Management Strategies for Hepatotoxicity Associated With Elexacaftor/Tezacaftor/Ivacaftor: Results of a Provider Survey.

BACKGROUND: Elexacaftor/tezacaftor/ivacaftor (ETI) has significantly improved lung function and the clinical course of people with cystic fibrosis (PwCF) but also can cause elevations in liver function tests (LFTs). While the prescribing information (PI) provides some recommendations for holding ETI or hepatically adjusting doses based on certain criteria, clinicians are often faced with clinical scenarios that do not meet those criteria (e.g. PwCF with CF liver disease [CFLD, now termed CF hepatobiliary involvement (CFHBI)] that do not meet Child-Pugh Class B or C criteria). The purpose of th

The causal associations of serum uric acid levels and gout with the risk of acute pancreatitis: A bidirectional Mendelian randomization study.

This study investigates the causal links between serum uric acid (UA), gout, and the risk of acute pancreatitis (AP) using a bidirectional 2-sample Mendelian randomization analysis of genome-wide association studies data. The primary inverse variance weighted method indicated that genetically predicted higher serum UA levels are causally associated with an increased risk of AP (odds ratio [OR] = 1.194, 95% confidence interval [CI]: 1.020-1.399, P = .027). Conversely, the reverse Mendelian randomization analysis suggested a potential weak inverse association, with genetically predicted AP linke

Effectiveness and Safety of Enteroscopy-Assisted ERP-Guided Versus EUS-Guided Pancreatic Duct Drainage for Pancreaticojejunostomy Strictures: A Multicenter Observational Study.

OBJECTIVES: Enteroscopy-assisted endoscopic retrograde pancreatography-guided pancreatic duct drainage (eERP-PDD) and endoscopic ultrasound-guided pancreatic duct drainage (EUS-PDD) are minimally invasive alternatives to surgery for pancreaticojejunostomy stricture (PJS); however, comparative data remain limited. We compared the effectiveness and safety of these approaches and identified factors associated with technical failure. METHODS: This multicenter retrospective study included 88 patients (111 procedures) who underwent endoscopic intervention for PJS at 13 Japanese tertiary centers. We

Gastrointestinal and Pancreatic Manifestations of Cystic Fibrosis in the Era of Highly Effective Modulators.

This review synthesizes emerging evidence on gastrointestinal (GI) disorders, specifically luminal and pancreatic function in the era of triple cystic fibrosis transmembrane conductance regulator (CFTR) modulator therapy. Elexacaftor-tezacaftor-ivacaftor has been associated with mild improvements in GI symptoms and disease burden in patients' reported outcomes and has shown a significant increase in growth and body mass index reflecting improved nutritional status, even among individuals who remain pancreatic insufficient. Long-term data on triple CFTR modulators remain limited, highlighting t

Hypokalemic Hypochloremic Metabolic Alkalosis in Cystic Fibrosis: A Retrospective Observational Study with Clinical Characterization, CFTR Genetic Profiling, and a Comparative Genetic Analysis with Patients from a Brazilian Reference Center.

Cystic fibrosis is a genetic disorder caused by pathogenic variants in the Cystic Fibrosis Transmembrane Conductance Regulator (CFTR) gene and may be complicated by hypokalemic hypochloremic metabolic alkalosis (HHMA), resulting from excessive electrolyte loss through sweat. This retrospective study aimed to clinically characterize individuals with cystic fibrosis who developed HHMA, characterize their CFTR genetic profile and compare it with that of other individuals with cystic fibrosis followed at a Brazilian tertiary referral center, and contextualize these findings within the available na

Clinical Utility of the Apolipoprotein A2 Isoform Index as a Tumor Marker for Pancreatic Cancer.

BACKGROUND AND AIMS: We investigated the clinical utility of the apolipoprotein A2 isoform index (apoA2-i index), a novel tumor marker for pancreatic cancer, in clinical practice, particularly for CA19-9-negative pancreatic cancer. METHODS: Between May 2024 and July 2025, patients who underwent abdominal computed tomography (CT) for the differential diagnosis of pancreatic diseases had their CEA, CA19-9, and apoA2-i indexes measured. We evaluated the sensitivity and specificity for pancreatic cancer and analyzed the relationship between the apoA2-i index and pancreatic atrophy or main pancrea

Safety of Biologic Therapy for Inflammatory Bowel Disease in Liver Transplant Recipients.

Background/Objectives: Liver diseases and chronic inflammatory bowel diseases (IBD) are closely associated, particularly primary sclerosing cholangitis (PSC) and ulcerative colitis (UC). In severe cases of PSC, liver transplantation (LT) may be required, just as severe IBD may necessitate biologic therapy. Evidence on combining biologics with post-transplant immunosuppression is limited, particularly regarding hepatotoxicity, infections, and drug-drug interactions. We therefore assessed the safety of biologic therapy for IBD in LT recipients. Methods: In this retrospective, single-center analy

Pancreatic Colloid Carcinoma Arising From Intraductal Papillary Mucinous Neoplasm in the Setting of Gallbladder Agenesis, Ansa Pancreatica, and Santorinicele: Follow-Up of a Previously Reported Case.

This report details the long-term follow-up of a patient previously reported from our centre with recurrent acute pancreatitis and episodic jaundice in the setting of three rare anatomic variants: gallbladder agenesis, ansa pancreatica, and a santorinicele. The index report described progressive main pancreatic duct dilatation and endoscopic findings consistent with intraductal papillary mucinous neoplasm (IPMN) despite non-diagnostic cross-sectional imaging. We now present the subsequent clinical course, including elective pancreaticoduodenectomy, definitive histopathology, and surveillance o

In vitro alpha amylase and protein denaturation inhibitory activity of newer generation Schiff base Oxazolone derivatives: Synthesis, chemical characterization, molecular docking, MD simulation, DFT, ADMET, and bioactivity.

Chronic inflammation and diabetes mellitus are closely linked diseases and involved in metabolic dysfunction and disease pathogenesis. So, three new Oxazolone Schiff base derivatives (JS1A, JS2A and JS3A) were synthesized by a multi-step procedure, and then characterized by FTIR, 1H NMR, 13C NMR, mass spectrometry and elemental analysis. A series of synthetic compounds was screened by integrated computational and experimental studies to assess their antidiabetic and anti-inflammatory properties. Molecular docking studies with pancreatic α-amylase (4GQR) and cyclooxygenase-2 (6COX) revealed goo

Groove area involvement predicts post-ERCP pancreatitis after 8-mm fully-covered metal stent placement in resectable pancreatic cancer.

BACKGROUND AND STUDY AIMS: Fully covered self-expandable metal stents (FCSEMSs) provide durable preoperative biliary drainage in pancreatic cancer but may increase risk of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP). We evaluated whether groove involvement was an independent anatomical PEP risk factor and compared PEP incidence after 8-mm FCSEMS and plastic stent (PS) placement using propensity-score-based inverse probability of treatment weighting (IPTW). PATIENTS AND METHODS: Sixty-two consecutive patients with resectable or borderline resectable pancreatic

Role of Abdominal Contrast-Enhanced Computed Tomography in Diagnosing Gangrenous Gall Bladder Wall Perforation.

BACKGROUND: Complicated acute cholecystitis, particularly gangrenous cholecystitis and gallbladder perforation, represents a radiologic and surgical emergency because delayed recognition may lead to sepsis, abscess formation, and worse operative outcomes. Although ultrasonography is the first-line imaging modality, its ability to detect mural necrosis and perforation remains limited. This study evaluated the role of contrast-enhanced computed tomography (CECT) in identifying advanced gallbladder inflammation in patients with surgically confirmed complicated disease. METHODS: This retrospectiv

Adverse Events Associated with Incretin-Based Therapies: A Narrative Review on Mechanisms, Clinical Management, and Risk Mitigation.

BACKGROUND: Incretin-based therapies, including glucagon-like peptide-1 receptor agonists (GLP-1RAs), are established therapies for obesity and type 2 diabetes mellitus, providing sustained weight reduction, improved glycaemic control, cardiovascular and renal benefits. Despite these established benefits, safety concerns remain an important clinical issue because adverse events may affect treatment persistence, patient acceptability, and clinical decision-making. This review evaluates a number of safety profiles of GLP-1RAs using clinical, mechanistic, and pharmacovigilance evidence, with atte

Hypoxia-Induced KDM3A Binding With AP-1 Promotes Angiogenesis by Mediating ANGPT2 in the Development of Small Bowel Angiodysplasia.

BACKGROUND AND AIMS: Small bowel angiodysplasia (SBAD) is the leading cause of small bowel hemorrhage, with recurrent lesions that severely affect the lives of patients. Pathological angiogenesis induced by chronic hypoxia is a key component in the pathogenesis of SBAD, but the underlying epigenetic mechanisms remain unclear. METHODS: KDM3A expression was evaluated in SBAD specimens by immunohistochemistry. Functional studies in human umbilical vein endothelial cells (HUVECs) under hypoxia included migration, invasion, and tube formation assays. RNA sequencing and chromatin immunoprecipitatio

Gallbladder volvulus: Recognizing the rare 'beak' and 'whirl' signs.

Gallbladder volvulus is a rare surgical emergency caused by torsion of the gallbladder on its mesentery, leading to cystic duct and vascular obstruction with rapid progression to ischemia and gangrene, and it often mimics acute acalculous cholecystitis, making preoperative diagnosis challenging. A 80-year-old woman presented with acute right upper quadrant pain and vomiting. Contrast-enhanced CT demonstrated a distended, horizontally displaced gallbladder with gallstones and the "whirl" and "beak" signs consistent with cystic pedicle torsion. Urgent surgery confirmed ischemic volvulus; detorsi

Abscisic acid ameliorates inflammation-related diseases by inhibiting NLRP3 inflammasome activation.

The NLRP3 inflammasome has been implicated in the pathogenesis of various human diseases; however, no specific NLRP3-targeted therapies have yet been approved for routine clinical use. Abscisic acid (ABA), a well-characterized plant hormone, has recently demonstrated efficacy in alleviating both chronic and acute inflammation. Here, we show that ABA dose-dependently inhibits caspase-1 activation (CASP1 p20), interleukin-1β (IL-1β) secretion, and NLRP3 inflammasome assembly in macrophages. Mechanistically, we identified PDZD8 as a direct target of ABA. ABA-mediated activation of PDZD8 promotes

Cost-Effectiveness and Efficacy of Biodegradable Pancreatic Stents for Preventing Post-ERCP Pancreatitis: A Retrospective Multi-Center Study.

BACKGROUND: Post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) occurs in up to 10% of cases. Prophylactic pancreatic stenting reduces PEP risk; however, plastic stents frequently require an additional procedure for removal, increasing cost and patient burden. Biodegradable pancreatic stents negate the need for retrieval, but data on cost-effectiveness and clinical outcomes remain limited. METHODS: We conducted a two-center retrospective cohort study of adults undergoing ERCP with biodegradable pancreatic stent placement for PEP prophylaxis between January 2021 and A
