Early Versus Delayed Laparoscopic Cholecystectomy in Acute Cholecystitis: A Comparative Study.

BACKGROUND: Laparoscopic cholecystectomy is the gold standard for treating symptomatic cholelithiasis. However, the optimal timing of surgery in acute calculous cholecystitis remains a subject of debate. This study aims to compare early versus delayed laparoscopic cholecystectomy to determine the timing that yields better clinical outcomes. METHODS: A prospective comparative study was conducted on 100 patients diagnosed with acute calculous cholecystitis at a tertiary healthcare centre in Punjab, India. Patients were randomized into two groups: Group A underwent early laparoscopic cholecystec

Gallstones: Physiopathology and treatment of cholelithiasis (part 1).

Cholelithiasis (gallstones) is a frequent disease, with an estimated annual risk of 1 to 4% of presenting clinical manifestations. It mainly results from imbalanced bile composition: cholesterol oversaturation, low levels of bile acids and gallbladder stasis resulting in the formation of gallstones. Two main types of gallstones exist: cholesterol gallstones (the most frequent), and pigment gallstones. The risk factors for lithiasis are multiple: age, female sex, rapid weight fluctuations, certain medical treatments, pregnancy, and a number of pathologies (diabetes, obesity, cirrhosis, Crohn's

Cholecystectomy versus conservative management for patients with uncomplicated symptomatic gallstones and cholecystitis: an updated systematic review and meta-analysis.

BACKGROUND: Gallstone disease affects 10-25% of the population, with higher prevalence in women over 40 years of age. Laparoscopic cholecystectomy is the standard treatment for symptomatic or complicated cases. However, it is costly, can cause adverse effects, and can result in post-cholecystectomy syndrome. Conservative management remains an option, especially for high surgical risk patients. The optimal management of uncomplicated gallstones remains controversial. This updated systematic review and meta-analysis incorporates recently published randomized controlled trials (RCTs) comparing th

Factors influencing the difficulty of laparoscopic cholecystectomy: A review.

Laparoscopic cholecystectomy (LC) is the preferred treatment for symptomatic cholelithiasis, yet conversion to open surgery occurs in a notable proportion of cases due to factors such as dense adhesions, unclear anatomy, or intraoperative complications. Therefore, it is of interest to identify preoperative predictors of difficult LC to guide surgical planning. Across the included studies, factors consistently associated with conversion included gallbladder wall thickness greater than 4-5 mm, contracted gallbladder, acute cholecystitis, age over 60 years and male gender. Other factors such as d

Surgical Complications in Conventional and Laparoscopic Cholecystectomy in an Ecuadorian Reference Hospital.

Cholecystectomy is a very common surgical intervention that consists of the removal of the gallbladder, especially when complications such as cholecystitis exist. The laparoscopic technique has been associated with greater postsurgical benefits; however, no local research has been conducted comparing these two techniques, considering that in poor-income countries, there are hospitals that do not have the technological and economic resources available to perform laparoscopic surgeries. This project aimed to determine the differences between open versus laparoscopic cholecystectomy, in terms of

Eosinophilic cholecystitis associated with vanishing bile duct syndrome: a rare case report of drug-induced hepatobiliary injury.

BACKGROUND: Eosinophilic cholecystitis (EC) is a rare histopathological variant, defined by dense transmural eosinophilic infiltration (>90% of inflammatory cells). It is typically an incidental postoperative diagnosis, as its clinical and radiological features mimic those of conventional acute cholecystitis. CASE SUMMARY: A 38-year-old female with recurrent pregnancy loss presented with acute cholecystitis and markedly deranged liver function tests in a cholestatic pattern. Magnetic resonance cholangiopancreatography revealed cholelithiasis with an impacted neck calculus without biliary obst

Diagnostic Challenge of Small Bowel Neuroendocrine Tumour Mimicking Biliary Disease: A Case Report.

Small bowel neuroendocrine tumours (NETs) are uncommon neoplasia that often occur with non-specific gastrointestinal symptoms, leading to considerable delays in diagnosis and treatment, and overlapping findings that often distract the clinical focus from the underlying pathology. We present the case of a 49-year-old woman who reported to us with a one-month history of epigastric pain, nausea, vomiting, and unintentional weight loss of about two stones. Initial imaging showed deranged liver markers and cholelithiasis on ultrasound without biliary enlargement, which resulted in a tentative diagn

Cholecystectomy vs Endoscopic Retrograde Cholangiopancreatography or No Intervention After Gallstone-Related Acute Pancreatitis.

IMPORTANCE: Same-admission cholecystectomy is recommended by guidelines for mild gallstone-related acute pancreatitis, yet surgery is often deferred. Endoscopic retrograde cholangiopancreatography (ERCP) is often used as an interim strategy, but its effectiveness compared with cholecystectomy is uncertain. OBJECTIVE: To compare cholecystectomy, ERCP only, and no intervention in association with recurrent acute pancreatitis and other gallstone-related complications, accounting for death as a competing event. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study used nationwide

United States benign gallbladder disease mortality, 1999-2024: rising cholecystitis and persistent demographic and geographic disparities.

BACKGROUND: Mortality patterns of benign gallbladder diseases in the USA remain poorly characterized, particularly across disease subtypes and demographic and geographic groups. METHODS: We used the Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemiologic Research (CDC WONDER) Underlying Cause of Death database to identify deaths from benign gallbladder diseases from 1999 to 2024. Eligible deaths were defined by ICD-10 codes K80-K82, including cholelithiasis, cholecystitis, and other diseases of the gallbladder. Age-adjusted mortality rates (AAMRs) were standardiz

Migration of a Giant Gallstone Through a Therapeutic Cholecystocutaneous Fistula: A Case Report.

Percutaneous cholecystostomy tubes (PCTs) are employed in the management of acute cholecystitis in patients unfit for surgery and may be utilised to create a temporary or permanent therapeutic cholecystocutaneous fistula. While spontaneous passage of small gallstones through such fistulae has been described, the migration of a giant gallstone (≥5 cm) has not previously been reported. We present a case involving an 82-year-old woman with a gallstone measuring 65x40 mm that had traversed a cholecystocutaneous fistula and become lodged subcutaneously. The stone was successfully extracted through

A Rare Presentation of Native Aortic Valve Endocarditis Secondary to Streptococcus gordonii: A Case Report.

Streptococcus gordonii is an alpha-hemolytic Gram-positive bacterium most commonly present in the oral cavity and gastrointestinal tract. Despite an increase in the incidence of infective endocarditis, S. gordonii cases reported on a native aortic valve are relatively uncommon. Reported cases have been linked to spondylodiskitis, intra-abdominal infection, and oral procedures. A 79-year-old male was admitted for acute kidney injury, diarrhea, and dehydration, and was found to have S. gordonii bacteremia with endocarditis. A 1.7 cm × 0.9 cm vegetation on the native aortic valve was revealed, an

Safe Laparoscopic Cholecystectomy in Mirror-Image Anatomy: Acute Biliary Pancreatitis and Acute Calculous Cholecystitis in Situs Inversus Totalis.

Situs inversus totalis (SIT) is a rare congenital laterality anomaly characterized by the complete mirror-image transposition of the thoracic and abdominal viscera. Although it is usually asymptomatic, the occurrence of acute biliary disease poses significant diagnostic and surgical challenges because the atypical location of pain and reversed anatomy may complicate clinical assessment, interpretation of imaging studies, and laparoscopic planning. We present the case of a patient with acute biliary pancreatitis associated with acute calculous cholecystitis in the setting of SIT, whose manageme

Safety and Efficacy of Single-Session Endoscopic Retrograde Cholangiopancreatography and Laparoscopic Cholecystectomy for the Treatment of Concomitant Gallbladder and Common Bile Duct Stones: A Single-Centre Experience.

Background and objectives Cholelithiasis is a commonly encountered biliary disorder in the Asian subcontinent, with a substantial burden observed in the Indian population. A significant proportion of patients admitted with gallbladder stones also present with choledocholithiasis. In individuals with acute calculous cholecystitis who are discharged without surgical intervention, the likelihood of subsequent gallstone-related complications increases progressively over time. The objective of the present study was to evaluate the safety, efficacy, and perioperative outcomes of performing single-se

Causal effects of circulating cathepsins on cholecystitis, cholelithiasis, primary biliary cholangitis, primary sclerosing cholangitis, acute pancreatitis, and chronic pancreatitis: A Mendelian randomization study.

Gallbladder, biliary, and pancreatic disorders are nonneoplastic diseases marked by inflammation, epithelial injury, and fibrosis. Cathepsins are lysosomal proteases involved in inflammatory activation, immune regulation, extracellular matrix turnover, and pancreatic enzyme activation. Observational links between dysregulated cathepsins and hepatopancreatobiliary pathology could reflect confounding or reverse causation. This study used Mendelian randomization (MR) to assess whether circulating cathepsins causally influence gallbladder, biliary, and pancreatic diseases. We conducted a 2-sample

Gallbladder Volvulus: A Great Mimicker of Acute Cholecystitis.

Gallbladder volvulus is a rare disease process, with approximately 500 reported cases in the current literature. Preoperative diagnosis is challenging, with only about 80 cases reported as being diagnosed prior to surgery, as it is frequently misdiagnosed as acute cholecystitis. Prompt recognition is vital, as the mainstay of treatment is emergent surgical intervention. This case describes an 84-year-old female who presented with right upper quadrant abdominal pain. Initial computed tomography (CT) demonstrated biliary anatomic abnormalities with relatively unremarkable laboratory values, and

The Role of Preoperative Neutrophil-to-Lymphocyte Ratio and C-reactive Protein in Predicting the Severity of Cholelithiasis With Cholecystitis.

INTRODUCTION: Acute calculous cholecystitis is a common surgical condition with variable clinical severity. Early identification of severe disease remains challenging. Inflammatory biomarkers such as neutrophil-to-lymphocyte ratio (NLR) and C-reactive protein (CRP) have been proposed as simple indicators of systemic inflammation and disease severity. The present study aimed to evaluate the association of preoperative NLR and CRP levels with the severity of calculous cholecystitis and their relationship with clinical outcomes. MATERIALS AND METHODS: This prospective observational study was con

Case Report: Multiple complications after laparoscopic cholecystectomy with perforation and spilled gallstones.

INTRODUCTION: Perforation of the gallbladder is a benign and common complication during laparoscopic cholecystectomy. However, it may result in stone spilling, which potentially can lead to serious postoperative complications. CASE REPORT: A 70-year-old male underwent laparoscopic cholecystectomy for acute cholecystitis. The procedure was complicated by perforation of the gallbladder and spilling of gallstones. More than a year after the procedure, the patient developed subcutaneous abscesses containing some of the spilled stones, a computed tomography revealed a complex intraabdominal and in

When Silence Speaks: A Perforated Hepatic Cyst, Intact Omental Cyst, and Cholelithiasis.

A 39-year-old man from an echinococcosis-endemic region presented with acute left upper quadrant pain. Imaging revealed cholelithiasis, a hepatic and a large abdominal cystic masses, and significant free peritoneal fluid. Urgent laparotomy unexpectedly disclosed three concurrent pathologies: chronic calculous cholecystitis, a perforated hepatic hydatid cyst, and an intact omental hydatid cyst. The exact source of his symptoms remains uncertain. This case highlighted the diagnostic challenge of coexisting surgical diseases in endemic regions.
