Tacrolimus-Induced Gallstone Formation in the Early Post-renal Transplant Period: A Case Report.

Gallstone disease (GSD) is a known but relatively underrecognized complication in renal transplant (RT) recipients, particularly in those receiving cyclosporine. However, there is scarce data on the association between tacrolimus use and cholelithiasis. We present the case of a 33-year-old female with end-stage renal disease who underwent an ABO-compatible RT and was initiated on a standard triple immunosuppressive regimen consisting of tacrolimus (3.5 mg/day), mycophenolate mofetil (1.5 g/day), and prednisolone (20 mg/day). Six weeks post-transplant, she developed acute abdominal pain, nausea

Cystic Duct Stenting Versus Other Treatment Modalities for the Management of Acute Cholecystitis in Patients with Decompensated Cirrhosis.

BACKGROUND AND AIMS: The incidence of cholecystitis and cholelithiasis is higher in patients with cirrhosis. Decompensated liver disease places them at higher risk for morbidity and mortality from cholecystectomy, and many providers prefer non-surgical approaches. We compared cystic duct stenting (CDS) to other modalities mainly percutaneous cholecystostomy (PC), cholecystectomy, and medical management. METHODOLOGY: We performed a retrospective cohort study. After obtaining IRB approval, we gathered records of all patients at our health care system who had acute cholecystitis on presentation

Managing Hepatic Cysts in the Setting of Surgery for Cholecystitis.

Hepatic cysts are relatively common and often present with non-specific symptoms such as nausea and abdominal pain. While most are benign simple cysts, the differential diagnosis includes infectious, inflammatory, and malignant etiologies. Imaging, including ultrasound and cross-sectional modalities, is key to diagnosis. Hepatic cysts can occasionally present with symptoms that overlap with gallbladder disease, complicating the clinical picture. We present the case of a patient with chronic cholecystitis who was found to have an incidental hepatic cyst obstructing the gallbladder. Given its si

Biliary Hyperkinesia: An Overlooked Cause of Right Upper Quadrant Pain.

Biliary colic, characterized by intermittent right upper quadrant (RUQ) abdominal pain, is a common clinical presentation worldwide. The most frequent underlying causes include acute or chronic cholecystitis and cholelithiasis. In cases where ultrasound findings are unremarkable, patients sometimes undergo a hepatobiliary iminodiacetic acid (HIDA) scan to evaluate gallbladder and biliary tree function. Traditionally, the results are categorized into two outcomes based on the gallbladder ejection fraction (GBEF): biliary dyskinesia or normal function. Biliary dyskinesia, or hypokinesia, is char

A 20-Year Analysis of Risk Factors for Complicated Postoperative Course Following Laparoscopic Cholecystectomy.

BackgroundCholecystectomy is the most common surgical intervention performed in the United States with over 300,000 annual cases. We aim to describe risk factors for complications and prolonged stay after cholecystectomy in a 20-year analysis of a national database.MethodsThe Nationwide Inpatient Sample (NIS) was queried for years 2000-2019 for patients who underwent cholecystectomy within 7 days of admission. Complicated stay was defined as postoperative stay longer than 3 days or an ICU admission. Outcomes were postoperative complications, hospital length of stay, hospital charges, and morta

Learning Curve for da Vinci Single-Port Robotic System Cholecystectomy.

BACKGROUND: Minimally invasive surgery is recognized as the gold standard for cholecystectomy, with various approaches having been implemented over time. Since 2018, the da Vinci Single-Port (DVSP) robotic system (Intuitive) has been used in experimental surgical contexts, revitalizing interest in single-site surgical techniques. The authors' aim is to describe the learning curve associated with single-port robotic cholecystectomy (SPRC). METHODS: A prospective series of 266 consecutive off-label SPRC performed by a single surgeon experienced in laparoscopy, single-site surgery, and multiport

Incidental diagnosis of gallbladder carcinoma during or after routine cholecystectomy: A retrospective study with emphasis on clinicopathologic findings.

BACKGROUND: Cholecystectomy is a common surgical procedure routinely performed for patients with benign gallbladder disease. The most common indications for cholecystectomy are acute or chronic cholecystitis with or without cholelithiasis. However, in rare instances, incidental findings ranging from benign to malignant conditions are encountered, of which gallbladder adenocarcinoma is an aggressive and fatal disease. AIM: To determine the prevalence of all incidental diagnoses in routinely performed cholecystectomy specimens, with a particular emphasis on adenocarcinoma, and to characterize t

Eosinophilic infiltration in gallbladder pathologies: A clinical dilemma.

OBJECTIVES: To determine the clinicopathological features and prevalence of Eosinophilic cholecystitis (EC) with a single-center experience. Eosinophilic cholecystitis is an exceedingly uncommon pathologic issue caused by examining cholecystectomy specimens. METHODS: A retrospective analysis was performed on 8,342 cholecystectomy specimens. Of these, 33 pathology results confirmed EC and were re-examined by pathologists. This study obtained demographic data, medical histories, comorbidities, clinical laboratory results, radiological findings, and pathological information from an electronic re

Cholecystoduodenal fistula presenting with refractory upper gastrointestinal bleeding: retrospective analysis: a case report.

BACKGROUND: Spontaneous cholecystoduodenal fistula presents an uncommon complication arising from cholelithiasis. Its symptoms are typically nonspecific and can closely mimic those of other chronic biliary diseases, therefore increasing the likelihood of misdiagnosis. Massive upper gastrointestinal bleeding complicated with cholecystoduodenal fistula is an extremely rare condition with only a few documented cases. CASE PRESENTATION: Here we discuss the case of a 77-year-old Han Chinese female who presented with refractory upper gastrointestinal bleeding due to a cholecystoduodenal fistula. Th

Two-stage treatment of cholecystobronchial fistula secondary to gallbladder perforation: a case report.

BACKGROUND: Gallbladder perforation is a serious complication of acute cholecystitis. Among its rarest manifestations, cholecystobronchial fistula represents an exceptionally uncommon form of type III perforation and is frequently misdiagnosed due to its nonspecific presentation. A 67-year-old Chinese (East Asian) male presented with a chief complaint of cough and hemoptysis. His medical history was significant for nephrotic syndrome, for which he had been receiving long-term corticosteroid therapy. Imaging demonstrated cholelithiasis, a cholecystobronchial fistula, and a right-sided lung absc

Preoperative C-reactive protein as a predictor of conversion and complications in laparoscopic cholecystectomy for acute cholecystitis.

INTRODUCTION: Laparoscopic cholecystectomy (LC) is the gold standard for gallbladder surgery but may require conversion to open surgery in complicated cases. While preoperative C-reactive protein (CRP) is known to reflect inflammation, its predictive role in surgical outcomes remains debated. This study evaluated CRP as a predictor of surgical outcomes in acute cholecystitis and uncomplicated gallbladder stones, explicitly excluding choledocholithiasis to isolate gallbladder-specific inflammatory predictors. MATERIALS AND METHODS: This retrospective cohort study analyzed 180 patients undergoi

Single-Operator Multi-Port cholecystectomy (SOMP) - technique, safety, and efficacy.

BACKGROUND: The standard technique of laparoscopic cholecystectomy requires four ports and at least two operators (surgeon and assistant). The Single-Operator Multi-Port (SOMP) technique, wherein the surgeon independently manipulates all four ports using self-retaining retractors, remains poorly characterized in the literature. AIM: To describe the SOMP cholecystectomy technique and evaluate its feasibility, safety, and efficacy through analysis of surgical outcomes in comparison to the standard Multiple-Operator Multi-Port (MOMP) cholecystectomy. METHODS: We performed a retrospective review o

Idiopathic Eosinophilic Cholecystitis in the Setting of Cholelithiasis: A Case Report and Brief Literature Review.

Eosinophilic cholecystitis (EC) is a rare inflammatory condition characterized by prominent eosinophilic infiltration of the gallbladder wall. It often mimics typical acute cholecystitis and is usually diagnosed only after histopathological examination. This case report discusses a 69-year-old woman presenting with symptoms of acute cholecystitis who was later found to have EC. The report highlights the clinical presentation, diagnosis, treatment, and relevant literature on this uncommon condition.

Biliary Dyskinesia - Is It Real?

INTRODUCTION: Biliary dyskinesia is a functional gallbladder disorder characterized by altered gallbladder motility. This often presents as history of biliary symptoms without imaging evidence of gallstones, sludge, or other structural pathology. The diagnosis is made by demonstration of abnormal gallbladder emptying on hepatic iminodiacetic acid analogue scan with cholecystokinin administration (HIDA-CCK). The aim of this study was to determine if patients with biliary dyskinesia who undergo cholecystectomy have abnormal pathology and resolution of symptoms. METHODS: This is a retrospective

Symptomatic Gallbladder Duplication With Mucocele and Calculi: An Anatomic Variant Diagnosed by CT.

Gallbladder duplicates are generally asymptomatic and discovered incidentally; meanwhile, complications with gallbladder duplication, such as cholelithiasis and mucocele, can result in acute symptoms in the abdominal region with both complicated diagnostic and/or surgical difficulties. This report describes a 51-year-old male patient who presented to our facility with colicky right upper quadrant abdominal pain. Contrast-enhanced CT demonstrated true gallbladder duplication with two separate cystic ducts, one of which was distended secondary to a mucocele from an obstructing calculus. An elect

Heterotopic Pancreas in the Gallbladder: An Incidental Finding Following Laparoscopic Cholecystectomy in a Patient With Cholelithiasis.

Heterotopic pancreas (HP)-defined as ectopic pancreatic tissue lacking vascular or ductal connections to the main organ-represents an exceedingly rare anatomical variant in the gallbladder. This developmental anomaly warrants careful clinical attention due to its potential surgical implications. We report a case of a 38-year-old female who presented to the emergency department with progressive abdominal pain, nausea, and vomiting. Ultrasonographic evaluation revealed a solitary 15-mm gallstone without evidence of gallbladder wall inflammation. Following laparoscopic cholecystectomy, histologic

Hot Gallbladders, Cool Outcomes: Bailout Outcomes of Early Laparoscopic Cholecystectomy for Acute Cholecystitis.

Introduction Laparoscopic cholecystectomy for acute cholecystitis (hot cholecystectomy) is known to be associated with increased operative difficulty and a higher risk of intraoperative biliovascular complications. Certain bailout strategies have been formulated to mitigate these risks. Most guidelines recommend surgery within seven days of onset of inflammation. The objective of this study is to assess the bailout rate of early cholecystectomy for acute cholecystitis, performed irrespective of the time of presentation but with a target of six weeks from the time of diagnosis. Methods This ret
