A Survey on Terminology and Diagnostic Features of a Subgroup of Non-cirrhotic Portal Hypertension Patients: A Report from Asia-Pacific Experts.

Background: There have been recent proposals to replace non-cirrhotic portal fibrosis (NCPF), idiopathic portal hypertension (IPH), idiopathic non-cirrhotic portal hypertension, nodular regenerative hyperplasia, etc. with an all-encompassing term "porto-sinusoidal vascular disorder." However, the Asia-Pacific Association for Study of the Liver recommended continued use of the term NCPF/IPH. Methods: We sent this questionnaire-based online survey to capture the clinical perspectives of 300 specialized experts in the Asia-Pacific region with complete anonymisation. Results: The survey (n = 242 r

Asymptomatic Portomesenteric Venous Thrombosis After Colorectal Cancer Surgery: Incidence, Natural History, and Implications for Management.

BACKGROUND: Portomesenteric venous thrombosis (PMVT) after colorectal cancer (CRC) surgery is most often reported in symptomatic patients, leaving its frequency and natural history across the asymptomatic spectrum poorly defined. When encountered incidentally, it frequently prompts anticoagulation, hypercoagulability evaluation, or delays in oncologic treatment, despite limited evidence that intervention alters its course. Using a cohort with routine postoperative imaging, we characterized the incidence, anatomic distribution, risk factors, and clinical course of asymptomatic PMVT. METHODS: W

Life-Threatening Aberrant Omental Variceal Bleeding with Interventional Treatment: A Case Report.

Ectopic varices resulting from portal hypertension can cause fatal bleeding. Intra-abdominal hemorrhage from omental varices is rare, and no previous reports have described a direct connection between the splenic and ovarian veins. We report a case of intra-abdominal hemorrhage caused by an omental varix in a 49-year-old women with liver cirrhosis. Because she was hemodynamically unstable, surgical treatment was not feasible; therefore, an interventional procedure was planned. The varices originated from the splenic vein and drained into the ovarian vein. Through percutaneous portal venous acc

Changes in the apoptotic index, along with gene and protein expression levels, in splenomegaly due to portal vein stenosis in rats.

BACKGROUND: The spleen is an important peripheral immune organ, and splenomegaly (SM) is a common complication of portal hypertension. However, the regulatory mechanisms underlying splenic apoptosis remain poorly understood. MATERIALS AND METHODS: A portal vein stenosis-induced SM model was established in Sprague-Dawley rats, using a sham-operated (SO) group as the control. Apoptotic changes in splenic tissue cells were evaluated using the terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay, immunohistochemistry staining, quantitative real-time polymerase chain reaction

Baveno VI criteria validation and development of a new non-invasive strategy for varices rule-out in patients with HCC (SAPHE-R: Stiffness, ALBI, Platelets, HEpatocellular carcinoma Rule).

BACKGROUND AND AIMS: Accuracy of Baveno criteria for avoiding endoscopy in patients with hepatocellular carcinoma (HCC) is uncertain, and systematic upper gastrointestinal endoscopy (UGE) may not be necessary. We evaluated Baveno VI criteria performance in this population and developed an alternative non-invasive rule to exclude esophagogastric varices of any grade, rather than high-risk varices (HRV). METHODS: Patients with BCLC 0-C HCC and compensated advanced chronic liver disease (cACLD) from Italian Liver Cancer (ITA.LI.CA) database, with available liver stiffness measurement (LSM), UGE,

Endoscopic ultrasound in hepatology: A European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review.

BACKGROUND: Endoscopic ultrasound (EUS)-guided techniques are increasingly applied in hepatology, including for tissue acquisition, vascular access, pressure measurement, and selected therapeutic interventions. However, adoption remains heterogeneous because of variability in technical approaches, devices, outcome definitions, operator expertise, and the quality of available evidence. METHODS: This European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review summarizes the current evidence on selected EUS-guided techniques in liver disease and portal hypertension, inc

Assessment and Reversibility of Hepatic Abnormalities in Heart Failure: Liver in Heart Failure.

Hepatic abnormalities in non-Fontan heart failure result from congestive hepatopathy and hypoperfusion-related ischemic injury, compounded by neurohormonal activation and systemic inflammation. Assessment of liver disease severity is challenging because standard liver biochemistries, serum fibrosis markers, severity score models, elastography, and imaging modalities have limited diagnostic accuracy in congestive hepatopathy, and hepatic fibrosis is often heterogeneous on histology in this population. Management focuses on optimization of cardiac function through medical therapy or mechanical c

Pharmacotherapy for MASLD: current evidence and challenges in clinical practice.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is characterized by a complex pathophysiology driven by intertwined metabolic and hepatic pathways, enabling the identification of multiple therapeutic targets and a rapidly expanding drug development pipeline. This review summarizes the current and emerging therapeutic landscape for MASLD and discusses key challenges for clinical implementation. The imminent availability of several new therapies is expected to pose important clinical challenges, particularly in accurately identifying patients most likely to benefit. Currently, r

Study protocol of a prospective, non-interventional cohort study evaluating the benefits of transjugular intrahepatic portosystemic shunt in patients with hepatocellular carcinoma receiving first-line atezolizumab plus bevacizumab (NISTIPS TRITICC-4, IKF092).

BACKGROUND: Hepatocellular carcinoma (HCC) is a leading cause of cancer-related mortality, with most patients diagnosed at an advanced stage. Atezolizumab plus bevacizumab is a current standard first-line therapy; however, outcomes are strongly influenced by underlying cirrhosis and clinically significant portal hypertension (CSPH). Complications such as ascites and variceal bleeding may limit treatment continuation, particularly as bevacizumab increases bleeding risk. Transjugular intrahepatic portosystemic shunt (TIPS) reduces portal pressure and may improve outcomes, but its role in patient

Effects of Aerobic Exercise on Autonomic and Endothelial Function and Splanchnic Hemodynamics in Compensated Cirrhosis: RCT Study.

BACKGROUND: Patients with cirrhosis often develop autonomic dysfunction (AD) and endothelial dysfunction (ED), which correlate with higher mortality. Aerobic exercise (AE) is a potential intervention, yet its effects on the autonomic nervous system (ANS) and hemodynamics remain underinvestigated in cirrhosis. METHODS: We conducted a parallel-group randomized controlled trial including 50 sedentary patients with compensated cirrhosis. Participants were randomized to a 16-week moderate-intensity AE plus nutritional consultation (AE group; n = 25) or nutritional consultation alone (control group

Biopsy-Proven Hepatic Extramedullary Hematopoiesis With Chronic Portal Vein Thrombosis in JAK2 V617F-Positive Prefibrotic Myelofibrosis: Contributors to Noncirrhotic Portal Hypertension.

Noncirrhotic portal hypertension warrants evaluation for vascular and infiltrative etiologies when hepatic fibrosis is absent. We describe a 48-year-old man with JAK2 V617F-positive prefibrotic primary myelofibrosis who developed chronic portal vein thrombosis with cavernous transformation and gastroesophageal varices despite preserved liver stiffness (4.2 kPa). Endoscopic ultrasound-guided liver biopsy demonstrated grade 2 hepatic extramedullary hematopoiesis (4 foci across 10 high-power fields) without fibrosis. Chronic portal vein thrombosis was the principal driver of portal hypertension.

Impact of body composition on clinical outcomes in patients with hepatocellular carcinoma: A systematic review and meta-analysis.

BACKGROUND: Abnormalities in body composition are common in patients with hepatocellular carcinoma (HCC); however, their impact across different treatment modalities remains poorly understood. We systematically reviewed the prognostic value of body composition in patients with HCC, stratified by curative, locoregional, and systemic therapies. METHODS: Following PRISMA 2020 guidelines, we searched PubMed, Embase, the Cochrane Library, and Web of Science (January 2015-March 2025) for studies reporting associations between CT/MRI or bioelectrical impedance analysis (BIA)-assessed body compositio

fiLiver and spleen elastography-based prediction models for clinically significant portal hypertension and high-risk esophageal varices: a systematic review of conventional and machine-learning approaches.

The presence of clinically significant portal hypertension (CSPH) and high-risk esophageal varices (HREV) are major predictors of decompensation and mortality in chronic liver disease. Liver and spleen elastography are validated noninvasive markers of portal pressure, and increasing numbers of studies have integrated elastography measurements with conventional statistical and machine-learning (ML) models to predict these outcomes. This systematic review identified, critically appraised and synthesized studies developing or validating prediction models incorporating liver and/or spleen elastogr

Endothelin-1 and Anticardiolipin Antibodies in Primary Biliary Cholangitis Patients with Portal Hypertension: A Pilot Retrospective Study.

Primary biliary cholangitis (PBC) is a chronic autoimmune, cholestatic liver disease leading to fibrosis, cirrhosis, and portal hypertension. Endothelin-1 (ET-1), a potent vasoconstrictor with profibrotic properties, has been implicated in the pathogenesis of liver fibrosis and portal hypertension. Anti-cardiolipin antibodies (aCL) may further contribute to disease progression by promoting thrombotic complications. Our aim was to evaluate serum ET-1 concentrations and anti-cardiolipin antibody levels in PBC patients and their association with portal hypertension. Serum samples from 80 patients
