MASLD: Spatial Mechanisms and New Therapeutics.

Metabolic dysfunction-associated steatotic liver disease (MASLD) encompasses a spectrum of liver injury ranging from simple steatosis to metabolic dysfunction-associated steatohepatitis (MASH), a progressive inflammatory state associated with hepatocyte injury, fibrosis, cirrhosis, and hepatocellular carcinoma. Increasing evidence suggests that progression to MASH is not spatially uniform across the liver. Instead, it reflects region-specific metabolic stress, inflammatory signaling, and fibrogenic remodeling along the porto-central axis of the hepatic lobule. Recent advances in spatial transc

Polystyrene microplastics-induced gut-liver axis injury in mice: Protective effect and mechanisms of sodium humate.

Microplastics (MPs), as widespread and increasingly prevalent environmental pollutants, pose a persistent threat to aquatic organisms and mammals, including humans. Sodium humate (HNa), a naturally derived humate salt that is widely available, low-cost, and has a favorable safety profile, exhibits multiple biological activities, including antimicrobial, anti-inflammatory, and antioxidant properties. This study was designed to evaluate whether HNa could protect against intestinal and hepatic injury caused by polystyrene microplastics (PS-MPs) exposure in mice and to clarify the involvement of t

Global Prevalence, Incidence, and Outcomes of Coexisting MASLD and Chronic Kidney Disease: A Meta-Analysis.

BACKGROUND & AIMS: Metabolic dysfunction-associated steatotic liver disease (MASLD) is highly prevalent and increasingly recognized as a multisystem cardiometabolic disorder. The global burden, incidence and prognostic implications of coexisting chronic kidney disease (CKD) in MASLD remain uncertain. METHODS: In this meta-analysis, we searched MEDLINE and EMBASE from inception through September 25, 2025, for observational studies reporting CKD prevalence and/or incidence among adults (≥ 18 years) with MASLD. The primary outcomes were pooled CKD prevalence, estimated using random-effects model

GGT-triggered self-immolative dexamethasone-oligopeptide prodrug nanoparticles ameliorate hepatic fibrosis via dual anti-inflammatory/immunomodulatory pathways with minimized systemic toxicity.

Liver fibrosis remains a critical unmet medical challenge, where glucocorticoids' therapeutic potential is constrained by systemic toxicity. We innovatively address this through an amphiphilic prodrug (DEI) engineered by Michael addition-conjugating dexamethasone with I-C-F-6, which self-assembles into γ-glutamyltranspeptidase (GGT)-responsive nanoparticles for liver-targeted delivery. This nanoplatform achieves spatial precision through enzyme-triggered drug self-immolative release exclusively in fibrotic livers, minimizes systemic toxicity, and modulates fibrosis pathogenesis via dual pathwa

Competing risks of treatment discontinuation of atezolizumab plus bevacizumab in unresectable hepatocellular carcinoma.

BACKGROUND & AIMS: Atezolizumab plus bevacizumab (Atezo/Bev) is considered standard first-line therapy for unresectable hepatocellular carcinoma (HCC), yet reasons for treatment discontinuation vary, and conventional survival endpoints do not capture the time-varying probability of each cause. This multicentre retrospective study aimed to quantify the cause-specific discontinuation and its predictors. METHODS: We analysed 685 consecutive patients treated with Atezo/Bev (10 excluded). Time-to-discontinuation was defined from treatment initiation to cessation and right-censored at last follow-u

Senescence-Related SOCS1, SOCS2, and GADD45G Identify an Immune-Associated Molecular Signature in Metabolic Dysfunction-Associated Steatotic Liver Disease.

Senescence-related molecular nodes linking hepatic stress with immune dysregulation in metabolic dysfunction-associated steatotic liver disease (MASLD) remain poorly defined. This study aimed to identify and experimentally validate a senescence-related gene (SRG) signature associated with immune dysregulation in MASLD. Four transcriptomic datasets (127 controls, 127 MASLD) were integrated as the discovery cohort. Candidate SRGs were identified through differential expression analysis, weighted gene co-expression network analysis, and three machine learning algorithms. Immune infiltration was a

The mitochondrial protein Timm13 promotes liver fibrosis by activating TGFβ signaling through Hsp90aa1.

Liver fibrosis is a pathological consequence of chronic liver injury that is reversible but can potentially progress to serious complications. Currently, due to the incomplete elucidation of its key molecular driving mechanisms, effective targeted therapeutic approaches remain lacking in clinical practice. The mitochondrial inner membrane transporter Timm13 has been implicated in cellular stress and disease, yet its role in liver fibrosis remains unclear. This study found that Timm13 is significantly upregulated in human fibrotic liver tissue. Functional studies demonstrated that Timm13 overex

Noncirrhotic Portopulmonary Hypertension Due to Hepatoportal Sclerosis in Adams-Oliver Syndrome.

A 13-year-old girl presented for evaluation of pulmonary hypertension after symptoms of dyspnea and exercise intolerance. Full evaluation was negative except for abdominal ultrasonography with splenomegaly and esophageal varices suggestive of portal hypertension. Cardiac and hepatic vein catheterization confirmed portopulmonary hypertension. Liver biopsy demonstrated sinusoidal dilatation, nodularity, and minimal fibrosis, which was interpreted as possible nodular regenerative hyperplasia but not cirrhosis. Radiographic imaging, including computed tomography venography, demonstrated an elongat

The Scavenger Function of LSECs, a Regulator of Liver Diseases.

Liver sinusoidal endothelial cells are highly specialized endothelial cells that contribute to liver homeostasis through their remarkable scavenger function. Through a broad repertoire of scavenger receptors, including mannose receptor, scavenger receptor H, fragment crystallizable gamma receptor IIb, LSECs efficiently clear waste and toxins, altering drug distribution and metabolism. However, this scavenger function can be disrupted by aging, toxins, and transcription factors, thereby promoting the accumulation of harmful substances within the hepatic microenvironment. Impaired LSEC scavenger

Obesity history exacerbates liver macrophage-mediated fibrosis via IGFBP7.

Weight regain is a frequent outcome after lifestyle-induced weight loss and is closely linked to the progression of metabolic dysfunction-associated steatotic liver disease (MASLD). However, the mechanisms by which prior obesity predisposes to metabolic dysfunction-associated steatohepatitis (MASH) development remain unclear. This study investigates how obesity history influences hepatic steatosis and fibrosis during weight regain. A diet-switch model was used to establish obesity, weight loss (WL), weight regain (RCHFD), and short-term obesity (STHFD) conditions, enabling comparisons between

Clinically Diagnosed Decompensated Chronic Liver Disease With Suspected Portal Hypertension in a One-Year-Old Infant: Diagnostic Challenges in a Resource-Limited Setting.

Chronic liver disease in infancy is uncommon, diagnostically demanding, and rarely reported from sub-Saharan Africa, where advanced hepatologic evaluation is seldom accessible. We describe a one-year-old boy from rural Somaliland who presented with a two-week history of progressive abdominal distension, followed by jaundice, hematemesis and bloody diarrhea. Examination showed wasting, deep scleral icterus, tense ascites, caput medusae and splenomegaly. Investigations demonstrated severe anemia (hemoglobin 3.8 g/dL), thrombocytopenia (130 × 109/L), conjugated hyperbilirubinemia (total 2.45 mg/d

Protective Effects of Allicin and Garlic-Derived Interventions Against Metabolic Dysfunction-Associated Steatotic Liver Disease: Current Evidence and Mechanistic Insights.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most prevalent chronic liver disease worldwide and poses a significant public health challenge. Allicin, a major bioactive organosulfur compound derived from garlic, has emerged as a promising candidate for MASLD prevention and management due to its antioxidant, anti-inflammatory, and metabolic regulatory properties. Despite its potential, evidence regarding allicin's efficacy on MASLD remains limited and heterogeneous because existing studies have evaluated different garlic-derived interventions across diverse experimenta

Insulin Resistance: Current State of Knowledge and Clinical Implications-Toward a Better Diagnostic Framework and the Question of Its Disease Status.

Insulin resistance (IR) represents one of the most pressing problems in contemporary public health. Its estimated global prevalence ranges from approximately 15.5% to over 61%, depending on the population studied, the diagnostic criteria applied, and the method used for its assessment. Despite the scale of the problem, IR remains underrecognized and lacks formal definition as a distinct disease entity, even as a growing number of clinicians and researchers worldwide describe it as such. Its asymptomatic or mildly symptomatic course allows it to remain undetected for years, during which it make

Voriconazole Dosing and Therapeutic Drug Monitoring in Patients Before and After Liver Transplantation.

BACKGROUND: Voriconazole is a triazole antifungal, primarily metabolized by the liver, and poses a dosing challenge in patients with liver impairment. Although the manufacturer recommends a maintenance dose reduction for mild-to-moderate hepatic dysfunction, no standardized recommendations exist for severe liver impairment. Prior evidence suggests a need for significant dose reduction in the setting of severe liver failure; however, there is limited data on empirical dosing in patients undergoing orthotopic liver transplant (OLT) evaluation and surgery. METHODS: This is a single-center, retro

Fondaparinux attenuates methotrexate-induced hepatotoxicity by regulating coagulation, endothelial dysfunction, and inflammatory signaling via the TLR4/NLRP3 and NF-κB/IL-1β/MCP-1 pathways.

This study evaluates the protective potential of Fondaparinux (Fond), a selective antithrombin-mediated Factor Xa inhibitor, in methotrexate-induced hepatotoxicity. The work explores its ability to correct coagulation imbalance, improve endothelial function, and attenuate oxidative and inflammatory cascades (TLR4/NLRP3, NF-κB p65/IL-1β/MCP-1). Animals were allocated into 4 groups. A control group was given distilled water via the intraperitoneal route (i.p.); an MTX group was given a single intraperitoneal injection of MTX (20 mg/kg) on the seventh experimental day; and two groups received pri

Modulatory Effects of Piper sarmentosum Roxb. Aqueous Extract on Gut Microbiota and Obesity in a High-Fat Diet Rat Model.

Background/Objectives: Obesity and its metabolic comorbidities, including dyslipidemia and non-alcoholic fatty liver disease (NAFLD), are a growing global health burden, and current pharmacotherapies have limited efficacy and notable adverse effects. The gut microbiome has emerged as a key regulator of host metabolism, and its dysregulation is implicated in obesity. This study investigated the therapeutic potential of Piper sarmentosum Roxb. aqueous extract (PSRW) on metabolic dysfunction and gut microbiota composition in a high-fat diet (HFD)-induced obese rat model. Methods: HFD-induced obes

Deletion of phosphatidylethanolamine methyltransferase promotes the spontaneous development of hepatic steatosis, inflammation, and fibrosis in young mice.

Phosphatidylethanolamine N-methyltransferase (PEMT) catalyzes the transfer of methyl groups to phosphatidylethanolamine to generate phosphatidylcholine (PC). PC produced de novo through this pathway is preferentially used for very-low-density lipoprotein assembly and is essential for its normal hepatic secretion as well as for bile acid detoxification. While human PEMT loss-of-function polymorphisms are linked to increased metabolic dysfunction-associated steatotic liver disease risk, the enzyme's role as a primary driver of progressive liver disease remains underexplored. We utilized PEMT kno

Ultrasound assessment of diaphragmatic structure and function before and after orthotopic liver transplantation in MNGIE.

BACKGROUND: Mitochondrial Neurogastrointestinal Encephalomyopathy (MNGIE) is an ultra-rare autosomal recessive disorder caused by thymidine phosphorylase (TP) deficiency, leading to systemic nucleoside accumulation and multisystemic dysfunction. While respiratory failure is not a primary feature, severe cachexia, sarcopenia, and respiratory muscle weakness may contribute to morbidity, particularly during acute illnesses. Orthotopic liver transplantation (OLT) has emerged as a disease-modifying therapy, restoring TP activity and normalizing circulating nucleosides, though its impact on respirat
