Association between triglyceride-glucose index-related indices and gallstone disease in US adults: A cross-sectional analysis of NHANES.

The triglyceride-glucose (TyG) index and its obesity-related derivatives-TyG-body mass index, TyG-waist circumference, TyG-waist-to-height ratio (TyG-WHtR), and TyG-a body shape index (TyG-ABSI)-are emerging surrogates of insulin resistance. Their relationships with gallstone disease in the general population remain unclear. We investigated the associations between TyG-related indices and gallstone disease and compared their discriminatory performance. We performed a cross-sectional analysis of 6551 adults from the 2017 to 2023 National Health and Nutrition Examination Survey. Gallstone diseas

Use of genetic analysis in adult cholestatic liver disease: lessons from progressive paediatric syndromes and cohort studies.

The increasing availability and decreasing costs of DNA sequencing have resulted in the re-grouping of rare, severe paediatric cases of progressive familial intrahepatic cholestasis (PFIC) with more frequent, later-onset cases of cholestasis (eg, intrahepatic cholestasis of pregnancy, benign recurrent intrahepatic cholestasis, low phospholipid-associated cholelithiasis) under the umbrella of genetic cholestasis. The common denominator is the presence of functional variants in the PFIC-associated genes, predominantly in ABCB4, ABCB11 and ATP8B1, which cause PFIC types 1-3. Several other congeni

Mirizzi Syndrome Mimicking Cholangiocarcinoma Complicated by Infected Biloma Formation Following Complex Biliary Intervention: A Case Report.

Mirizzi syndrome is a rare but clinically significant complication of chronic cholelithiasis. It is characterized by extrinsic compression of the common hepatic duct (CHD) by an impacted gallstone in the gallbladder neck or cystic duct. Although rare, this condition can closely mimic cholangiocarcinoma, particularly in the presence of elevated tumor markers or atypical imaging findings. Timely recognition is essential, as misdiagnosis may lead to inappropriate surgical or oncologic interventions. We present a diagnostically challenging case of Type I Mirizzi syndrome complicated by infected bi

Clinical characteristics, etiology, and outcomes of acute pancreatitis at a tertiary referral center in Latvia: a retrospective cohort study.

BACKGROUND: Acute pancreatitis is a common cause of gastrointestinal hospitalization, but detailed clinical cohort data from the Baltic region are limited. We aimed to characterize the clinical features, etiologies, comorbidities, and in-hospital outcomes of acute pancreatitis at a tertiary-center in Latvia. METHODS: We conducted a retrospective single-center cohort study of adults hospitalized with acute pancreatitis between 2020 and 2024, including only the index admission for each patient. Cases were identified using ICD-10 codes and confirmed according to the Revised Atlanta Classificatio

Biliary tract microbes and common bile duct stones: current status and prospects.

Common bile duct stones is a common digestive system disease, and about 5%-30% of patients with cholelithiasis are complicated with common bile duct stones. It poses significant challenges to clinical diagnosis and treatment. Although its occurrence is related to traditional factors such as abnormal bile composition and biliary dynamics disorders, the exact pathogenesis has not been fully clarified. In recent years, with the rapid development of high-throughput sequencing and metagenomics and other microbiome technologies, researchers have begun to pay attention to the role of biliary microbio

Focal Nodular Hyperplasia Presenting With Biliary Obstruction and Intrahepatic Gallstones: A Case Report and Literature Review.

Focal nodular hyperplasia (FNH) is a benign liver lesion that is most often asymptomatic and does not require surgical intervention. Rarely, FNH may cause biliary obstruction and stasis, which can lead to intrahepatic gallstone (hepatolithiasis) formation. We describe a case of a 44-year-old female patient who was incidentally found to have biliary ductal dilation with an associated malignant-appearing 3.5 cm left liver mass. Workup included contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI). A biopsy was deferred, given the high clinical suspicion. An open anatomi

Fulminant Mantle Cell Lymphoma Presenting With Hepatic Infiltration and Spontaneous Tumor Lysis Syndrome.

Mantle cell lymphoma (MCL) is an uncommon, aggressive B-cell non-Hodgkin lymphoma that typically presents with lymphadenopathy, splenomegaly, and gastrointestinal (GI) tract involvement, most often as multiple lymphomatous polyps. While hepatic involvement by MCL is rare and usually asymptomatic, hepatic lesions in clinical practice more often represent metastases from GI, breast, lung, or pancreatic primaries. We report a case of MCL initially presenting with tumor lysis syndrome (TLS) and cholestatic liver injury, mimicking hepatobiliary malignancy. A 90-year-old male with coronary artery di

GLP-1 Receptor Agonists for Weight Loss and Risk of Major Safety Outcomes: A Multicentre Cohort Study.

AIMS: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are increasingly prescribed for weight loss, yet their safety in clinical practice remains uncertain. We evaluated associations between weight-management semaglutide or liraglutide initiation and safety outcomes in a multicentre cohort. MATERIALS AND METHODS: We used electronic health records from 13 South Korean hospitals (2018-2025) mapped to the OMOP Common Data Model. Adults initiating semaglutide or liraglutide were compared with propensity score-matched non-initiators in a new-user design. Ten safety outcomes were assessed. Sit

A Pilot Study of Plasma Cell-Free DNA Fragmentomics in Gallbladder Cancer.

BACKGROUND & AIMS: Gallbladder cancer (GBC) is a rare and highly lethal biliary tract cancer with limited treatment options and lack of diagnostic and prognostic noninvasive biomarkers. Circulating cell-free DNA (cfDNA) offers a noninvasive means to capture fragmentomics alterations that may assist with diagnosis and biological characterization. This pilot study aimed to identify cfDNA-based features that differentiate GBC from individuals with gallstones and healthy controls. METHODS: cfDNA was extracted from archived plasma samples from 67 individuals in two case-control studies from China

Upfront laparoscopic cholecystectomy versus preoperative MRCP or ERCP in patients with suspected choledocholithiasis.

BACKGROUND: The optimal diagnostic and therapeutic strategy for patients with intermediate-to-high-risk of choledocholithiasis and stable or decreasing total bilirubin levels remains unclear. Although magnetic resonance cholangiopancreatography (MRCP) and endoscopic retrograde cholangiopancreatography (ERCP) are used preoperatively, their diagnostic yield, complication risks, and cost-effectiveness in this subgroup remain debated. METHODS: This retrospective cohort study included patients who underwent laparoscopic cholecystectomy (LC) for suspected choledocholithiasis between January 2018 an

Spontaneous ovarian hyperstimulation syndrome in early singleton pregnancy with hepatic involvement: two case reports of atypical presentations.

BACKGROUND: Ovarian hyperstimulation syndrome (OHSS) is typically an iatrogenic complication of assisted reproductive techniques. Spontaneous OHSS is rare, particularly in singleton pregnancies and in the absence of known risk factors, and may present with atypical systemic manifestations such as hepatic involvement. CASE REPORT: We report two cases of spontaneous OHSS in early singleton pregnancies with hepatic involvement. The first case was a 21-year-old woman presenting with abdominal distension, ascites, and hyperbilirubinemia, with imaging revealing markedly enlarged ovaries and no evid

Cholestatic Jaundice Without Hepatomegaly as the Initial Manifestation of Hepatic Amyloid Light-Chain (AL) Amyloidosis: A Case of Rapid Multiorgan Failure.

Systemic amyloid light-chain (AL) amyloidosis is a rare plasma cell disorder characterized by extracellular deposition of misfolded immunoglobulin light chains, resulting in progressive organ dysfunction. Although hepatic involvement is recognized in AL amyloidosis, it is often clinically silent or presents with hepatomegaly (liver enlargement) and cholestatic liver enzyme abnormalities. Severe cholestatic jaundice as the initial and dominant manifestation is uncommon, particularly in the absence of hepatomegaly or radiographic biliary obstruction. We report a 63-year-old woman with limited pr

Association between endoscopic sphincterotomy and the development of hepatopancreatobiliary cancer.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic sphincterotomy (ES) has been hypothesized to increase the risk of hepatopancreatobiliary (HPB) cancer, but prior studies have shown conflicting results. This study aimed to determine whether ERCP with ES is associated with the longitudinal development of HPB cancer. METHODS: This matched cohort study conducted from 2010 to 2022 used linked medical record data from clinics, hospitals, and other facilities in Southeast Minnesota and Southwest Wisconsin. A total of 188,243 patients were included: 4003 with cholelit

Clinical Complexity of Acute Pancreatitis in a Patient With Jaundice Associated With Acute Hepatitis A, Cholelithiasis, and Recent Endoscopic Retrograde Cholangiopancreatography: A Case Report.

The coexistence of gallstone disease, post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis, and acute hepatitis A is uncommon and presents significant diagnostic and therapeutic challenges. We reported a 26-year-old man who initially presented with a three-week history of jaundice and pruritus. He was diagnosed with acute pancreatitis associated with acute hepatitis A, ERCP, and gallstones. Following initial clinical improvement, the patient's condition deteriorated, and contrast-enhanced CT demonstrated complications in the form of infected walled-off necrosis in the lesser

Endoscopic Sphincterotomy Plus Large Balloon Dilation Versus Endoscopic Sphincterotomy Alone for Patients with Common Bile Duct Stones: A GRADE Evaluation of Systematic Review and Meta-Analysis.

INTRODUCTION: Endoscopic sphincterotomy (ES) and endoscopic papillary balloon dilation (EPBD) are established treatments for common bile duct (CBD) stones, each with limitations. Endoscopic sphincterotomy with large-balloon dilation (ESBD) combines both techniques, but its efficacy versus ES alone remains controversial. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing ESBD with ES alone for the management of CBD stones. METHODS: We searched PubMed, Scopus, Embase, Ovid, Web of Science, and Cochrane Library for RCTs comparing ESBD versus ES al

Single-Stage Laparoscopic-Endoscopic Rendezvous Technique for Choledocholithiasis: Clinical Outcomes From 1008 Consecutive Patients.

BACKGROUND: Two-stage management of choledocholithiasis may be associated with procedure-related complications, repeated interventions, and longer hospital stays. While single-stage laparoscopic common bile duct exploration avoids these risks, it requires advanced laparoscopic expertise. We evaluated the outcomes of the single-stage laparoscopic-endoscopic rendezvous (LER) technique in a large, consecutive cohort. METHODS: A STROBE-compliant retrospective analysis was conducted on 1,008 consecutive adult patients with symptomatic cholelithiasis and magnetic resonance cholangiopancreatography-

Historical heritage: abdominoscopy in the early 20th century.

This historical study examines the contribution of Calabrian surgeon Gino Iannelli to the development of endoscopic surgery through his abdominoscopy technique, based on a recently discovered twelve-page manuscript. Operating in the first half of the 20th century in a small hospital in Cetraro, Southern Italy, Iannelli independently developed a method for exploring the peritoneal cavity using gas insufflation and endoscopic visualization under local anesthesia. Notably, his technique included the use of the instrument's tip as a palpator for diagnosing cholelithiasis and other abdominal pathol
