Endoscopic Ultrasound-Guided Gallbladder Drainage Using Lumen-Apposing Metal Stents for Non-Surgical Candidates With Cholecystitis: A Prospective Cohort Study.

INTRODUCTION: Management of acute cholecystitis (AC) in high-risk surgical candidates is challenging. Drainage via percutaneous transhepatic gallbladder drainage (PT-GBD) has been standard of care, with high complication and re-intervention rates. Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) using lumen-apposing metal stents (LAMS) is proposed as an alternative. This study evaluates the efficacy and safety of LAMS in EUS-GBD for non-surgical candidates presenting with AC. METHODS: An institution-based prospective database has tracked all patients who undergo EUS-GBD using LAMS

Case Report: A case study of geriatric acute cholecystitis complicated by coinfection with Shewanella putrefaciens and Enterococcus faecium.

BACKGROUND: Acute cholecystitis is a common abdominal condition mainly caused by enteric Gram-negative bacilli and Enterococcus species. Advances in microbial detection have highlighted infections by rare pathogens like Shewanella putrefaciens (S. putrefaciens), an opportunistic bacterium from aquatic environments affecting mainly immunocompromised or comorbid patients. Its clinical features, antibiotic resistance, and treatment remain unclear. CASE PRESENTATION: This article presents a case study of an 87-year-old female patient with a medical history of gallstones and previous endoscopic re

Radiologic predictors of early recurrence of acute cholecystitis following percutaneous cholecystostomy tube removal.

BACKGROUND: Percutaneous cholecystostomy (PC) is commonly utilized in high-risk patients with acute cholecystitis as either a bridge to surgery or as definitive therapy. However, early recurrence following tube removal remains a significant clinical concern. This study aimed to identify imaging predictors associated with early recurrence after PC tube removal. METHODS: A total of 1,002 patients who underwent PC between 2008 and 2024 were retrospectively reviewed. Patients who subsequently underwent cholecystectomy (n = 606), were discharged with the tube in place (n = 221), or did not undergo

Endoscopic management of biliary stricture in primary sclerosing cholangitis.

Primary sclerosing cholangitis (PSC) is a progressive fibroinflammatory disease characterized by multifocal biliary strictures, recurrent cholangitis, and a markedly increased lifetime risk of cholangiocarcinoma (CCA). Endoscopic retrograde cholangiopancreatography (ERCP) remains central to the diagnosis of CCA and management of PSC-related complications. This review synthesizes current evidence guiding the use of ERCP in patients with PSC, highlighting the importance of careful patient selection to mitigate adverse events. We review traditional ERCP techniques for the evaluation of dominant s

Spinal Cord Injury Without Radiographic Abnormality Complicated by Acute Cholecystitis: A Case Report Highlighting Diagnostic and Therapeutic Challenges.

A man in his early 90s was brought to our emergency department after a fall in which he struck his left shoulder and subsequently became unable to move his left upper extremity. On arrival, neurological examination revealed muscle weakness and paresthesia predominantly affecting the left upper and lower extremities below the C6 level. Cervical magnetic resonance imaging demonstrated narrowing of the spinal canal at the C4/5 and C5/6 levels, corresponding to the level of neurological deficits, along with multilevel left-sided foraminal stenosis, leading to a diagnosis of spinal cord injury with

Robotic reoperative management of recurrent gallbladder disease after subtotal cholecystectomy: a contemporary single-institution experience.

BACKGROUND: The increasing adoption of incomplete cholecystectomy in severe cholecystitis has created a growing population of patients who later develop recurrent symptoms from remnant gallbladder or cystic duct pathology. Reoperative cholecystectomy is definitive but technically challenging, and the role of robotic surgery in this setting remains poorly defined. METHODS: A retrospective cohort study was performed of adult patients undergoing robot-assisted completion or remnant cholecystectomy for recurrent gallbladder disease at a single tertiary institution between 2022 and 2024. Intramural Duodenal Abscess Masquerading as Malignancy: Diagnosed and Managed by Endoscopic Ultrasound.

Subepithelial lesions of the gastrointestinal tract are frequently encountered and are most often benign. Among the etiologies of subepithelial lesions, duodenal abscesses are exceptionally uncommon. Duodenal abscesses may have imaging characteristics that overlap with malignancy. A patient with history of necrotizing cholecystitis presented with abdominal pain 3 months after his cholecystectomy. Cross-sectional imaging revealed suspected duodenal malignancy. Endoscopic ultrasound identified a subepithelial lesion allowing for diagnosis and treatment of an intramural duodenal abscess. This cas

Incidentally Detected Gallbladder Adenocarcinoma Presenting as Acute Calculous Cholecystitis: A Case Report and Management Considerations.

Gallbladder cancer is an aggressive malignancy of the biliary tract with a poor prognosis and variable geographic distribution. It may be identified incidentally after a cholecystectomy performed for presumed benign disease, with diagnosis established on final histopathological examination. Surgeons should be aware of this possibility, as incidental detection has important implications for staging and subsequent management. Early recognition and appropriate evaluation are essential to guide treatment and improve outcomes.

Association between the Parkland grading scale score and postoperative complications according to the Clavien-Dindo classification in emergency laparoscopic cholecystectomy.

Laparoscopic cholecystectomy is the standard treatment for acute calculous cholecystitis. The Parkland grading scale was developed to assess intraoperative severity; however, its association with postoperative outcomes remains underexplored in low- and middle-income countries. This study aimed to evaluate the associations between the Parkland grading scale and postoperative complications according to the Clavien–Dindo classification in emergency laparoscopic cholecystectomy. A retrospective observational study was conducted including patients who underwent emergency laparoscopic cholecystectom

Endoscopic ultrasound-guided gallbladder drainage in acute cholecystitis with contained perforation: a prospective cohort study.

BACKGROUND: Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) is usually contraindicated in perforated cholecystitis, although initial reports suggest feasibility in contained perforation. METHODS: This prospective single-center cohort study aimed to explore EUS-GBD outcomes in patients with acute cholecystitis with contained perforation (cp-AC; Niemeier classification type 2). A secondary exploratory comparison was performed with a contemporaneous cohort of patients with intact gallbladder walls. RESULTS: Between January 2021 and March 2025, 23 patients with cp-AC (39.7% of the ov

"Results of the structured implementation of a robotic acute care surgery program".

The adoption of robotic surgery in emergency general surgery has traditionally been limited to sporadic use, mainly because of organizational, logistical, and training-related barriers. Evidence regarding the structured implementation of robotic platforms in a continuous 24/7 emergency setting is scarce. This study aimed to evaluate the implementation and early operational performance of a structured robotic acute care surgery program. We conducted a prospective observational study to describe the implementation and early outcomes of a structured 24/7 robotic acute care surgery program at a te

Parallel Configuration of the Cystic and Common Hepatic Ducts: a Surgical Odyssey.

INTRODUCTION: Laparoscopic cholecystectomy is among the most frequently performed surgical procedures worldwide; however, it carries a persistent risk of iatrogenic bile duct injury, particularly in the presence of anatomical variations. Misinterpretation of biliary anatomy remains a leading cause of serious complications, underscoring the importance of recognizing rare variants and adopting safe surgical strategies. CASE REPORT: We report the case of a 78-year-old patient undergoing elective laparoscopic cholecystectomy for recurrent cholecystitis and cholangitis. Given the history of repeat
