Lipid metabolism and gallstone disease risk: a multicenter study.

This study aimed to explore the associations between various lipid parameters and the risk of gallstone disease (GSD). A multicenter cross-sectional study was conducted to explore the associations between nine lipid parameters and the risk of GSD. Multivariable logistic regression models, adjusted for covariates, were employed to evaluate these associations. Stratified analyses by age and sex were performed to assess population-specific effects. Data from multiple centers were integrated using meta-analysis to generate pooled odds ratios with 95% confidence intervals. Trial Sequential Analysis

Digital modeling of the gallbladder for revealing microanatomical features and optimizing surgical approaches in gallbladder-preserving cholelithotomy.

With the advancement of minimally invasive techniques and the reevaluation of surgical indications, gallbladder-preserving cholecystolithotomy is anticipated to serve as an alternative to cholecystectomy in specific cases. The choice of surgical incision is critical for optimizing gallbladder function preservation, which is a pivotal factor influencing the prognosis of gallbladder-preserving cholecystolithotomy. Consequently, a comprehensive understanding of the distribution of microstructures, including gallbladder blood vessels and nerves, is of substantial significance. For this study, we s

Machine learning-based clinical prediction model and multi-omics integration for assessing pancreatic cancer risk in new-onset diabetes.

BACKGROUND: Given that pancreatic cancer (PC) is typically diagnosed at an advanced stage but is often preceded by new-onset diabetes mellitus (NODM), providing a window for early detection, we sought to develop and validate an interpretable machine-learning model integrated with multi-omics profiling to identify early biomarkers of NODM-associated PC. METHODS: In a population-based cohort, individuals with NODM-associated PC and NODM without PC were identified and randomly divided (70:30) into training and validation sets after feature selection. Eight machine learning (ML) classifiers were c

ERBB2 I655V mutation correlates with efficacy of immunotherapy in gallbladder cancer.

BACKGROUND: Gallbladder cancer (GBC) is a highly aggressive malignancy of the digestive system with a poor prognosis. Therefore, the development of effective targeted therapeutic strategies is critical for improving survival outcomes in patients with GBC. Erb-B2 receptor tyrosine kinase 2 (ERBB2) is a proto-oncogene whose overexpression or mutation has been closely linked to the initiation and progression of various cancers. METHODS: Whole-exome sequencing (WES) was performed on tumor tissue from a patient with advanced GBC who underwent conversion surgery following combination therapy with t

Emergence of Liquid Biopsy in Precision Oncology: A Review of its diagnostic, Predictive and Therapeutic Applications in Gallbladder Cancer.

BACKGROUND: Gallbladder cancer (GBC) is associated with a poor prognosis due to the challenges in early diagnosis. The gallbladder's anatomical constraints make it difficult to obtain tumor tissue for histological analysis and diagnostic monitoring. Consequently, liquid biopsy (LB) has emerged as a vital technique for minimally invasive extraction of tumor-derived materials. This review explores various non-invasive methods for GBC screening, with a particular focus on liquid biopsy. METHODS: A comprehensive search was conducted on PubMed, Scopus, Web of Science, and Google Scholar for articl

Cholecystectomy Is Linked With Lower Respiratory Exchange Ratio and Higher Lipid Oxidation and Sleep Energy Expenditure.

OBJECTIVE: Cholecystectomy (GBX) may alter energy metabolism, but human evidence is limited. We examined whether GBX alters energy expenditure (EE), respiratory exchange ratio (RER), and substrate oxidation. METHODS: A total of 384 healthy Southwestern Indigenous American adults (222 males, age 28 ± 6 years) were studied, including individuals with a history of gallbladder surgery [GBX(+), n = 39] and without surgery [GBX(-), n = 345]. In addition, 24-h energy metabolism was measured in a respiratory chamber. General linear models were adjusted for age, sex, body composition, and glucose regu

Twenty-year temporal trends, risk profiles, and prognostic value of clinicopathological features in gallbladder cancer following cholecystectomy.

BACKGROUND: Gallbladder cancer (GBC) is a rare but highly aggressive malignancy often diagnosed at an advanced stage. Owing to its relatively low incidence, the clinicopathologic features of GBC and its precursor lesions remain poorly understood and insufficiently explored. AIM: To examine temporal trends and clinicopathological features in post-cholecystectomy GBC to identify risks profiles and prognostic value. METHODS: In this retrospective study, 15785 patients who underwent cholecystectomy over a 20-year period at a single center were reviewed, among whom 285 were diagnosed with GBC. Cl

Causality between Cholecystectomy, Blood Lipids, and Major Adverse Cardiac and Cerebrovascular Events: A Multimodal Approach Highlighting Lipid Regulation.

Cholecystectomy alters lipid profiles and is associated with the risk of major adverse cardiac and cerebrovascular events (MACCE), yet the results are ambiguous. To assess the causal effects of cholecystectomy on blood lipid levels and risks of MACCE, we performed Mendelian randomization (MR) aiming to reduce confounding. We used genetic data on gallbladder removal, lipid levels, and MACCE from public databases. MR analysis estimated causal effects using genetic variants as instruments. Enrichment analysis identified relevant metabolic pathways, while multivariable MR evaluated specific lipid

Gallbladder and Sphincter of Oddi Disorders.

Dysfunctional gallbladder disorder (DGBD) and sphincter of Oddi disorder (SOD) are possible causes of abdominal pain, biliary obstruction, and acute pancreatitis, and are often invoked when a structural etiology is not obvious. Diagnosis was traditionally based on gallbladder scintigraphy and sphincter of Oddi manometry, both of which have fallen out of favor and are no longer part of the Rome Diagnostic Criteria. For DGBD, the presence of typical biliary pain and persistence of symptoms despite watchful waiting, and for SOD, objective evidence of biliary obstruction and pancreatitis are now c

Surgical Techniques: Robotic versus conventional Laparoscopic cholecystectomy IN benign Gallbladder disease: a randomized controlled, open, parallel, non-inferiority, single-center trial (STaRLING trial).

BACKGROUND: Robotic surgery is increasingly being adopted for gallbladder removal, despite the absence of randomized trials comparing its safety and patient outcomes to the current gold standard, laparoscopic cholecystectomy (LC). Existing literature reviews have found several case series showing no significant differences in postoperative complications or recovery between robotic cholecystectomy (RC) and LC. This trial aims to evaluate whether RC is non-inferior to conventional LC. METHODS: This is a prospective single-center, randomized controlled, parallel, non-inferiority trial. All adult

Preoperative Advanced Lung Cancer Inflammation Index as a Potential Marker for Incidental Gallbladder Carcinoma: A Matched Case-Control Study.

Background and Objectives: Incidental gallbladder carcinoma (IGBC) is an uncommon but clinically significant finding after elective cholecystectomy, as failure to recognize malignancy preoperatively may lead to inadequate initial surgical management. Inflammation-based hematological indices have been explored in various gastrointestinal malignancies; however, data focusing on preoperative discrimination of IGBC are limited. This study aimed to evaluate the diagnostic performance of the Advanced Lung Cancer Inflammation Index (ALI) and other inflammatory and immunonutritional indices in disting
