A rare twist in the abdomen: diagnostic and therapeutic approaches to omental infarction - a case report and literature review.

Intraperitoneal focal fat infarction (IFFI) encompasses a group of rare conditions that are clinically and radiologically similar, arising from focal fatty tissue necrosis. These entities often mimic other acute abdominal conditions such as acute appendicitis or cholecystitis. We present the case of a 65-year-old female with progressive abdominal pain, ultimately diagnosed with IFFI using contrast-enhanced computed tomography (CT). Omental infarction (OI) was the leading diagnosis, though a definitive distinction from epiploic appendagitis (EA) could not be made on imaging. Conservative manage

Endoscopic Transpapillary Gallbladder Drainage Reduces the Frequency of Biliopancreatic Events in the Waiting Period for Surgery, Even in Patients without Comorbid Diseases.

INTRODUCTION: In patients with acute cholecystitis and choledocholithiasis undergoing endoscopic retrograde cholangiopancreatography (ERCP) followed by interval cholecystectomy (6 weeks-3 months), preoperative biliopancreatic events are reported in 18.3-41.8%. Endoscopic transpapillary gallbladder drainage (ET-GBD) is indicated in this patient group if there are comorbidities preventing surgery. This study compared biliopancreatic events during the interval to cholecystectomy in patients who underwent ET-GBD despite being surgically fit versus those treated with ERCP alone. METHODS: In this r

Endoscopic Ultrasound-Guided Lumen-Apposing Metal Stent Drainage in Benign Pancreatobiliary and Gastrointestinal Disease: Evolving Techniques and Clinical Outcomes.

Interventional endoscopic ultrasound (EUS) has become a cornerstone in the management of malignant pancreatobiliary diseases, offering minimally invasive alternatives to traditional surgical approaches. More recently, accumulating evidence supports its expanding role in the treatment of benign pancreatobiliary conditions, including acute cholecystitis and pancreatitis, benign gastric outlet obstruction, and scenarios involving altered gastrointestinal anatomy. This narrative review provides an overview of key EUS-guided drainage techniques utilizing lumen-apposing metal stents (LAMSs) in benig

Multi-Hole Self-Expandable Metallic Stent for Malignant Distal Biliary Obstruction: A Literature Review.

Endoscopic biliary drainage using self-expanding metal stents (SEMSs) is a standard palliative therapy for cholangitis and obstructive jaundice caused by malignant distal biliary obstruction (MDBO). Fully-covered SEMSs (FC-SEMSs) prevent tumor ingrowth and provide longer patency; however, recent advances in chemotherapy have increased stent migration due to tumor shrinkage, resulting in reduced functional patency compared with uncovered SEMSs. Partially covered SEMSs can reduce migration but are often difficult to remove after deployment. In addition, adverse events such as acute pancreatitis

Point-of-Care Ultrasound Diagnosis of Emphysematous Cholecystitis.

Emphysematous cholecystitis (EC) is a form of cholecystitis characterized by the presence of intramural or intraluminal gallbladder gas secondary to infection by gas-forming bacteriaand is considered a surgical emergency. Computed tomography (CT) is considered the gold standard for the diagnosis of EC, but it can result in increased time to diagnosis and may not be readily available in the emergency department (ED). This delay in diagnosis may contribute to increased rates of gallbladder perforation and associated mortality. As emergency medicine door-to-first-provider times continue to increa

Outcomes and Risk Factors for Bacteriobilia in Cancer Patients Undergoing Endoscopic Retrograde Cholangiopancreatography of Malignant Biliary Obstruction at a Teaching Hospital in Northern India.

BACKGROUND: Malignant biliary obstruction (MBO) is a common complication in cases of locally or systemically advanced gastrointestinal malignancies. Placing a biliary stent is a method to relieve blockages in the biliary tract with fewer chances of bacteriobilia after endoscopic retrograde cholangiopancreatography (ERCP). MATERIALS AND METHODS: This retrospective study was carried out in the Department of Microbiology at a teaching hospital. The hospital information system and the case files for the ward data entry were used to collect the clinical data of every patient. Records of management

Adoption of robotic interval cholecystectomy: a retrospective comparison with the laparoscopic approach at a single center.

BACKGROUND: Robotic-assisted cholecystectomy has gained popularity due to its purported advantages over the traditional laparoscopic technique. However, studies to date have not consistently demonstrated improved patient outcomes. Concerns regarding increased costs, particularly when the platform's clinical benefit is unclear, have limited its adoption. To our knowledge, there has been no study focusing on the role of robotic technique in interval cholecystectomy, which may present greater technical challenges, and the advantages offered by robotic technology may lead to improved outcomes. Post-cholecystectomy Choleperitoneum Due to Cystic Duct Stump Leak: A Case Report Emphasizing Diagnostic Challenges and Multidisciplinary Management.

Biliary peritonitis, also known as choleperitoneum, is a rare but severe complication of cholecystectomy. The symptoms are non-specific, which can cause a delay in the diagnosis, leading to potential sepsis and multi-organ failure. A 45-year-old female underwent robotic cholecystectomy for gallstone pancreatitis and cholecystitis. One week post-operatively, she presented again with abdominal pain and distention, rapidly progressing to septic shock with respiratory, renal, and neurological failure. Initial imaging showed only mild ascites. Imaging showed a biliary leak, and paracentesis confirm

Incidental Discovery and Removal of an Appendiceal Mucocele in a Patient With Acute Cholecystitis.

Appendiceal mucoceles (AMs) are a rare clinical entity among appendiceal tumors. They are characterized by dilation of the appendix and accumulation of mucus within the lumen. These lesions are often discovered incidentally during imaging or surgery for unrelated abdominal complaints. We report a case of a 55-year-old male who presented with right upper quadrant pain due to acute cholecystitis. An AM and a hepatic lesion were incidentally identified on abdominal CT. The patient underwent a robotic-assisted laparoscopic cholecystectomy and appendectomy, with pathology confirming a benign AM and
