Case Report: Gallbladder perforation after endoscopic retrograde cholangiopancreatography-a rare complication.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is a state-of-the-art diagnostic and therapeutic procedure for various pancreatic and biliary problems. In spite of the well-established safety of the procedure, there is still a risk of complications such as pancreatitis, cholangitis, bleeding and perforation. Gallbladder perforation has rarely been reported in association with ERCP. CASE PRESENTATION: A 58-year-old male with pancreatic cancer was admitted for jaundice and underwent ERCP with fully covered self-expanding metal stent (FCSEMS) placement for obstructive jaundice,

Defining endpoints in percutaneous cholecystostomy: Catheter management, patient survival, and long-term outcomes from a twelve-year retrospective study.

BACKGROUND: Percutaneous cholecystostomy (PCT) is widely used for high-risk acute cholecystitis as an alternative to emergent cholecystectomy. Despite its effectiveness, the optimal timing, catheter management, and long-term outcomes remain to be elucidated. AIM: To characterize timing, catheter management, survival, and follow-up outcomes after PCT in a high-risk cohort. METHODS: This single center retrospective cohort study included consecutive adult patients undergoing PCT placement for acute cholecystitis at a community hospital setting in New York between January 2012 and December 2024.

Advances in Imaging and Diagnosis of Emphysematous Cholecystitis.

Emphysematous cholecystitis is a rare but severe variant of acute cholecystitis characterized by gas-forming organisms within the gallbladder wall or lumen. It progresses rapidly and carries substantial mortality, making early and accurate recognition essential. Although its pathogenesis involves gallbladder wall ischemia with superimposed infection by gas-producing bacteria-most commonly Clostridium species-the clinical presentation is often nonspecific, particularly in patients with diabetes mellitus or immunosuppression. Imaging therefore serves as the cornerstone of diagnosis. Abdominal ra

Adverse hepato-biliary-pancreatic events in acromegaly patients treated with first generation somatostatin receptor ligands.

First-generation somatostatin receptor ligands (SRLs) are first-line medical therapy for acromegaly. During long-term treatment, hepato-biliary-pancreatic adverse events can occur. This study aimed to evaluate the prevalence and predictors of hepato-biliary-pancreatic adverse events during SRL-treatment.In this multicenter study, data of 371 acromegaly patients (223 females) were retrospectively collected at the start of SRL therapy (T0), and at the last follow-up visit (120 ± 97.31 months). The occurrence of hepato-biliary-pancreatic adverse events and their relationship with features at T0 w

Left-Sided Abdominal Pain Revealing Acute Cholecystitis in Situs Inversus Totalis: Diagnostic Pitfalls and Surgical Considerations.

Situs inversus totalis is a rare congenital condition that can complicate the diagnosis and management of acute abdominal pathology due to atypical symptom localization. Acute cholecystitis in this setting may present with left-sided abdominal pain, increasing the risk of misdiagnosis and delayed treatment. The scientific rationale for this case report is to emphasize the importance of recognizing anatomical variants when evaluating atypical abdominal presentations. We report the case of a 65-year-old woman who presented with left upper quadrant pain, nausea, and vomiting. Imaging studies demo

Acute acalculous cholecystitis and dengue fever: a case report from Australia.

INTRODUCTION AND IMPORTANCE: Acute acalculous cholecystitis (AAC) can be a life-threatening condition, particularly in critically ill patients. The pathophysiology appears to be secondary to cholestasis, hypoperfusion, or ischemia. Dengue fever is a rare cause of AAC. PRESENTATION OF CASE: We present the case of a 44-year-old female with non-specific symptoms including right upper quadrant abdominal pain, fatigue, anorexia, vomiting, and fevers. Her clinical examination was consistent with tenderness in the right upper quadrant, with a positive Murphy's sign. Given her deranged biochemical ma

Single-Incision Laparoscopic Cholecystectomy in a Patient with a Left Ventricular Assist Device.

INTRODUCTION: Laparoscopic cholecystectomy (LC) is the standard treatment for benign gallbladder diseases, including acute cholecystitis (AC). However, performing LC in patients with a left ventricular assist device (LVAD) presents specific technical and physiological challenges related to driveline preservation and hemodynamic stability. We report the first case of single-incision laparoscopic cholecystectomy (SILC) via the right lower abdomen in a patient with an LVAD after treatment for biliary tract infection, following prior surgery for strangulated ileus and umbilical wound infection. Cholecystectomy or non-operative management for cholecystitis in elderly patients: An analysis based on hospital practice patterns.

BACKGROUND AND AIMS: Early cholecystectomy improves outcomes in acute cholecystitis, yet surgeons often hesitate to operate on elderly and multimorbid patients, and prospective data in this group are lacking. This cohort study compared outcomes across hospitals with differing operative policies to estimate the effect of surgery among the elderly. METHODS: Retrospective data using the Swedish National Inpatient Register, including 12,481 patients aged ⩾ 70 years treated for cholecystitis between 2015 and 2019, were analyzed. Outcomes included all-cause and gallstone-related mortality, length o

Acute abdomen secondary to perforated jejunal diverticulitis: an unusual clinical presentation - case report.

INTRODUCTION: Jejunal diverticulosis is an uncommon condition, usually asymptomatic that could lead to severe complications such as perforation, associated with high mortality, and its presentation can mimic other acute abdominal or even urologic disorders, often requiring urgent surgery for both diagnosis and treatment. CASE REPORT: A 68-year-old woman with no significant medical history presented with sudden-onset severe colicky pain in the left lumbar and iliac regions, initially resembling renal colic. Laboratory tests revealed leukocytosis and elevated CRP. A non-contrast abdominal CT ru

Endoscopic Ultrasound-Guided Gallbladder Drainage Using Lumen-Apposing Metal Stents for Non-Surgical Candidates With Cholecystitis: A Prospective Cohort Study.

INTRODUCTION: Management of acute cholecystitis (AC) in high-risk surgical candidates is challenging. Drainage via percutaneous transhepatic gallbladder drainage (PT-GBD) has been standard of care, with high complication and re-intervention rates. Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) using lumen-apposing metal stents (LAMS) is proposed as an alternative. This study evaluates the efficacy and safety of LAMS in EUS-GBD for non-surgical candidates presenting with AC. METHODS: An institution-based prospective database has tracked all patients who undergo EUS-GBD using LAMS

Case Report: A case study of geriatric acute cholecystitis complicated by coinfection with Shewanella putrefaciens and Enterococcus faecium.

BACKGROUND: Acute cholecystitis is a common abdominal condition mainly caused by enteric Gram-negative bacilli and Enterococcus species. Advances in microbial detection have highlighted infections by rare pathogens like Shewanella putrefaciens (S. putrefaciens), an opportunistic bacterium from aquatic environments affecting mainly immunocompromised or comorbid patients. Its clinical features, antibiotic resistance, and treatment remain unclear. CASE PRESENTATION: This article presents a case study of an 87-year-old female patient with a medical history of gallstones and previous endoscopic re

Displacement of Hem-o-lok clips into the common bile duct after LCBDE: a case report.

BACKGROUND: Laparoscopic cholecystectomy (LC) combined with laparoscopic common bile duct exploration (LCBDE) is the standard treatment for hepatobiliary calculi. While postoperative complications have decreased with technical maturation, migration of polymer ligation clips (e.g., Hem-o-lok) remains an underrecognized yet serious complication that can serve as a nidus for recurrent stone formation and cholangitis, necessitating heightened clinical awareness. METHODS: We present a case of a 76-year-old female who presented with fever and chills three years after initial LCBDE. Imaging studies

Radiologic predictors of early recurrence of acute cholecystitis following percutaneous cholecystostomy tube removal.

BACKGROUND: Percutaneous cholecystostomy (PC) is commonly utilized in high-risk patients with acute cholecystitis as either a bridge to surgery or as definitive therapy. However, early recurrence following tube removal remains a significant clinical concern. This study aimed to identify imaging predictors associated with early recurrence after PC tube removal. METHODS: A total of 1,002 patients who underwent PC between 2008 and 2024 were retrospectively reviewed. Patients who subsequently underwent cholecystectomy (n = 606), were discharged with the tube in place (n = 221), or did not undergo

Endoscopic management of biliary stricture in primary sclerosing cholangitis.

Primary sclerosing cholangitis (PSC) is a progressive fibroinflammatory disease characterized by multifocal biliary strictures, recurrent cholangitis, and a markedly increased lifetime risk of cholangiocarcinoma (CCA). Endoscopic retrograde cholangiopancreatography (ERCP) remains central to the diagnosis of CCA and management of PSC-related complications. This review synthesizes current evidence guiding the use of ERCP in patients with PSC, highlighting the importance of careful patient selection to mitigate adverse events. We review traditional ERCP techniques for the evaluation of dominant s

Ventriculo-gallbladder shunt reflux: A case study and review of the literature.

BACKGROUND: Ventriculo-gallbladder (VGB) shunt represents an uncommon yet well-documented surgical intervention for the treatment of hydrocephalus through cerebrospinal fluid (CSF) diversion into the gastrointestinal tract through the gallbladder. When other ventricular shunt types have failed or are contraindicated, VGB shunts are effective. In exceptionally rare circumstances, biliary reflux can occur and cause accumulation of bile in the ventricular system. The authors report a case of bile reflux through a ventricular gallbladder shunt with eventual patient recovery after surgical interven

Spinal Cord Injury Without Radiographic Abnormality Complicated by Acute Cholecystitis: A Case Report Highlighting Diagnostic and Therapeutic Challenges.

A man in his early 90s was brought to our emergency department after a fall in which he struck his left shoulder and subsequently became unable to move his left upper extremity. On arrival, neurological examination revealed muscle weakness and paresthesia predominantly affecting the left upper and lower extremities below the C6 level. Cervical magnetic resonance imaging demonstrated narrowing of the spinal canal at the C4/5 and C5/6 levels, corresponding to the level of neurological deficits, along with multilevel left-sided foraminal stenosis, leading to a diagnosis of spinal cord injury with

Robotic reoperative management of recurrent gallbladder disease after subtotal cholecystectomy: a contemporary single-institution experience.

BACKGROUND: The increasing adoption of incomplete cholecystectomy in severe cholecystitis has created a growing population of patients who later develop recurrent symptoms from remnant gallbladder or cystic duct pathology. Reoperative cholecystectomy is definitive but technically challenging, and the role of robotic surgery in this setting remains poorly defined. METHODS: A retrospective cohort study was performed of adult patients undergoing robot-assisted completion or remnant cholecystectomy for recurrent gallbladder disease at a single tertiary institution between 2022 and 2024.
