Clinical Outcomes of Patients With Inflammatory Bowel Disease After Liver Transplantation for Primary Sclerosing Cholangitis: A Systematic Review and Meta-analysis.

BACKGROUND: The clinical course of primary sclerosing cholangitis (PSC)-associated inflammatory bowel disease (IBD) after liver transplantation (LTx) varies significantly across studies. This meta-analysis aims to provide a comprehensive estimation of incidence rates (IR) for IBD-related outcomes and PSC recurrence. METHODS: Online databases (January 2000-June 2025) were searched for studies on PSC patients with or without IBD at LTx. Studies reporting posttransplant outcomes of interest were included. The pooled IR were calculated with the "meta" package (R Studio [V.4.3.2]) using a random e

A liquid fiducial marker for image-guided proton beam therapy of abdominal tumors.

PURPOSE: This study compared a liquid to solid fiducial markers regarding visibility, artifact characteristics on X-ray imaging and dose perturbation for image-guided proton therapy (IGPT) of abdominal tumors. Additionally, the radio-opaque composition of the liquid fiducial marker was optimized, and the chemical stability after proton beam irradiation was analyzed. MATERIAL AND METHODS: The fiducial markers inserted into an anthropomorphic abdominal phantom were exposed to X-ray, single-energy computed tomography (SECT), dual-energy CT (DECT) and cone-beam CT (CBCT) to determine contrast-to-

Host-pathogen pyrimidine metabolism drives cystic fibrosis lung disease.

Cystic fibrosis (CF), caused by CFTR mutations, is characterized by progressive pulmonary remodeling, particularly during bacterial infection. This pathological environment, exacerbated by altered epithelial growth, leads to thickened airway walls, inducing permanent narrowing and long-term breathing difficulties. However, what drives this adverse milieu remains unclear. Here, we identify activation of the de novo pathway of pyrimidine synthesis (DNPPS) as a central contributor of CF lung remodeling. In response to impaired mitochondrial bioenergetics, CFTR-mutant epithelial cells engage compe

Structure and functional divergence of the non-canonical BoNT-like toxin PG1 and PG2.

Botulinum neurotoxins (BoNTs) are potent bacterial toxins. Distinct BoNT-like toxins, including BoNT/X, BoNT/En, and PMP1, form a distinct lineage. Here we characterize two additional members of this group, PG-toxin 1 and 2 (PG1 and PG2). Their light-chain (LC) and heavy-chain (HC) domains are encoded by adjacent genes. PG1 and PG2 LCs cleave Drosophila SNAP-25 at distinct sites (N196-E197 and R191-K193, respectively); PG1 is more potent, while PG2 activity depends on pH and ionic strength. Both LCs enter Sf9 cells via clathrin-mediated endocytosis. PG1 LC is toxic to Drosophila and northern a

Dramatic regression of mass-forming gastric IgG4-related disease with inebilizumab: a case report.

BACKGROUND: Gastric immunoglobulin G4-related disease (IgG4-RD) is exceedingly rare, with fewer than 50 cases reported worldwide. Its mass-forming presentation frequently mimics malignancy, leading to high rates of unnecessary surgical resection. Inebilizumab, a CD19-directed monoclonal antibody and the first FDA-approved therapy for IgG4-RD, has not previously been described in gastric IgG4-RD. We report the first case of mass-forming gastric IgG4-RD treated with inebilizumab, achieving clinical, endoscopic, and histologic remission without surgical intervention. CASE PRESENTATION: A 52-year

ARID1A Stimulates Notch-TLL1 Signaling to Induce Cancer-Associated Fibroblast Activation and Intrahepatic Cholangiocarcinoma Development.

UNLABELLED: Intrahepatic cholangiocarcinoma (ICC) is a lethal hepatic malignancy characterized by a prominent desmoplastic stroma. In this study, we demonstrated that AT-rich interactive domain-containing protein 1A (ARID1A), a core component of the Switch/Sucrose (SWI/SNF) protein complex, is upregulated in ICC and plays an oncogenic role. Mechanistically, ARID1A stabilized NICD1 protein by inhibiting AMPK-dependent autophagy and lysosomal degradation, thereby sustaining Notch signaling. Tolloid-like 1 (TLL1), a metalloproteinase that promotes collagen maturation, was found to be a downstream

Toripalimab-induced isolated ACTH deficiency associated with gastroparesis: a case report.

Immune checkpoint inhibitors can cause isolated ACTH deficiency, a rare condition often misdiagnosed due to nonspecific symptoms. We report a cholangiocarcinoma patient on toripalimab and lenvatinib who experienced recurrent gastrointestinal symptom and precordial discomfort for 2 years, mistakenly attributed to drug toxicity or functional disorders. Following surgery, the patient developed refractory delayed gastric emptying, during which IAD was identified through negative imaging examinations, low ACTH and cortisol levels. Glucocorticoid replacement rapidly resolved symptoms. This case high

Clinical benefit of locoregional therapy for oligometastasis after surgery for pancreatic cancer: a single center retrospective study.

BACKGROUND: The prognosis of patients with pancreatic cancer (PC) is generally poor. However, favorable outcomes have recently been reported in selected patients with metastatic PC who receive multidisciplinary treatment. Oligometastasis (OM) represents a limited metastatic state and may be associated with better prognosis than multiple metastases (MM). The clinical role of locoregional therapy (LT) for postoperative OM remains unclear. METHODS: We included all consecutive patients with pancreatic malignancies who underwent macroscopically curative resection between 2013 and 2018. Patients wi

Impairment of preoperative activities of daily living predicts poor prognosis after surgery for perihilar cholangiocarcinoma.

PURPOSES: Perihilar cholangiocarcinoma (PHC) is a rare malignancy with rising incidence and mortality. With global population aging, more patients present with impaired activities of daily living (ADL), potentially influencing surgical outcomes. This study aimed to assess the association between preoperative Barthel Index (BI) scores, a measure of ADL, and survival outcomes after PHC surgery. METHODS: We retrospectively analyzed 88 patients who underwent PHC resection from 2005 to 2020. Preoperative ADL was evaluated using the BI (range 0-100), with scores ≤ 95 defined as impaired. Cox propor

Mechanisms of synergistic action between olaparib and KPT-330 in pancreatic cancer.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) is a highly aggressive and prognostically poor cancer driven by multifaceted mechanisms, notably immune evasion and drug resistance. The majority of PDAC patients present with unresectable locally advanced or metastatic disease, emphasizing a critical unmet need for effective therapies against advanced-stage pancreatic cancer. Despite the proven efficacy of the PARP inhibitor Olaparib against pancreatic cancer in patients carrying BRCA mutations, its practical use is limited by the development of acquired resistance. The clinical application

Determinants of Non-Receipt of Guideline-Concordant Treatment in Pancreatic Cancer: A Classification Tree Analysis.

BACKGROUND: Non-receipt of guideline-concordant treatment (GCT) in pancreatic ductal adenocarcinoma (PDAC) is associated with worse outcomes. Studies have identified associated factors, but less is known about how these factors co-occur within patient subgroups at the time of presentation to oncology care. METHODS: We conducted a retrospective cohort study of patients with PDAC treated at two academic centers. Candidate variables available at presentation were selected a priori across demographic, clinical, socioeconomic, and healthcare utilization domains. Classification and regression tree

Management and outcomes of patients undergoing high-risk oncologic pancreatic surgery: do patients presented at pancreatic multidisciplinary tumor board fare better?

BACKGROUND: Few studies have examined the impact of multidisciplinary tumor board (MDT) discussion on pancreatic cancer treatment and diagnosis. We aimed to determine whether these patients who are presented at MDT meetings have improved outcomes compared with those who are not presented. METHODS: A retrospective chart review was performed of patients diagnosed with pancreatic cancer at our tertiary referral center between 2019 and 2022. RESULTS: Of the 190 patients included in the study, 166 (87.4%) were presented at MDT. Patients presented were more likely to receive appropriate pretherapy

Long-term mortality trends of pancreatic cancer and its comorbidities with diabetes mellitus and metabolic disorders, 1999-2023.

BACKGROUND: The coexistence of Pancreatic cancer (PC) with diabetes mellitus (DM) and metabolic disorders (MDs) may further exacerbate the disease burden; however, the long-term mortality trends remain unclear. METHODS: We utilized the CDC WONDER database to extract mortality data for PC and its comorbidities with diabetes mellitus and metabolic disorders among the U.S. population aged 25 and older from 1999 to 2023. We calculated the Age-Adjusted Mortality Rate (AAMR) and employed Joinpoint regression analysis to assess the average annual percentage change (AAPC), stratifying the data by sex

The ongoing first-line treatment race for metastatic pancreatic ductal adenocarcinomas: will a tough disease lose its horror?

Pancreatic ductal adenocarcinomas (PDACs) are found to account for more than 5% of all cancer-related deaths worldwide. The 5-year survival rates are still low (<5% for in the metastatic setting). In patients with metastatic disease, combination chemotherapy regimens are currently standard of care. PDACs are K-RAS addicted (incidence >90%), however, RAS oncogenes have been regarded to be "undruggable." Several highly specific inhibitors have been developed with atebimetinib (MEK inhibitor), daraxonrasib (tri-complex pan-K-RAS inhibitor), zoldonrasib (tri-complex K-RAS G12D inhibitor), INCB1617

Five-Year Follow-Up of the Phase III ASCOT Trial of Adjuvant S-1 After Resection for Biliary Tract Cancer.

The ASCOT phase III trial demonstrated that adjuvant S-1, an oral fluoropyrimidine, improved 3-year overall survival (OS) from 67.6% to 77.1% in patients with resected biliary tract cancer (BTC). We report the prespecified final 5-year follow-up analysis. In this multicenter, open-label, randomized trial conducted in Japan, 440 patients with resected BTC were assigned to observation (n = 222) or S-1 (n = 218). S-1 (40 mg/m2 twice daily, 4 weeks on/2 weeks off, for four cycles) was orally administered. The primary end point was OS in the intention-to-treat population. At a median follow-up of 6

Transcriptomic profiling reveals autophagy-associated programs and prognostic biomarkers in anoikis-resistant pancreatic cancer cells.

Pancreatic ductal adenocarcinoma (PDAC) is characterized by aggressive metastatic progression and poor clinical outcomes. During metastatic dissemination, detached tumor cells must overcome anoikis, a form of apoptosis induced by loss of extracellular matrix attachment. Although anoikis resistance is considered a critical prerequisite for metastasis, the adaptive mechanisms supporting detached survival in pancreatic cancer remain incompletely understood. In this study, anoikis-resistant (AR) pancreatic cancer cell models were established through repeated long-term suspension culture using Capa
