Quantitative Analysis of Bronchiectasis in Alpha-1 Antitrypsin Deficiency.

INTRODUCTION: Alpha-1 antitrypsin deficiency (AATD) may be directly related to bronchiectasis. However, radiological diagnosis of mild bronchiectasis is subjective, and visual assessment of airway tapering is time-consuming. CT algorithms have been used to quantify airway tapering in other bronchiectasis aetiologies. We assessed whether this process can be used in AATD. METHODS: Historic CT scans from patients with severe AATD in the Birmingham registry were processed with YACTA software (Weinheimer et al., 2017), and data on bronchiectasis index (BEI), airway wall thickness (AWPi10) and emph

Systematic screening identifies USP20 as an accelerator of metabolic dysfunction-associated steatotic liver disease via driving de novo lipogenesis.

BACKGROUND AND AIMS: Excessive hepatic lipid production drives disrupted lipid homeostasis. Although deubiquitinases have emerged as key modulators of metabolic enzymes, their involvement in de novo lipogenesis (DNL) remains elusive. RESULTS: Hepatic USP20 is identified by the multi-screening system based on biological assays of the constructed 105 DUB-plasmids library, combined with transcript analysis of multi-species with MASLD. USP20 activates the DNL pathway through multi-omics integrated analysis and isotope-metabolic flux tracing. The most potential USP20-interacting protein, FASN is s

A systems pharmacology model for paracetamol overdose and treatment.

Paracetamol overdose is the leading cause of acute liver failure in many countries, including North America and the United Kingdom. While timely administration with the standard treatment is generally effective, the optimal treatment for scenarios such as "massive" overdose or modified release paracetamol overdose remains unknown. In this work we aim to develop and calibrate a systems pharmacology model for paracetamol-induced hepatotoxicity with the influence of administration of the antidote N-acetylcysteine (NAC). The system model integrates five components, including (1) input of paracetam

Computed Tomographic Findings of Primary Hepatic Extraskeletal Osteosarcoma With Peritoneal Metastasis via Periportal Space in a Dog.

A 15-year-old castrated male Shih Tzu presented with anorexia and lethargy. Computed tomography (CT) revealed a hepatic mass with extensive central mineralization and an ill-defined hypoattenuating peripheral zone. The periportal halo sign extending to the lesser omentum was compatible with neoplastic infiltration of the periportal space, suggesting a potential metastatic pathway via this anatomical route. Whole-body CT excluded primary skeletal involvement, and histopathology confirmed extraskeletal osteosarcoma of hepatic origin. This case illustrates the value of CT in characterizing minera

Bellidifolin alleviates doxorubicin-induced hepatotoxicity: a study integrating network pharmacology, molecular docking, and experimental validation.

This study aimed to investigate the protective role of bellidifolin (BEL) in doxorubicin (DOX)-induced liver injury and explore its underlying mechanisms. Through network pharmacology, potential targets and pathways of BEL against DOX-induced liver injury were identified. Molecular docking and molecular dynamics simulation were employed to further verify the binding activity of BEL to the core targets. Mice were randomly divided into three groups: control (CON), DOX (DOX) and BEL treated group (BEL). Liver pathological changes were evaluated through HE and Masson staining, serum ALT and AST we

Preventive effects of Faecalibacterium prausnitzii on chronic alcoholic liver injury and associated alterations in gut microbiota and host phenotypes in mice.

BACKGROUND AND OBJECTIVE: Alcoholic liver disease (ALD) is a complex disorder with limited effective interventions; gut dysbiosis plays a pivotal role in its pathogenesis. Faecalibacterium prausnitzii , a core commensal of the healthy human gut, exhibits potent anti-inflammatory properties and supports intestinal barrier integrity; however, its role in ALD remains unclear. This study aimed to investigate the preventive effects of Faecalibacterium prausnitzii against chronic alcoholic liver injury and to explore associated host and microbial alterations. METHODS: Chronic ALD was induced in C57

Clinical severity score-guided metagenomic analysis identifying gut microbial taxonomic and functional markers for severe hepatitis E progression.

Although gut microbiome alterations have been reported in hepatitis E (HE), the taxonomic and functional determinants of disease severity remain poorly defined. Here, we performed shotgun metagenomic sequencing of fecal samples from 125 individuals spanning acute non-icteric hepatitis (ANIH), acute icteric hepatitis (AIH), acute liver failure (ALF), and healthy controls. We constructed a surrogate clinical severity index from routine blood parameters and developed an integrative analytical framework that combined XGBoost-based taxonomic modeling with LASSO-driven functional feature selection t

PLGA-loaded puerarin attenuates arsenic-induced liver injury through modulation of ferroptosis, oxidative stress, inflammation, and apoptotic signaling.

BACKGROUND: Arsenic (AR) is an environmental and mining occupational toxicant with reported hepatotoxic effects. Ferroptosis has recently emerged as a critical mechanism involved in arsenic-induced hepatotoxicity. This study investigated the hepatoprotective effects of puerarin (PU) and its PLGA-loaded nanoparticle formulation (PU-PLGA) against arsenic-induced liver injury in rats, with particular emphasis on ferroptosis pathways modulation. METHODS: Sixty adult male Sprague-Dawley rats were randomly assigned into six groups: control, PU, PU-PLGA, arsenic (AR), AR + PU, and AR + PU-PLGA. Suba

Construction of a self-amplifying mRNA-LNP complex for phenylalanine hydroxylase and its efficacy evaluation in gene therapy for phenylketonuria.

Phenylketonuria (PKU) is an autosomal recessive disorder caused by deficient phenylalanine hydroxylase (PAH) activity, with current pharmacotherapies showing limited response rates and age-dependent restrictions. We compared two liver-targeted lipid nanoparticle formulations delivering PAH-encoding mRNA-a self-amplifying construct (saRNA) and a conventional non-replicating counterpart (nr-mRNA)-in Pah-knockout mice. Both constructs reduced circulating phenylalanine (Phe) after a single intravenous injection. The saRNA formulation achieved comparable efficacy at one-sixth the nr-mRNA dose and m

Microbiome alternative treatments for hepatic encephalopathy: Reassessment of the potential use of lactose in lactase non-persistent cirrhosis patients.

BACKGROUND: Hepatic encephalopathy is secondary to liver failure and is prevalent in 20%-40% of cirrhosis patients. The cause involves ammonia toxicity, gut-brain interactions, and inflammation usually involving the microbiome. The aim was to review succinct management of microbiome disturbances. The purpose includes an argument for further research into the possible selective benefit of lactose and dairy products in managing chronic hepatic encephalopathy in lactose maldigesters. METHODS: Articles from 1970 to June 2025 were sought on PubMed and Google Scholar, as well as individual articles

Multi-target modulation of traditional Chinese medicine based on the liver-gut axis: a new therapeutic strategy for NAFLD.

As one of the most common liver diseases, non-alcoholic fatty liver disease (NAFLD) has attracted widespread international attention given its rising incidence. The current mainstay of treatment for NAFLD is controlled diet with controlled exercise. Significantly, the development of NAFLD is proposed to be related to the liver-gut axis. The gut barrier function may be disrupted in the case of gut microbial dysbiosis, which may increase the entry of bacteria, lipopolysaccharides and endotoxins into the liver, causing an inflammatory response, increasing the load on the liver, and triggering lip

Analytical Identification of Novel Furan Metabolites in Human Hepatocytes and Urine: Implications for Liver and Respiratory Disease Research.

The gas phase of tobacco smoke has been implicated as a major cause of cancer and chronic obstructive pulmonary disease. Furan, a component of the gas phase, is highly toxic and carcinogenic in rodents; however, its toxicity in humans has not been well investigated. Furan's toxicity results from cytochrome P450 2E1-catalyzed oxidation to cis-2-butene-1,4-dial (BDA). The structure of furan metabolites demonstrates that BDA reacts with glutathione (GSH) to form a reactive GSH conjugate, 2-(S-glutathionyl)succinaldehyde (GSH-BDA), which reacts with protein lysine groups as well as other cellular

Adverse childhood experiences and chronic liver diseases: Dose-response, type-specific associations, and mediation analyses.

BACKGROUND: Adverse childhood experiences (ACEs) are linked to long-term health risks, yet their association with chronic liver diseases (CLDs) and mediating roles of mental health, lifestyles, and socioeconomic status remain undefined. We aimed to investigate these associations. METHODS: This prospective cohort study included 107,363 UK Biobank participants free from CLDs at baseline. CLD diagnoses were ascertained through linkage to multiple sources. The primary outcomes were any CLD and metabolic dysfunction-associated steatotic liver disease (MASLD). Physical and emotional neglect, sexual

Comprehensive Overview of Gut Microbiota Alterations in Obesity and Related Chronic Diseases: A Narrative Review.

BACKGROUND: Metabolic disorders, including obesity, type 2 diabetes (T2D), metabolic syndrome, Non-Alcoholic Fatty Liver Disease (NAFLD), and Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD), represent major global health challenges. Increasing evidence suggests that gut microbiota dysbiosis contributes to the development and progression of these disorders through mechanisms involving inflammation, insulin resistance, altered energy metabolism, and impaired intestinal barrier function. This review aimed to evaluate recent clinical evidence regarding the effects of gut microbiot

[Non-cirrhotic portal hypertension complicated by portal biliopathy].

The earliest references to association between mechanical jaundice and non-cirrhotic portal hypertension (NCPH) are by Fraser et al. (1944) and Gibson et al. (1965). The term "portal hypertension biliopathy" was proposed by Dhiman et al. (1999). The incidence of asymptomatic form of portal hypertension biliopathy (PHB) in patients with NCPH ranges from 81% to 100% that is significantly higher compared to patients with hepatic block (0-33%). Symptomatic form of PHB occurs only in 5-33% of patients. Any intervention on biliary tract in patients with PHB may result in significant blood loss due t
