This article systematically reviews the management of key complications in hematopoietic stem cell transplantation (HSCT), including infections, graft-versus-host disease (GVHD), and hepatic sinusoidal obstruction syndrome (VOD/SOS). It highlights the importance of optimizing conditioning regimens to reduce infection risk and discusses the role of novel antiviral agents like letermovir in transforming infection control. For GVHD, the pathogenesis involving effector and regulatory T-cell imbalances is analyzed, together with prevention strategies such as post-transplant cyclophosphamide with an

Late-onset sinusoidal obstruction syndrome (SOS) after allogeneic hematopoietic cell transplantation remains difficult to diagnose and treat. We retrospectively analyzed 14 umbilical cord blood transplant recipients who developed late-onset SOS and received defibrotide at Toranomon Hospital between 2019 and 2021. All patients presented with very severe SOS accompanied by multiorgan failure. The cumulative incidence of complete remission was 35.7% at one year after initiation of defibrotide therapy. Overall survival was 57.1% at day 100 and 35.7% at two years after SOS diagnosis. Patients with

Radiation-induced liver disease (RILD) poses a major clinical challenge in radiotherapy, transplantation preconditioning, or radiation accidents, yet its pathogenesis is poorly understood due to limited animal models. Here, we establish a translational pig model recapitulating human RILD pathology within 4 weeks post-40 Gy irradiation, featuring veno-occlusive disease (VOD) and centrilobular necrosis. Single-cell atlas analyses identify ferroptosis as a key driver of hepatocyte death during RILD initiation. Ferroptosis inhibition with liproxstatin-1 (Lip-1) not only prevents RILD progression b

Sinusoidal obstruction syndrome/veno-occlusive disease (SOS/VOD) after allogeneic hematopoietic transplant (HCT) is associated with high mortality. We evaluated a cohort from 2 Canadian transplant centers to identify predictors of death among patients treated with defibrotide. We conducted a retrospective study of 40 patients with SOS/VOD treated with defibrotide at 2 Canadian centers between January 2016 and June 2023. The primary outcome was overall survival at D+100. Cox proportional hazards regression was used to explore factors associated with mortality. Median patient age was 55 years (i

A 57 -year -old woman with relapsed Philadelphia chromosome -negative B -cell acute lymphoblastic leukemia developed diffuse non -blanching erythematous skin lesions following allogeneic hematopoietic stem cell transplant. The lesions appeared concomitantly with hepatic sinusoidal obstruction syndrome and persisted despite engraftment and supportive therapy. Skin biopsy excluded graft -versus -host disease. Upon initiation of defibrotide for moderate hepatic sinusoidal obstruction syndrome, both hepatic and cutaneous findings resolved completely. This case underscores the systemic endothelial

Relapse of B-cell acute lymphoblastic leukemia (B-ALL) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) remains a major clinical challenge, and the use of inotuzumab ozogamicin (InO) as a bridge to second (2nd) HSCT is often constrained by concerns regarding sinusoidal obstruction syndrome/veno-occlusive disease (SOS/VOD). We retrospectively analyzed 25 patients with post-allo-HSCT relapsed or persistent MRD-positive B-ALL treated with InO. The primary objective was to assess SOS/VOD risk and safety of InO as a bridging strategy to 2nd allo-HSCT, with secondary endpoints in

Background and Methods: Sinusoidal obstruction syndrome/veno-occlusive disease (SOS/VOD) is a severe complication of allogeneic stem cell transplant (allo-SCT). Given the increased use of allo-SCT and variability of SOS/VOD incidence in published reports, cases of allo-SCT from two major transplant centers in Ontario, Canada (2019-2021), were reviewed to identify risk factors prognostic for SOS/VOD onset and to assess outcomes. Results: This study included 536 allo-SCT cases, with a mean age of 53.4 years [min-max: 17-76], including 322 male recipients and 214 female recipients. There were 17

BACKGROUND: Hepatic veno-occlusive disease refers to a toxic liver injury leading to obstruction and significant morbidity and mortality. Pyrrolizidine alkaloids are among the commonest causes of toxins. In the Tigray region of Ethiopia, this disease linked to toxic plant Ageratum conyzoides. Disruption interventions caused by the COVID-19 pandemic and the Tigray war, potentially leading to escalated reports of the disease. Therefore, this study aims to assess the burden and distribution of hepatic veno-occlusive disease in the northwestern zone of the Tigray region of Ethiopia.

INTRODUCTION: Sinusoidal obstruction syndrome (SOS) and early portal hypertension (ePHT) represent significant hepatic vascular complications following preoperative chemotherapy and liver resection for colorectal liver metastases (CRLM). This study aimed to identify predictors of SOS and ePHT and to develop a risk model for preoperative stratification.
METHODS: A retrospective analysis was conducted in 108 patients undergoing hepatectomy for CRLM after ≥ 4 cycles of neoadjuvant chemotherapy combined with anti-EGFR or anti-VEGF therapy. SOS severity was graded histologically, and ePHT was defi

We report a pooled analysis of real-world data of adult patients from two European registries of defibrotide use (DEFIFrance and EBMT Post-Authorization Safety Study [EBMT PASS]) to provide insights into the duration of defibrotide therapy to achieve resolution of veno-occlusive disease/sinusoidal obstruction syndrome (VOD/SOS) symptoms by severity. Overall, 275 defibrotide-treated adults with VOD/SOS post-hematopoietic cell transplantation (HCT) were included (severe VOD/SOS, n = 215; nonsevere VOD/SOS, n = 60). Median time from the start of defibrotide treatment to VOD/SOS resolution was 18.

Inotuzumab ozogamicin (InO) is a humanized, anti-CD22 antibody-drug conjugate with demonstrated use in the treatment of children with relapsed or refractory B-cell acute lymphoblastic leukemia (B-ALL). Sinusoidal obstruction syndrome (SOS), a life-threatening complication of hematopoietic stem cell transplant (HSCT), arises from injury to the sinusoidal endothelium and occurs at higher frequency and severity after treatment with InO. Defibrotide is widely used in the treatment of transplant-associated SOS; however, its role in prophylaxis is less clear. In this single-center retrospective anal

BACKGROUND: Hepatic veno-occlusive disease (VOD) is an endothelial injury seen after stem cell transplantation that can also occur with conventional chemotherapies. Morbidity can be significant, highlighting the need for effective supportive care.
CASE REPORT: We present a pediatric patient with Wilms tumor who developed VOD after dactinomycin and vincristine. Management included a molecular adsorbent recirculating system (MARS) for acute liver failure and irinotecan with vincristine (VI) as bridging chemotherapy before gradual reintroduction of dactinomycin.
CONCLUSION: The patient remains

Hepatic focal nodular hyperplasia (FNH)-like lesions mimicking metastasis have been reported as rare complications of cytotoxic chemotherapy, but they have not previously been described in association with CDK4/6 inhibitors alone. We present the case of a 32-year-old chemotherapy-naïve woman with de novo hormone receptor (HR)-positive/HER2-negative metastatic breast cancer who was treated with first-line ribociclib and letrozole. After four cycles, imaging revealed a complete response of her bone metastases alongside a newly developed 5.5-cm hepatic lesion suspicious for visceral progression.

Sinusoidal obstruction syndrome is a well-known hepatic vascular injury associated with oxaliplatin-based chemotherapy and can lead to non-cirrhotic portal hypertension. Although splenomegaly and thrombocytopenia are common clinical manifestations, clinically significant gastrointestinal varices may persist or newly develop after oxaliplatin cessation. The optimal therapeutic strategy for multiple gastrointestinal varices caused by oxaliplatin-induced sinusoidal obstruction syndrome remains unclear. Here, we report a case of a 71-year-old man who developed non-cirrhotic portal hypertension aft

Liver vascular diseases include portal vein thrombosis (with or without cirrhosis), portal-sinusoidal vascular disease, Budd-Chiari syndrome, hepatic sinusoidal obstruction syndrome, non-obstructive sinusoidal dilatation and peliosis hepatis, visceral artery aneurysms, and hepatic arteriovenous fistulas. Importantly, with the exception of portal vein thrombosis in cirrhosis, the others are rare conditions. A substantial body of evidence has been published on the diagnosis, pharmacological treatment, and interventional therapy of liver vascular disorders since the European Association for the S

Cytomegalovirus (CMV) infection is one of the most prevalent severe complications post hematopoietic stem cell transplantation (HSCT), which significantly increases the morbidity and mortality risks among transplant recipients. Despite the continuous advancement of prevention and treatment strategies for CMV infection, refractory CMV infection remains a major clinical challenge in diagnosis and treatment, and this issue is even more prominent in pediatric patients due to the limited therapeutic options available. This paper reports the diagnosis and treatment of two pediatric cases with refrac

BACKGROUND: Individuals with anorexia nervosa frequently feel ambivalent about treatment and weight restoration, and drop out and relapse rates in treatment are high. Increased insight into the function of the eating disorder is considered essential for achieving long-lasting, meaningful change. However, research investigating the functions of anorexia nervosa tends to focus on the role of the disease per se. Distinctions are rarely made across features. In particular, the subjective experience, understanding and sense making of the engagement in compulsive exercise in individuals with anorexi
