Lycorine ameliorates MASH by targeting ACSL4 to regulate lipid metabolism and inhibit ferroptosis.

Metabolic dysfunction-associated steatohepatitis (MASH), a progressive stage of metabolic dysfunction-associated steatotic liver disease (MASLD), has emerged as one of the most prevalent chronic liver diseases worldwide, yet therapeutic options remain limited. In this study, we found that lycorine effectively alleviated MASH both in vitro and in vivo. RNA-seq analysis revealed that lycorine suppressed lipid uptake and mitigated ferroptosis in MASH models. Using drug affinity responsive target stability coupled with mass spectrometry (DARTS-MS), we identified acyl-CoA synthetase long-chain fami

Prevalence of adeno-associated virus type 3B neutralizing antibodies in cohorts of healthy donors and Wilson's disease adults.

Adeno-associated virus (AAV) is widely used for liver-directed gene therapy, with AAV serotype 3B (AAV3B) showing particularly high tropism for human hepatocytes. However, pre-existing neutralizing antibodies (NAbs) against AAV represent a major barrier to efficient gene transfer and patient eligibility. We have recently demonstrated that subjects with intermediate NAb levels (1:5-1:45), that would be excluded from clinical trials, can be converted in eligible patients by the administration of IgG-cleaving enzymes. It is thus crucial to assess not only the seroprevalence but also titer distrib

Spatial multi-omic atlas profiles early organogenesis of a Carnegie stage 12 human embryo.

Organogenesis marks one of the most dynamic periods of early human embryonic development. Although the morphological features of this period have been described, the underlying transcriptional and metabolic landscapes remain poorly understood owing to the lack of an integrated spatial strategy. We present spatial transcriptomic and metabolomic maps of a Carnegie stage 12 (CS12) human embryo, co-registered with embryonic anatomy to generate a unified spatial multi-omic framework. Unsupervised clustering of the spatial transcriptomic data identified 17 cell populations covering the major organ p

Brucella melitensis infection complicated by secondary hemophagocytic lymphohistiocytosis, meningitis, and multiorgan damage: a case report and literature review.

BACKGROUND: Brucellosis is one of the most widespread zoonotic diseases globally. Of the twelve known Brucella species that affect mammals, Brucella melitensis is the most significant cause of human infection. Others include B. abortus, B. suis, and B. canis, with transmission occurring via unpasteurized dairy, direct animal contact, or occupational exposure (veterinarians, farmers, and laboratory workers). Although hemophagocytic lymphohistiocytosis (HLH) and neurological complications are known sequelae of brucellosis, their simultaneous occurrence with multiorgan failure is rarely reported.

GlycA-gene interactions in cardiovascular-kidney-metabolic syndrome: evidence from genetic association.

OBJECTIVE: Cardiovascular-Kidney-metabolic (CKM) syndrome is a concept introduced by the American Heart Association, describing systemic diseases resulting from the pathophysiological interaction among obesity, stroke, type 2 diabetes, chronic kidney disease, and cardiovascular disease. Glycoprotein acetyls (GlycA) is a novel biomarker for systemic inflammation and cardiovascular risk. This study aims to uncover the shared genetic architecture between GlycA and CKM, identify common risk loci and key tissues, and clarify the mechanisms linking these conditions. METHODS: Using summary data from

Total and beverage-specific alcohol use at mid-life and all-cause and cause-specific mortality: a prospective cohort study.

BACKGROUND: While detrimental health impacts of excessive alcohol use are well-established, potential effects of low-to-moderate drinking remain controversial. We examined the associations between total and beverage-specific alcohol intake and mortality, including cardiovascular disease (CVD), cancer (e.g., 20 site-specific cancers), and other mortality. METHODS: We included 340,924 participants from UK Biobank (2006-2022). Alcohol use was categorized as: never or occasional alcohol consumption (≤ 20 g/week), low consumption (> 20 g/week to ≤ 20 g/d for men, > 20 g/week to ≤ 10 g/d for women)

Identification of a putative founder variant in SLC30A10 associated with hypermanganesemia with dystonia 1 in Iranian patients: a case series and literature review.

Hypermanganesemia with dystonia 1 (HMNDYT1) is an rare autosomal recessive condition resulting from pathogenic variants in the SLC30A10 gene. The presence of biallelic variants in this gene results in systemic accumulation of manganese, which leads to dystonia, polycythemia, and liver dysfunction. We conducted clinical and genetic evaluations of seven affected individuals from six Iranian consanguineous families, emphasizing the identification of potential founder mutations, and integrated this information with a total of 80 previously reported cases. Analysis of exome sequencing in all seven

Study protocol of a prospective, non-interventional cohort study evaluating the benefits of transjugular intrahepatic portosystemic shunt in patients with hepatocellular carcinoma receiving first-line atezolizumab plus bevacizumab (NISTIPS TRITICC-4, IKF092).

BACKGROUND: Hepatocellular carcinoma (HCC) is a leading cause of cancer-related mortality, with most patients diagnosed at an advanced stage. Atezolizumab plus bevacizumab is a current standard first-line therapy; however, outcomes are strongly influenced by underlying cirrhosis and clinically significant portal hypertension (CSPH). Complications such as ascites and variceal bleeding may limit treatment continuation, particularly as bevacizumab increases bleeding risk. Transjugular intrahepatic portosystemic shunt (TIPS) reduces portal pressure and may improve outcomes, but its role in patient

Examination of factors affecting adropin levels in healthy and obese children.

INTRODUCTION: There are very different reports regarding whether adropin levels are low, moderate, or high in childhood obesity and obesity-related comorbidities. However, data on the effects of age, sex, and puberty are insufficient. MATERIAL AND METHODS: We planned a case-control study consisting of 59 obese patients (female [F] = 35/male [M] = 24) and 40 healthy controls (F = 21/M = 19). Additionally, obese children were re-evaluated following a 12-week diet intervention. Serum glucose, insulin, lipid profiles, liver function tests, and adropin levels were examined. RESULTS: While age (12

Altered OXPHOS potential in ascites immune cells is linked to CD4 T-cell exhaustion and a higher risk of spontaneous bacterial peritonitis in cirrhosis.

BACKGROUND AND AIMS: Ascites marks advanced cirrhosis and increases the risk of complications including SBP. Systemic inflammation in liver disease is well characterized, but the immunometabolic properties of the peritoneal compartment remain undefined. This study evaluated the metabolic characteristics of ascites-derived immune cells. METHOD: Matched blood and ascites samples from patients with cirrhosis without SBP (n = 21) were analyzed. OXPHOS and glycolysis were assessed using the Seahorse XFe96, lactate concentrations were measured, and immune cell subsets were characterized by spectral

Association of Prophylactic Abdominal Drain Placement With Short-Term Postoperative Outcomes After Hepatectomy: A Nationwide Inpatient Database Study in Japan.

AIM: Although randomized trials have discouraged routine prophylactic abdominal drainage after uncomplicated hepatectomy, these drains are still widely used in clinical practice. This study evaluated the association between prophylactic abdominal drain placement and short-term outcomes after hepatic resection using a nationwide inpatient database. METHODS: We identified patients who underwent initial hepatic resection for liver tumors from the Japanese Diagnosis Procedure Combination database. Patients were classified into drain and no-drain groups according to whether a prophylactic abdomina

TSPO in systemic-ocular immunometabolic crosstalk: mechanisms and therapeutic implications.

The eye is increasingly recognized as a tissue shaped by systemic immune and metabolic states rather than an isolated immune-privileged organ. Translocator protein (TSPO), an 18-kDa outer mitochondrial membrane protein enriched in myeloid and glial cells, links mitochondrial homeostasis, redox signaling, lipid metabolism, and immune polarization. This review synthesizes evidence for TSPO involvement in systemic-ocular immunometabolic crosstalk and organizes it into a three-stage working model: peripheral immunometabolic priming, blood-retinal barrier vulnerability, and intraocular glial amplif

Anti-Tumor Necrosis Factor-Alpha Associated Kaposi Sarcoma in a Patient with Ulcerative Colitis: A Case Report and Literature Review.

Kaposi sarcoma (KS) is a rare vascular malignancy associated with human herpesvirus-8 (HHV-8). Tumor necrosis factor-alpha inhibitors (anti-TNF-α agents) are widely used in inflammatory bowel diseases such as ulcerative colitis (UC). While these biologics are effective, they carry a risk of opportunistic infections and, rarely, malignancies. We report a rare case of KS developing in a patient with UC following infliximab treatment. A 67-year-old man with a 3-year history of moderate to severe UC, maintained on azathioprine and mesalazine, initiated induction therapy with infliximab (four 100 m

Blood Neutrophil to Lymphocyte Ratio as a Marker of Inflammatory Bowel Disease Severity in Iranian Adults: a Cross-Sectional Survey.

INTRODUCTION: Given the increase in the prevalence of inflammatory bowel diseases (IBD) in developing countries like Iran, studies to find new markers for disease monitoring seem necessary. Thus, we aimed to evaluate the value of a novel non-invasive marker, the neutrophil-to-lymphocyte ratio (NLR), in assessing the severity and activity of IBD in adults. METHODS: We collected retrospective data from adult patients with a history of ulcerative colitis (UC) or Crohn's disease (CD) who attended the clinic, gathering information from one month prior to their enrollment. The data included demogra

The multifaceted role of mesenchymal stem cells in infectious diseases: antimicrobial properties, immunomodulation and tissue repair.

BACKGROUND: Mesenchymal stem cells (MSCs) have emerged as promising therapeutic agents for infectious diseases owing to their antimicrobial, immunomodulatory, and tissue regenerative properties. Beyond their well-established role in tissue repair, MSCs and their secretome directly influence pathogen clearance through the secretion of antimicrobial peptides, extracellular vesicles, iron-sequestration mechanisms, and modulation of both innate and adaptive immune responses. KNOWLEDGE GAP: Despite extensive preclinical investigations, MSC responses are highly pathogen-dependent, with outcomes tha
