Elective transabdominal preperitoneal repair of an incidentally detected spigelian hernia with massive sigmoid colon protrusion through a small fascial defect: a case report.

INTRODUCTION AND IMPORTANCE: Spigelian hernia is a rare type of ventral hernia. The hernia orifice is generally narrow, and incarceration of the contents - usually omentum or small bowel - is a common problem. CASE PRESENTATION: A 79-year-old man underwent laparoscopic cholecystectomy for acute cholecystitis. Preoperative computed tomography (CT) incidentally revealed a left Spigelian hernia with protrusion of the sigmoid colon through a small fascial defect. Considering the potential risk of future incarceration, elective hernia repair was planned, and laparoscopic transabdominal preperitone

Cholecystocolonic fistula with ruptured cystic artery pseudoaneurysm presenting as massive per rectal bleeding: a case report.

INTRODUCTION AND IMPORTANCE: Cholecystocolonic fistula and cystic artery pseudoaneurysm are rare complications of gallbladder disease. Their coexistence with massive lower gastrointestinal bleeding is extremely rare and potentially fatal. CASE PRESENTATION: A 47-year-old male presented with severe acute cholecystitis, followed by massive per rectal bleeding and hypovolemic shock. Contrast enhanced CT revealed a cholecystocolonic fistula with a cystic pseudoaneurysm. The patient underwent emergency open cholecystectomy, primary colon repair, and diversion ileostomy. Postoperative histopatholog

Robotic Cholecystectomy Remains Safe and Effective After Regular Hours: A 3-Year Update.

BACKGROUND AND OBJECTIVES: Despite growing popularity in robotic-assisted surgery, access during evenings and weekends for nonelective operations may be restricted. A prior smaller comparison study in 2024 found no basis for safety concerns in managing acute cholecystitis in this setting. We sought to provide additional data to further evaluate whether restriction of robotic cholecystectomy during "after-hours" is justified. METHODS: We performed a retrospective analysis of 161 patients undergoing robotic cholecystectomy for acute cholecystitis according to 2018 Tokyo criteria by a single sur

Beyond individual complications: textbook outcome in elective minimally invasive cholecystectomy and the role of robotic approach.

Textbook outcome (TO) summarizes several desirable perioperative results within a single endpoint, but its use in cholecystectomy remains uncommon. Comparisons between robotic cholecystectomy (RC) and laparoscopic cholecystectomy (LC) have generally relied on individual outcomes. We therefore examined TO after elective minimally invasive cholecystectomy and assessed whether surgical approach was associated with overall perioperative performance and postoperative recovery. We retrospectively reviewed consecutive patients who underwent elective minimally invasive cholecystectomy from January 202

Impact of puncture site location in percutaneous transhepatic gallbladder drainage on outcomes of subsequent laparoscopic cholecystectomy: a multicenter retrospective clinical study.

BACKGROUND: Percutaneous transhepatic gallbladder drainage (PTGD) followed by laparoscopic cholecystectomy (LC) is an effective treatment strategy for acute cholecystitis (AC). However, the impact of the PTGD puncture site on subsequent LC outcomes has not been systematically investigated. METHODS: This retrospective study included patients diagnosed with AC who underwent PTGD followed by LC at the First Affiliated Hospital of Dalian Medical University (January 2018-May 2025) and Yingkou Central Hospital (January 2020-May 2025). PTGD was performed according to the Tokyo Guidelines 2018 (TG18)

Successful endoscopic retrieval of a plastic gallbladder stent that had completely migrated into the gallbladder.

Endoscopic gallbladder stenting is a useful transpapillary drainage technique for acute cholecystitis, especially in patients who are poor candidates for cholecystectomy or percutaneous gallbladder drainage. However, long-term placement may cause late adverse events, including migration. Complete migration of a gallbladder stent into the gallbladder is rare and may make retrieval challenging. A 77-year-old woman was referred to our hospital with epigastric pain. Imaging studies suggested biliary tract inflammation with gallbladder debris and bile duct dilatation, but no gallstones or common bi

Biliary Microbiology and Antimicrobial Treatment Modification in Patients Undergoing Percutaneous Cholecystostomy for Acute Cholecystitis: A Single-Centre Retrospective Cohort Study.

OBJECTIVE: To describe bile culture yield, organism distribution and antimicrobial treatment modification in patients undergoing percutaneous cholecystostomy (PC) for acute cholecystitis who were unsuitable for early cholecystectomy. METHODS: We retrospectively reviewed 84 consecutive patients who underwent PC between December 2021 and December 2025. Index bile cultures were classified as positive, no growth, or not obtained. We analysed organisms, polymicrobial growth, empirical antimicrobial therapy, treatment modification and clinical outcomes by culture status and disease severity. RESUL

Incidental Gallbladder Cancer After Laparoscopic Cholecystectomy: A Comparison Between Patients With and Without Acute Cholecystitis at a High-Volume Center.

INTRODUCTION: Incidental gallbladder cancer (iGBC) after laparoscopic cholecystectomy (LC) remains clinically relevant, and preoperative recognition may be especially difficult in acute cholecystitis (AC). We reviewed iGBC cases diagnosed after LC and descriptively compared cases with and without AC. METHODS: We retrospectively reviewed 2572 consecutive LCs performed between January 2013 and November 2025. After excluding patients with preoperatively detected gallbladder polyps or suspected malignancy, 2224 patients were analyzed (AC, n = 663; non-AC, n = 1561). The denominator cohort was use

Does ICG-NIRF cholangiography replace the critical view of safety in laparoscopic cholecystectomy? A prospective evaluation on biliary visualization dynamics and the impact of inflammation.

BACKGROUND: While indocyanine green near-infrared fluorescence (ICG-NIRF) cholangiography is increasingly used to facilitate real-time biliary visualization, its ability to substitute for the Critical View of Safety (CVS) as a primary anatomical endpoint remains unclear. This study aimed to characterize the intraoperative dynamics of ICG-NIRF and evaluate the impact of acute cholecystitis (AC) on its reliability. METHODS: In this prospective study of 96 consecutive patients (48 AC; 48 non-AC), biliary visualization was assessed at three milestones: T1 (dissection onset), T2 (CVS declaration),

Laparoscopic Cholecystectomy versus Percutaneous Drainage for Acute Cholecystitis in Elderly.

BACKGROUND: There is ongoing debate regarding the best management strategy for acute calculous cholecystitis (ACC) in the oldest old population. The present study sought to evaluate clinical outcomes following laparoscopic cholecystectomy (LC) versus percutaneous transhepatic gallbladder drainage (PTGD) in elderly individuals diagnosed with ACC. METHODS: This retrospective study included 135 elderly ACC patients (mean age >80 years) treated from January 2023 to December 2024. Of these, 68 underwent LC and 67 received PTGD. Between-group comparisons covered baseline profiles, a primary composi

Association between mean platelet volume and acute cholecystitis in pregnant patients with right upper quadrant pain: A multicenter retrospective cohort study.

BACKGROUND: Physiological and hematological changes during pregnancy may complicate the interpretation of conventional inflammatory markers in patients with suspected acute cholecystitis (AC). This study aimed to evaluate the association between mean platelet volume (MPV), derived from routine complete blood count analysis, and AC in pregnant patients and to investigate whether this association remained consistent across pregnancy trimesters. METHODS: This retrospective multicenter exploratory cohort study included 112 pregnant patients presenting with right upper quadrant pain between 2022 a

Advances in Indocyanine Green Near-Infrared Fluorescence for Extrahepatic Biliary Identification in Laparoscopic Cholecystectomy for Acute Cholecystitis: A Narrative Review.

Laparoscopic cholecystectomy, a minimally invasive procedure with rapid recovery, remains the standard surgical intervention for acute cholecystitis. Acute inflammation of the gallbladder can cause severe edema, dense adhesions, and anatomical distortion within Calot's triangle, frequently obscuring tissue planes and increasing the risk of iatrogenic bile duct injury, a significant surgical complication. Indocyanine green fluorescence serves as a valuable intraoperative navigation tool. Its hepatobiliary metabolic characteristics enable real-time near-infrared visualization of extrahepatic bil

The biliary microbiome in symptomatic cholelithiasis: a prospective comparison across distinct bacterial niches.

BACKGROUND: Cholelithiasis is highly prevalent in the Western world and frequently requires surgical or endoscopic intervention. However, the composition and role of the biliary microbiome remain poorly understood. This study aimed to characterize and compare bacterial communities of the biliary niche with saliva and faeces in patients with cholelithiasis. METHODS: In this prospective observational study, patients with symptomatic cholecystolithiasis, chronic or acute cholecystitis, and choledocholithiasis were included. Bacterial community composition was characterized across bile, gallstone

[The "simple" laparoscopic cholecystectomy as a hybrid DRG in 2026 : Critical appraisal and practical orientation].

BACKGROUND: With approximately 175,000 procedures per year (IQTIG, reporting year 2024), laparoscopic cholecystectomy is among the most common visceral surgical procedures in Germany. Since 2026, "simple" laparoscopic cholecystectomy (H08M) has been included in the cross-sectoral hybrid diagnosis-related groups (DRG) scheme under § 115f of the German Social Code Book V (SGB V). OBJECTIVE: To critically analyze the hybrid DRG classification with respect to the underlying evidence, coding/grouping logic, real-world care patterns and economic implications, including a model calculation of the ex

Pyloric Pseudonecrosis Caused by Biliary Peritonitis: A Rare Intraoperative Pitfall.

Biliary peritonitis can cause subserosal bile staining that mimics pyloric or duodenal necrosis. When endoscopy or ICG fluorescence is unavailable, pseudonecrosis remains an operative diagnosis of exclusion, supported by preserved serosal integrity, absence of visible perforation, careful inspection, and uneventful 1-month follow-up.

Endoscopic Ultrasound-Guided Gallbladder Drainage as a Bridge to Surgery for Acute Cholecystitis: A Comparative Study.

OBJECTIVES: Gallbladder drainage is used for patients with acute cholecystitis (AC) who are unsuitable for emergency cholecystectomy. Percutaneous transhepatic gallbladder drainage (PTGBD), endoscopic ultrasound-guided gallbladder drainage (EUS-GBD), and endoscopic transpapillary GBD (ETGBD) are used as bridge-to-surgery (BTS) approaches, but comparative data are limited. We compared clinical and perioperative outcomes among these strategies. METHODS: This retrospective, single-center study included 84 patients with AC who underwent GBD between January 2016 and January 2026 (PTGBD, n = 26; EU

Bailout Procedures, Downstream Outcomes, and Cost After Robotic vs Laparoscopic Cholecystectomy for Acute Cholecystitis.

BACKGROUND: Robotic cholecystectomy use is increasing, but comparisons with laparoscopic cholecystectomy have largely excluded bailout procedures, limiting assessment of difficult acute cholecystitis. We compared bailout use, downstream outcomes, and 1-year costs between approaches. STUDY DESIGN: This retrospective cohort study used Healthcare Cost and Utilization Project databases from New York and Florida (2012-2021) to identify adults undergoing inpatient, unscheduled robotic or laparoscopic cholecystectomy for acute cholecystitis. Bailout included conversion to open, subtotal cholecystect

Caring for patients with multimorbidity: moral distress and life satisfaction among doctors and nurses in Portugal.

BACKGROUND: The prevalence of multimorbidity (MM), defined as the co-occurrence of two or more chronic conditions in an individual, presents complex challenges for healthcare providers (HCPs). In Portugal, nearly half of the general population experiences MM, intensifying demands on HCPs. Doctors and nurses often encounter fragmented care pathways, inadequate guidelines, and frequent ethical dilemmas, which can lead to moral distress (MD). MD arises when HCPs are unable to act in accordance with their ethical beliefs due to institutional barriers, and it has consequences for well-being and emp
