Evaluating periportal fibrosis in schistosomiasis mansoni using the ELF [Enhanced Liver Fibrosis] score: A pilot study.

BACKGROUND: Measuring the periportal fibrosis pattern (PPF) in schistosomiasis mansoni (SM) is essential for diagnosing portal hypertension. The Enhanced Liver Fibrosis (ELF) test can be used to quantify liver fibrosis, however, few studies have been done in SM. This study evaluated serum ELF value according to the PPF pattern established by the Niamey ultrasound classification (US) and history of upper gastrointestinal bleeding (UGIB) in patients with SM. METHODOLOGY: A cross-sectional, bidirectional analytical study was carried out with patients from an endemic area and from a reference out

Mapping the preclinical therapeutic landscape of cirrhotic portal hypertension: a systematic evidence map.

INTRODUCTION AND OBJECTIVES: Portal hypertension has a limited pharmacological armamentarium despite decades of preclinical therapeutic research. We mapped the experimental architecture of this literature, independently of reported treatment efficacy. MATERIALS AND METHODS: PubMed/MEDLINE was searched from inception to March 31, 2026. Two independent title-and-abstract screening procedures identified 833 records retained by at least one screen; 540 formed a concordant core retained by both. Records were classified using a prespecified taxonomy of therapeutic strategy, intervention modality, a

Native Spleen T1 Mapping for the Assessment of Hepatic Congestion in Children With Congenital Heart Disease.

BACKGROUND: Congestive hepatopathy in congenital heart disease (CHD) can lead to portal hypertension. Liver native T1 relaxation time (LT1) was reported to be associated with elevated central venous pressure and liver injury in CHD. This study investigated the utility of spleen native T1 relaxation time (ST1) in children with CHD, particularly those with Fontan circulation, and its association with hemodynamic parameters and biomarkers of liver injury. METHODS AND RESULTS: This study included 120 patients with CHD (biventricular repair, 39; bidirectional Glenn shunt, 31; and Fontan circulatio

From Resection to Transplantation: Rethinking Training and Certification for the Modern HPB Surgeon.

Hepatopancreatobiliary (HPB) surgery has evolved beyond the confines of traditional liver and pancreatic resection. Contemporary practice increasingly encompasses advanced vascular reconstruction, extended hepatectomy in compromised livers, transplantation for malignancy, and complex operations following multimodality therapy. These developments have fundamentally altered the technical and cognitive demands placed upon HPB surgeons, raising an important question: which training pathway best prepares surgeons for the future of the specialty? Dedicated HPB, surgical oncology, and integrated tran

Validation of Baveno VII Criteria using Liver and Splenic Elastography for Predicting Esophageal Varices Severity in Chronic Liver Disease: A Multiparametric, Observational Study.

BACKGROUND AND AIMS: Baveno VII criteria prioritize Liver Stiffness Measurement (LSM) for portal hypertension risk stratification, primarily using transient elastography. We aimed to evaluate two-dimensional-shear wave elastography (2D-SWE) cutoffs, assess the etiology-dependent utility of spleen stiffness (SSM), and introduce a novel Combined Hepato-splenic Stiffness Index (CHSSI). MATERIALS AND METHODS: This ambispective, observational study enrolled 100 chronic liver disease patients. Participants underwent 2D-SWE for LSM and SSM, alongside endoscopy as the reference standard. A novel mult

Portopulmonary hypertension: an integrated approach to diagnosis, risk stratification, and management.

Portopulmonary hypertension (PoPH) is a complex and underrecognized form of pulmonary arterial hypertension arising in the setting of portal hypertension, in which outcomes are driven by the interplay between pulmonary vascular disease and liver dysfunction. Despite advances in hemodynamic definitions and targeted therapies, PoPH remains associated with significant morbidity and mortality, largely due to delayed diagnosis, challenges in risk stratification, and the absence of disease-specific management strategies. Recent insights support a paradigm shift from a purely hemodynamic view toward

Emergency surgical management for complicated schistosomal massive splenomegaly with portomesenteric thrombosis and bowel ischemia: a case report.

INTRODUCTION: Schistosomal periportal fibrosis is a major cause of non-cirrhotic portal hypertension and massive splenomegaly. Rarely, superimposed portomesenteric venous thrombosis (PMVT) may precipitate bowel ischemia. Managing this combination in non-endemic settings presents significant diagnostic and therapeutic challenges. We report successful bowel salvage and definitive portal decompression during the same surgical course. CASE PRESENTATION: A 43-year-old man with schistosomal periportal fibrosis and massive splenomegaly presented with partial PMVT-associated bowel ischemia. Following

Anticoagulation in cirrhosis: Indications, drug selection, and disease-modifying potential.

Anticoagulation in cirrhosis has evolved from a perceived contraindication to an integral component of modern liver disease management. Recognition of the fragile rebalanced hemostatic state has challenged the traditional concept of cirrhosis as an exclusively hemorrhagic disorder and provided the biological rationale for anticoagulation in selected patients. Beyond preventing thrombus extension and promoting portal vein recanalization, accumulating experimental and clinical evidence suggests that anticoagulants may exert disease-modifying effects by attenuating intrahepatic coagulation, hepat

Large esophageal varices predict hepatocellular carcinoma in newly diagnosed cirrhosis: insights from machine learning and propensity score analysis.

INTRODUCTION AND OBJECTIVES: Hepatocellular carcinoma (HCC) is a major complication of chronic liver disease (CLD) requiring improved risk stratification. Esophageal varices (EVs), as manifestations of severe portal hypertension, may promote HCC development through vascular remodeling and hypoxia-related pathways. We aimed to evaluate whether variceal size independently predicts HCC risk and whether specific biomarker profiles can be identified. PATIENTS AND METHODS: In this 12-year prospective cohort study, 545 patients with newly diagnosed compensated advanced chronic liver disease (cACLD)

When Liver Disease Misleads the Diagnosis: Two Cases of TAFRO Syndrome With Porto-Sinusoidal Vascular Disease.

BACKGROUND: TAFRO syndrome is a rare inflammatory disorder whose hepatic manifestations may mimic decompensated cirrhosis. CASES: We report two women with TAFRO syndrome associated with portal-sinusoidal vascular disorder (PSVD), a rare cause of non-cirrhotic portal hypertension. In both cases, liver biopsy excluded cirrhosis and confirmed PSVD. RESULTS: Clinical outcomes were markedly different. One patient achieved sustained disease control with anti-IL-6 therapy and remained stable for more than 6 years, whereas the other developed rapidly progressive and fatal disease. CONCLUSION: PSVD

Hepatic manifestations of ciliopathies: genetic causes and clinical update.

UNLABELLED: Ciliopathies are genetically heterogeneous disorders of primary cilia that frequently involve the liver in childhood, producing a spectrum of hepatobiliary pathology. Primary cilia on cholangiocytes and hepatic progenitors are mechano-, chemo-, and osmo-sensors essential for remodelling of the ductal plate, the embryologic precursor of the liver's intrahepatic bile duct system. Disruption of the components of primary cilia (basal body, intraflagellar transport (IFT), transition-zone, trafficking, and endoplasmic reticulum (ER)/Golgi protein homeostasis components) leads to ductal p

An international multicenter analysis of the outcome of pediatric first variceal hemorrhage.

BACKGROUND AIMS: Management of portal hypertension in children including primary prophylaxis of variceal bleeding is controversial due to limited understanding of mortality risk after first variceal hemorrhage (1VH). This international multicenter study was designed to advance understanding of outcomes after pediatric 1VH. APPROACH RESULTS: Center-specific characteristics and approaches to portal hypertension were determined at 44 international sites. K-means clustered the centers based upon volume and complexity of approaches [cluster 1 high (n=5), cluster 2 low (n=21), cluster 3 intermediat

Endoscopy timing in managing acute esophagogastric variceal bleeding: a multicenter retrospective study.

BACKGROUND: Patients suffering from acute esophagogastric variceal bleeding (AEGVB) frequently face unfavorable prognoses. Although endoscopy is the preferred intervention for AEGVB, the optimal timing for its administration to enhance patient outcomes remains a subject of ongoing debate. METHODS: This study encompassed patients with cirrhosis who presented with AEGVB and received endoscopic treatment at three major tertiary hospitals in Shandong Province, China, from January 2017 to July 2024. Participants were stratified based on the time interval from hospital admission to endoscopic inter

Safety and efficacy of robotic repeat hepatectomy compared to laparoscopic and open repeat hepatectomy: a retrospective comparative study.

Repeat hepatectomies are challenging due to adhesions and altered anatomy. Robotic surgery, with enhanced dexterity and visualization, may expand the role of minimally invasive liver surgery (MILS) in this setting. The aim of this study was to explore the safety and efficacy of robotic repeat hepatectomy. We conducted a retrospective cohort study comparing repeat robotic (R-RLR), laparoscopic (L-RLR), and open liver resections (O-RLR) performed between January 2019 and December 2024. The primary outcome was postoperative morbidity. Secondary outcomes included conversion to laparotomy, use of P

A Giant Intra-Abdominal Plasmablastic Neoplasm Favoring Lymphoma Mimicking an Exacerbation of Severe Fistulizing Crohn's Disease: A Case Report.

BACKGROUND Plasmablastic lymphoma (PBL) is a rare, aggressive large B-cell lymphoma associated with immunosuppression. In Crohn's disease, it can mimic disease exacerbation or infection. We describe a rapidly enlarging intra-abdominal plasmablastic neoplasm in a medically complex patient, emphasizing diagnostic vigilance without inferring a causal association with biological therapy. CASE REPORT A 44-year-old woman with severe fistulizing Crohn's disease treated sequentially with adalimumab, infliximab, and ustekinumab developed progressive systemic deterioration despite partial intestinal sta

Portosystemic Anastomoses: A Narrative Review of Anatomy, Clinical Significance of Spontaneous Portosystemic Shunts, and Contemporary Therapeutic Approaches.

Portosystemic anastomoses represent natural or acquired vascular communications between the portal venous system and the systemic circulation, which under physiological conditions constitute vessels of minimal flow and no clinical significance, yet become dilated and develop in states of portal hypertension, predominantly in the context of liver cirrhosis. This narrative review aims to elucidate the complex and essentially dual role of these anastomoses, with particular focus on the spontaneous portosystemic shunts (SPSS) that develop in cirrhotic patients, their clinical significance, and the

Clinical and pathophysiological perspectives on haemostasis, inflammation, and thrombo-haemorrhagic risk in cirrhosis.

Haemostasis in cirrhosis is a dynamic process characterised by a fragile equilibrium. Haemostatic alterations become more pronounced during acute decompensation and acute-on-chronic liver failure, conditions driven by increasing systemic inflammation. When compounded by bacterial infection or acute kidney injury, additional alterations may emerge, predisposing to bleeding and thrombosis. Recent data indicate an interplay between haemostasis and inflammation in chronic liver disease, with implications for disease progression and thrombo-haemorrhagic complications. In this review, we discuss evo

Incidence and Risk Factors for Development of Portal Hypertension and Sinusoidal Liver Injury Associated with T-DM1 in HER2-Positive Breast Cancer.

PURPOSE: While an association between trastuzumab emtansine (T-DM1) and development of portal hypertension has been suggested, clinical studies on the incidence and risk factors remain limited. MATERIALS AND METHODS: HER2-positive breast cancer patients treated with T-DM1 as adjuvant or palliative therapy were retrospectively enrolled. The composite endpoint of portal hypertension and sinusoidal liver injury was defined as development of splenomegaly and at least one among gastroesophageal varices, spontaneous portosystemic shunt, or portal hypertensive ascites; or biopsy-confirmed sinusoidal
