Gallbladder duplication is a rare congenital anomaly that may complicate the diagnosis and surgical management of biliary disease. We report the case of a 42-year-old gentleman presenting with the sudden onset of right upper abdominal pain, nausea, and emesis. Imaging revealed a duplicated gallbladder with cholelithiasis and sonographic features consistent with acute cholecystitis. The patient underwent a laparoscopic cholecystectomy, which revealed a bilobed gallbladder with a single cystic duct. This case highlights the diagnostic and operative challenges associated with gallbladder duplicat

Ectopic gallbladders are uncommon congenital variants that can complicate management of acute cholecystitis. They are rare, and their unusual positioning can pose diagnostic and intraoperative challenges. Because of their rarity, many ectopic gallbladders are not suspected until the time of surgery, increasing the potential risk of bile duct injury and prolonging operative time. Here, we present the case of a 41-year-old woman presenting with severe overnight epigastric pain following six months of intermittent pain. Examination revealed right upper quadrant and epigastric tenderness with a po

INTRODUCTION: Cholecystectomy is one of the most common operations in surgical departments. Complications after gallbladder removal are mainly bleeding, infection including abscess in the gallbladder bed or in the abdominal wall, wound dehiscence, acute pancreatitis or injury of the bile ducts. In the further course, hernias in the scar may appear after both laparoscopic and open cholecystectomy, strictures of the bile ducts and symptoms of the so-called postcholecystectomy syndrome. The presence of residual gallstones is rare, statistically reported in 0.08-0.3%.

IntroductionBile spillage (BS) is common during laparoscopic cholecystectomy (LC) and has been shown to be associated with an increased risk of surgical site infection (SSI). We hypothesized that positive bile cultures (PBCs) increase the risk of postoperative complication including SSI.MethodsA retrospective chart review was conducted including all patients older than 18 years undergoing urgent LC from January to September 2019. Charts were reviewed for the index admission and postoperative visits. We compared those who had PBCs with those who did not. Our primary endpoint was the rate of SSI

Gallstone disease (GSD) is a common gastrointestinal disorder affecting approximately 10-20% of the global adult population, characterized by the presence of gallstones, predominantly cholesterol-based, in the gallbladder and/or biliary ducts. While many patients remain asymptomatic, more than 20% develop clinical symptoms such as abdominal pain, nausea, vomiting, jaundice, and anorexia, potentially leading to severe complications like acute cholecystitis and biliary pancreatitis. GSD has a significant genetic predisposition, with the variable prevalence of the disease according to ethnicity b

We present the case of a 60-year-old woman who was admitted with acute pancreatitis, cholelithiasis, and cholecystitis, and subsequently underwent an uneventful laparoscopic cholecystectomy. Two weeks postoperatively, she returned to the hospital with severe epigastric pain radiating to the back, accompanied by nausea and vomiting. Contrast-enhanced computed tomography (CT) imaging revealed acute necrotizing pancreatitis with early pancreatic pseudocyst formation at the junction of the pancreatic body and tail, without evidence of biliary obstruction or retained stones. She was managed with in

Background Gallstones represent a common hepatobiliary disease, with the most common being cholesterol stones. This study aimed to determine the prevalence of cholesterol gallstones and evaluate their relationship with body mass index (BMI) and other risk factors. Methodology From March to August 2025, a total of 300 adult patients with ultrasonographically proven cholelithiasis were admitted to the Department of Surgery of Jinnah Medical and Dental College in Karachi and participated in this descriptive cross-sectional study. Patients with hemolytic disorders, chronic liver disease, acute cho

Hepatic subcapsular biloma is a rare complication of gallbladder perforation in acute cholecystitis. We report the case of an 87-year-old man who presented with right upper quadrant pain, a new oxygen requirement, and deranged liver function tests, with a recent history of conservatively managed acute calculous cholecystitis. Magnetic resonance cholangiopancreatography (MRCP) demonstrated a thick-walled gallbladder containing a 2 cm calculus with focal fundal perforation and a large subcapsular fluid collection extending beneath the hepatic capsule, along with an associated right pleural effus

Gallbladder torsion is a rare but potentially life-threatening surgical emergency that poses a significant diagnostic challenge due to the condition's non-specific clinical presentation and often inconclusive imaging findings. We present the case of a 53-year-old woman with sudden-onset right upper quadrant abdominal pain and unintentional weight loss. Her low body mass index and initial clinical findings suggested acute cholecystitis. Ultrasonography revealed a distended gallbladder without cholelithiasis. Despite conservative management, her symptoms worsened, prompting an emergency diagnost

BACKGROUND: Identifying patients with common bile duct (CBD) stones in the setting of acute calculous cholecystitis (ACC) is challenging. Three prediction models were previously developed to streamline this process. This study externally validates these models and evaluates whether a new model is needed.
METHODS: In this retrospective single-centre cohort study, patients who underwent early cholecystectomy for ACC between 2015 and 2021 were included. The previously published models were assessed for discriminative performance, calibration, and two pre-defined evaluation criteria. These criter

BACKGROUND: The aim of the study was to map current practices in the management of gallstone disease at Norwegian hospitals providing acute care surgery.
MATERIAL AND METHOD: A questionnaire was distributed to all surgical departments in Norway providing acute care for cholecystectomy.
RESULTS: A total of 41 hospitals met the inclusion criteria and 40 responded to the questionnaire. Institutions were categorised as low, medium or high-volume hospitals (< 50, 50-200 and > 200 cholecystectomies annually, respectively). Six, 20 and 14 hospitals were classified as low, medium and high-volume cen

Metastatic spread to the gallbladder is exceedingly rare, representing less than 5% of all gallbladder malignancies, with secondary involvement from head and neck squamous cell carcinoma (SCC) being exceptionally uncommon. We present the case of a 68-year-old male with recurrent metastatic head and neck SCC who arrived with right upper-quadrant pain, nausea, and vomiting. Imaging demonstrated gallbladder distension and wall thickening without cholelithiasis, and subsequent hepatobiliary iminodiacetic acid (HIDA) scanning and magnetic resonance cholangiopancreatography (MRCP) findings supported

An adolescent female presented with acute cholecystitis associated with obstructive choledocholithiasis. As she was being prepared to undergo urgent endoscopic retrograde cholangiopancreatography and cholecystectomy, it was noted that she was profoundly bradycardic with resting heart rate as low as 33 beats per minute. Though her body mass index was within normal limits at presentation, her bradycardia prompted review of her growth curve which showed almost 100 pounds of weight loss in the previous year. History revealed severe disordered eating behaviours and led to a diagnosis of atypical an

We present a case of a woman in her 70s who arrived in the emergency department with signs of small-bowel obstruction. CT scanning revealed acute cholecystitis with a cholecystoduodenal fistula, pneumobilia and small-bowel obstruction possibly secondary to gallstone ileus although no radio-opaque gallstones were seen. The patient underwent an emergency operation and intra-operative findings revealed mechanical small-bowel obstruction of the proximal jejunum where a 4×2 x 3 cm gallstone was impacted. A laparoscopic-assisted enterotomy was performed with stone extraction and primary closure. The

BACKGROUND: Strongyloides stercoralis (S. stercoralis), is a prevalent parasitic worm that infects humans. It is found all over the world, particularly in tropical and subtropical areas. Strongyloidiasis is caused mostly by the parasitic nematode S. stercoralis. Filariform larvae typically infest humans by coming into contact with dirt, such as by walking barefoot or through exposure to human waste or sewage.
CASE SUMMARY: A 35-year-old male presented to our department with a 10-year history of abdominal pain and diarrhea, which had recently recurred for the past 3 months. A computed tomograp

BACKGROUND A cholecystoenteric fistula (CEF) is a rare complication of gall stone disease. While a cholecystoduodenal fistula is the most commonly occurring bilioenteric fistulous communication, cholecystocolonic, cholecystogastric and choledochoduodenal fistulas have also been described. CASE REPORT A 73-year-old woman presented with a 1-week history of melena on a background of acid reflux and no abdominal pain. A gastroscopy revealed kissing D1 ulcers with excretion of pus. Following a CT scan of the abdomen, the patient was referred to the surgical team for the management of acute cholecys

Right upper quadrant pain is one of the most frequent reasons for consultations in the emergency room. Gallbladder pathology is among the most common etiologies and can include cholecystitis, cholelithiasis, choledocholithiasis, and cholangitis, among others. Mirizzi syndrome is a complication that manifests as hepatobiliary dysfunction due to a gallstone causing extrinsic compression of the common bile duct. However, acute cholecystitis can externally obstruct the common bile duct and mimic Mirizzi syndrome in the absence of a causative gallstone. The relevance of this syndrome lies in its ti

Cytokeratins (CKs) 8 and 18 are normally expressed in simple epithelia. This unique pair of CKs is believed to be involved in hepatic diseases and many human cancers. Little is known about the role of tissue expression of both CKs in patients with cholelithiasis (CH). The aim of the study was to analyse tissue expression of CK8 and 18 in the specimens of gallbladder mucosa in 35 young (up to 25 years of age) and 20 older patients (approximately 50 years of age) with CH. An immunocytochemical ABC method and the spatial visualization technique were conducted. Our study demonstrated significantly
