Comprehensive insight into the anti-dyslipidemic and pancreatic lipase inhibition activities of Clematis flammula L. and Fraxinus angustifolia Vahl. extracts using two animal models.

Clematis flammula and Fraxinus angustifolia leaves are commonly used in Algeria to treat inflammatory diseases, with potential hypolipidemic effects that have not been investigated so far. The present study explored the in vitro, ex vivo, and in vivo hypolipidemic effects of ethanolic leaf extracts from both plants using two animal models (mice and rabbits) with distinct lipid metabolism. Dyslipidemia was induced by oral administration of Triton X-100, once daily for three consecutive days. Immediately after each Triton X-100 administration, animals received plant extracts (100 and 200 mg/kg b

Venous Thromboembolism in Acute Necrotizing Pancreatitis Is Associated With Worse Outcomes, With Subtype-Specific Patterns: A Nationwide Analysis, 2016-2022.

OBJECTIVES: Venous thromboembolism (VTE) complicates acute necrotizing pancreatitis (ANP), but outcomes across extra-splanchnic deep vein thrombosis (DVT), portal vein thrombosis (PVT), and splenic vein thrombosis (SVT) remain poorly defined. We evaluated national trends and in-hospital outcomes in ANP with versus without VTE and across VTE subtypes. METHODS: Using the National Inpatient Sample (2016-2022), adults hospitalized with principal ANP were identified. Secondary diagnosis codes captured VTE subtypes. Primary analyses compared mortality and major complications in ANP with versus with

Endoscopic ultrasound in hepatology: A European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review.

BACKGROUND: Endoscopic ultrasound (EUS)-guided techniques are increasingly applied in hepatology, including for tissue acquisition, vascular access, pressure measurement, and selected therapeutic interventions. However, adoption remains heterogeneous because of variability in technical approaches, devices, outcome definitions, operator expertise, and the quality of available evidence. METHODS: This European Society of Gastrointestinal Endoscopy (ESGE) Technical and Technology Review summarizes the current evidence on selected EUS-guided techniques in liver disease and portal hypertension, inc

Confluence Stones at the Junction of the Cystic Duct and an Aberrant Anterior Sectoral Bile Duct: A Case Report.

INTRODUCTION: Confluence stones usually arise when gallstones impacted in the cystic duct migrate toward the biliary confluence. Surgical treatment can be technically demanding because of chronic inflammation and distorted anatomy. Confluence stones at the junction between the cystic duct and an aberrant bile duct are extremely rare. We report a successfully treated case of confluence stones arising at the junction of the cystic duct and an aberrant anterior sectoral bile duct with a communicating accessory bile duct. CASE PRESENTATION: A 38-year-old woman presented with abdominal pain, and b

Decoding Atrial Fibrillation Heterogeneity: A Nationwide Phenome-Wide Association Study in England.

BACKGROUND: Age and sex influence atrial fibrillation (AF) prevalence and outcomes, yet comprehensive data on sex- and age-specific health care utilization in patients newly diagnosed with AF remain limited. OBJECTIVES: We aimed to address this gap using a phenome-wide association study (Phe-WAS) approach. METHODS: We conducted a retrospective cohort study using nationwide Clinical Practice Research Datalink data linked to Hospital Episodes Statistics. Adults diagnosed with AF between 1998 and 2016 (n = 199,433) were matched 1:1 by age and sex to non-AF control subjects. Primary and secondar

MAF Transcription Factors: Emerging Key Players in Cardiovascular Health and Disease.

Cardiovascular diseases are a leading cause of morbidity and mortality worldwide, and their complex pathophysiological mechanisms remain incompletely understood. The regulatory role of transcription factors provides a crucial avenue for investigating the mechanisms underlying these diseases. The MAF transcription factor family is a key member of the AP-1 superfamily, comprising two subfamilies: large MAFs (including MAFA, MAFB, MAF [c-MAF], and NRL) and small MAFs (including MAFK, MAFG, and MAFF). Different members exhibit distinct cellular expression patterns and functions within tissues: sma

The Clinical Significance of Hyperkinetic Biliary Dyskinesia on HIDA Scan: A Systematic Review and Meta-analysis.

BACKGROUND: Hyperkinetic biliary dyskinesia, defined as a gallbladder ejection fraction (EF) ≥80% on cholecystokinin (CCK)-stimulated hepatobiliary iminodiacetic acid scans in the setting of biliary-type pain, has been increasingly recognized as a potential functional gallbladder disorder. The clinical significance of this entity and the role of cholecystectomy remain uncertain. PATIENTS AND METHODS: We conducted a systematic review and meta-analysis of studies reporting outcomes in patients with CCK-hepatobiliary iminodiacetic acid EF ≥80% who underwent cholecystectomy due to non-acute bilia

Mechanistic insights into TLR2-mediated acute pancreatitis exacerbation via macrophage-ferroptotic acinar cell crosstalk.

Ferroptosis is closely linked to increased inflammatory responses and plays a vital role in the development of acute pancreatitis. We investigated how this process contributes to pancreatic injury in AP and examined the interactions between ferroptotic acinar cells and macrophages. Our study profiles the crosstalk mechanisms between ferroptotic acinar cells and macrophages. These findings, derived from human peripheral blood cohorts, were validated in vivo and in vitro. Multi-omics analyses revealed a reduction in pancreatic acinar cells owing to abnormal ferroptosis activation, as confirmed i

Nutritional-inflammatory indices and gallstone risk: complementary insights from hospital-based and community-based cohorts in China.

BACKGROUND: Gallstone is a common digestive disorder closely associated with inflammatory responses and nutritional metabolic disturbances. However, evidence regarding the association between composite nutritional-inflammatory indices and gallstone risk remains limited. This study aims to investigate the association of albumin/neutrophil to lymphocyte ratio (ANLR) and neutrophil percentage to albumin ratio (NPAR) with gallstone prevalence using two independent cohorts from China. METHODS: This cross-sectional study analyzed two cohorts: a hospital-based cohort (Zibo Central Hospital, 2014-202

Impact of Obesity on Chemotherapy Pharmacokinetics, Toxicity, and Survival Outcomes in Pediatric Acute Lymphoblastic Leukemia.

Obesity is increasingly prevalent among children and adolescents and may influence chemotherapy pharmacokinetics, treatment-related toxicity, and clinical outcomes in pediatric acute lymphoblastic leukemia (ALL). This narrative review evaluates current evidence regarding the effects of obesity on drug disposition, chemotherapy-associated toxicity, dosing considerations, and survival outcomes in pediatric ALL. Obesity alters body composition and may affect hepatic and renal physiology; however, its pharmacokinetic effects vary among therapeutic agents. Studies demonstrate associations between g

A gastric dieulafoy lesion presenting with life-threatening upper gastrointestinal hemorrhage during recovery from hypertriglyceridemic acute pancreatitis: a case report.

BACKGROUND: Dieulafoy lesions are an uncommon but potentially life-threatening cause of non-variceal upper gastrointestinal bleeding, caused by rupture of a large-caliber submucosal artery through a minute mucosal defect; diagnosis is difficult because bleeding is intermittent and the surrounding mucosa appears nearly normal. Hemorrhage from a gastric Dieulafoy lesion during recovery from hypertriglyceridemic acute pancreatitis (HTG-AP) has rarely been reported. CASE PRESENTATION: A man in his twenties with severe obesity presented with acute epigastric pain after alcohol and a high-fat meal.

Metabolic Syndrome Is Associated With Worsened Clinical Outcomes in Individuals With Necrotizing Pancreatitis.

GOALS: Evaluate the relationship between metabolic syndrome (MetS) and clinical outcomes in individuals with necrotizing pancreatitis (NP) and infected pancreatic necrosis (IPN). BACKGROUND: MetS affects a significant proportion of the global population and is associated with low-grade systemic inflammation and immune dysregulation. This has implications for several disease processes, but the impact of MetS on patients with NP is not well characterized. STUDY: A retrospective cohort study of patients admitted with NP at a single tertiary center was performed. MetS was defined as meeting any

A novel dual-epitope ELISA reveals heterogeneity of circulating lipoprotein lipase: distinguishing intact LPL from total LPL.

BACKGROUND: Conventional immunoassays measure total circulating lipoprotein lipase (LPL) but do not distinguish molecular species retaining both terminal regions. We developed a dual-epitope E LISA that measures LPL simultaneously recognized through defined N- and C-terminal epitopes, operationally termed "intact LPL." METHODS: The F1A/R23B ELISA was evaluated using recombinant full-length LPL, analytical validation, immunoblotting, reciprocal immunoprecipitation-immunoblotting, post-heparin kinetics, LPL activity measurements, and ex vivo tetrahydrolipstatin treatment. Developing a cps-genotyping scheme for the emerging zoonotic pathogen Streptococcus parasuis through genetic analysis of capsular polysaccharide synthesis gene loci.

BACKGROUND: Streptococcus parasuis is an emerging zoonotic pathogen responsible for pneumonia, meningitis, peritonitis, pleuritis, nephritis, cellulitis, cholecystitis, sepsis, and arthritis in humans. The continuous increase in reported human cases of S. parasuis infection indicates its growing threat to public health and highlights the urgent need to monitor its epidemiology. The lack of high-throughput typing tools has seriously impeded the epidemiological surveillance of S. parasuis. Serotyping based on antigenic differences in capsular polysaccharides (CPSs) synthesized by cps loci is wid

Pancreatic Neuroendocrine Microtumors: Etiology, Diagnostic Criteria, and Differential Diagnosis.

BACKGROUND: Pancreatic neuroendocrine microtumors represent small, well-differentiated neuroendocrine neoplasms (<5 mm) that are widely regarded as early lesions within the spectrum of pancreatic neuroendocrine tumorigenesis; however, their biological behavior remains a matter of an ongoing debate. SUMMARY: Although frequently associated with hereditary tumor syndromes such as multiple endocrine neoplasia type 1 and von Hippel-Lindau disease, pancreatic neuroendocrine microtumors also occur sporadically and are increasingly detected incidentally. In this review, we summarize their histopathol
