Intramural Duodenal Abscess Masquerading as Malignancy: Diagnosed and Managed by Endoscopic Ultrasound.

Subepithelial lesions of the gastrointestinal tract are frequently encountered and are most often benign. Among the etiologies of subepithelial lesions, duodenal abscesses are exceptionally uncommon. Duodenal abscesses may have imaging characteristics that overlap with malignancy. A patient with history of necrotizing cholecystitis presented with abdominal pain 3 months after his cholecystectomy. Cross-sectional imaging revealed suspected duodenal malignancy. Endoscopic ultrasound identified a subepithelial lesion allowing for diagnosis and treatment of an intramural duodenal abscess. This cas

Incidentally Detected Gallbladder Adenocarcinoma Presenting as Acute Calculous Cholecystitis: A Case Report and Management Considerations.

Gallbladder cancer is an aggressive malignancy of the biliary tract with a poor prognosis and variable geographic distribution. It may be identified incidentally after a cholecystectomy performed for presumed benign disease, with diagnosis established on final histopathological examination. Surgeons should be aware of this possibility, as incidental detection has important implications for staging and subsequent management. Early recognition and appropriate evaluation are essential to guide treatment and improve outcomes.

Evaluation of microbiological cultures in patients with acute cholecystitis.

BACKGROUND AND AIMS: Acute cholecystitis (AC) requires emergency care. Besides surgical interventions, adequate empirical antibiotic therapy is indispensable. We aimed to analyze bile samples obtained during surgical interventions for AC. The primary aim was determine sampling frequency, pathogen prevalence (including ESKAPE strains), and to create the cumulative antibiogram of the most common bacteria (including MDR pathogens). A secondary aim was to explore the potential relationship between pathogen type (ESKAPE, MDR) and different patient characteristics in acute cholecystitis. METHODS: A

Association between the Parkland grading scale score and postoperative complications according to the Clavien-Dindo classification in emergency laparoscopic cholecystectomy.

Laparoscopic cholecystectomy is the standard treatment for acute calculous cholecystitis. The Parkland grading scale was developed to assess intraoperative severity; however, its association with postoperative outcomes remains underexplored in low- and middle-income countries. This study aimed to evaluate the associations between the Parkland grading scale and postoperative complications according to the Clavien–Dindo classification in emergency laparoscopic cholecystectomy. A retrospective observational study was conducted including patients who underwent emergency laparoscopic cholecystectom

Endoscopic ultrasound-guided gallbladder drainage in acute cholecystitis with contained perforation: a prospective cohort study.

BACKGROUND: Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) is usually contraindicated in perforated cholecystitis, although initial reports suggest feasibility in contained perforation. METHODS: This prospective single-center cohort study aimed to explore EUS-GBD outcomes in patients with acute cholecystitis with contained perforation (cp-AC; Niemeier classification type 2). A secondary exploratory comparison was performed with a contemporaneous cohort of patients with intact gallbladder walls. RESULTS: Between January 2021 and March 2025, 23 patients with cp-AC (39.7% of the ov

"Results of the structured implementation of a robotic acute care surgery program".

The adoption of robotic surgery in emergency general surgery has traditionally been limited to sporadic use, mainly because of organizational, logistical, and training-related barriers. Evidence regarding the structured implementation of robotic platforms in a continuous 24/7 emergency setting is scarce. This study aimed to evaluate the implementation and early operational performance of a structured robotic acute care surgery program. We conducted a prospective observational study to describe the implementation and early outcomes of a structured 24/7 robotic acute care surgery program at a te

Parallel Configuration of the Cystic and Common Hepatic Ducts: a Surgical Odyssey.

INTRODUCTION: Laparoscopic cholecystectomy is among the most frequently performed surgical procedures worldwide; however, it carries a persistent risk of iatrogenic bile duct injury, particularly in the presence of anatomical variations. Misinterpretation of biliary anatomy remains a leading cause of serious complications, underscoring the importance of recognizing rare variants and adopting safe surgical strategies. CASE REPORT: We report the case of a 78-year-old patient undergoing elective laparoscopic cholecystectomy for recurrent cholecystitis and cholangitis. Given the history of repeat

Outcomes of Spiral Versus Conventional Double Pigtail Plastic Stents in Permanent Transpapillary Gallbladder Drainage for Acute Cholecystitis.

PURPOSE: Endoscopic gallbladder stenting is an important definitive treatment option for acute cholecystitis in high-risk patients who are poor surgical candidates. In this study, we aimed to compare the outcomes between a novel 5-Fr spiral-shaped plastic stent (IYO-stent) and conventional 7-Fr double-pigtail stent (C-stent) for permanent transpapillary gallbladder drainage. METHODS: This retrospective cohort study included 56 patients with acute cholecystitis who underwent permanent endoscopic gallbladder stenting at 2 Japanese centers between 2019 and 2023 (C-stent, n = 27; IYO-stent, n = 2

A Case of Remnant Cholecystitis with an Endoscopically Achieved Definitive Diagnosis.

Remnant cholecystitis caused by cystic duct stones is a rare cause of post-cholecystectomy syndrome and it is often difficult to diagnose. We report the case of a man in his seventies with recurrent epigastric pain one year after laparoscopic cholecystectomy. Computed tomography revealed a 10-mm cystic duct stone. Endoscopic retrograde cholangiography (ERC) demonstrated macroscopic purulent bile from the remnant cystic duct, whereas bile from the common bile duct was grossly normal. This confirmed the diagnosis of localized remnant cholecystitis, without the need for cholangioscopy. ERC-guided

Outcomes are equivalent between robotic and laparoscopic cholecystectomy in all grades of acute cholecystitis.

BACKGROUND: Robotic surgery is increasingly being adopted in emergency general surgery. The impact of robotic surgery on outcomes based on disease severity is unknown. We hypothesize that robotic cholecystectomy would be associated with better outcomes than laparoscopic cholecystectomy in severe acute cholecystitis. METHODS: A single-center, retrospective analysis was conducted involving patients undergoing either laparoscopic or robotic cholecystectomy for acute cholecystitis from November 2022 to July 2024. Disease severity was categorized by the American Association for the Surgery of Trau

Operative Decision Discordance in Geriatric Emergency General Surgery.

Nearly 1 million older adults are hospitalized annually for emergency general surgery (EGS) conditions in the United States. Recent literature has demonstrated the impact of discordant care in EGS, defined as a mismatch between treatment choice and patients' clinical risk profile. In the present work, we characterize the association between preoperative decision variation and outcomes in elderly EGS patients.MethodsAll geriatric (≥65 years) hospitalizations with EGS conditions (appendicitis, diverticulitis, cholecystitis, hernia, bowel obstruction, perforated peptic ulcer, intestinal ischemia,

Endoscopic Ultrasound-Guided Gallbladder Drainage Using a 19-Gauge Needle and a Modified Slim Metal Stent: A Simplified Approach (With Video).

Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) is a promising therapeutic option for surgically unfit patients with acute cholecystitis. Data on the use of fully covered self-expandable metal stents (FCSEMSs) in this setting remain limited, especially in the cancer population. We describe a simplified needle-to-stent EUS-GBD technique and report its early clinical outcomes using a 19-gauge Franseen-tip FNB needle and a modified slim FCSEMS. We applied this technique in consecutive patients with acute cholecystitis secondary to malignant biliary obstruction between September 2022 a

Alpha-ketoglutarate alleviates acute pancreatitis by restoring energy metabolism.

Acute pancreatitis (AP) lacks specific therapies. This study identified dysregulated metabolites in AP and investigated the therapeutic potential of α-ketoglutaric acid (AKG). Metabolomic profiling of human serum revealed 76 AP-specific metabolites compared to acute cholecystitis and normal control, with AKG significantly elevated and correlated with disease severity. In a severe AP mouse model, oral AKG supplementation attenuated pancreatic injury, systemic inflammation, and multi-organ damage. Mechanistically, AKG restored pancreatic energy metabolism and mitochondrial structure while reacti
