Clinical Significance of Emergency EUS-Guided ERCP in the Treatment of Common Bile Duct Stones.

BACKGROUND: Common bile duct stones (CBDS) can rapidly lead to cholangitis or biliary pancreatitis, making timely and accurate diagnosis essential. Although EUS provides high diagnostic accuracy, its value in emergency settings remains unclear. This study aimed to evaluate the clinical value of emergency EUS-guided ERCP. METHODS: A retrospective analysis was performed on the clinical data of 215 patients diagnosed with CBDS at the First Affiliated Hospital of Anhui Medical University between June 2018 and June 2024. Patients were assigned to either the emergency EUS group or the elective EUS

Construction and validation of a diagnostic model for acute suppurative cholecystitis in older patients: LASSO-Logistic regression analysis using readily available clinical indicators.

OBJECTIVES: Acute suppurative cholecystitis (ASC) is a severe form of acute cholecystitis (AC) that disproportionately affects older adults, often leading to delayed diagnosis and increased morbidity. Early identification of ASC in older adults over 80 years is crucial to guide timely and appropriate management. This study aimed to develop a diagnostic model based on readily available clinical indicators to identify whether older adults with acute cholecystitis have progressed to ASC. METHODS: Older adults (aged 80 to 98 years) diagnosed with AC between January 2017 and December 2024 were retr

Small bowel obstruction from jejunal gastrointestinal stromal tumour in a young adult with neurofibromatosis type 10: a case report.

Neurofibromatosis type 1 is an autosomal dominant disorder associated with an increased risk of tumours, including gastrointestinal stromal tumours. These are usually asymptomatic and discovered incidentally. Small bowel obstruction due to gastrointestinal stromal tumour in neurofibromatosis 1 is rare, especially in young adults. We report a 24-year-old man with neurofibromatosis type 1 who presented with right upper quadrant pain and biochemical features of acute cholecystitis. During laparoscopic cholecystectomy, unexpected small bowel dilatation prompted conversion to laparotomy, revealing

Outcomes of Percutaneous Cholecystostomy as a Bridging or Definitive Treatment for Acute Cholecystitis.

Percutaneous cholecystostomy (PC) is a minimally invasive intervention for managing acute cholecystitis in patients unfit for immediate cholecystectomy. Although it is widely used, its outcomes and factors influencing clinical success are not well understood. Objectives The goal of this study was to assess the outcomes of PC by examining various factors such as gender, type of cholecystitis, and procedure timing to correlate with symptom improvement, mortality, and the need for additional medical interventions. Materials and methods A cross-sectional study was conducted at The Royal Oldham Hos

Operative strategies for the acute difficult gallbladder: a Society for Surgery of the Alimentary Tract state-of-the-art systematic review and meta-analysis of subtotal cholecystectomy outcomes.

BACKGROUND: The "difficult gallbladder" in acute cholecystitis can preclude safe achievement of the critical view of safety, prompting subtotal cholecystectomy (STC) as a guideline-supported bailout. We performed a contemporary synthesis to clarify STC's safety effectiveness trade-offs vs total cholecystectomy (TC) and delineate technique-specific outcomes and patient factors that may influence risk. METHODS: We conducted a systematic review and meta-analysis of studies from 2010 to June 2025. Three comparisons were evaluated: (i) single-arm outcomes after STC, (ii) STC vs TC, and (iii) fenes

Somatostatin analog therapy in delaying progression of polycystic liver disease: A meta-analysis with trial sequential analysis.

BACKGROUND AND AIMS: Polycystic liver disease (PLD) can occur independently or in association with autosomal dominant polycystic kidney disease and has no effective medical therapy. This meta-analysis evaluated whether somatostatin analog therapy slows progression of PLD. METHODS: The PubMed, Scopus, Web of Science, and Cochrane databases were searched through to May 1, 2025 to identify randomized controlled trials that evaluated somatostatin analogs in PLD. The primary endpoints were percentage change from baseline in total liver volume (TLV), total kidney volume (TKV), and estimated glomeru

Assessing the role of the triglyceride-glucose index in the diagnosis and risk stratification of acute cholecystitis: A retrospective study.

Accurate severity assessment is crucial in acute cholecystitis, yet commonly used inflammatory markers such as C-reactive protein, white blood cell count, and bilirubin have limited predictive accuracy. The triglyceride-glucose (TyG) index has emerged as a potential biomarker reflecting metabolic stress and inflammation. This retrospective observational study included 134 patients admitted with acute cholecystitis between 2020 and 2024. Disease severity was classified according to the Tokyo Guidelines 2018, and patients were grouped as non-severe (mild-moderate) or severe. Laboratory parameter

18 F-FDG PET/CT Five Days Post First Administration of Semaglutide : Side Effects in One Shot.

We present the case of a 60-year-old woman with metastatic breast cancer who underwent 18 F-FDG PET/CT for treatment response assessment. Beyond oncologic findings, the scan revealed abnormal gastric retention, gallbladder, colonic, and renal uptake patterns suggestive of Semaglutide-induced gastroparesis, cholecystitis, colitis, and possible acute kidney injury secondary to dehydration. Suspicions were confirmed by clinical evaluation. Symptoms resolved after supportive therapy and withholding of semaglutide. To our knowledge, this is the first report of multi-organ side effects seen on 18 F-

Acquired factor V deficiency in an elderly hemodialysis patient: a case report and literature review.

BACKGROUND: Acquired factor V deficiency (AFVD) is a rare hemorrhagic disorder caused by autoantibodies against coagulation factor V. Clinical manifestations vary widely, ranging from asymptomatic cases to severe hemorrhage or even thrombosis, and diagnosis is often challenging due to its rarity and nonspecific presentation. Reported triggers include bovine thrombin, antibiotics, surgery, autoimmune diseases, infections, and malignancy, although up to 20% of cases are idiopathic. We report a case of AFVD in an elderly hemodialysis patient, in whom corticosteroid therapy was effective but ultim
