Case Report: Successful R0 resection following conversion immunochemotherapy in a patient with biliary tract cancer and multiple metastases discovered after laparoscopic gallstone surgery.

Gallbladder cancer is usually discovered as an incidental finding during or after cholecystectomy for gallstones. Most patients have advanced gallbladder cancer at diagnosis and have lost the opportunity for radical surgery, leading to dismal prognosis. Standard conversion therapy regimen is still in exploration. We reported a patient with unresectable biliary tract cancer (most likely gallbladder cancer) and multiple metastases (TxN2M1) discovered one month after laparoscopic gallstone surgery. The patient received margin-free resection after 9 cycles of immunotherapy (camrelizumab) combined

Evaluating the Efficacy of Body-First Technique Compared to the Conventional Technique in Achieving a Critical View of Safety in Laparoscopic Cholecystectomy: A Randomized Controlled Trial.

BACKGROUND: Laparoscopic cholecystectomy (LAP-C) is the definitive treatment for symptomatic cholelithiasis, yet bile duct injury (BDI), primarily caused by anatomical misidentification, remains a formidable risk. The Critical View of Safety (CVS) achievement will prevent BDI, but its consistent attainment is technically demanding in inflamed or distorted anatomy. The body-first dissection technique has been proposed as an alternative approach to improve CVS achievement, though high-quality prospective evidence remains limited. This randomized controlled trial (RCT) was designed to systematica

Mirizzi Syndrome With a Hepatic Duct Confluence Fistula and Stone Migration Into the Left Hepatic Duct: A Case Report.

Mirizzi syndrome (MS) is an uncommon complication of cholelithiasis, in which an impacted gallstone causes extrahepatic biliary obstruction via extrinsic duct compression. Advanced disease may progress to cholecystobiliary fistula formation. We report the case of a 75-year-old man with cardiometabolic comorbidities who presented with cholestatic liver biochemistry and imaging findings consistent with chronic calculous cholecystitis. Computed tomography and magnetic resonance cholangiopancreatography revealed a distended gallbladder packed with stones, hilar biliary narrowing, and a variant bil

Metaheuristic-based gallstone classification using rotational forest explained with SHAP.

INTRODUCTION: Cholelithiasis, commonly known as Gallstone disease, occurs when hardened deposits form in the gallbladder or bile ducts. It affects millions of people worldwide and is especially common in women. While many people may not experience any symptoms, symptomatic cases can present with acute cholecystitis and other complications such as pancreatitis and even gallbladder cancer. However, this disease presents a clinical challenge due to its variable symptoms and risk of serious complications. Therefore, early prediction of gallstones is essential for timely intervention. METHOD: Thus

Gallstone Disease and Its Association with Hypothyroidism: A Case-Control Study from a Tertiary Care Centre in Puducherry, South India.

BACKGROUND AND OBJECTIVE: Biliary stasis is a principal driver of gallstone formation. Hypothyroidism promotes lithogenesis through altered lipid metabolism, sphincter of Oddi dysfunction, and impaired gallbladder contractility, thereby promoting bile cholesterol supersaturation. The objectives of this study were (1) to determine whether a statistically significant association exists between gallstone disease and hypothyroidism and (2) to identify whether gallstone composition correlates with thyroid status. METHODS: A prospective case-control study was conducted in the Department of General

Incidental Chilaiditi Sign Identified During Evaluation for Symptomatic Cholelithiasis: Implications for Surgical Planning.

The Chilaiditi sign is a rare radiographic finding characterized by the interposition of the colon between the liver and the right hemidiaphragm. Although typically asymptomatic, it may complicate upper abdominal procedures or mimic pneumoperitoneum. We present a 73-year-old male with symptomatic cholelithiasis whose preoperative computed tomography (CT) imaging incidentally revealed the Chilaiditi sign. Given this finding, the standard umbilical approach for peritoneal access was avoided, and pneumoperitoneum was instead established via left upper quadrant (LUQ) Veress needle insertion to red

Incidental finding of "H"-type duplex gallbladder: a case report.

BACKGROUND: Gallbladder duplication is a rare congenital anomaly resulting from abnormal bifurcation of the caudal bud during embryogenesis. It may cause diagnostic and surgical challenges. Classification systems of gallbladder duplication are based on cystic duct anatomy. Though often asymptomatic, it can be associated with complications such as cholelithiasis. CASE PRESENTATION: We report the case of a 22-year-old North African male presenting to the emergency department with a left renal colic episode. He complained of left lumbar pain with typical radiation and no other associated symptom

Shifting Paradigms in Pediatric Cholelithiasis: A 13-Year Retrospective Study of the Clinicodemographic Profile, Risk Factors, and Outcomes.

BACKGROUND: Pediatric cholelithiasis is becoming a more common disease all over the world, and its epidemiology is changing, with a decrease in the incidence of hemolytic causes and a rise in idiopathic and metabolic risk factors. However, long-term data is still limited in the Indian subcontinent. The aim of the present study was to assess the clinicodemographic profile, risk factors, presentation, management, and outcome of cholelithiasis in the pediatric age group in a tertiary care teaching hospital in North India for 13 years. MATERIALS AND METHODS: This was a retrospective observational

Correlation of Postoperative Shoulder Tip Pain in Low- Versus Standard-Pressure Pneumoperitoneum During Laparoscopic Cholecystectomy.

Introduction During laparoscopic cholecystectomy, pneumoperitoneum is created to facilitate exposure of the operative site. It provides the surgeon with adequate visualization; however, it has been associated with postoperative shoulder tip pain. As an alternative, low-pressure pneumoperitoneum (8-10 mmHg) has been proposed. This study compared the postoperative shoulder tip pain between standard- and low-pressure pneumoperitoneum created during laparoscopic cholecystectomy. Methods This was a prospective comparative study conducted in the Department of General Surgery over 18 months, followin

Case report of pyloric obstruction caused by giant gallbladder compression with literature review.

Giant gallbladder is a rare clinical condition characterized by abnormal gallbladder enlargement, typically defined as a longitudinal diameter exceeding 14 cm or a volume surpassing 1.5 L. The most common pathological etiologies are cholelithiasis and chronic cholecystitis, followed by neoplastic factors, while congenital developmental anomalies and other causes are relatively uncommon. Herein, we report a case of giant gallbladder-induced pyloric obstruction in a patient who presented with nausea and vomiting for two weeks-symptoms that recurred after initial resolution of diabetic ketoacidos

Severe Gallbladder Dyskinesia With 2% Ejection Fraction: A Comprehensive Clinicoradiologic and Pathologic Case Correlation.

Gallbladder dyskinesia (GBD) is a functional biliary disorder characterized by biliary colic in the absence of cholelithiasis or mechanical obstruction and is identified by reduced gallbladder ejection fraction (EF) on cholecystokinin (CCK)-stimulated hepatobiliary scintigraphy. Patients may experience significant symptoms due to impaired gallbladder contractility and bile stasis despite the absence of gallstones or inflammatory structural abnormalities on imaging. We present a 42-year-old male with a three-month history of episodic right upper quadrant (RUQ) pain triggered by fatty meals. Abd

Recurrent Acute Calculous Cholecystitis in a Retained Gallbladder Remnant: A Case Report.

We present the case of a 67-year-old woman with a history of prior laparoscopic cholecystectomy who presented with right upper quadrant (RUQ) pain and nausea and was found to have recurrent acute cholecystitis. Review of the operative note from her previous cholecystectomy revealed that a rim of gallbladder tissue had been left behind due to anatomical complications. CT of the abdomen and pelvis demonstrated a distended gallbladder with cholelithiasis, wall thickening, and pericholecystic fluid, consistent with acute calculous cholecystitis despite her history of prior cholecystectomy. The pat

Early Versus Delayed Laparoscopic Cholecystectomy in Acute Cholecystitis: A Comparative Study.

BACKGROUND: Laparoscopic cholecystectomy is the gold standard for treating symptomatic cholelithiasis. However, the optimal timing of surgery in acute calculous cholecystitis remains a subject of debate. This study aims to compare early versus delayed laparoscopic cholecystectomy to determine the timing that yields better clinical outcomes. METHODS: A prospective comparative study was conducted on 100 patients diagnosed with acute calculous cholecystitis at a tertiary healthcare centre in Punjab, India. Patients were randomized into two groups: Group A underwent early laparoscopic cholecystec

Conquering the Challenge: Difficult Gallbladders and Delicate Solutions.

Background Laparoscopic cholecystectomy is the accepted standard treatment for symptomatic gallstone disease. However, severe inflammation, fibrosis, adhesions, or anatomical distortion can obscure critical landmarks and increase the risk of bile duct injury. In such cases, timely recognition of operative difficulty and appropriate modification of technique are essential to ensure patient safety. Methods This prospective case series was conducted at a tertiary care center, R L Jalappa Hospital & Research Center, Tamaka, Karnataka, India, in 2024 and included seven adult patients whose intraope

Factors influencing the difficulty of laparoscopic cholecystectomy: A review.

Laparoscopic cholecystectomy (LC) is the preferred treatment for symptomatic cholelithiasis, yet conversion to open surgery occurs in a notable proportion of cases due to factors such as dense adhesions, unclear anatomy, or intraoperative complications. Therefore, it is of interest to identify preoperative predictors of difficult LC to guide surgical planning. Across the included studies, factors consistently associated with conversion included gallbladder wall thickness greater than 4-5 mm, contracted gallbladder, acute cholecystitis, age over 60 years and male gender. Other factors such as d

Surgical Complications in Conventional and Laparoscopic Cholecystectomy in an Ecuadorian Reference Hospital.

Cholecystectomy is a very common surgical intervention that consists of the removal of the gallbladder, especially when complications such as cholecystitis exist. The laparoscopic technique has been associated with greater postsurgical benefits; however, no local research has been conducted comparing these two techniques, considering that in poor-income countries, there are hospitals that do not have the technological and economic resources available to perform laparoscopic surgeries. This project aimed to determine the differences between open versus laparoscopic cholecystectomy, in terms of

Diagnostic Challenge of Small Bowel Neuroendocrine Tumour Mimicking Biliary Disease: A Case Report.

Small bowel neuroendocrine tumours (NETs) are uncommon neoplasia that often occur with non-specific gastrointestinal symptoms, leading to considerable delays in diagnosis and treatment, and overlapping findings that often distract the clinical focus from the underlying pathology. We present the case of a 49-year-old woman who reported to us with a one-month history of epigastric pain, nausea, vomiting, and unintentional weight loss of about two stones. Initial imaging showed deranged liver markers and cholelithiasis on ultrasound without biliary enlargement, which resulted in a tentative diagn

Cholecystectomy vs Endoscopic Retrograde Cholangiopancreatography or No Intervention After Gallstone-Related Acute Pancreatitis.

IMPORTANCE: Same-admission cholecystectomy is recommended by guidelines for mild gallstone-related acute pancreatitis, yet surgery is often deferred. Endoscopic retrograde cholangiopancreatography (ERCP) is often used as an interim strategy, but its effectiveness compared with cholecystectomy is uncertain. OBJECTIVE: To compare cholecystectomy, ERCP only, and no intervention in association with recurrent acute pancreatitis and other gallstone-related complications, accounting for death as a competing event. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study used nationwide

United States benign gallbladder disease mortality, 1999-2024: rising cholecystitis and persistent demographic and geographic disparities.

BACKGROUND: Mortality patterns of benign gallbladder diseases in the USA remain poorly characterized, particularly across disease subtypes and demographic and geographic groups. METHODS: We used the Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemiologic Research (CDC WONDER) Underlying Cause of Death database to identify deaths from benign gallbladder diseases from 1999 to 2024. Eligible deaths were defined by ICD-10 codes K80-K82, including cholelithiasis, cholecystitis, and other diseases of the gallbladder. Age-adjusted mortality rates (AAMRs) were standardiz
