A Mendelian randomization study exploring the genetic associations between biliary system disorders and brain structural changes.

The objective was to evaluate potential genetic associations between biliary system disorders and cortical and subcortical brain structural changes using Mendelian randomization analyses. This study focused on 4 diseases, including primary sclerosing cholangitis, cholecystitis, intrahepatic cholangiocarcinoma, and gallstone disease, and total bilirubin levels as indicators of biliary system disorders. Genome-wide association studies summary statistics from the ENIGMA consortium were utilized to assess the directional associations between these biliary system diseases and changes in brain struc

[Bacterial cholecystitis with concurrent hypothyroidism in a mixed-breed dog].

A six-year-old, neutered male mixed-breed dog was presented with jaundice, fever, and abdominal pain. A bacterial cholecystitis was diagnosed and subsequently treated with antimicrobials. However, follow-up evaluation showed inadequate disease control. Over time, the dog gained weight and developed dermatological changes indicative of an underlying endocrinopathy. Subsequent diagnostics confirmed hypothyroidism, necessitating levothyroxine supplementation. In the course of the treatment of hypothyroidism the dog developed complete resolution of the bacterial cholecystitis. This case report desc

Antibiotic treatment for 1 day versus 4-7 days in patients with acute cholangitis after adequate endoscopic biliary drainage (COBRA): study protocol for a randomized controlled trial.

BACKGROUND: Acute cholangitis is an infection of the biliary tract that is managed with adequate biliary drainage combined with antibiotic treatment. The international Tokyo Guidelines 2018 recommend 4 to 7 days of antibiotic treatment after adequate biliary drainage, but observational data suggest shorter treatment may be sufficient. We assessed whether 1 day of antibiotic treatment is non-inferior to 4-7 days of antibiotic treatment for acute cholangitis after adequate biliary drainage. METHODS: The COBRA-trial is a multicentre, open-label, parallel group randomized controlled non-inferiori

Percutaneous vs. surgical management in acute cholecystitis: addressing selection bias in meta-analyses.

This commentary addresses the recent research titled "Comparing percutaneous treatment and cholecystectomy outcomes in acute cholecystitis patients: a systematic review and meta-analysis" published in the World Journal of Emergency Surgery. The review confirms that cholecystectomy (CC), particularly laparoscopic, is associated with lower mortality and readmission rates compared to percutaneous cholecystostomy (PC). However, this commentary emphasizes that the interpretation of these findings must account for inherent selection bias: in clinical practice, PC is typically reserved for higher-ris

Guideline-based, but not error-free: Multilingual risks in AI-powered patient counseling on gallstones.

BACKGROUND: Patients increasingly use large language models (LLMs) for health information, yet the guideline concordance and safety of patient-facing outputs-particularly across languages-remain uncertain. We evaluated three widely used LLM platforms (web interfaces) and their underlying default models for gallstone-related counseling in Turkish and English. METHODS: In this cross-sectional content analysis, 14 real-world, guideline-mappable patient questions were developed in Turkish and translated into semantically equivalent English. Each question was submitted once to ChatGPT (ChatGPT-4o

Acute Acalculous Cholecystitis as the First Manifestation of Systemic Lupus Erythematosus in a Young Man: A Case Report and Review of Literature.

Acute acalculous cholecystitis can rarely be the initial manifestation of systemic lupus erythematosus. Awareness of this atypical presentation is essential to avoid unnecessary surgical intervention. Prompt recognition and initiation of immunosuppressive therapy may resolve symptoms and help establish the correct underlying diagnosis.

Squamous Cell Carcinoma of the Gallbladder With Recurrence at the Tumor Spillage Site Following Laparoscopic Cholecystectomy.

Squamous cell carcinoma of the gallbladder is a rare and aggressive biliary malignancy, typically associated with locally invasive disease and poor outcomes, whereas most gallbladder cancers are adenocarcinomas staged primarily by depth of invasion. We report the case of a 76-year-old woman who presented with acute calculous cholecystitis and choledocholithiasis and underwent laparoscopic cholecystectomy followed by endoscopic retrograde cholangiopancreatography. During cholecystectomy for a severely inflamed and friable gallbladder, intraoperative gallbladder perforation with bile and gallsto

Laparoscopic Cholecystectomy for Acute Cholecystitis: Defining the Golden Period.

Background/aims Laparoscopic cholecystectomy (LC) performed within the first week of acute cholecystitis (AC) is generally regarded as safe and effective, while delayed surgery is often recommended for unresolved or complicated cases. This study assessed perioperative outcomes of LC performed during the first week versus two to six weeks following symptom onset. Methods This prospective observational comparative study was conducted at a tertiary care center from January 2021 to December 2024 using data from a prospectively maintained database. Patients with acute calculous cholecystitis who un

Case Report: Congenital gallbladder agenesis.

The congenital gallbladder agenesis presents a diagnostic challenge due to its atypical clinical features, often leading to misdiagnosis or delayed diagnosis and ultimately unnecessary surgeries. The role of MRI in this condition cannot be overstated. We report a case where MRI preoperatively diagnosed congenital gallbladder agenesis. Initially, routine physical examination ultrasonography misdiagnosed the patient with a gallbladder full of stones and atrophic cholecystitis, leading the patient to seek surgical treatment and get admitted to the hospital. After admission, further abdominal MRI

Interventional management of acute perforated cholecystitis: When is percutaneous transhepatic cholecystostomy a reasonable therapeutic option?

In the case of acute perforated cholecystitis, rapid control of the infection site is important for patient outcomes. For free perforation with generalized peritonitis, emergency laparoscopic cholecystectomy remains the gold standard therapy. Percutaneous transhepatic cholecystostomy (PTC) is an alternative damage-control option, particularly for frail, septic, or profoundly comorbid patients, either as a bridge to (early or interval) cholecystectomy or, in truly inoperable patients, as definitive palliation. Gallbladder perforations are categorized on the basis of the Niemeier classification.

Feasibility and safety of endoscopic ultrasound-guided gallbladder drainage for Niemeier type II acute perforated cholecystitis: a multicenter retrospective pilot study.

BACKGROUND: Acute perforated cholecystitis (APC) requires timely intervention, and endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) has emerged as a minimally invasive option for high-risk surgical candidates. This multicenter study evaluated the feasibility and safety of EUS-GBD in patients with Niemeier type II APC. METHODS: Patients with type II APC who underwent EUS-GBD between January 2017 and December 2024 at five tertiary centers were retrospectively analyzed. The primary outcome was technical success; secondary outcomes included clinical success, adverse events, stent paten

The 48-h window of opportunity: patient and health system-related predictors of surgical severity in symptomatic cholelithiasis.

INTRODUCTION: Cholelithiasis is a primary cause of surgical morbidity in Ecuador. While the Parkland Grading Scale (PGS) is a validated tool for intraoperative severity, the influence of time-to-surgery and healthcare system barriers remains poorly understood in regional contexts. This study aimed to evaluate the association between patient-dependent factors, health system-related factors, and surgical severity in Cuenca, Ecuador. METHODS: This analytical, retrospective cross-sectional study included 259 patients (> 15 years) who underwent laparoscopic cholecystectomy at a tertiary hospital i

Artificial intelligence in the recognition of stages in live laparoscopic cholecystectomy.

Laparoscopic cholecystectomy is the gold standard for treating symptomatic gallstones. Despite established safety strategies, variability in their adoption leads to inconsistent outcomes. Artificial intelligence (AI) offers potential for real-time recognition of procedural stages, contributing to surgical standardization and safety. A prospective observational study was conducted between February 2023 and March 2024. Patients undergoing laparoscopic cholecystectomy for symptomatic gallstones or class 1 cholecystitis (Tokyo classification) were included. From 141 surgical videos, 131 were used

Perioperative Antimicrobial Prophylaxis in Elective and High-Risk Laparoscopic Cholecystectomy: A Narrative Review.

Laparoscopic cholecystectomy (LC) is one of the most frequently performed general surgical procedures. Surgical site infection (SSI) rates are low in low-risk elective cases, yet practice variation in perioperative antimicrobial prophylaxis remains considerable. Questions persist regarding the necessity of prophylaxis. Optimising antimicrobial use in LC may represent an area of interest for antimicrobial stewardship within general surgery. A narrative review of literature published between 2010 and 2025 was conducted using MEDLINE, Embase and Cochrane databases. Randomised controlled trials, o

Management of percutaneous cholecystostomy drains: a survey of real-world practices across Ireland and the UK.

INTRODUCTION: Acute calculous cholecystitis (ACC) is a common surgical emergency with varying severity. The Tokyo Guidelines stratified ACC into grades I-III based on severity. Patients with grade III ACC and high ASA scores can be managed with percutaneous cholecystostomy drain (PCD) insertion to control sepsis. There are currently no guidelines in the literature concerning PCD management. This questionnaire highlights the current real-life practices of PCD across Ireland and the UK. METHODS: The Irish Surgical Research Collaborative sought to explore PCD practices in Ireland and the UK. Thi
