Cross-sectional association between gallstone disease and nonalcoholic fatty liver disease: Exploring the mediating roles of hematological and biochemical markers.

Nonalcoholic fatty liver disease (NAFLD) is the most common chronic liver disorder worldwide and is closely associated with metabolic comorbidities. Gallstone disease (GSD), another prevalent hepatobiliary condition, frequently coexists with NAFLD; however, their relationship and underlying mediating mechanisms remain unclear. This study aimed to investigate the association between a history of GSD and NAFLD using data from National Health and Nutrition Examination Survey 2017-March 2020. Restricted cubic spline models were employed to assess the linearity of this association, stratified analy

Association between the remnant cholesterol inflammatory index and gallstone disease: A cross-sectional analysis based on NHANES 2017 to 2020.

Gallstone disease is a common gastrointestinal disorder influenced by lipid metabolism and systemic inflammation. The remnant cholesterol inflammatory index (RCII), derived from remnant cholesterol (RC) and high-sensitivity C-reactive protein, integrates both components, yet its relationship with gallstones remains unclear. This study aimed to investigate the association between the RCII and gallstone disease in a nationally representative US population. We conducted a cross-sectional analysis using data from 2120 adults in the 2017 to 2020 cycle of the National Health and Nutrition Examinatio

Thyrotoxicosis-Associated Cholestatic Jaundice in a Patient With Incidental Gallstone Disease: A Case Report and Literature Review.

BACKGROUND Thyrotoxicosis is associated with well-recognized and clinically significant complications, including congestive heart failure and cardiac arrhythmias. The relationship between hyperthyroidism and hepatic dysfunction is complex, with several proposed pathophysiological mechanisms; however, direct cholestatic injury is frequently overlooked. Although mild liver enzyme elevation is common in hyperthyroidism, clinically overt jaundice is rare. Nevertheless, increasing evidence from case reports and case series suggests that thyrotoxicosis, especially Graves disease, can result in chole

Sex differences in gallstone formation in mice on a lithogenic diet are driven by gut microbiota-mediated bile acid metabolism.

Sex differences in gallstone formation involve the gut microbiota, but the specific mediating mechanisms remain unclear. This study established gallstone and pseudo-germ-free mouse models and found that male mice maintained higher gut microbiota diversity, whereas females showed an increased Firmicutes/Bacteroidota ratio, with enrichment of Clostridium_sensu_stricto_13 and Lactobacillus. This shift led to increased production of toxic secondary bile acids-deoxycholic acid and lithocholic acid-resulting in elevated bile hydrophobicity and higher cholesterol saturation, which significantly incre

Cholecystostomy Drainage - Current Practices in a Tertiary Hepatobiliary Unit.

BACKGROUND: Acute calculous cholecystitis (ACC) is a common general surgical emergency and accounts for 20% of symptomatic gallstone presentations. According to the Tokyo Guidelines, severe ACC necessitates cholecystostomy drainage. Percutaneous Cholecystostomy Drains (PCD) are a temporary intervention in acutely unwell patients with gallbladder-related disease who are unfit to undergo surgery. PCD is used with a therapeutic or palliative intent for the management of gallbladder disease based on the condition and the patient's fitness. This study aims to assess the current role of PCDs in a te

p53 and β-Catenin Expression in Gallbladder Carcinoma: Translational Insights for Pediatric Cancer Biology.

BACKGROUND: The tumor suppressor p53 and the Wnt pathway effector β-catenin represent two of the most frequently dysregulated molecular components across human malignancies. Although gallbladder cancer (GBC) predominantly affects adults, the pattern and clinicopathological correlates of p53 and β-catenin dysregulation in GBC may illuminate how these pathways behave in different biological contexts, including developmental and pediatric oncological settings, where analogous pathway alterations drive distinct tumor types. OBJECTIVE: To characterize p53 and β-catenin expression patterns in gallb

Management of concomitant cholecystoduodenal and cholecystocolonic fistulas with sigmoid colonic gallstone: a case report.

Cholecystoduodenal fistula and cholecystocolonic fistula are rare complications of gallstone pathology. A 62-year-old female presented to our surgical assessment unit with abdominal pain and vomiting. A computed tomographic scan revealed complex biliary-enteric fistulas. The fistulas involved the gallbladder, duodenum, and colon, with evidence of partial gastric outlet obstruction secondary to a gallstone in the proximal duodenum. Furthermore, imaging revealed a large gallstone impacted proximal to the diverticular stricture in the sigmoid colon. A minimally invasive approach to resolve the fi

Mapping Mutations and Signaling Network Interactions to Guide Precision Therapy in Gallbladder Cancer.

BACKGROUND: Gallbladder cancer (GBC) is a biologically complex malignancy arising from the epithelial lining of the gallbladder, with adenocarcinoma constituting the major histological subtype. Early detection is challenging because of vague clinical manifestations and the organ's deep-seated anatomical location, resulting in diagnosis at advanced stages and limited treatment options. Consequently, patients often experience poor prognosis with low 5-year survival rates. Clinical and translational studies have demonstrated marked biological heterogeneity in GBC, primarily driven by diverse mole

Extracellular Vesicle-Mediated O-GlcNAcase Transfer Drives Neuronal Necroptosis to Facilitate Gallbladder Cancer Perineural Invasion.

UNLABELLED: Peripheral nerve invasion (PNI) is an early and decisive step in gallbladder cancer progression that strongly predicts poor postsurgical outcome. The tumor-neuron interactions that drive PNI could represent potential targets and biomarkers to improve treatment of gallbladder cancer. In this study, we demonstrated that gallbladder cancer provoked necroptosis of neurons to enable PNI. Gallbladder cancer cells transferred extracellular vesicles (EV) containing O-GlcNAcase (OGA) to neurons, which activated RIPK1-dependent necroptosis. Mechanistically, EV-derived OGA suppressed RIPK1 gl

Gallbladder disease diagnosis from ultrasound using squeeze-and-excitation capsule network with convolutional bidirectional long short-term memory.

The gallbladder is a small, hollow organ positioned beneath the liver, primarily responsible for temporarily storing bile. Bile is a fluid formed by the liver that helps digestion. There are various types of gallbladder disease. Early diagnosis and identification are crucial for effective treatment of gallbladder disorders. Poor medical outcomes and improved patient symptoms may result from errors or delays in diagnosis. Numerous symptoms and signs, particularly those associated with gallbladder disorder, might be blurred. As a result, medical specialists must understand and interpret ultrasou

Untargeted Serum Metabolomics Reveals Differential Signatures in Gallstone-Associated and Gallstone-Free Gallbladder Cancer Variants.

Gallbladder cancer (GBC) is an aggressive malignancy often associated with gallstones (GBCGS), a condition distinct from gallstone disease (GSD). Both GBC and GBCGS are rare, with unclear pathogenesis and no established biomarker-based diagnostics. This pilot study aimed to identify distinct metabolic signatures in GBC and GBCGS for early diagnosis and stratification of high-risk GSD patients. Comparative untargeted serum metabolomic profiling was performed across three groups: GBC (n1 = 9), GBCGS (n2 = 11), and GSD (n3 = 10). A total of 35,385 mass features with MS/MS characteristics were det
