The Author describes 151 cases of acute cholecystitis in patients ranging from 70 to 95 years of age. He examines then the literature on the subject paying attention to the treatment of cases of acute cholecyst disease in aged patients. The group of conditions variously termed biliary dyskinesia, acalculous cholecystitis, biliary pain without stones, or functional disorders of the biliary tract, is poorly defined clinically, and no consistent pathological abnormalities have been previously described in patients with this diagnosis. During 5 years 1311 operations for benign diseases of the biliary tract were performed. 281 patients were more than 70 years old, on the average 76,1 years. There is a definite clinical entity of chronic acalculous cholecystitis that can be cured by cholecystectomy. More research needs to be done on how this condition could be more accurately and earlier diagnosed. The majority of chronic gallbladder symptoms are not dependent on the presence of calculi. Gallbladder x-ray films are subject to significant diagnostic error, as are all other diagnostic tests. There is a need for more uniformity among pathologists as to what microscopic findings substantiate a diagnosis of chronic cholecystitis.

The morbidity of complications and mortality following surgical operations for nonmalignant disease of the biliary tract is greatest among those 65 years and older. The reported overall mortality regardless of age ranges upward from 0.88%. At The New York Hospital-Cornell Medical Center for a 43-year period (1932-1975) it was 1.77%. Recurrent surgical interventions on the biliary system for benign biliary tract diseases carry high morbidity and mortality. Choledochoduodenostomy creates a large and easily performed biliodigestive anastomosis enabling good drainage of the biliary system. Among 27 patients undergoing choledochoduodenostomy for benign biliary tract diseases, recurrent cholangitis occurred in only one patient, in whom a stenosed anastomosis was probably the culprit. A series of 412 personally performed cholecystectomies is analyzed and discussed. Surgical cases were divided into groups based on whether or not the gallbladder was visualized by cholecystography. Clinical evaluation of hepatobiliary scanning using 99mTc-PG was done in twenty normal volunteers and eighty-three patients with liver and biliary tract disease. An unusual case of melanoma of the gallbladder is reported. It is the fourth reported case with roentgenographic demonstration. It presented clinically as cholecystitis and radiologically as a larger solitary defect within the gallbladder accompanied by cholelithiasis. Eighty-seven patients who had sonographic examinations at the Orange County Medical Center were studied retrospectively to evaluate the usefulness and reliability of this noninvasive technique, and sonographic results were compared with findings at operation or findings made using other diagnostic methods.
