Expression of Vascular Endothelial Growth Factor A in Gallbladder Cancer Cells: A Clinicopathological Study.

BACKGROUND/AIM: Vascular endothelial growth factor (VEGF) is a key mediator of tumor angiogenesis. However, the clinicopathological and prognostic significance of VEGF expression gallbladder cancer (GBC) remains incompletely defined. This study evaluated the significance of VEGF expression in resected GBC specimens. PATIENTS AND METHODS: We retrospectively reviewed 53 patients who underwent curative resection for GBC between 2001 and 2019. VEGF expression was evaluated using immunohistochemistry (IHC) in carcinoma tissues and paired non-cancerous mucosa. RESULTS: VEGF positivity was more fre

Lymph node metastasis, dissection efficacy, and prognostic differences between direct and incidental gallbladder carcinoma: A retrospective study.

AIMS: Gallbladder carcinoma (GBC) has a poor prognosis, with lymph node metastasis (LNM) being a key prognostic factor. Controversies remain regarding lymph node dissection (LND) utility and prognostic differences of direct (DGBC) vs. incidental (IGBC) GBC. METHODS: Retrospective analysis of 193 GBC patients who underwent radical resection (2012.2-2024.9). Patients were categorized in two ways: first by LNM status (LNM group vs. LN0 group) and second by GBC type (DGBC group vs. IGBC group). Clinical, pathological, and follow-up data were collected; survival analyses used Kaplan-Meier (KM) cur

Macrophage-rich niches regulate T cell dynamics at the liver invasive margin during gallbladder cancer progression.

Liver invasion is one of the most frequent events in the progression of gallbladder cancer (GBC). However, the cellular and pathological role of the tumor-liver-interface microenvironment in liver invasion is still enigmatic. Here, we applied single-cell and spatial transcriptomics to systematically investigate the cellular component and gene expression regulation of the microenvironment from the tumor to the liver, specifically the invasive boundary. Our analyses revealed that CXCL9+ macrophage-rich immune cell niches were accumulated in the tumor-liver invasive margin, where 2 subclasses of

Bariatric Surgery With or Without Concomitant Laparoscopic Cholecystectomy in Morbidly Obese Patients With Gallbladder Stone Disease: A Prospective Randomized Controlled Pilot Study.

INTRODUCTION: Imagine a surgeon's critical decision: Should the gallbladder be removed now, along with the planned bariatric surgery, or risk the complication and necessity of a second surgery later? This clinical dilemma is central to treating morbidly obese patients, who face a high prevalence of gallstone disease exacerbated by rapid postoperative weight loss. The best approach to managing existing gallstones in bariatric candidates remains debated, with debate focusing on whether combining laparoscopic cholecystectomy (LC) with bariatric surgery is both safe and advantageous. In this pilot

Perioperative approaches for patients with biliary tract cancer.

Biliary tract cancers, encompassing intrahepatic, perihilar and distal cholangiocarcinoma and gallbladder cancer, are a heterogeneous group of highly aggressive malignancies. Most patients have unresectable disease at first presentation, and even those who undergo surgery are likely to have disease recurrence. Newer approaches have included liver transplantation for selected patients, and the integration of locoregional and systemic therapies has expanded the number of patients who can benefit from surgery. The advent of immune-checkpoint inhibitors and targeted therapies for patients with adv

Port Site Excision During Completion Extended Cholecystectomy for Incidental Gallbladder Cancer - A Scoping Review.

BACKGROUND: Completion extended cholecystectomy (CEC) is the standard treatment for incidental gallbladder cancer (iGBC) to reduce early recurrence and improve survival. However, the necessity of laparoscopic port site excision (PSE) during CEC remains controversial. This scoping review evaluates whether routine PSE is warranted in the management of iGBC. METHODS: A systematic search was conducted across PubMed, Google Scholar, Scopus, and the Cochrane Library using keywords such as "incidental gallbladder carcinoma," "radical cholecystectomy," "extended cholecystectomy," "port site excision,

Integrative Molecular Insights Into Epidemiological, Genetic, and Metabolic Risk Factors of Gallbladder Cancer: Implications for Biomarkers, Therapeutic Targeting, and Future Perspectives.

Gallbladder cancer (GBC) is a rare yet highly aggressive malignancy of the biliary tract, characterized by a five-year survival rate of less than 5%. Its asymptomatic onset and the lack of reliable early diagnostic tools contribute to delayed detection and poor clinical outcomes. Although epidemiological and genetic studies have identified numerous risk factors, the molecular mechanisms linking these factors to tumor initiation and progression remain incompletely understood. This review integrates current evidence on the multifactorial etiology of GBC-including geographic variation, genetic pr

Microcystin-LR Disrupts Bile Acid Homeostasis, Driving Cholestatic Liver Injury and Gallstones via FXR/SHP Antagonism.

Bile acids (BAs) are vital for liver health, and their dysregulation causes hepatobiliary disorders. Microcystins (MCs) are pervasive hepatotoxins in freshwaters worldwide that threaten public health; yet, their effects on BA homeostasis─particularly regulation mechanisms─remain poorly defined. Here, we show that chronic exposure to environmentally relevant doses of microcystin-LR (MC-LR) induces not only cholestatic liver injury but also gallstone formation in mice. Targeted metabolomics revealed that MC-LR strikingly altered BA profiles across the liver, gallbladder bile, and serum, characte
