A rare case of median arcuate ligament syndrome presenting with chronic abdominal pain managed surgically: A case report.

The disorder median arcuate ligament syndrome (MALS) develops when the median arcuate ligament compresses the celiac artery and accompanying nerve fibers leading to postprandial abdominal pain, nausea and bloating symptoms that are commonly mistaken for other gastrointestinal or vascular conditions. A 29-year-old female patient experienced right hypochondrium pain after eating combined with burning stomach pain and debilitating bloating episodes for 5 months. Computed tomography (CT) angiography scan and Doppler ultrasonography revealed celiac artery stenosis due to pressure from the median ar

Management of Bile Leak Post Minimally Invasive Subtotal Cholecystectomy: A Review.

OBJECTIVE: Minimally invasive subtotal cholecystectomy (STC) is a safe alternative to minimally invasive cholecystectomy that prevents bile duct injury. Nonetheless, it is associated with higher rates of other complications, namely postoperative bile leak and symptomatic remnant cholelithiasis. BACKGROUND: Bile leak presents as biloma requiring drainage and subsequently high bilious drain output. It is more strongly associated with fenestrating STC. Fortunately, the majority are self-limited and do not require endoscopic intervention. Symptomatic remnant cholelithiasis presents as recurrent g

Diet-induced inflammation is associated with fatty pancreas in patients with common bile duct stones.

Inflammation has been proven to be associated with chronic diseases. We hypothesized that higher diet-induced inflammation is associated with increased risk of fatty pancreas (FP). Among 278 patients with common bile duct (CBD) stones, 89 patients were diagnosed with fatty pancreas (case group) during endoscopic ultrasonography and the other 189 patients were healthy in this regard (control group). Dietary inflammatory index (DII), empirical dietary inflammatory pattern (EDIP) and dietary inflammatory score (DIS) were calculated based on a 168-question valid food frequency questionnaire. Dieta

Primary Closure Versus T-Tube Drainage on Common Bile Duct Exploration for Choledocholithiasis: An Updated Systematic Review and Meta-Analysis.

Introduction: Laparoscopic common bile duct exploration (LCBDE) is a critical procedure for managing choledocholithiasis, with primary closure (PC) and T-tube drainage (TTD) as common methods for common bile duct closure. However, the substantial number of new studies comparing PC and TTD underscores the need for an updated meta-analysis. Therefore, this study aims to compare surgery-related outcomes in PC and TTD for biliary duct closure following LCBDE. Methods: We searched PubMed, Embase, and Cochrane Library databases on June 20, 2024. Mean differences (MDs) and risk ratios with 95% confid

Management of a Patient with Severe Osteogenesis Imperfecta Undergoing Laparoscopic Cholecystectomy.

Individuals with severe osteogenesis imperfecta who require surgery often present a difficult management problem due to limb deformity and shortening, kyphoscoliosis, and deformity of the rib cage. All of these features may be associated with respiratory problems and impaired cardiovascular reserve. Surgical procedures and anaesthetic management represent a substantial challenge in these individuals. Here, we describe the clinical outcome of laparoscopic surgery to remove multiple gallstones in an individual with severe osteogenesis imperfecta. Meticulous pre-operative planning, combined with

Primary intrahepatic lithiasis in western countries: role of ultrasound and comparison with MRI with MRCP.

OBJECTIVES: To study hepatobiliary expert ultrasound (US) in a cohort of suspected primary intrahepatic lithiasis (IHL) and to compare these findings with MRI/MRCP data. METHODS: All patients with a diagnosis of primary-IHL based upon biological and, or, clinical symptoms who underwent US between 2008 and 2023 were retrospectively enrolled in two tertiary hepatobiliary centers. Clinical characteristics, available genetic and radiological features (expert US and MRCP) were recorded. Data were compared for the presence of comet-tail images or stones on US, between US and MRCP features and betwe

Cholelithiasis With a Rare Branched Accessory Hepatic Duct: A Case Report.

The patient was a 71-year-old male who underwent laparoscopic cholecystectomy. Preoperative ERCP (Endoscopic Retrograde Cholangiopancreatography) and MRCP (Magnetic Resonance Cholangiopancreatography) did not reveal any obvious biliary tract anomalies. However, during surgery, a vascular structure was observed while dissecting the dorsal side of the gallbladder. The blade of the ultrasonic coagulation and cutting device came into contact with this structure, resulting in bile leakage. The posterior sectoral hepatic duct was found to merge with the common bile duct dorsally, distal to the confl

A Report of a Rare Case of Choledocholithiasis Complicated by Proximal Basket Rod Rupture Requiring Surgical Intervention.

We report a rare case of choledocholithiasis in which mechanical lithotripsy led to rupture of the proximal rod of the Dormia basket, complicating the removal of calculi via endoscopic retrograde cholangiopancreatography (ERCP) and necessitating surgical intervention. ERCP plays a key role in the management of biliopancreatic disorders. Although this approach has improved in recent decades, it remains one of the endoscopic procedures with the highest complication rates. These complications may arise due to the presence of large or impacted stones, resulting in excessive stress on the basket wi

Genetic Analysis Reveals a Protective Effect of Sphingomyelin on Cholelithiasis.

BACKGROUND: Cholelithiasis is the most common disorder affecting the biliary system. Choline is an essential nutrient in the human diet and is crucial for the synthesis of neurotransmitters. Previous studies have suggested an association between choline metabolites and cholelithiasis. However, the underlying mechanisms remain unclear. This research aims to fill the knowledge gap regarding the role of choline metabolites in cholelithiasis. METHODS: Genetic data related to choline metabolites and other covariates were retrieved from the U.K. Biobank and IEU OpenGWAS database. Two-sample (TSMR)

Association between C-reactive protein to lymphocyte ratio and gallstones: a cross-sectional study.

BACKGROUND: Inflammation plays a key role in the development of gallstones, and the C-reactive protein to lymphocyte ratio (CLR) has been introduced as a promising biomarker for evaluating inflammatory processes. Nonetheless, its correlation with gallstone prevalence remains ambiguous. This study aims to evaluate the potential link between CLR levels and gallstone prevalence. METHODS: This study utilized data from the National Health and Nutrition Examination Survey, covering the periods from March 2017 to 2020 and 2021 to 2023. Multivariate logistic regression was employed to examine the ass

Analysis of risk factors in breast cancer patients with hand-foot syndrome and oral mucositis caused by pegylated liposomal doxorubicin.

INTRODUCTION: Doxorubicin (DOX) is a primary treatment for breast cancer (BC), but its widespread use is hindered by cardiotoxicity. Pegylated liposomal doxorubicin (PLD) has been developed to enhance the efficacy of DOX and mitigate its cardiotoxic effects. However, PLD is associated with adverse reactions (ADRs) such as hand-foot syndrome (HFS) and oral mucositis (OM), which have garnered significant attention. Although not life-threatening, HFS and OM can cause severe discomfort and functional impairment. Severe cases may necessitate a reduced PLD dose or even delay or interrupt chemotherap

Prospects of Mendelian randomization in hepatology: a comprehensive literature review with practice guidance.

Mendelian randomization (MR), a powerful statistical tool for causal inference, has been widely applied in various fields of medical research, even extending to economics and psychology. In hepatology, MR has been utilized to identify risk factors and potential therapeutic targets for liver diseases, including metabolic dysfunction-associated steatotic liver disease, cholestatic and autoimmune liver diseases, and hepatobiliary cancer. MR can provide evidence of causation via associations between genetic variants, modifiable exposures and liver disease occurrence or outcomes, using large existi

Minimal Endoscopic Sphincterotomy with Papillary Balloon Dilation Using a Novel Non-slip Balloon Catheter for the Treatment of Common Bile Duct Stones: A Prospective Pilot Study.

BACKGROUND: Endoscopic sphincterotomy (ES) with minimal incision followed by balloon dilation (ESBD) is a promising hybrid technique for the treatment of common bile duct (CBD) stones. However, balloon slippage during dilation can occur easily following ES due to partially opened papillae. This study aimed to examine the clinical utility of ESBD using a novel non-slip balloon catheter. METHODS: This prospective pilot study included 100 eligible patients with CBD stones measuring ≤ 11 mm. The study outcomes included the rates of technical success, adverse events (AEs), use of mechanical lithot

High Prevalence of Cholelithiasis in Umbilical Hernia Patients and Vice Versa: A Prospective Multicenter Study.

Background Cholelithiasis (CL) and umbilical hernia (UH) often coexist, but their exact relationship remains underexplored. Both conditions share common risk factors such as obesity, female gender, and metabolic disorders, suggesting a potential pathophysiological link. Despite numerous retrospective analyses, robust prospective multicenter data are lacking. This study aims to provide a more accurate estimation of the prevalence of UH in CL patients and vice versa, identify independent risk factors, and assess the predictive value of BMI in this association. Methods In this prospective, multic
