Impact of Bedside Ultrasound on Patient Anxiety in Acute Cholecystitis: A Comparative Observational Study.

INTRODUCTION: Acute cholecystitis is a common condition requiring prompt diagnosis and management in emergency settings. Traditional diagnostic pathways often involve patient transfer for imaging, potentially increasing anxiety and discomfort. This study aimed to evaluate the impact of bedside ultrasound compared with standard ultrasound on anxiety and pain levels in patients with suspected acute cholecystitis in the emergency department. METHODS: A comparative observational study was conducted enrolling 100 adult patients with suspected acute cholecystitis. Patients were assigned to either b

Prognostic value of the acute-phase reactant index in predicting severity and clinical outcomes in acute cholecystitis patients presenting to the emergency department: A retrospective observational study.

Acute cholecystitis is a common inflammatory condition encountered in emergency departments (EDs), with severity varying widely among patients. Rapid risk stratification and appropriate management are critical in preventing complications and mortality. This study aimed to evaluate the association between the acute-phase reactant index (APhRI) and disease severity, and to investigate the potential prognostic value of APhRI in clinical decision-making. We conducted a single-center, retrospective observational study of adult patients diagnosed with acute cholecystitis in a tertiary ED between Jan

Is robotic surgery ready for emergency cholecystectomy? A systematic review and meta-analysis of robotic versus laparoscopic approach in acute cholecystitis.

INTRODUCTION: Acute cholecystitis is typically managed with laparoscopic cholecystectomy, though inflammation and distorted anatomy can increase operative difficulty. Robotic cholecystectomy may offer technical advantages through improved visualisation and instrument dexterity, yet current evidence is limited, heterogeneous, and entirely observational, with no randomized trials comparing the two approaches in the emergency setting. This systematic review and meta-analysis synthesises existing comparative data to determine whether robotic assistance confers meaningful operative or postoperative

Multicenter Analysis of the Learning Curve for Endoscopic Ultrasound-Guided Gallbladder Drainage: A Cumulative Sum Approach.

OBJECTIVES: Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) is effective and safe for poor surgical candidates; however, it remains technically challenging with potential serious adverse events. This study evaluated the learning curve for EUS-GBD for safe and effective procedures. METHODS: From January 2016 to January 2024, 146 patients underwent EUS-GBD performed by three endosonographers. We analyzed the baseline characteristics, procedural outcomes, and learning curves using cumulative sum (CUSUM) analysis. RESULTS: No significant differences were found among the three operato

[Acute cholecystitis associated with IgG4-related sclerosing cholangitis: a case report].

A 72-year-old male patient with jaundice and hilar bile duct stenosis, initially suspected to have cholangiocarcinoma, was referred to our hospital. Subsequent cholangiography revealed bile duct stenosis improvement, and intraductal ultrasonography demonstrated homogeneous bile duct wall thickening. The increased serum immunoglobulin G4 (IgG4) levels and the presence of other organ lesions possibly indicated to IgG4-related sclerosing cholangitis (IgG4-SC). After 2 months, the patient developed acute cholecystitis. Endoscopic ultrasonography revealed circumferential wall thickening that extend

An intraoperative bile culture does not improve the outcomes or guide antibiotic management in acute cholecystitis: A Propensity-Weighted analysis.

PURPOSE: The Tokyo Guidelines of 2018 recommend performing an intraoperative bile culture for acute cholecystitis. However, their clinical significance remains unclear. We evaluated the impact of performing a bile culture on the perioperative outcomes. METHODS: We retrospectively analyzed 344 patients who underwent cholecystectomy for acute cholecystitis between 2015 and 2024. The patients were classified into a culture group (n = 248) and a non-culture group (n = 96). The inverse probability of treatment weighting was applied to adjust for baseline differences. The perioperative outcomes wer

Construction and validation of a diagnostic model for acute suppurative cholecystitis in older patients: LASSO-Logistic regression analysis using readily available clinical indicators.

OBJECTIVES: Acute suppurative cholecystitis (ASC) is a severe form of acute cholecystitis (AC) that disproportionately affects older adults, often leading to delayed diagnosis and increased morbidity. Early identification of ASC in older adults over 80 years is crucial to guide timely and appropriate management. This study aimed to develop a diagnostic model based on readily available clinical indicators to identify whether older adults with acute cholecystitis have progressed to ASC. METHODS: Older adults (aged 80 to 98 years) diagnosed with AC between January 2017 and December 2024 were retr

Small bowel obstruction from jejunal gastrointestinal stromal tumour in a young adult with neurofibromatosis type 10: a case report.

Neurofibromatosis type 1 is an autosomal dominant disorder associated with an increased risk of tumours, including gastrointestinal stromal tumours. These are usually asymptomatic and discovered incidentally. Small bowel obstruction due to gastrointestinal stromal tumour in neurofibromatosis 1 is rare, especially in young adults. We report a 24-year-old man with neurofibromatosis type 1 who presented with right upper quadrant pain and biochemical features of acute cholecystitis. During laparoscopic cholecystectomy, unexpected small bowel dilatation prompted conversion to laparotomy, revealing

Outcomes of Percutaneous Cholecystostomy as a Bridging or Definitive Treatment for Acute Cholecystitis.

Percutaneous cholecystostomy (PC) is a minimally invasive intervention for managing acute cholecystitis in patients unfit for immediate cholecystectomy. Although it is widely used, its outcomes and factors influencing clinical success are not well understood. Objectives The goal of this study was to assess the outcomes of PC by examining various factors such as gender, type of cholecystitis, and procedure timing to correlate with symptom improvement, mortality, and the need for additional medical interventions. Materials and methods A cross-sectional study was conducted at The Royal Oldham Hos

Operative strategies for the acute difficult gallbladder: a Society for Surgery of the Alimentary Tract state-of-the-art systematic review and meta-analysis of subtotal cholecystectomy outcomes.

BACKGROUND: The "difficult gallbladder" in acute cholecystitis can preclude safe achievement of the critical view of safety, prompting subtotal cholecystectomy (STC) as a guideline-supported bailout. We performed a contemporary synthesis to clarify STC's safety effectiveness trade-offs vs total cholecystectomy (TC) and delineate technique-specific outcomes and patient factors that may influence risk. METHODS: We conducted a systematic review and meta-analysis of studies from 2010 to June 2025. Three comparisons were evaluated: (i) single-arm outcomes after STC, (ii) STC vs TC, and (iii) fenes
