Bouveret Syndrome Mimicking Gallbladder Malignancy: A Diagnostic Challenge in an 85-Year-Old Woman.

Bouveret syndrome is a rare complication of cholelithiasis in which a gallstone enters the duodenum via a cholecystoenteric fistula, causing gastric outlet obstruction. Because of chronic inflammation and nonspecific symptoms, the condition can easily be misdiagnosed as a gastrointestinal malignancy. An 85-year-old woman presented with severe weight loss (10 kg), night sweats, and non-bloody vomiting. CT imaging revealed massive aerobilia, a thickened gallbladder wall, and an ill-defined mass at the gastroduodenal junction. These findings raised strong suspicion for a malignant process in the

Abdominal Wall Abscess Due to Retained Gallstone After Laparoscopic Cholecystectomy: A Case Report.

Laparoscopic cholecystectomy (LC) is the gold standard treatment for cholelithiasis and cholecystitis; however, gallbladder perforation with spillage of gallstones may occur and result in delayed complications. We present the case of a 47-year-old woman who developed a chronic right abdominal wall mass and pain eight years after LC. Imaging revealed a right subdiaphragmatic collection extending into the abdominal wall, initially raising concern for a neoplastic process. Biopsies were inconclusive, and due to persistent symptoms, laparoscopic exploration was performed, revealing severe adhesion

Gastrointestinal adverse effects of GLP-1 receptor agonists and dual GIP/GLP-1 receptor agonists: A narrative expert review of approved therapies, oral formulations, and pipeline agents.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and dual glucose-dependent insulinotropic polypeptide (GIP)/GLP-1 receptor agonists have transformed the management of type 2 diabetes and obesity. Gastrointestinal (GI) adverse events are the most common limitation of these therapies and a leading cause of discontinuation. METHODS: This narrative expert review synthesizes evidence from randomized controlled trials, observational studies, meta-analyses, and pharmacovigilance data to characterize the incidence, mechanisms, and clinical implications of GI adverse effects across a

Current Status of PSMA-Targeted Agents Based on Small Molecules for Prostate Cancer in Preclinical Studies.

BACKGROUND: National conversation has justifiably been concerned with firearm-related deaths and much less attention has been paid to the consequences of surviving a firearm injury. We assessed the risk of hospital readmission, length of stay (LOS) during hospitalization, and costs within 90-days after surviving an index firearm injury and compared these data with pedestrians and occupants involved in motor vehicle crash (MVC). METHODS: Nationwide Readmission Database, a nationally representative readmission database from 2013 and 2014 was used to create a retrospective cohort study. The prim

Using Remote Behavioral Skills Training to Teach University Students Literature Searching Skills.

BACKGROUND: The presence of bacteria in bile is an important factor in the formation of pigment gallstones. The bile of healthy people is sterile and bacteria in the biliary system come from endogenous infection from the gut. Yet, the route of bacterial translocation into the bile duct is still unclear. Theoretically, two routes exist: one is through the intestinal barrier and the other is by direct reflux from the sphincter of Oddi. This study was undertaken to explore the relationship between the effectiveness of intestinal barrier and the formation of pigment gallstones in hamsters. METHOD

Transcystic papillary balloon dilatation vs. laparoscopic choledochoscopy for choledocholithiasis: a retrospective observational propensity score-matched cohort study.

BACKGROUND: Single-stage management of choledocholithiasis during laparoscopic cholecystectomy can be performed using transcystic papillary balloon dilatation (TCPBD) or laparoscopic choledochoscopy (LCC). Direct comparative data between intraoperative clearance techniques remain limited. METHODS: We conducted a retrospective single-center observational cohort study of 132 consecutive adult patients undergoing documented one-stage minimally invasive treatment for choledocholithiasis between January 2020 and January 2026. Ninety-three patients underwent TCPBD and 39 underwent LCC. The primary

HMGCR-related SNPs and cholelithiasis: statins may exert effects beyond lipid-lowering.

BACKGROUND: As traditional lipid-lowering drugs, the role of statins in reducing the incidence of cholelithiasis remains controversial. The aim of this study is to utilize the single nucleotide polymorphisms (SNPs) related to the target of statins, HMGCR, to simulate the effect of statins and explore the causal relationship between statins and cholelithiasis. METHODS: Drug-targeted Mendelian randomization (MR) was first applied to study the causal relationship between the application of statins and the risk of cholelithiasis. Subsequently, we explored the causal relationship between other lip

The Scope of Percutaneous Biliary Cholangioscopy: Beyond Choledocholithiasis.

OBJECTIVE: To discuss less well-defined indications for percutaneous cholangioscopy (PC) that have been described in case reports and other studies. The role of PC is expanding in the treatment of biliary and pancreatic pathologies and presents unique advantages and drawbacks when compared to other endovascular diagnostic and therapeutic modalities. The utility of cholangioscopy in the treatment of choledocholithiasis has been well defined, and therefore here we focus on other indications. METHODS: A thematic review was conducted by searching PubMed for studies reporting indications for PC si

Reasons for delayed medical treatment among cholelithiasis patients: a qualitative study based on the health belief model.

BACKGROUND: Delayed treatment of cholelithiasis can lead to serious complications, missed optimal intervention windows, and increased treatment complexity, thereby imposing a greater disease burden on patients. Understanding the reasons for delayed healthcare-seeking from the patient’s perspective is therefore essential. This study aimed to explore the multidimensional factors contributing to delayed medical treatment among patients with cholelithiasis—guided by the Health Belief Model (HBM)—to identify potential targets for intervention and provide a theoretical foundation for optimizing care

Fluoroscopy-free direct solitary cholangioscopy versus endoscopic retrograde cholangiography for clearance of noncomplex biliary stones: a noninferiority randomized controlled trial.

BACKGROUND AND AIMS: Endoscopic removal of noncomplex bile-duct stones is typically accomplished by endoscopic retrograde cholangiography (ERC), requiring fluoroscopy-associated radiation exposure. Fluoroscopy exposure must be limited or is unavailable for some high-risk patients. We conducted a direct comparison of the efficacy and safety of ERC versus fluoroscopy-free direct solitary cholangioscopy (DSC) for noncomplex bile-duct stone removal. METHODS: Eligible patients were aged 18 years or older with noncomplex common bile-duct or common hepatic-duct stones. The primary outcome was comple

Potential of lysine succinylation as a therapeutic target for gallstone formation: An insightful strategy.

Cholelithiasis has a complex pathogenesis, necessitating better therapeutic and preventive strategies. We recently read with interest Wang et al's study on lysine acetyltransferase 2A (KAT2A)-mediated adenosine monophosphate-activated protein kinase (AMPK) succinylation in cholelithiasis. Using mouse models and gallbladder mucosal epithelial cells, they found that KAT2A inhibits gallstones through AMPK K170 succinylation, thereby activating the AMPK/silent information regulator 1 pathway to reduce inflammation and pyroptosis. This study is the first to connect lysine succinylation with choleli

The Association Between the C-Reactive Protein-Albumin-Lymphocyte Index and Gallstone Disease: A Cross-Sectional Study.

INTRODUCTION: This study used the novel C-reactive protein-albumin-lymphocyte (CALLY) index to explore its relationship with the risk of developing gallstones. METHODS: This study conducted a cross-sectional analysis of the National Health and Nutrition Examination Survey data from 2017 to 2020; multivariable logistic regression examined the CALLY-gallstone association. Restricted cubic splines tested nonlinearity. Subgroup and mediation analyses explored population variations and mediating effects (Conicity, lipid accumulation product, and a body shape index). Comparative Evaluation of Intraperitoneal Instillation of Bupivacaine With Magnesium Sulfate Versus Bupivacaine With Dexmedetomidine for Postoperative Relief After Laparoscopic Cholecystectomy.

BACKGROUND: Laparoscopic cholecystectomy (LC) is the most common surgical treatment for cholelithiasis and offers advantages over open surgery. However, postoperative pain remains a significant concern, arising from both somatic and visceral components. Intraperitoneal instillation of local anesthetics with adjuvants has been explored as a simple and effective method of pain control. OBJECTIVE: This study compared intraperitoneal bupivacaine with magnesium sulfate versus bupivacaine with dexmedetomidine for postoperative pain relief in LC. Pain assessed using the Visual Analog Scale (VAS), an

Preoperative Prediction of Difficult Laparoscopic Cholecystectomy Using a Scoring System: A Prospective Cohort Study.

BACKGROUND: Laparoscopic cholecystectomy (LC) is now considered the gold standard for the treatment of symptomatic cholelithiasis. However, conversion to open surgery is occasionally required due to intraoperative difficulties and complications. Early preoperative identification of difficult LC cases can be of help in surgical preparedness and patient counselling. OBJECTIVE: To evaluate the sensitivity, specificity, and predictive value of a preoperative scoring system in the prediction of difficult LC. METHODS: A prospective cohort study was conducted on 150 patients undergoing elective LC

A Case Report of Laparoscopic Cholecystectomy in an Obese Patient With Situs Inversus Totalis.

Situs inversus totalis (SIT) is a rare congenital condition characterized by a complete mirror-image arrangement of the visceral organs. We report a laparoscopic cholecystectomy performed on an obese patient with a body mass index (BMI) over 30 kg/m² and SIT. An 80-year-old female patient with SIT and a BMI of 30.1 kg/m² was diagnosed with cholecystolithiasis. Laparoscopic cholecystectomy was performed using a mirror-image port placement relative to the standard configuration. The postoperative course was uneventful. Laparoscopic cholecystectomy can be safely performed in patients with SIT, ev

Biloma in a cat with cholelithiasis and suspected previous biliary peritonitis due to gallbladder rupture.

CASE SUMMARY: A 14-year-old, female spayed domestic shorthair cat presented with a 2-day history of lethargy and intermittent vomiting. Abdominal ultrasonography identified biliary obstruction with several choleliths, dilation and thickening of the intrahepatic biliary ducts and a mass-like lesion was identified in the right medial liver lobe, attached to the gallbladder. Cholecystectomy and partial liver lobectomy were performed to treat biliary obstruction and remove the mass lesion, respectively. Histopathological examination of the hepatic mass confirmed the diagnosis of biloma. RELEVANCE

Double Gallbladder Identified and Managed by Laparoscopic Cholecystectomy: A Case Report.

Gallbladder duplication is a rare congenital biliary anomaly that can pose diagnostic and surgical challenges due to anatomical variations, particularly when duplicated cystic ducts are present. We report the case of a patient who presented with recurrent right upper quadrant pain associated with cholelithiasis and was scheduled for elective laparoscopic cholecystectomy. During surgery, a duplicated gallbladder with two cystic ducts was unexpectedly identified. Intraoperative findings confirmed the anomaly, and careful dissection of Calot's triangle allowed safe identification and ligation of
