Evolution of gallbladder drainage: a comprehensive review from percutaneous to endoscopic ultrasound-guided approaches.

Acute cholecystitis frequently occurs in elderly or medically fragile patients. While early cholecystectomy remains the standard treatment for operable patients, gallbladder drainage plays a critical role in those who are poor candidates for immediate surgery. Over the past decade, gallbladder drainage has evolved from reliance on percutaneous transhepatic gallbladder drainage (PT-GBD) to a spectrum of endoscopic alternatives, including endoscopic transpapillary gallbladder drainage (ET-GBD) and endoscopic ultrasound-guided gallbladder drainage (EUS-GBD). Although PT-GBD provides rapid and rel

Timing of laparoscopic cholecystectomy after percutaneous transhepatic gallbladder drainage and the risk of bailout surgery: a restricted cubic spline analysis.

BACKGROUND: The optimal timing of laparoscopic cholecystectomy (LC) after percutaneous transhepatic gallbladder drainage (PTGBD) for acute cholecystitis remains controversial. Previous studies have often categorized surgical timing using arbitrary cutoffs and have not adequately evaluated potential non-linear associations. In particular, the interval from PTGBD to LC has rarely been analyzed as a continuous variable when assessing operative difficulty. This study aimed to assess the relationship between the interval from PTGBD to LC and intraoperative technical difficulty using restricted cubi

Dedicated day surgery supported by remote patient monitoring for appendicectomy and cholecystectomy procedures: A cost-effectiveness analysis.

BackgroundManaging inpatient demand is challenging, especially for high-volume emergency surgical casemixes such as acute appendicitis and cholecystitis. These conditions drive unplanned admissions and consume significant bed capacity, despite their suitability for accelerated pathways. We evaluated a bundled care model for this casemix, incorporating same-day discharge protocols and remote patient monitoring to reduce unnecessary inpatient stays and optimize hospital resources.MethodsA decision tree model compared the proposed same-day discharge surgical care model supported by remote patient

Predictive value of biochemical markers and clinical scoring systems for outcomes in nonagenarians and centenarians admitted to the ICU after cholecystectomy for acute cholecystitis: a binational multicenter analysis.

INTRODUCTION: With the global rise in nonagenarian and centenarian populations, demand for surgical and critical care in this age group is increasing. Acute biliary disease, particularly cholecystitis, often necessitates surgery, yet outcomes following cholecystectomy in the oldest-old ICU patients remain poorly characterized. This study aimed to identify clinical and biochemical predictors of mortality and resource use in this cohort. METHODS: In this retrospective binational cohort study, we used the Australian and New Zealand Intensive Care Society Adult Patient Database (2010–2024) to iden

Isolated IgG4-related cholecystitis mimicking locally advanced gallbladder cancer: a case report.

Immunoglobulin G4 (IgG4)-related disease is a fibroinflammatory condition that can involve multiple organs. Isolated involvement of the gallbladder is exceptionally uncommon and often radiologically indistinguishable from gallbladder carcinoma, resulting in diagnostic uncertainty before surgery. A 77-year-old man presented with right upper abdominal discomfort lasting 1 month. Cross-sectional imaging revealed irregular gallbladder wall thickening with contrast enhancement, raising suspicion for resectable gallbladder cancer. The patient underwent a radical cholecystectomy with partial hepatect

Electrical risk score and electrocardiographic markers in acute cholecystitis: a prospective cross-sectional study using explainable machine learning.

BACKGROUND: This study investigated the association between the Electrical Risk Score (ERS), derived from 12-lead electrocardiography (ECG), and electrocardiographic markers related to ventricular arrhythmia risk in patients with acute cholecystitis (AC), using conventional statistical analyses and machine learning–based exploratory analyses. Given the cross-sectional study design, clinical cardiovascular outcomes and longitudinal prognostic effects were not assessed. METHODS: In this prospective cross-sectional study, 153 patients diagnosed with AC and 100 age- and sex-matched healthy control

Acalculous cholecystitis with gallbladder perforation in a critically ill trauma patient: An unexpected complication.

Gallbladder perforation is a rare complication following blunt abdominal trauma, largely due to its anatomical protection within the liver parenchyma. We present the case of a male in his 50s who sustained multiple traumatic injuries following high-impact motor vehicle collision. He was initially admitted with haemodynamic compromise necessitating emergency trauma laparotomy, splenectomy and a traumatic diaphragmatic hernia repair. There was no evidence of gallbladder or liver injury in the preoperative abdominal CT scan or laparotomy. Despite this, he developed new RUQ pain, anaemia and early

Sarcopenia independently predicts acute cholecystitis in older patients with gallstones: a retrospective cohort study.

BACKGROUND: This study investigated whether sarcopenia independently predicts incident acute cholecystitis (AC) in geriatric patients with gallstones. METHODS: This retrospective cohort study included 1,177 patients aged ≥65 years with incidentally detected gallstones and no prior biliary disease. Sarcopenia was diagnosed using Asian Working Group for Sarcopenia 2019 criteria. The primary outcome was AC development during follow-up, adjudicated using Tokyo Guidelines 2018. Multivariate logistic and Cox regression models adjusted for age, BMI, sex, alcoholism, and Charlson Comorbidity Index.7.

A case of gallbladder adenoma causing gallbladder haemorrhage.

Gallbladder haemorrhage is a rare but life-threatening condition. We report a unique case of a 50-year-old female who presented with acute cholecystitis due to massive intracholecystic haemorrhage. A 17-mm pedunculated gallbladder polyp had been identified, but the patient opted for observation. Three weeks later, she presented to the emergency department with a chief complaint of epigastric pain. Computed tomography showed a distended gallbladder filled with hyperdense material, and magnetic resonance imaging confirmed a haematoma. Semi-urgent cholecystectomy was performed on the third hospit

SURGICAL TREATMENT OF OBSTRUCTIVE JAUNDICE IN BENIGN DISEASES OF THE BILIARY TRACT.

INTRODUCTION: Obstructive jaundice (OJ) remains a significant clinical and diagnostic challenge due to the high risk of hepatic failure and homeostasis disturbances. The main causes of OJ include biliary calculi (≈50%), malignant tumors (up to 40%), and biliary strictures (≈10%). Postoperative complications occur in 15.6-63.4% of cases, with mortality rates reaching 25-30%, highlighting the need for timely biliary decompression and the use of minimally invasive treatment methods to improve outcomes. AIM: To analyze the outcomes of the implementation and optimization of surgical treatment meth

Acute cholecystitis in an intrahepatic duplicated gallbladder mimicking a liver abscess: The pivotal diagnostic role of percutaneous drainage.

Gallbladder duplication is a rare congenital anomaly that poses significant diagnostic challenges, particularly when one moiety is intrahepatic. Herein, we report a case of acute cholecystitis in a 40-year-old man with a duplicated gallbladder that closely mimicked a liver abscess. Initial contrast-enhanced computed tomography revealed a rim-enhancing fluid collection in segment 4 of the liver, which was distinct from the normal gallbladder. Based on the intrahepatic location and signs of inflammation, a liver abscess was initially suspected. Ultrasound-guided percutaneous drainage was perform

Early and Long-Term Outcomes of Laparoscopic Reconstituting Subtotal Cholecystectomy: Impact of Surgical Approach and Stump Closure Method.

PURPOSE: Laparoscopic cholecystectomy is common, but bile duct injury occurs in 0.3% of cases. Moderate to severe cholecystitis increases operative difficulty, and subtotal cholecystectomy is recommended as a preventive procedure. This study evaluated the outcomes of reconstituting subtotal cholecystectomy for cholecystitis at our institution. METHODS: We retrospectively analyzed 2707 patients who underwent cholecystectomy between January 2010 and December 2022. Of these, 109 patients underwent reconstituting subtotal cholecystectomy. Patients were classified into a laparoscopic group (36 cas

Impact of percutaneous cholecystostomy timing on clinical outcomes in moderate-to-severe acute cholecystitis.

OBJECTIVES: The optimal timing of percutaneous cholecystostomy (PC) in moderate-to-severe acute cholecystitis remains uncertain. This study evaluated the relationship between PC timing and clinical outcomes, particularly hospital length of stay (LOS), in patients with Tokyo Grade II-III acute cholecystitis. METHODS: This retrospective single-center study included 102 patients with Tokyo Grade II-III acute cholecystitis who underwent PC between January 2022 and January 2025. Patients were classified as early (≤24 h), intermediate (25-48 h), or delayed (≥49 h) according to the timing of drainag

Chest Pain and Palpitations After Spinal Surgery: An Atypical Presentation of Acute Cholecystitis.

Acute cholecystitis is a common disease that affects ~200,000 people in the United States each year. Early diagnosis and gallbladder removal are associated with improved patient outcomes, including fewer postoperative complications, shorter length of stay, and lower hospital costs. Acute cholecystitis typically presents with right upper quadrant (RUQ) pain associated with nausea and vomiting. The pathophysiological mechanism of RUQ pain corresponds with dermatomes that arise from thoracic spinal cord levels T6 to T8. We describe a case of a 40-year-old male who presented with a 2-day history o

Modeling Post-cholecystitis Complication Risk from Perioperative Liver Function and Immune-inflammation Indicators.

Acute calculous cholecystitis (ACC) often triggers transient perioperative elevations in liver enzymes and systemic inflammation, yet existing complication-prediction tools seldom incorporate dynamic biomarker changes. Our goal was to establish and develop, using internal validation, a multivariable risk model that incorporates perioperative variations in liver function tests (LFTs) and the Systemic Immune-Inflammation Index (SII) in order to predict Clavien-Dindo grade ≥II complications following cholecystectomy for ACC. In this retrospective cohort study at a tertiary academic center (Januar

Recurrent Chest Pain as the Sole Initial Manifestation of Acute Cholecystitis: A Case Report and Diagnostic Challenge.

Acute cholecystitis can present atypically, mimicking acute coronary syndrome (ACS) and leading to diagnostic delays. We report a case of a 49-year-old man with diabetes, hypertension, and dyslipidemia who presented three times over 18 days with episodic chest pain radiating to the neck and back. Serial electrocardiograms and high-sensitivity troponin assays were normal. On the third presentation, he developed epigastric tenderness. A right upper quadrant ultrasound was unremarkable; however, a CT aortogram revealed a 1.7 cm impacted gallstone. A prior normal coronary CT angiogram effectively

Successful ERCP guided rescue cystic duct embolization for the persistent bile leak after subtotal cholecystectomy: a case report.

INTRODUCTION AND IMPORTANCE: Bile leakage is a recognized complication of subtotal cholecystectomy, particularly when using the fenestrating technique. Persistent leaks may not respond to standard endoscopic stenting. In refractory cases, non-surgical embolization using agents such as coils, glue, or gelatin sponge has emerged as a potential alternative to reoperation. CASE PRESENTATION: We present a case of a 25-year-old male who developed persistent bile leakage following laparoscopic fenestrating STC for severe cholecystitis. The initial management with endoscopic retrograde cholangiopancr
