Metagenomic Analysis of Biliary Microbial Flora in Patients with Gallbladder Cancer or Gallstones-Associated Chronic Cholecystitis.

Biliary dysbiosis is associated with gallbladder cancer (GBC). We aimed to look for biliary bacteria specifically detected in GBC patients. We used 16S rRNA-based metagenomic analysis to elucidate biliary microbiota in 30 GBC and 30 gallstones-associated chronic cholecystitis patients. Relative abundance of five genera, Streptococcus, Enterococcus, Halomonas, Escherichia and Caulobacter was significantly associated with GBC. Of 15-species, 7 were detected significantly higher in GBC, Streptococcus anginosus, Streptococcus constellatus, Streptococcus intermedius, Actinomyces bowdenii, Actinomyc

Hyperkinetic Biliary Dyskinesia: An Underrecognized Problem With Good Surgical Outcomes After Cholecystectomy.

Introduction While surgical indications for symptomatic cholelithiasis and biliary hypokinesia are clear, hyperkinetic biliary dyskinesia (HBD) is an underrecognized condition with poorly defined symptomology and management guidelines. HBD is typically defined as a gallbladder ejection fraction (EF) ≥ 80% on a hepatobiliary iminodiacetic acid (HIDA) scan. We aimed to identify the prevalence and radiographic reporting of HBD, physician referral patterns, and clinical outcomes following cholecystectomy. Methods  A retrospective cohort study of patients with HIDA scans completed over 21 years at

Navigating Challenges: Managing Upper Gastrointestinal Bleeding From Cholecystoduodenal Fistula in an Elderly Patient.

Cholecystoduodenal fistula (CDF) is an uncommon condition characterized by an abnormal connection between the gallbladder and the duodenum, often linked to cholelithiasis. It typically presents with nonspecific symptoms such as abdominal pain and jaundice but can occasionally result in severe upper gastrointestinal (GI) bleeding. This report describes the case of a 94-year-old female who presented with hypovolemic shock and multiple episodes of hematemesis. An upper GI endoscopy confirmed a CDF with active hemorrhage. Due to her comorbidities and poor performance status, an endoscopic approach

Single-Incision Combined Laparoscopic Right Hemicolectomy and Cholecystectomy: A Case Report.

Minimally invasive surgery has transformed the management of complex procedures, offering reduced postoperative pain, faster recovery time, and improved cosmetic outcomes. Despite the growing use of minimally invasive techniques, literature specifically addressing single-incision laparoscopic surgery (SILS) for combined hemicolectomy and cholecystectomy is limited. This report seeks to fill this gap by detailing the successful management of a patient case through a single-incision combined laparoscopic right hemicolectomy and cholecystectomy. A 66-year-old female was referred to surgical consu

Pancreatic cancer risk after benign gallbladder disease: A Swedish population-based cohort study.

AIM: The purpose of this nationwide registry-based cohort study was to outline pancreatic cancer risk after benign gallbladder disease (GBD). Anatomically adjacent cancers were investigated to address incidental findings. METHODS: We included all Swedes aged 20-79 years with histologically confirmed GBD (cholecystitis and/or cholecystectomy) in 1992-2016 and five matched non-exposed population comparators. Follow-up started one month after GBD and incidence rates (IR) and hazard ratios (HR) with 95 % confidence intervals (CI) up to 15 years after GBD were estimated using Poisson and Cox regre

Assessment of Predictors of Difficult Laparoscopic Cholecystectomy by Clinico-Radiological Parameters at a Tertiary Hospital in Eastern India.

BACKGROUND: Laparoscopic cholecystectomy has become the standard treatment for gallbladder (GB) stones, favored for its minimally invasive approach. Despite its benefits, the procedure sometimes requires conversion to open cholecystectomy due to intra-operative challenges, with conversion rates varying between 1% and 13%. There are various preoperative predictors that help in identifying such difficult cases and help to proceed safely. This study aims to identify the preoperative factors that could predict the difficulty of laparoscopic cholecystectomy, thus anticipating the need for conversio

A Case of Intracholecystic Papillary Neoplasm Extending Through a Cholecysto-Colonic Fistula.

We report the case of a 76-year-old man who presented with pain in the right upper abdomen. Laboratory and radiological examinations revealed cholangitis, cholelithiasis, and a gallbladder tumor adhering to the transverse colon. After receiving conservative therapy for cholangitis, the patient underwent surgery for the gallbladder disease. On surgery, a cholecysto-colonic fistula communicating the gallbladder and transverse colon was revealed, and combined resection of the gallbladder and partial transverse colon was performed. The resected sample revealed an intracholecystic papillary neoplas

Identifying the Prognostic Factors Affecting the Conversion From Laparoscopic Cholecystectomy to Open Cholecystectomy in Acute Cholecystitis.

Introduction Acute cholecystitis, commonly caused by gallstones, is a prevalent surgical emergency worldwide. Laparoscopic cholecystectomy (LC) is the gold standard for treatment, but the timing is crucial, with early surgery (within seven days) reducing complications. Identifying prognostic factors such as age, sex, white blood cell (WBC) count, C-reactive protein (CRP), and gallbladder wall thickness can help predict outcomes and reduce the need for conversion to open surgery. Methods A prospective longitudinal observational study was conducted at Sree Balaji Medical College and Hospital, Ch

Laparoscopic common bile duct exploration for choledocholithiasis on an emergency setting.

INTRODUCTION: Choledocholithiasis is a common clinical condition that may present with severe complications such as acute cholecystitis or cholangitis, requiring treatment on an emergency setting. This situation is frequently managed following an endoscopic approach by ERCP. However, access to emergent endoscopic biliary decompression is lacking in most centers. In addition, experience in laparoscopic common bile duct exploration (LCBDE) in elective patients is increasing revealing good outcomes. This study analyzes the outcomes of LCBDE management of choledocholithiasis performed on an emerge

A focused point-of-care abdominal ultrasound is an examination performed at the patient's location and interpreted within the clinical context. This review gives an overview of this examination modality. The objective is to rapidly address predefined dichotomised questions about the presence of an abdominal aortic aneurysm, gallstones, cholecystitis, hydronephrosis, urinary retention, free intraperitoneal fluid, and small bowel obstruction. FAUS is a valuable tool for emergency physicians to promptly confirm various conditions upon the patients' arrival, thus reducing the time to diagnosis and

Risk Factors for Difficult Three-Port Laparoscopic Cholecystectomy.

Background Standard laparoscopic cholecystectomy (LC) is a four-port technique in which a camera port and three additional ports are used. The advantages of minimally invasive surgery with reduced-port surgery have been reported. However, evidence on the indications for minimally invasive surgery in patients with severe acute cholecystitis or previous upper abdominal surgery in whom laparoscopic surgery is considered challenging is limited. Therefore, this study aimed to explore the factors that complicate reduced-port LC. Methods Data from 47 consecutive patients who underwent three-port LC u

Role of Intraoperative Near-Infrared Indocyanine Green Fluorescence Cholangiography in the Management of Acute Gangrenous Cholecystitis Secondary to Empyema of the Gallbladder in Mirizzi's Syndrome.

Mirizzi syndrome, although rare, is a potential complication of long-standing gallstone disease, particularly cholecystolithiasis. Due to the nonspecific nature of its symptoms, this condition often remains undiagnosed prior to surgery in most cases. While minimally invasive approaches are generally safe in expert hands, they can be challenging and entail the risk of bile duct injuries, often necessitating conversion to bail-out procedures. Delayed management of Mirizzi syndrome can lead to serious consequences, such as empyema of the gallbladder (GB), gangrene of the GB wall, perforation, and

The Presence of Type Va Double Common Bile Duct in a Male Patient: A Case Report.

Double common bile duct (DCBD) is a rare congenital anomaly of the biliary system, characterized by the presence of two common bile ducts. The condition can be classified into five distinct types. Type Va DCBD is one of the rare congenital variations where both bile ducts open separately into the duodenum: one opens normally at the major duodenal papilla, and the second opens independently into the duodenum at a different site, usually proximal to the major duodenal papilla. DCBD is often associated with biliary pathologies such as cholelithiasis, choledocholithiasis, choledochal cysts, abnorm
