Gallbladder osseous metaplasia mimicking a focal lesion on ultrasound: a case report of a rare benign condition.

Focal gallbladder wall thickening may raise suspicion for inflammatory, hyperplastic, or malignant processes. We report a rare case of chronic cholecystitis associated with adenomyosis and osseous metaplasia in a 43-year-old woman with multiple comorbidities. The patient presented with right upper quadrant abdominal pain. Ultrasonography revealed focal gallbladder wall thickening without calculi. She underwent uneventful laparoscopic cholecystectomy. Gross and microscopic pathological examination demonstrated chronic cholecystitis with mucosal hyperplasia, intramural diverticula consistent wit

Simultaneous laparoscopic adrenal and gallbladder surgery: a case of combined minimally invasive success.

Concomitant indications for adrenalectomy and cholecystectomy are rare, and simultaneous laparoscopic performance of both procedures is infrequently reported. Combined surgery may reduce anesthetic exposure, hospital stay, and recovery time. A 57-year-old male was admitted with intermittent pain in the right upper quadrant and abdominal distension. Imaging confirmed cholelithiasis with chronic cholecystitis, along with an incidental, nonfunctional 8-cm right adrenal mass. The patient underwent an uneventful simultaneous laparoscopic right adrenalectomy and cholecystectomy. Histopathology confi

Ketorolac-induced peptic ulcer bleeding after cholecystectomy in a "healed" ulcer patient: A CARE case report.

RATIONALE: Ketorolac tromethamine injection-induced peptic ulcer with bleeding remains rarely reported in clinical case studies. Controversy persists, particularly regarding its analgesic use in patients with a history of gastric ulcer, even after clinical cure. Herein, this article presents a case of peptic ulcer with bleeding after supra-therapeutic dosage of ketorolac tromethamine injection for post‑cholecystectomy analgesia in a patient with a clinically "healed" gastric ulcer, emphasizing the need for cautious and rational prescribing. PATIENT CONCERNS: A 70-year-old male patient with a

Chronic Cholecystitis: Anatomical Variants, Pediculitis, and a Candidate Preoperative Framework for Difficult Laparoscopic Cholecystectomy.

Preoperative risk stratification for laparoscopic cholecystectomy (LC) remains imperfect, particularly in patients with chronic inflammatory remodeling and biliary anatomic variants. Existing tools often focus on acute presentations or intraoperative variables, resulting in uncertainty on how congenital anatomy, recurrent biliary colic, and cystic pediculitis interact. We synthesize a hypothesis-generating conceptual framework and propose an illustrative candidate preoperative rubric for future validation. We performed a structured narrative review of PubMed, Scopus, and Web of Science (Januar

Histopathological findings in gallbladders of asymptomatic living liver donors at a liver transplant center.

BACKGROUND & OBJECTIVE: In low and middle-income countries (LMICs), living donor liver transplant (LDLT) is common due to limited cadaveric grafts. Cholecystectomy during donor hepatectomy allows evaluation of gallbladder pathologies in healthy donors. This study examined the prevalence of gallbladder pathologies in healthy living liver donors. METHODOLOGY: This study used retrospective data from March 2019 to March 2023 and prospective data from March 2023 to July 2024, collected from consecutive liver donors at a single liver transplant center using a prospectively maintained database. Risk factors for complications after laparoscopic surgery in children with congenital choledochal cysts.

BACKGROUND: Laparoscopic Roux‑en‑Y hepaticojejunostomy is the standard treatment for pediatric congenital choledochal cysts (CCCs). However, postoperative complications remain a significant clinical challenge. This study aimed to identify independent risk factors and develop a predictive model for complications following laparoscopic CCC excision in children. METHODS: We conducted a retrospective cohort study of 362 pediatric patients who underwent laparoscopic surgery for CCC between August 2019 and July 2024 at two tertiary centers. Preoperative variables were systematically collected. Univ

Comparative Early Postoperative Outcomes in Acute Calculous vs. Acute Acalculous Cholecystitis: A Retrospective Analysis.

Background and Objectives: Acute cholecystitis is a common indication for emergency surgery. While acute calculous cholecystitis (ACC) is most common, acute acalculous cholecystitis (AAC) occurs without gallstones and is often associated with severe systemic illness. We compared early postoperative outcomes after cholecystectomy for AAC versus ACC, with emphasis on complication severity and overall morbidity burden. Materials and Methods: We performed a single-center retrospective cohort study of consecutive adults undergoing urgent or emergent cholecystectomy for acute cholecystitis between D

Clinical and Molecular Signatures of Gallbladder Lesions: Insights into Metabolic and Inflammatory Pathways.

Background: Gallbladder carcinoma (GBC) represents one of the most aggressive malignancies of the hepatobiliary system, evolving along a continuum from chronic inflammation to preneoplastic lesions and invasive cancer. This progression is frequently associated with gallstones and chronic cholecystitis and shares common pathogenic mechanisms with systemic inflammatory and metabolic disorders. Despite its relatively low incidence, GBC is characterized by poor prognosis, largely due to late-stage diagnosis and limited understanding of its molecular underpinnings. Methods: We conducted an observat

Diagnostic value of cystic artery colour Doppler ultrasound indices in acute cholecystitis.

OBJECTIVE: To evaluate the diagnostic performance of cystic artery colour Doppler ultrasound indices in differentiating acute from chronic cholecystitis and to assess their added value over clinical and grayscale findings. METHODS: In this prospective study, 101 adults with clinically suspected cholecystitis underwent ultrasound before cholecystectomy. Grayscale features and colour Doppler measurements of cystic and hepatic artery peak systolic velocity and resistive index were recorded; histopathology was the reference standard. Logistic regression models using clinical and grayscale variabl

Clinical, gross, and microscopic findings in 7 owl monkeys (Aotus spp) with chronic biliary tree disease.

Owl monkeys are nocturnal primates in the genus Aotus that range across a wide variety of arboreal habitats from Central to South America. Owl monkeys are an important animal model for malaria research. Despite decades of use as a laboratory animal model, disease of the hepatobiliary system is rarely described in this species. The exception is cholelithiasis, which has been reported in the species as an incidental finding with minimal serum chemistry derangements or histologic evidence of disease. However, a retrospective analysis of clinical histories and necropsy reports from owl monkeys hou

High throughput screening of nuclear receptors identifies NR4A1 as a novel tumor suppressor in gallbladder cancer.

Gallbladder cancer (GBC) is a major malignancy of the hepato-biliary tract, often detected at advanced, inoperable stages with limited therapeutic options. Nuclear receptors (NRs), a family of ligand-dependent transcription factors, play key roles in cancer biology and represent promising drug targets. mRNA expression of 48 NRs was profiled in GBC (n = 13) and chronic cholecystitis tissues using the Nanostring nCounter platform, identifying NR4A1 as significantly downregulated. NR4A1 activity was modulated in GBC cell lines using the agonist Cytosporone B (CSNB) or siRNA knockdown, and effects

Management of an unsuspected double gall bladder.

Gall bladder duplication is a rare congenital anomaly that is often asymptomatic and frequently goes undiagnosed until surgery. We present the case of a 47-year-old female who reported abdominal pain, vomiting, loss of appetite and generalised weakness. Initial imaging, including ultrasound and magnetic resonance cholangiopancreatography, identified cholelithiasis but did not reveal the presence of a duplicated gall bladder. During a laparoscopic cholecystectomy, a double gall bladder with two separate cystic ducts was unexpectedly discovered. Both gall bladders were successfully removed using

A prospective observational study of the relationship between Helicobacter pylori and symptomatic gallstone disease.

In this prospective observational study, we aimed to evaluate the incidence of Helicobacter pylori infection in gallstone disease and to assess its association with different types of gallstones and histopathological findings. It was conducted on 200 patients undergoing cholecystectomy. H. pylori detection was performed using the rapid urease test and histopathological examination. Cholesterol stones constituted 70% of cases, while pigment stones accounted for 30%. The overall incidence of H. pylori infection was 38.5%. A significantly higher prevalence of H. pylori was observed in cholesterol

Duplicate gallbladder masquerading as a choledochal cyst: a case of single-port robotic surgery.

Gallbladder duplication is a rare congenital anomaly (1 in 4000 births) that can mimic other biliary pathologies, risking iatrogenic injury. A 61-year-old male presented with fever, right upper quadrant pain, and elevated aminotransferases. Contrast-enhanced computed tomography and magnetic resonance imaging initially suggested a 3.6-cm Todani type II choledochal cyst posterior to the gallbladder. Suspecting duplication, surgeons utilized a single-port robotic approach via the da Vinci SP system. Intraoperatively, two distinct gallbladders with independent cystic ducts were identified and succ

Synchronous Acute Cholecystitis and Perforated Acute Appendicitis in a 49-Year-Old Male Patient in Jordan: A Case Report.

Acute appendicitis (AAP) and acute cholecystitis (AC) are two of the most common surgical emergencies, yet their synchronous occurrence is exceptionally rare and diagnostically challenging. We report the case of a 49-year-old male patient who presented with a 10-day history of diffuse abdominal pain, nausea, vomiting, anorexia, and dark urine. Examination revealed abdominal tenderness with positive Murphy's and rebound signs. Laboratory tests demonstrated leukocytosis, neutrophilia, and elevated liver enzymes. Imaging with combined ultrasound and computed tomography confirmed both an inflamed,

Case report of pyloric obstruction caused by giant gallbladder compression with literature review.

Giant gallbladder is a rare clinical condition characterized by abnormal gallbladder enlargement, typically defined as a longitudinal diameter exceeding 14 cm or a volume surpassing 1.5 L. The most common pathological etiologies are cholelithiasis and chronic cholecystitis, followed by neoplastic factors, while congenital developmental anomalies and other causes are relatively uncommon. Herein, we report a case of giant gallbladder-induced pyloric obstruction in a patient who presented with nausea and vomiting for two weeks-symptoms that recurred after initial resolution of diabetic ketoacidos

Severe Gallbladder Dyskinesia With 2% Ejection Fraction: A Comprehensive Clinicoradiologic and Pathologic Case Correlation.

Gallbladder dyskinesia (GBD) is a functional biliary disorder characterized by biliary colic in the absence of cholelithiasis or mechanical obstruction and is identified by reduced gallbladder ejection fraction (EF) on cholecystokinin (CCK)-stimulated hepatobiliary scintigraphy. Patients may experience significant symptoms due to impaired gallbladder contractility and bile stasis despite the absence of gallstones or inflammatory structural abnormalities on imaging. We present a 42-year-old male with a three-month history of episodic right upper quadrant (RUQ) pain triggered by fatty meals. Abd
