with a painless right cervical mass. Biopsy showed tumor cells arranged in nests with necrosis, enlarged hyperchromatic nuclei, and frequent mitoses. Immunohistochemistry showed CK positivity, focal CK5/6 and CK7 positivity, a Ki-67 index greater than 90%, focal TTF-1 positivity, and negativity for CD56

A dual recombinant strategy to quantify PreS1 antibody in hepatitis B patients with genotype B or C virus by ELISA.

Elevated levels of anti-preS1 antibodies in patients correlate with faster hepatitis B virus (HBV) clearance, since preS1 is vital for viral entry. However, accurate quantification of preS1 antibodies is hindered by the variation of HBV genotypes. Herein, a quantitative preS1 antibody assay was developed for Chinese patients with predominant HBV genotypes B and C. First, to address genotype diversity, we constructed a dual recombinant protein combining genotype B and C preS1. Second, a mouse-human chimeric antibody with high and comparable affinity for both B and C preS1 is developed as the st

Golgi-derived COPI vesicles fuel the biogenesis of coronavirus replication organelles.

Positive-strand RNA viruses remodel host endomembranes to form replication organelles (ROs). However, the membrane sources for coronavirus double-membrane vesicle (DMV) biogenesis remain unclear. Here, we used proteomic, genetic, pharmacological, and imaging approaches to identify the central membrane supply route for DMV formation. APEX2-based proximity proteomics first identified candidate pathways, followed by functional validation using siRNA knockdown, CRISPR/Cas9 editing, and Golgicide A (GCA) inhibition. DMV formation and viral RNA synthesis were assessed by electron microscopy, immunof

Early initiation of antiviral treatment for two infants with acute HBV infection: Beneficial or unfavorable?

Mother-to-child transmission is the major route of hepatitis B virus (HBV) infection in children, and antiviral therapy is generally deferred in infancy due to presumed immune tolerance. Here, we report the clinical course and treatment response of two infants younger than 6 months with HBV infection who initiated antiviral therapy due to elevated serum transaminase levels indicative of an immune-active status. Notably, Case #01 achieved a functional cure after 25 months of antiviral treatment. Case #02 did not achieve a functional cure but attained HBV DNA clearance and HBeAg seroconversion a

Rapidly Progressive Buschke-Löwenstein Tumor Managed With Wide Local Excision and Healing by Secondary Intention: A Case Report and Literature Review.

Buschke-Löwenstein tumor (BLT) is a rare, locally aggressive anogenital tumor associated predominantly with low-risk human papillomavirus (HPV) types 6 and 11. BLT is classically described as a slow-growing lesion predominantly affecting middle-aged men who are immunocompromised; cases of rapid progression are rarely reported. We present the case of a 23-year-old male with a progressively enlarging perianal mass over 1 year that was accompanied by rectal pain and intermittent bleeding. He reported prior unprotected heterosexual intercourse, though serologic testing for HIV, syphilis, and hepat

Magnitude and Associated Factors of Depressive Symptoms Among Adult Patients With Cirrhosis: An Experience From a Resource-Limited Setting.

OBJECTIVE: Depression is a common comorbidity in cirrhosis, yet its burden and determinants are poorly characterized in sub-Saharan Africa. This study aimed to determine the magnitude and factors associated with depression among adult patients with cirrhosis at a tertiary hospital in Ethiopia. METHODS: In a hospital-based cross-sectional study, 134 adult patients with cirrhosis were consecutively recruited from a hepatology clinic in Addis Ababa. Depressive symptoms were assessed using the patient health questionnaire-9 (PHQ-9), with a score ≥10 indicating clinically significant depressive sy

Current progress in the pathophysiology and potential therapeutic targets of acute-on-chronic liver failure through multiomics approaches.

Acute-on-chronic liver failure (ACLF) is a severe clinical syndrome that develops in patients with chronic hepatitis or liver cirrhosis. It is characterized by a high mortality rate, rapid progression, and complex pathological mechanisms. As it often involves multiorgan failure (e.g., of the liver, kidneys, and brain), traditional clinical indicators have limited predictive value and cannot support accurate, personalized treatment. However, the rapid development of omics sequencing technologies in recent years (e.g., genomics, transcriptomics, proteomics, metabolomics, and microbiomics) has pr

The aMAP Score Effectively Stratifies Hepatocellular Carcinoma Risk in the Steatotic Liver Disease Population.

BACKGROUND & AIMS: Steatotic liver disease (SLD) is an increasingly important cause of hepatocellular carcinoma (HCC), yet validated risk stratification tools remain limited. We aim to evaluate the aMAP score, initially developed for HCC risk prediction mainly in viral hepatitis across international cohorts, in the SLD population. METHODS: 156 406 SLD participants were included from the UK Biobank (UKB) and 3166 SLD patients with virally suppressed chronic hepatitis B from a Chinese cohort. The predictive and stratification performance of the aMAP score was assessed and compared with the PAGE

Exploratory associations of gut microbiota composition with HBeAg and HBsAg status in nucleos(t)ide analog-treated chronic hepatitis B.

HBeAg and HBsAg reflect viral replication and host immune status and are important determinants of treatment responsiveness in chronic hepatitis B (CHB). The gut microbiota, a key component of the gut-liver axis, may influence immune regulation and antiviral outcomes. This study characterized gut microbiota profiles across different HBeAg and HBsAg statuses in CHB patients receiving nucleos(t)ide analog (NA) therapy. Fecal shotgun metagenomic sequencing was performed in 29 healthy controls (HC) and 101 NA-treated CHB patients without cirrhosis or hepatocellular carcinoma, including 30 HBeAg-po

A case of acute autoimmune hepatitis complicated by autoimmune neutropenia and autoimmune hemolytic anemia.

An 84-year-old woman was admitted to her referring hospital after blood tests revealed severe liver dysfunction. During follow-up, her neutrophil count, which had been normal at admission, significantly decreased, prompting transfer to our hospital. Laboratory evaluation revealed profound neutropenia (22/μL) and persistent severe liver dysfunction. An antinuclear antibody titer of 1:320 and elevated immunoglobulin G levels raised concern for autoimmune hepatitis (AIH). Her anemia subsequently worsened, with hemoglobin decreasing to 6.9 g/dL. Haptoglobin was undetectable, and the direct Coombs

How do we monitor doxy-PEP impact and when should we consider revisiting guidelines?

The 2026 WHO guidelines recommending doxycycline post-exposure prophylaxis (doxy-PEP) represent the first global antibiotic prophylaxis recommendation for bacterial sexually transmitted infections (STIs), with provisions for antimicrobial resistance (AMR) monitoring.Doxy-PEP should be governed as an adaptive intervention subject to periodic, transparent review, but establishing decision rules faces challenges.Doxy-PEP is expected to select for resistance, including in Neisseria gonorrhoeae, Staphylococcus aureus, and the gut and skin microbiomes; the scope of clinical consequences is incomplet

Recommendations to Enhance Engagement in Perinatal Research With People Who Inject Drugs.

Women of reproductive age are significantly less likely than men to initiate hepatitis C virus (HCV) treatment, with the lowest uptake among women with children. Women who inject drugs who have children are a highly marginalised and stigmatised population who experience inequitable access to healthcare and poorer health outcomes. In the BUMP-C qualitative study-Bridging Unified Management of Pregnancy in hepatitis C care-we sought to investigate current HCV care pathways and perceptions of HCV management during pregnancy among women who inject drugs. During this study, several lessons emerged

Equitable outcomes for critically urgent living donor liver transplantation in acute alcoholic hepatitis.

BACKGROUNDS/AIMS: The justification for living donor liver transplantation (LDLT) for acute alcoholic hepatitis (AAH) remains debated, particularly in urgent settings. We compared outcomes of recipients undergoing urgent LDLT for AAH versus other causes and identified predictors of mortality. METHODS: We analyzed 317 adults in the Korean national registry who underwent urgent LDLT from 2017 to 2021 within seven days of emergency approval. AAH was defined using National Institute on Alcohol Abuse and Alcoholism criteria, excluding coexisting liver disease. Recipient, donor, and perioperative v

Protective mechanisms against murine coronavirus infection in individual mice and its transmission between mice.

Murine coronavirus, commonly known as mouse hepatitis virus (MHV), is a representative pathogenic virus in mice. MHV infection in mice is used as a model for viral infections and is utilized to analyze the mechanisms of protection and transmission. By establishing MHV-specific T cell clones and conducting adoptive transfer experiments in infected mice, it was demonstrated that T cells play a crucial role in the protection against MHV infection and that cytotoxic T lymphocytes induce apoptosis in infected cells and suppress viral production. Furthermore, studies using IFN-γ-deficient C57BL/6 mi
