Liver transplantation for adult presentations of MDR3 deficiency.

BACKGROUND: MDR3 deficiency is associated with end-stage liver disease including liver failure, portal hypertension and primary liver cancer, with increasing recognition of adult presentations. We report our single centre experience of liver transplantation (LT) for adult presentations of MDR3 deficiency. METHODS: A single centre retrospective study of patients who underwent LT for MDR3 deficiency from 2013-2024 and were first diagnosed above the age of 16 years. We identified demographic and clinical data from the electronic patient records. RESULTS: There were 7 patients with genetically c

Thinking outside the box: leveraging endoscopic ultrasound-guided portal pressure gradient for comprehensive risk stratification in liver transplantation candidates.

BACKGROUND AND AIMS: Coronary artery disease (CAD) is common in liver transplantation (LT) candidates and may require coronary artery bypass grafting (CABG). Risk stratification is challenging in cirrhosis because histology alone does not reliably predict clinically significant portal hypertension (CSPH). Endoscopic ultrasound-guided portal pressure gradient (EUS-PPG) provides minimally invasive, real-time hemodynamic assessment. The aim of this study was to evaluate the role of EUS-PPG in perioperative risk stratification among LT candidates requiring CABG. METHODS: We retrospectively review

Case Report: Early-onset portal hypertension revealing pediatric ADPKD with a truncating PKD1 variant.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is a multisystemic ciliopathy whose clinical spectrum extends into childhood. Although hepatic cysts are the most common extrarenal manifestation, early portal hypertension associated with congenital hepatic fibrosis (CHF)-like features is uncommon and likely underrecognized. CASE PRESENTATION: We report a 10-year-old girl with no known family history of ADPKD who presented at 9 years of age with upper gastrointestinal bleeding secondary to grade III esophageal varices. The diagnostic evaluation established non-cirrhotic portal

Serum Uric Acid and Disease Severity in Chronic Liver Disease: A Cross-Sectional Study of Its Association With Child-Pugh Class.

BACKGROUND: Chronic liver disease (CLD) is a major cause of morbidity and mortality worldwide, with India accounting for the highest absolute number of cirrhosis-related deaths globally. Serum uric acid (SUA), the end product of purine metabolism, is increasingly recognised as a marker of oxidative stress and metabolic derangement and has been reported to be elevated in CLD. The Child-Pugh score remains the most widely used bedside tool for assessing hepatic functional reserve. Data regarding the association of SUA with Child-Pugh class and CLD-related complications in Indian tertiary care set

Heart Transplantation in Adult Fontan Patients: New Insights from Fontan-Associated Liver Disease Beyond Traditional Predictors.

BACKGROUND: Fontan patients are considered high risk for heart transplantation (HT) owing to their complex anatomy, multiple prior chest entries, and universal Fontan-associated liver disease (FALD). The impact of FALD and other preoperative factors remains, however, uncertain. OBJECTIVES: We sought to identify factors associated with 90-day mortality after isolated HT in adults with Fontan circulation, with particular focus on the relationship between liver-related parameters and post-transplant outcomes. METHODS: A retrospective cohort study of all adult Fontan patients undergoing isolated

Gut microbiota and innate immune barrier dysfunction in hepatic portal venous gas: a hypothesis-generating review.

Hepatic portal venous gas (HPVG) is an uncommon radiographic sign whose prognosis is driven by bowel necrosis and shock rather than by intraportal gas. Immunological events along the gut-liver axis have not been synthesized for adult HPVG, and dedicated microbiome, metabolomic, gut-vascular barrier (GVB), and immune-profiling data are lacking. This hypothesis-generating review proposes gut dysbiosis as a putative contributor to mucosal and hepatic innate immune failure, inferred from cirrhosis, porto-sinusoidal vascular disease (PSVD), and intestinal ischemia/reperfusion. In those proxy states

Altered T cell immune profile and its prognostic implications in children with portal hypertension: A pilot study.

OBJECTIVES: Immune dysregulation in paediatric portal hypertension (PHT) is poorly understood. We aimed to characterise T cell phenotype and function and circulating immune mediators in children with PHT according to disease aetiology. METHODS: Forty-four children with PHT (27 chronic liver disease [CLD]-related and 17 portal vein thrombosis [PVT]-related) and 15 healthy controls were prospectively enrolled. Peripheral and portal blood samples were analysed using multiparameter flow cytometry and ELISA to assess T cell subsets, chemokine receptors, cytokines and soluble immune markers. Analys

Proximal Splenorenal Shunt in Non-cirrhotic Portal Hypertension: Our Experience.

Background Non-cirrhotic portal hypertension (NCPH) is characterized by an elevated portal pressure without liver disease and without obvious venous thrombosis. It is characterized by splenomegaly and gastroesophageal varices with intact liver histology. Extrahepatic portal vein obstruction (EHPVO) and non-cirrhotic portal fibrosis (NCPF) come under the umbrella of NCPH. EHPVO is further characterized by portal cavernoma, cholangiopathy, and growth retardation. Surgical shunts remain a definitive option for patients requiring frequent endoscopies. This study was done to evaluate the surgical o

Coil Embolization and Open Splenectomy for a Massive Splenic Vein Aneurysm: A Case Report.

BACKGROUND: Splenic vein aneurysms (SVAs) are a rare vascular anomaly of the portal system that are often asymptomatic and found incidentally on imaging. Intervention is reserved for aneurysms that are enlarging, thrombosed, symptomatic, or ruptured to avoid catastrophic complications. This can range from endovascular to open, complex procedures. Here, we present the case of a massive, symptomatic, partially thrombosed SVA that required a hybrid approach involving both minimally invasive and open surgery. CASE PRESENTATION: A 56-year-old woman presented to the emergency department for an infe

Cellular Crosstalk at the Gut-Liver Interface in Liver Cirrhosis: Epithelial Barrier Failure, Bacterial Translocation and Immune Activation as a Bidirectional Network.

The gut-liver axis is central to the systemic complications of liver cirrhosis, but it is usually described as a linear cascade in which barrier failure releases microbial products that activate a downstream immune response. This review proposes instead a four-compartment framework, in which the epithelial/barrier, innate immune, adaptive effector, and stromal-endothelial compartments-the last including the gut-vascular barrier-are treated as reciprocally interacting nodes, and pathological bacterial translocation is treated as both an input to and an output of the network rather than as its e

Acute colonic variceal bleeding treated with coil-assisted retrograde transvenous obliteration following atezolizumab-bevacizumab therapy for advanced hepatocellular carcinoma: A case report.

RATIONALE: Colonic variceal bleeding is a rare but potentially life-threatening manifestation of portal hypertension. Bevacizumab therapy is associated with an increased risk of variceal bleeding, but bleeding from ectopic varices is less well recognized. PATIENT CONCERNS: A 66-year-old man with alcohol-related cirrhosis and advanced hepatocellular carcinoma developed hematochezia on the day following the initiation of atezolizumab plus bevacizumab therapy. DIAGNOSES: Acute ascending colonic variceal bleeding in the setting of portal hypertension and portal vein thrombosis. INTERVENTIONS: C

Case Report: Pyrrolizidine alkaloid-induced hepatic sinusoidal obstruction syndrome misdiagnosed as ovarian malignancy.

BACKGROUND: Hepatic Sinusoidal Obstruction Syndrome (HSOS), formerly known as Hepatic Veno-Occlusive Disease (HVOD), is a rare and life-threatening liver condition. It is often caused by the ingestion of pyrrolizidine alkaloids (PAs), which are found in plants such as Gynura segetum (Tusanqi). Because its clinical presentation is non-specific, HSOS is frequently misdiagnosed. CASE PRESENTATION: A 72-year-old female with a 5-year history of type 2 diabetes presented with abdominal distension and anorexia for 2 weeks. Initial examinations revealed massive ascites and significantly elevated tumo

Long-term graft and patient survival of patients with portal vein stenosis after pediatric liver transplantation; results from the multicenter multinational PORTAL registry.

BACKGROUND AND AIMS: Pediatric liver transplantation (pLT) has become a standard treatment, but it is still burdened by complications such as portal vein stenosis (PVS). PVS can result in compromised graft function and portal hypertension if left untreated. We aimed to assess long-term patient and graft survival after PVS diagnosis after pLT. METHODS: Our study analyzed data from the Portal vein Obstruction Revascularisation Therapy After Liver transplantation (PORTAL) registry, which contains retrospective data from 21 centers, across 18 countries, over a 20-year period. Graft- and patient s

Clinically Significant Portal Hypertension in Hepatocellular Carcinoma: Non-Invasive Assessment Across the Continuum.

PURPOSE OF REVIEW: Clinically significant portal hypertension (CSPH) is a key determinant of surgical candidacy, locoregional therapy safety, and systemic treatment outcomes in hepatocellular carcinoma (HCC), yet its non-invasive assessment remains challenging because the Baveno criteria were derived in HCC-free cirrhosis. This review synthesises current evidence on non-invasive tests (NITs) for CSPH across the HCC care continuum, organised around three clinical scenarios: pre-operative assessment, on-treatment monitoring, and pre-immunotherapy evaluation. RECENT FINDINGS: Liver stiffness-bas

Lower circulating angiotensinogen levels across ACLD stages are associated with hepatic dysfunction and mortality.

INTRODUCTION: The liver produces angiotensinogen (AGT), the key upstream precursor of the renin-angiotensin system (RAS). We measured AGT levels in patients across distinct advanced chronic liver disease (ACLD) stages and investigated the impact of AGT levels on clinical outcomes. METHODS: We included clinically stable ACLD outpatients undergoing hepatic venous pressure gradient (HVPG) measurement between May 2018 and October 2023, as well as healthy controls (HCs) recruited in the prospective Vienna Cirrhosis Study (VICIS; NCT03267615). AGT was assessed by converting it to angiotensin I (Ang
