Cholecystocolonic fistula (CCF) is a rare type of cholecystoenteric fistula, which are sometimes diagnosed incidentally in patients undergoing cholecystectomy. They are usually a complication of chronic cholecystitis, often secondary to gallstones, and they are difficult to diagnose due to their rarity and vague clinical presentation. If untreated, they can lead to complications such as acute cholangitis, biliary peritonitis, and biliary cirrhosis. This highlights the importance of early recognition of CCF and timely treatment. While the generally accepted management is surgical, there are no

Cholelithiasis, characterized by the presence of gallstones, is a common condition in the United States, with 80% of affected individuals having no symptoms. Symptomatic gallstone disease encompasses symptomatic cholelithiasis, biliary dyskinesia, acute cholecystitis, choledocholithiasis, gallstone pancreatitis, and acute cholangitis. It often manifests as sharp, stabbing right upper quadrant or epigastric pain as well as nausea and vomiting. History, physical examination, and laboratory evaluation alone are insufficient to make an accurate diagnosis. Ultrasonography should be the first-line i

A biloma is a localized bile collection caused by bile leakage that is usually resolved with percutaneous drainage. In most cases, clinical manifestations are limited to local symptoms such as abdominal pain and distension. Although bilomas can occasionally cause septic shock attributable to secondary infection or obstructive jaundice attributable to direct compression of the bile duct, cases involving heart compression by a biloma that leads to circulatory failure have been rarely reported. We report the case of a 79-year-old man undergoing maintenance hemodialysis who developed gallstone-ass

BACKGROUND: Intraoperative cholangiography (IOC) during laparoscopic cholecystectomy (LC) helps to clarify the biliary anatomy, detect common bile duct (CBD) stones, and prevent or promptly detect bile duct injuries (BDI). However, the indication for routine or selective cholangiography remains controversial. We aimed to compare the results of a routine versus selective IOC policy during LC in an urban teaching hospital.
METHODS: A prospective cohort study was designed, including a consecutive series of patients undergoing LC with either routine IOC (RC) or selective IOC (SC). SC was performe

OBJECTIVE: This study investigated the prevalence and risk factors for acute cholecystitis following the placement of fully covered self-expandable metal stents (FC-SEMS) during endoscopic retrograde cholangiopancreatography into the common bile duct (CBD), across patients with various indications. We aimed to evaluate whether FC-SEMS placement increases the risk of acute cholecystitis and to identify associated risk factors.
METHODS: In this retrospective, multicenter study, 365 medical records from multiple hospitals were reviewed. All patients underwent FC-SEMS placement for biliary strict

BACKGROUND: The optimal timing for laparoscopic cholecystectomy (LC) following endoscopic retrograde cholangiopancreatography (ERCP) in patients with both gallbladder and common bile duct stones remains debated. This study aimed to evaluate clinical outcomes based on the timing interval of sequential LC performed within two weeks post-ERCP.
METHODS: We conducted a retrospective analysis of 241 patients with concomitant cholecystolithiasis and choledocholithiasis who underwent ERCP followed by LC at our institution between January 2018 and December 2023. Patients were stratified into three coh

Cholelithiasis (gallstones) is a frequent disease, with an estimated annual risk of 1 to 4% of presenting clinical manifestations. It mainly results from imbalanced bile composition: cholesterol oversaturation, low levels of bile acids and gallbladder stasis resulting in the formation of gallstones. Two main types of gallstones exist: cholesterol gallstones (the most frequent), and pigment gallstones. The risk factors for lithiasis are multiple: age, female sex, rapid weight fluctuations, certain medical treatments, pregnancy, and a number of pathologies (diabetes, obesity, cirrhosis, Crohn's

BACKGROUND: Gallstone disease affects 10-25% of the population, with higher prevalence in women over 40 years of age. Laparoscopic cholecystectomy is the standard treatment for symptomatic or complicated cases. However, it is costly, can cause adverse effects, and can result in post-cholecystectomy syndrome. Conservative management remains an option, especially for high surgical risk patients. The optimal management of uncomplicated gallstones remains controversial. This updated systematic review and meta-analysis incorporates recently published randomized controlled trials (RCTs) comparing th

BACKGROUND: Laparoscopic cholecystectomy (LC) is standard for gallstones, but residual cystic duct and common bile duct (CBD) stones occur due to lack of systematic intraoperative cystic duct exploration. This study evaluated the mid-to-long-term efficacy and safety of a modified procedure-Xinjiang-Gu Laparoscopic Cystic Duct Exploration and Lithotomy-in reducing residual biliary stones.
METHODS: This retrospective cohort study enrolled patients undergoing LC for gallstones (08/2021-09/2023).
EXCLUSION CRITERIA: biliary malignancy, intraoperative CBD exploration, prior bilioenteric anastomos

BACKGROUND: Eosinophilic cholecystitis (EC) is a rare histopathological variant, defined by dense transmural eosinophilic infiltration (>90% of inflammatory cells). It is typically an incidental postoperative diagnosis, as its clinical and radiological features mimic those of conventional acute cholecystitis.
CASE SUMMARY: A 38-year-old female with recurrent pregnancy loss presented with acute cholecystitis and markedly deranged liver function tests in a cholestatic pattern. Magnetic resonance cholangiopancreatography revealed cholelithiasis with an impacted neck calculus without biliary obst

Gallbladder duplication is an uncommon congenital anomaly (1 out of 4,000 autopsies) that poses significant diagnostic and surgical challenges. In the present case, a 70-year-old female presented with right hypochondrial pain, dyspepsia, and nausea for one month. Ultrasound revealed a bifid gallbladder with calculi in one pouch and a normal common bile duct. Intraoperatively, two separate gallbladders were found, one positioned normally and the other intrahepatic. Laparoscopic excision of the first was completed, but conversion to open surgery was required because of the unclear anatomy and th

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Endoscopic management of benign pancreaticobiliary disorders encompasses a range of procedures designed to address complications in gallstone disease, choledocholithiasis, and pancreatic disorders. Acute cholecystitis is typically treated with cholecystectomy or percutaneous drainage (PT-GBD), but for high-risk or future surgical candidates, alternative decompression methods, such as endoscopic transpapillary gallbladder drainage (ETP-GBD), and endoscopic ultrasound (EUS)-guided gallbladder drainage (EUS-GBD), are effective. PT-GBD is associated with significant discomfort as well as variable

We present laparoendoscopic hybrid treatment of a patient with choledocholithiasis, cholecystitis and large duodenal diverticula. A 69-year-old patient underwent one-stage hybrid laparoendoscopic intervention. Despite the difficult situation caused by large duodenal diverticula and calculus in terminal part of common bile duct, hybrid technique with rendez-vous procedure allowed successful lithoextraction and cholecystectomy. The follow-up examination found no residual biliary calculi. Hybrid laparoendoscopic intervention in the treatment of a patient with choledocholithiasis, cholecystitis an

INTRODUCTION: Cholecystectomy is one of the most common operations in surgical departments. Complications after gallbladder removal are mainly bleeding, infection including abscess in the gallbladder bed or in the abdominal wall, wound dehiscence, acute pancreatitis or injury of the bile ducts. In the further course, hernias in the scar may appear after both laparoscopic and open cholecystectomy, strictures of the bile ducts and symptoms of the so-called postcholecystectomy syndrome. The presence of residual gallstones is rare, statistically reported in 0.08-0.3%.

Gallstone disease (GSD) is a common gastrointestinal disorder affecting approximately 10-20% of the global adult population, characterized by the presence of gallstones, predominantly cholesterol-based, in the gallbladder and/or biliary ducts. While many patients remain asymptomatic, more than 20% develop clinical symptoms such as abdominal pain, nausea, vomiting, jaundice, and anorexia, potentially leading to severe complications like acute cholecystitis and biliary pancreatitis. GSD has a significant genetic predisposition, with the variable prevalence of the disease according to ethnicity b
